Acalculous Cholecystitis: An Unusual Manifestation of Cytomegalovirus Disease in Renal Transplant Recipient

Background: Common clinical manifestations of cytomegalovirus (CMV) infection include flu-like symptoms with fever, diarrhea, leukopenia, and elevated liver enzymes. Diagnosis is made by detection of the virus by buffy-coat blood culture or by polymerase chain reaction (PCR) analysis. Methods: Here we describe a women renal transplant recipient who presented with acalculous cholecystitis with CMV viremia, anemia and leucopenia tree months after she received a kidney from cadaveric donor. Results: Retrospective analysis of peripheral blood by PCR analysis was positive for CMV DNA. Treatement with Intravenous Ganciclovir was started after diagnosis. The role of cholecystectomy in patients diagnosed as having acalculous cholecystitis associated with systemic CMV disease remains unclear. Conclusions: Because CMV infection is common in transplant patients, the atypical manifestations of CMV should be considered in the differential diagnosis of posttransplant complications. Detection of CMV DNA in the peripheral blood by PCR analysis may help identify these patients.

The chemical composition of essential oils obtained from Artemisia herba-alba and Mentha pulegium were determined. The essential oils were analyzed by gas chromatography coupled with mass spectrometry （GC/MS）. Their antibacterial activity was studied in vitro against three standard strains： E. coli ATCC 25922, Staphylococcus aureus ATCC 29213, Pseudomonas aeruginosa ATCC 27853, and five clinical strains： Enterobacter cloacae, Staphylococcus aureus, Pseudomonas pyocyanique, Enterococcus faecium, and E. coli. Nineteen constituents were identified in A. herba-alba essential oil representing 99.57% of the total composition The major component was α-thujone （59.07%）. The bacterial strains were inhibited at concentrations ranging from 1.25 μL/mL to 5μL/mL and killed at concentrations ranging from 1.25 μL/mL to 10 μL/mL. M. pulegium resulted in the identification of eighteen constituents representing 99.48% of the total composition. The main component was pulegone （78.07%）. The minimal inhibitory （MIC） and bactericidal （MBC） concentrations were ranging from 1.25 μL/mL to 2.5 μL/mL.

The present study depicts the therapeutic effect of Zygophyllum cornutum methanolic extract （ZCME） on metabolic disturbances, oxidative stress in heart and myocardium histological changes in streptozotocin （STZ）-induced diabetic rats. Three days after diabetes induction, ZCME was administered orally for six weeks （700 mg/kg bw/day）. Serum glucose and lipid profile were evaluated. Reduced glutathione （GSH）, catalase （CAT） and thiobarbituric acid reactive substances （TBARS） were measured in heart tissue. The results showed increased levels of blood glucose, total cholesterol （TC）, LDL cholesterol （LDL-C） and triglyceride （TG） in the diabetic rats. On the other hand, the level of HDL cholesterol （HDL-C） decreased. Compared to the control normal rats, the level of TBARS in heart tissue was markedly increased while GSH and CAT were significantly modified in the diabetic rats. Oral administration of ZCME has normalized serum glucose and lipid profile. TBARS were significantly reduced in heart while CAT and GSH were markedly normalized. Myocardium sections showed the absence of histological changes observed in the diabetic rats. The study suggests that Zygophyllum cornutum may provide a useful therapeutic option in the reversal of metabolic disturbances and oxidative stress-induced cardiac dysfunction in diabetes mellitus.

Papillary Thyroid Carcinoma and Pregnancy: What Impact on Prognosis?

Introduction: The impact of pregnancy on the prognosis of papillary thyroid carcinoma (PTC) has been debated for several decades;however, no definitive conclusions have been reached. The main objective of this study is to demonstrate the short-term influence of pregnancy on the evolution and prognosis of PTC. Materials and Methods: A prospective descriptive and analytical study was conducted in the Endocrinology and Diabetology Department at the Hassan II University Hospital in Fez, including patients followed for papillary thyroid carcinoma presenting with a pregnancy during the year 2019 and 2020. The follow-up of these patients was continued until 1 year postpartum. We studied the clinical, paraclinical and therapeutic factors that could influence the prognosis of the disease. Results: We included 26 patients. Our study showed a significant correlation between recurrence and the presence of unfavorable histological signs (p = 0.02) as well as the initial Tg level (0.01). However, pregnancy was not an influencing factor (p = 0.41). Conclusion: Pregnancy does not seem to be a factor aggravating the prognosis of differentiated thyroid cancers.

Glucose Metabolism Disorders in Cirrhosis: Frequency and Risk Factors in Tunisian Population. Results of a Cross-Sectional Study

Background and aims: Alterations in carbohydrate metabolism are frequently observed in cirrhosis;to determine the frequency of diabetes mellitus and impaired glucose tolerance in Tunisian cirrhotic patients and identify risk factors. Patients and methods: Cross-sectional study;fasting plasma glucose levels were measured in consecutive patients with cirrhosis. Oral glucose tolerance test was performed if fasting plasma glucose level was normal. Glucose metabolism disorders were then classified as: impaired glucose tolerance and diabetes mellitus. Cirrhotics with glucose metabolism disorder were compared to those without. Results: Seventy-seven patients with cirrhosis were included: 68.8% were diagnosed as having glucose metabolism disorder;diabetes in 42.8% and impaired glucose tolerance in 26%. The tests were able to identify 60.4% of glucose metabolism disorders. Univariate analysis disclosed a higher proportion of female gender (p = 0.04) and more frequent familial history of diabetes mellitus (p = 0.005) in the group with glucose metabolism disorder. There were no statistically differences regarding age, etiology and severity of the cirrhosis, and dry body mass index. Multivariate analysis showed that familial history of diabetes was the only independent risk factor (OR = 5.1, p = 0.005). Conclusion: In our study, the frequency of glucose metabolism disorders was 68.8%. Oral glucose tolerance test allowed disclosing nearly half of them, pointing a high incidence of latent glucose metabolism disorders. In this way, it should be routinely evaluated in all patients with cirrhosis. Familial history of diabetes was the only independent risk factor, suggesting that other factors in addition to liver disease may play a role.

In this work, a waveguide structure consisting of a new artificial negative index material (NIM) surrounded by a nonlinear cover and a ferrite (YIG) substrate has been designed and investigated. We apply the boundary conditions and impose the condition of negative effective permeability of the ferrite slab to derive the dispersion relation related to the proposed structure. The NIM permittivity and permeability are not constant and depend on the operating frequency. The dispersion properties of the nonlinear electromagnetic surface waves (NEM) are analyzed and the associated propagation index is calculated. Results show that the dispersion could be tuned and controlled by selecting the NIM film thickness and the film-cover interface nonlinearity. The proposed structure is supporting unusual types of NEM surface waves having a non-reciprocal behavior widely used in designing optoelectronic devices.

Evaluation of 3D-CRT and VMAT Radiotherapy Plans for Left Breast Cancer with Regional Lymph Nodes Irradiation

Introduction: Radiation therapy after breast surgery is an integral part of the treatment of early breast cancer. The goal of radiation therapy is to achieve the best possible coverage of the planning target volume (PTV), while reducing the dose to organs at risk (OARs) which are normal tissues whose sensitivity to irradiation could cause damage that can lead to modification of the treatment plan. In the last decade, radiation oncologist started to use the Intensity Modulated Radiotherapy (IMRT) and Volumetric Modulated Arc Therapy (VMAT) for irradiating the breast, in order to achieve better dose distribution and target dose to the PTV and OAR. The aim of this study is to compare 2 external radiotherapy techniques (VMAT vs 3D) for patients with node-positive left breast cancer. Patients and Methods: We randomly selected 10 cases of postoperative radiotherapy for breast cancer in our hospital. The patients are all female, the average age was 45.4 years old, and the primary lesions are left breast. The ANOVA test was used to compare the mean difference between subgroups, and the p value Results: Dose volume histogram (DVH) was used to analyze each evaluation dose of clinical target volume (CTV) and organs at risk (OARs). Compared to 3DCRT plans, VMAT provided more uniform coverage to the breast and regional lymph nodes. The max point dose for tVMAT was lower on average (106.4% for VMAT versus 109% for 3DCRT). OAR sparing was improved with tVMAT, with a lower average V17Gy for the left lung (27.91% for VMAT versus 30.04% for 3DCRT, p and lower for V28Gy (13.75% for VMAT versus 22.34% for 3DCRT, p = 0.01). We also found a lower V35Gy for the heart on VMAT plan (p = 0.02). On the contrary, dose of contralateral breast was lower in 3DCRT than VMAT (0.59 Gy vs 3.65 Gy, p = 0.00). Conclusion: The both types of plans can meet the clinical dosimetry demands of postoperative radiotherapy for left breast cancer. The VMAT plan has a better conformity, but 3CDRT can provide a lo

Wegener’s Granulomatosis Presenting with Bilateral Excavated Nodules

Wegener’s granulomatosis (WG) is a systemic inflammatory disease whose histopathologic features often include necrosis, granuloma formation, and vasculitis of small-to-medium-sized vessels. We report a new case of Wegener’s granulomatosis occurring in a 41-year-old man who was admitted for respiratory symptoms, fever, asthenia, night sweats and weight loss since one month. Chest X-ray and computed tomography showed multiple bilateral pulmonary excavated lesions. A trransparietal lung biopsy was performed. Histological examination showed an area of necrosis, surrounded by a palisade of histiocytes. Some of the histiocytes were multinucleated and formed true granulomas within the wall of the adjacent pulmonary artery. Serology was strongly positive for ANCA. A diagnosis of Wegener granulomatosis was conducted and the patient was started on cyclophosphamide and methylprednisolone as an initial treatment, with a favorable evolution.

The increasingly strict regulation of the use of phytosanitary products and the will of the populations to move towards sustainable development allow bio-sourced products and more particularly for biocontrol in the field of pesticides to progress. The study carried out here concerns the evaluation of the potency of Bacillus amyloliquefaciens Bc2 and Trichoderma harzianum to fight against strawberry cultures infected with anthracnose (Fragaria × ananassa Duch.). The studies were carried out in the laboratory and in the field. The results indicate that B. amyloliquefaciens Bc2 and Trichoderma harzianum TR are effective for the biological control of anthracnose, gray mold and powdery mildew on strawberries grown in field conditions. The increase in plant size and the number of fruits produced with these control agents has also been observed.
