Outcome of In-Hospital Cardiac Arrest in Adult General Wards

Objective: Few studies have focused on factors influencing outcomes of patients with in-hospital cardiac arrest (IHCA) in general wards. The goal of this study was to report the outcomes of adult patients with IHCA in the general wards and identified the prognostic factors. Methods: Adult patients with IHCA having received cardiopulmonary resuscitation in general wards from January 2008 to December 2011 were retrospectively reviewed from our registry system. The primary outcome was survival to hospital discharge, while the secondary outcome was sustained return of spontaneous circulation (ROSC). Results: A total of 544 general ward patients were analyzed for event variables and resuscitation results. The rate of establishing a ROSC was 40.1% and the rate of survival to discharge was 5.1%. Ventricular tachycardia/ventricular fibrillation (VT/VF) was the initial rhythm in 3.9% of patients. Pre-arrest factors including a high Charlson comorbidity index (CCI) ≥ 9 (OR 0.251, 95% CI 0.098 - 0.646), cardiac comorbidity (OR 0.612, 95% CI 0.401 - 0.933), and arrest time on the midnight shift (OR 0.403, 95% CI 0.252 - 0.642) were independently associated with a low possibility of ROSC. The initial VT/VF presenting rhythms (OR 0.135, 95% CI 0.030 - 0.601) were independently associated with a high survival rate, whereas patients with deteriorated disease course were independently associated with a decreased hospital survival (OR 3.902, 95% CI 1.619 - 9.403). Conclusions: We demonstrated that pre-arrest factors can predict patient outcome after IHCA in general wards, including the association of a CCI ≥ 9 and cardiac comorbidity with poor ROSC, and deteriorated disease course as an independent predictor of a low survival rate.

Although the performance of perovskite solar cells(PSCs)has been dramatically increased in recent years,stability is still the main obstacle preventing the PSCs from being *** device instability can be caused by a variety of reasons,including ions diffusion,surface and grain boundary defects,*** this work,the cross-linkable tannic acid(TA)is introduced to modify perovskite film through post-treatment *** numerous organic functional groups(–OH and C=O)in TA can interact with the uncoordinated Pb^(2+)and I^(-)ions in perovskite,thus passivating defects and inhibiting ions *** addition,the formed TA network can absorb a small amount of the residual moisture inside the device to protect the perovskite ***,TA modification regulates the energy level of perovskite,and reduces interfacial charge ***,following TA treatment,the device efficiency is increased significantly from 21.31%to 23.11%,with a decreased hysteresis ***,the treated device shows excellent air,thermal,and operational *** light of this,the readily available,inexpensive TA has the potential to operate as a multipurpose interfacial modifier to increase device efficiency while also enhancing device stability.

Tourism-Oriented Cultural Product Design: A Case Study of Yami Culture in Lanyu

Due to the prevalence of modern commerce and mainstream culture, many indigenous cultures around the world are rapidly diminishing. These cultures, with their unique primitiveness distinct from modern culture, often attract tourists. This study integrates the indigenous Yami culture of Lanyu Island, Taiwan Region, into innovative designs from the perspective of tourists’ understanding of the local culture. In this way, tourists can experience the harmonious coexistence of the Yami tribe and nature, while these culturally rich products foster deeper experiential and emotional resonance. This study employs the Mandala thinking method for concept expansion and the Osborn checklist method for the analysis and transformation of design elements. Subsequently, designers were invited to conduct on-site travel experiences, leading to the creation of a series of Yami cultural products. Through the evaluation and validation of cultural product assessment indicators, functional commercialized products were ultimately produced. This study aims to establish a design process for cultural products through this design case. It seeks to help users recall their Lanyu Island travel experiences and appreciate the often-overlooked cultural beauty, thereby raising awareness of cultural preservation.

Prophylactic treatment of Lacticaseibacillus paracasei K56 ameliorates asthma by regulating short-chain fatty acid production and Treg differentiation

Many studies have shown that the development of allergic asthma is associated with intestinal microbiota dysbiosis. Based on the gut-lung axis theory， probiotic intervention may be a potential strategy for respiratory diseases. Lacticaseibacillus paracasei K56 was reported to regulate gut microbiota in mice. However， its effect on allergic asthma has not been reported. In this study， we investigated the effect of the K56 on ovalbumin-induced asthma and its possible mechanisms. Our results showed that K56 reduced asthma symptoms and inflammatory cell infiltration in the lung of asthmatic mice. And K56 regulated the differentiation of helper T cells in the lung and intestine. Results from 16S rRNA gene sequencing showed that K56 prophylaxis significantly elevated the richness of Akkermansia and Burkholderia. Meanwhile， K56 abrogated the decrease in the levels of short-chain fatty acids (SCFAs) in the feces. In addition， a rebound in the mRNA expression of the SCFA receptors was observed after K56 prophylaxis. Notably， the regulatory T cell (Treg) frequencies and the FFAR3 levels were positively correlated. These results suggest that K56 could attenuate asthma， possibly by modulating the intestinal microbiota and regulating Treg differentiation through SCFA metabolized. Our study showed that K56 may be used as a probiotic to prevent pulmonary inflammation and dysbiosis of the intestinal microbiota in allergic asthma.

Background:Nonalcoholic fatty liver disease(NAFLD)is associated with impaired renal function,and both diseases often occur alongside other metabolic ***,the prevalence and risk factors for impaired renal function in patients with NAFLD remain *** objective of this study was to identify the prevalence and risk factors for renal impairment in NAFLD ***:All adults aged 18-70 years with ultrasound-diagnosed NAFLD and transient elastography examination from eight Asian centers were enrolled in this prospective *** fibrosis and cirrhosis were assessed by FibroScan-aspartate aminotransferase(FAST),Agile 3+and Agile 4 *** renal function and chronic kidney disease(CKD)were defined by an estimated glomerular filtration rate(eGFR)with value of<90 mL/min/1.73 m^(2) and<60 mL/min/1.73 m^(2),respectively,as estimated by the CKD-Epidemiology Collaboration(CKD-EPI)***:Among 529 included NAFLD patients,the prevalence rates of impaired renal function and CKD were 37.4%and 4.9%,*** multivariate analysis,a moderate-high risk of advanced liver fibrosis and cirrhosis according to Agile 3+and Agile 4 scores were independent risk factors for CKD(P<0.05).Furthermore,increased fasting plasma glucose(FPG)and blood pressure were significantly associated with impaired renal function after controlling for the other components of metabolic syndrome(P<0.05).Compared with patients with normoglycemia,those with prediabetes[FPG≥5.6 mmol/L or hemoglobin A1c(HbA1c)≥5.7%]were more likely to have impaired renal function(P<0.05).Conclusions:Agile 3+and Agile 4 are reliable for identifying NAFLD patients with high risk of *** glycemic control in the prediabetic stage might have a potential renoprotective role in these patients.

BACKGROUND Benign rectal strictures can be categorized as primary(disease-related)and secondary(surgical anastomosis-related).Secondary strictures arise from surgical complications,whereas primary strictures have diverse etiologies,including various inflammatory *** strictures are usually managed by surgery and *** present an unusual etiology of benign rectal stricture caused by the repeated insertion of foreign objects into the rectum for sexual purposes,resulting in rectal injury and subsequent chronic *** SUMMARY A 53-year-old man presented to the outpatient clinic of the Colorectal Surgery Department with symptoms of chronic constipation and bloody *** patient previously experienced rectal injury due to foreign object insertion for sexual *** revealed benign circumferential narrowing of the *** underwent treatment by endoscopic argon plasma coagulation and balloon dilation and follow-up as an outpatient for 4 months.A colonoscopy at the end of the follow-up period revealed no evidence of rectal stricture *** A history of rectal injury,followed by chronic inflammation,should be considered in patients with benign rectal *** with endoscopic argon plasma coagulation and balloon dilation can prevent the need for surgical resection of benign rectal strictures.

Decision and Coordination of an E-commerce Supply Chain Considering Returns and Network Externalities

Considering the large number of returns in online sales and the network externalities of e-platforms, we develop a decentralized model and a centralized model to explore the impacts of returns and network externalities on e-commerce supply chain (ECSC) decisions. We show that in the decentralized model, the service level, price, market demand, and ECSC members' profits increase with the network externality strength. However, the service level and price increase, while the market demand and ECSC members' profits decrease with the product return rate. The centralized model is the optimal operating mode when it is properly coordinated. We design the "commission and return cost-sharing" contract to optimize ECSC, in which the proportion of the e-platform's sharing of the return handling cost is exactly equal to the proportion of the system profit after coordination. Based on the decentralized model, we develop two extended models in which we endogenize the impacts of the service level and return rate on the network externality strength. Through comparisons between the extended and decentralized models, we show that high-quality service can improve ECSC's profitability, while a high return rate hurts its economic performance.

On a Heuristic Point of View about the Generalization of Curie Law to Cosmic Higgs Fields with the Casimir Effect

Condensed state physics demonstrates that the Curie temperature is the point at which spontaneous magnetization drops to zero, marking the critical transition where ferromagnetic or ferrimagnetic materials transform into paramagnetic substances. Below the Curie temperature, a material remains ferromagnetic;above it, the material becomes paramagnetic, with its magnetic field easily influenced by external magnetic fileds. For example, the Curie temperature of iron (Fe) is 1043 K, while that of neodymium magnets ranges from 583 to 673 K. From both physics and mathematics perspectives, examining the temperature properties of materials is essential, as it provides valuable insights into their electromagnetic and thermodynamic behaviors. This paper makes a bold assumption and, for the first time, carefully verifies the existence of a Casimir temperature at 0.00206 K under conditions of one-atomic spacing.
