Some multimodal signals-that is,occurring in more than one sensory modality-appear to carry additional information which is not present when component signals are presented separately.To understand the function of male ring dove’s(Streptopelia risoria)multimodal courtship,we used audiovisual playback of male displays to investigate female response to stimuli differing in their audiovisual timing.From natural courtship recordings,we created a shifted stimulus where audio was shifted relative to video by aﬁxed value and a jittered stimulus,where each call was moved randomly along the visual channel.We presented 3 groups of females with the same stimulus type,that is,control,shifted,and jittered,for 7 days.We recorded their behavior and assessed pre-and post-test blood estradiol concentration.We found that playback exposure increased estradiol levels,conﬁrming that this technique can be efﬁciently used to study doves’sexual communication.Additionally,chasing behavior(indicating sexual stimulation)increased over experimental days only in the control condition,suggesting a role of multimodal timing on female response.This stresses the importance of signal conﬁguration in multimodal communication,as additional information is likely to be contained in the temporal association between modalities.

A 41-year-old man with a continuous- flow left ventricular assist device presented for evaluation of dysphagia and dark urine. He was found to have a significantly elevated L-lactate dehydrogenaseand an elevated plasma free hemoglobin consistent with intravascular hemolysis.After the hemolysis ceased,both the black urine and dysphagia resolved spontaneously.Transient esophageal dysfunction,as a manifestation of gastrointestinal dysmotility,is known to occur in the setting of hemolysis.Paroxysmal nocturnal hemoglobinuria is another recognized cause of massive hemolysis with gastrointestinal dysmotility occurring in25%-35%of patients during a paroxysm.Intravascular hemolysis increases plasma free hemoglobin,which scavenges nitric oxide(NO),an important second messenger for smooth muscle cell relaxation.The decrease in NO can lead to esophageal spasm and resultant dysphagia.In our patient the resolution of hemolysis resulted in resolution of dysphagia.

Developing countries shoulder a considerable burden of gastroenterological disease. Infectious diseases in particular cause enormous morbidity and mortality. Diseases which afflict both western and developing countries are often seen in more florid forms in poorer countries. Innovative techniques continuously improve and update gastroenterological practice. However, advances in diagnosis and treatment which are commonplace in the West, have yet to reach many developing countries. Clinical guidelines, based on these advances and collated in resource-rich environments,lose their relevance outside these settings. In this two-part review, we first highlight the global burden of gastroenterological disease in three major areas: diarrhoeal diseases, hepatitis B, and Helicobacter pylori . Recent progress in their management is explored, with consideration of future solutions. The second part of the review focuses on the delivery of clinical services in developing countries. Inadequate numbers of healthcare workers hamper efforts to combat gastroenterological disease. Reasons for this shortage are examined, along with possibilities for increased specialist training. Endoscopy services, the mainstay of gastroenterology in the West, are in their infancy in many developing countries. The challenges faced by those setting up a service are illustrated by the example of a Nigerian endoscopy unit. Finally, we highlight the limited scope of many clinical guidelines produced in western countries. Guidelines which take account of resource limitations in the form of "cascades" are advocated in order to make these guidelines truly global. Recognition of the different working conditions facing practitioners worldwide is an important step towards narrowing the gap between gastroenterology in rich and poor countries.

BACKGROUND People with diabetes and peripheral artery disease(PAD)have a high risk of major adverse cardiovascular events(MACE).Prior research suggests that medical therapies aimed to control modifiable risk factors are poorly implemented in patients with PAD.AIM To examine the association between the control of modifiable risk factors,estimated by the novel PAD-medical score,and the incidence of MACE in people with PAD and diabetes.METHODS Participants were recruited from out-patient clinics if they had a diagnosis of both PAD and diabetes.Control of reversible risk factors was assessed by a new composite measure,the PAD-medical score.This score takes into account the control of low-density lipoprotein cholesterol,blood pressure,blood glucose,smoking and prescription of an anti-platelet.Participants were followed to record incidence of myocardial infarction,stroke and cardiovascular death(MACE).The association of PAD-medical score with MACE was assessed using Cox proportional hazard analyses adjusting for age,sex and prior history of ischemic heart disease and stroke.RESULTS Between 2002 and 2020,a total of 424 participants with carotid artery disease(n=63),aortic or peripheral aneurysm(n=121)or lower limb ischemia(n=240)were prospectively recruited,and followed for a median duration(inter-quartile range)of 2.0(0.2–4.4)years.Only 33(7.8%)participants had the optimal PAD-medical score of five,with 318(75%)scoring at least three out of five.There were 89(21.0%)participants that had at least one MACE during the follow-up period.A one-unit higher PAD-medical score was associated with lower risk of MACE(HR=0.79,95%CI:0.63-0.98)after adjusting for other risk factors.CONCLUSION The PAD-medical score provides a simple way to assess the control of modifiable risk factors targeted by medical management aimed to reduce the incidence of MACE.

Relative methods, which are performed with the assistance of reference materials, are widely used in photon activation analysis(PAA). On the contrary, absolute methods, which are conducted without any reference material, are rarely applied due to the difficulty in obtaining photon flux. To realize absolute measurement in PAA,we retrieve photon flux in the sample via Monte Carlo simulation and raise a novel procedure—quasi-absolute method. With simulated photon flux and cross section data from existing databases, it is possible to calculate the concentration of target elements in the sample straightforwardly. A controlled experiment indicates that results from the quasi-absolute method for certain elements are nearly comparable to relative methods in practice.This technique of absolute measurement has room for improvement in the future and can serve as a validation technique for experimental data on cross sections as well.

Atypical Manifestations of Systemic Lupus Erythematosus (SLE) in Elderly Men

This case report highlights the rare occurrence of SLE in an elderly male patient. Despite the unusual presentation, the team was able to make a diagnosis of SLE using a combination of an atypical clinical picture, with initial signs and symptoms of weight loss, anemia, leucopenia and arthlagia, in combination with investigation results, including a positive test for anti-dsDNA antibodies. A more indolent disease course, combined with non-specific initial manifestations and low prevalence within the elderly population, often leads to delayed diagnosis. This case underscores the importance of considering SLE in the differential diagnosis of elderly patients, even in those who do not fit the typical epidemiological profile.

A family of tests for the presence of regression effect under proportional and non-proportional hazards models is described. The non-proportional hazards model, although not completely general, is very broad and includes a large number of possibilities. In the absence of restrictions, the regression coefficient, β(t), can be any real function of time. When β(t) = β, we recover the proportional hazards model which can then be taken as a special case of a non-proportional hazards model. We study tests of the null hypothesis;H0:β(t) = 0 for all t against alternatives such as;H1:∫β(t)dF(t) ≠ 0 or H1:β(t) ≠ 0 for some t. In contrast to now classical approaches based on partial likelihood and martingale theory, the development here is based on Brownian motion, Donsker’s theorem and theorems from O’quigley [1] and Xu and O’quigley [2]. The usual partial likelihood score test arises as a special case. Large sample theory follows without special arguments, such as the martingale central limit theorem, and is relatively straightforward.
