Comparison of chest CT scan findings between COVID-19 and pulmonary contusion in trauma patients based on RSNA criteria:Established novel criteria for trauma victims

Propose:In this study,we re-assessed the criteria defined by the radiological society of North America(RSNA)to determine novel radiological findings helping the physicians differentiating COVID-19 from pulmonary contusion.Methods:All trauma patients with blunt chest wall trauma and subsequent pulmonary contusion,COVID-19-related signs and symptoms before the trauma were enrolled in this retrospective study from February to May 2020.Included patients(Group P)were then classified into two groups based on polymerase chain reaction tests(Group Pa for positive patients and Pb for negative ones).Moreover,44 patients from the prepandemic period(Group PP)were enrolled.They were matched to Group P regarding age,sex,and trauma-related scores.Two radiologists blindly reviewed the CT images of all enrolled patients according to criteria defined by the RSNA criteria.The radiological findings were compared between Group P and Group PP;statistically significant ones were re-evaluated between Group Pa and Group Pb thereafter.Finally,the sensitivity and specificity of each significant findings were calculated.The Chi-square test was used to compare the radiologica丨findings between Group P and Group PP.Results:In the Group PP,73.7%of all ground-glass opacities(GGOs)and 80%of all multiple bilateral GGOs were detected(p<0.001 and p=0.25,respectively).Single bilateral GGOs were only seen among the Group PP.The Chi-square tests showed that the prevalence of diffused GGOs,multiple unilateral GGOs,multiple consolidations,and multiple bilateral consolidations were significantly higher in the Group P(p=0.001,0.01,0.003,and 0.003,respectively).However,GGOs with irregular borders and single consolidations were more significant among the Group PP(p=0.01 and 0.003,respectively).Of note,reticular distortions and subpleural spares were exclusively detected in the Group PP.Conclusion:We concluded that the criteria set by RSNA for the diagnosis of COVID-19 are not appropriate in trauma patients.The clinical signs and symptoms are not always usefu

Purpose Traumatic hemorrhagic shock is a life-threatening event worldwide.Severe brain trauma accompanying femoral fractures can trigger inflammatory responses in the body and increase pre-inflammatory cytokines such as TNF-α,IL-1.The primary treatment in these cases is hydration with crystalloids,which has both benefits and complications.The purpose of this study was to investigate the effects of fluid therapy on the hemodynamics,coagulation profiles,and blood gases in such patients.Methods In this cross-sectional study,patients were divided into two groups:femoral fracture group and non-femoral group.The hemodynamic status,coagulation profile,and blood gases of patients in both groups were evaluated upon arrival at the hospital and again 2 h later.Data were analyzed by t-test and ANOVA with repeated data and paired samples t-test.Results A total of 681 trauma patients(605 men and 76 women)participated in this study,including 69(86.3%)men and 11(13.8%)women in femoral fracture group and 536 men(89.2%)and 65 women(10.8%)in non-femoral group.The laboratory parameters were evaluated in response to the equal amount of crystalloid fluid given upon arrival and 2 h later.Blood gases decreased in the fracture group despite fluid therapy(p<0.003),and the coagulation profile worsened although the change was not statistically significant.Conclusion The treatment of multiple-trauma patients with femoral bone fractures should be more concerned with the need for the infusion of vasopressors such as norepinephrine.If there is evidence of clinical shock,excessive crystalloid infusion(limited to 1 L)should be avoided,and blood and blood products should be started as soon as possible.

Ergonomic assessment of drivers in MF285 and MF399 tractors during clutching using algometer

Tractor as one of the most widely used agricultural machinery should be considered from different aspects.The frequent application of clutch and brake pedals and also the steering wheel within farms,along with the unfavorable working conditions,cause negative effects on the occupational health of the tractor drivers.Thus,in this research,the imposed forces on three engaged muscles including:Gastrocnemius,Trapezius and Quadrate’s lumborum of the tractor drivers during clutching have been studied.In this regard,algometer device was used to determine the applied forces on selective muscles of drivers during clutching of MF285 and MF399 tractors.The experiments were performed employing sample of 30 drivers and were conducted on two Iranian frequently used tractors including:MF285 and MF399 models.The results showed that the clutching forces for MF285 and MF399 tractors were 340 N and 290 N,respectively.The knee angle of the drivers of the two tractors was statistically different at the one percent level of significance.The reduction of pain threshold after 30 and 60 s clutching and also 60 s rest after clutching in MF285 tractor,for all three muscles,were more than those of MF399 tractor.The impact of clutching on the average decreases of pain threshold,for all drivers and all clutching periods,during and after clutching,in the Quadratus lumborum muscle was more than the other two muscles,in both tractors.In order to reduce the clutching force for MF285 tractor some modifications is suggested.In this regard the force transfer joint between the pedal and the clutch release linkage may be replaced with one made of cast iron.

Assessment of land suitability and agricultural production sustainability using a combined approach (Fuzzy-AHP-GIS):A case study of Mazandaran province,Iran

This research was conducted to evaluate the sustainability of rice production using fuzzy logic in Mazandaran province,Iran.The untapped use of modern agricultural technology,such as agricultural machinery,has become a global dilemma,which has forced governments and environmentalists to take a serious stand against the dangers of massive non-standard agricultural practices in the environment.Three types of mechanization,socioeconomic,and economic modeling were used to determine the sustainability and their values were collected through interview method using a 320-item questionnaire from 2016 to 2018.Land evaluation was carried out using soil and climate characteristics,so that about 27.4%of the area was in the perfectly suitable range(S1),35.2%in the appropriate range(S2),19.5%in the critical range(S3),and 17.9%in the inappropriate range(N)for rice cultivation.Farmers were selected from two regions(R1 and R2)by the GIS on two lower levels and more than one hectare(A1,A2)according to agricultural methods(mechanical and traditional).The results showed that the highest and lowest means of the product were 4314.44 and 3501.95 kg ha1,respectively.The sustainability of production was within the range of“sustainability”and“almost sustainable”in terms of mechanistic and economic factors in the fuzzy range of“moderate sustainability”and also with respect to social factors that are in a better position than mechanistic and economic indicators.Results of final sustainability model showed that the average of the lasting sustainability index was in the range of“almost unsustainable”and“moderate sustainability”.

Influence of Moisture Content,Variety and Parboiling on Milling Quality of Rice Grains

The effects of moisture content （8%, 10%and 12%）, variety （Tarom and Fajr） and parboiling on milling quality of rice as a function of milling recovery （MR）, head rice yield （HRY）, degree of milling （DOM） and whiteness were investigated. The parboiled grains was prepared with three soaking temperatures of 25 oC, 50 oC and 75 oC and three steaming times of 10, 15 and 20 min. As a result of parboiling, the increasing rates of MR and HRY values were 7.8%and 14.3%for Tarom and 9.8%and 10.0%for Fajr, respectively, and the decreasing rates for DOM and whiteness were 6.6%and 10.8%for Tarom and 6.8% and 10.5% for Fajr, respectively. Moreover, decreasing moisture content to 8%maximized MR （75.8% for Tarom and 74.3% for Fajr） and HRY （65.8% for Tarom and 57.0% for Fajr） while increasing that to 12%revealed maximum values of DOM （6.1%for Tarom and 6.2%for Fajr） and whiteness （24.8%for Tarom and 28.2%for Fajr）.

Analgesic effects and hemodynamic mechanisms of perpendicular and transverse needling at Sanyinjiao(SP 6) in patients with primary dysmenorrhea: A randomized controlled trial

Objective: To explore the analgesic effects and uterine hemodynamics of perpendicular needling(PN)and transverse needling(TN) at SP 6 in patients with primary dysmenorrhea(PD).Methods: In this randomized controlled trial, patients with PD diagnosed with cold-dampness congealing pattern were randomly assigned in a ratio of 1:1 to receive PN or TN at bilateral SP 6 for 10 min.Acupuncture was performed when the menstrual pain score was over 40 mm on the first day of menstruation, as measured using the visual analog scale for pain(VAS-P). The primary outcome was average menstrual pain(VAS-P). Secondary outcomes included the pulsatility index(PI), resistance index(RI), and systolic-diastolic peaks ratio(S/D) in uterine arteries as measured using color Doppler ultrasonography;anxiety as assessed using the Hamilton Anxiety Rating Scale(HAMA), blood pressure(BP),and heart rate(HR).Results: Forty-eight patients completed the study. The TN group exhibited a significant reduction in VAS-P scores(-5.71 mm, 95% confidence interval(CI): -8.78, -2.63, P =.001), RI values(-0.05, 95% CI:-0.09, -0.01, P=.015), and HAMA values(-2.50, 95% CI: -4.78, -0.22, P=.032) when compared with the PN group. No significant differences in PI, S/D, BP, or HR values were observed between the two groups(P >.05).Conclusion: TN at SP 6 was superior to PN in alleviating menstrual pain and anxiety in patients with PD.This analgesic effect of TN may be due to its better ability to improve uterine arterial blood flow via decreases in RI values.

Background:Preoperative patient selection in Associating Liver Partition and Portal vein ligation for Staged hepatectomy(ALPPS)is not always reliable with currently available scores,particularly in patients with primary liver tumor.This study aims to(I)to determine whether comorbidities and patients characteristics are a risk factor in ALPPS and(II)to create a score predicting 90-day mortality preoperatively.Methods:Thirteen high-volume centers participated in this retrospective multicentric study.A risk analysis based on patient characteristics,underlying disease and procedure type was performed to identify risk factors and model the Comprehensive ALPPS Preoperative Risk Assessment(CAPRA)score.A nonparametric receiver operating characteristic analysis was performed to estimate the predictive ability of our score against the Charlson Comorbidity Index(CCI),the age-adjusted CCI(aCCI),the ALPPS risk score before Stage 1(ALPPS-RS1)and Stage 2(ALPPS-RS2).The model was internally validated applying bootstrapping.Results:A total of 451 patients were included.Mortality was 14.4%.The CAPRA score is calculated based on the following formula:(0.1×age)−(2×BSA)+1(in the presence of primary liver tumor)+1(in the presence of severe cardiovascular disease)+2(in the presence of moderate or severe diabetes)+2(in the presence of renal disease)+2(if classic ALPPS is planned).The predictive ability was 0.837 for the CAPRA score,0.443 for CCI,0.519 for aCCI,0.693 for ALPPS-RS1 and 0.807 for ALPPS-RS2.After 1,000 cycles of bootstrapping the C statistic was 0.793.The accuracy plot revealed a cut-off for optimal prediction of postoperative mortality of 4.70.Conclusions:Comorbidities play an important role in ALPPS and should be carefully considered when planning the procedure.By assessing the patient’s preoperative condition in relation to ALPPS,the CAPRA score has a very good ability to predict postoperative mortality.

Optical coherence tomography in ischemic optic neuropathy

Ischemic optic neuropathies are among the most common causes of sudden vision loss,especially in patients over age 50.The cause and prognosis of these disorders,and in particular non-arteritic anterior ischemic optic neuropathy,is poorly understood,and treatments remain poor in terms of restoring or preserving vision.Optical coherence tomography(OCT)and OCT angiography have allowed us to identify early and late structural changes in the optic nerve head and retina that may assist in predicting visual outcomes and may lead to greater understanding of pathogenesis and thus the development of effective medical interventions.
