Combined peripheral natural killer cell and circulating tumor cell enumeration enhance prognostic efficiency in patients with metastatic triple-negative breast cancer

Objective：Triple-negative breast cancer（TNBC）is a heterogeneous disease with poor *** tumor cells（CTCs）are a promising predictor for breast cancer prognoses but their reliability regarding progression-free survival（PFs）is *** aim to verify their predictive value in ***：In present prospective cohort study,we used the Pep@MNPs method to enumerate CTCs in baseline blood samples from 75 patients with TNBC（taken at inclusion in this study）and analyzed correlations between CTC numbers and outcomes and other clinical ***：Median PFs was 6.0（range：1.0–25.0）months for the entire cohort,in whom we found no correlations between baseline CTC status and initial tumor stage（P=0.167）,tumor grade（P=0.783）or histological type（P=0.084）.However,among those getting first-line treatment,baseline CTC status was positively correlated with ratio of peripheral natural killer（NK）cells（P=0.032）,presence of lung metastasis（P=0.034）and number of visceral metastatic site（P=0.037）.Baseline CTC status was predictive for PFs in first-line TNBC（P=0.033）,but not for the cohort as a whole（P=0.118）.This prognostic limitation of CTC could be ameliorated by combining CTC and NK cell enumeration（P=0.049）.Conclusions：Baseline CTC status was predictive of lung metastasis,peripheral NK cell ratio and PFs in TNBC patients undergoing first-line *** have developed a combined CTC-NK enumeration strategy that allows us to predict PFs in TNBC without any preconditions.

Objective:Fluzoparib(sHR3162)is a novel,potent poly(ADP-ribose)polymerases(PARP)1,2 inhibitor that showed anti-tumor activity in xenograft *** conducted a phaseⅠ,first-in-human,dose-escalation and expansion(D-Esc and D-Ex)trial in patients with advanced solid ***:This was a 3+3 phaseⅠD-Esc trial with a 3-level D-Ex at 5 hospitals in *** patients for DEsc had advanced solid tumors refractory to standard therapies,and D-Ex enrolled patients with ovarian cancer(OC).Fluzoparib was administered orally once or twice daily(bid)at 11 dose levels from 10 to 400 mg/*** included dose-finding,safety,pharmacokinetics,and antitumor ***:seventy-nine patients were enrolled from March,2015 to January,2018[OC(47,59.5%);breast cancer(BC)(16,20.3%);colorectal cancer(8,10.1%),other tumors(8,10.1%)];48 patients were treated in the D-Esc arm and 31 in the D-Ex *** maximum tolerated dose(MTD)was 150 mg bid,with a half-life of 9.14 *** 3/4 adverse events included anemia(7.6%)and neutropenia(5.1%).The objective response rate(ORR)was 30%(3/10)in patients with platinum-sensitive OC and 7.7%(1/13)in patients with *** patients treated with fluzoparib≥120 mg/d,median progression-free survival(m PFs)was 7.2[95%confidence interval(95%CI),1.8-9.3]months in OC,9.3(95%CI,7.2-9.3)months in platinum-sensitive OC,and 3.5(range,2.0-28.0)months in *** patients with germline BC susceptibility gene mutation(g BRCAMut)(11/43 OC;2/16 BC),m PFs was 8.9 months for OC(range,1.0-23.2;95%CI,1.0-16.8)and 14 and 28 months for BC(those two patients both also had somatic BRCAMut).Conclusions:The MTD of fluzoparib was 150 mg bid in advanced solid *** demonstrated single-agent antitumor activity in BC and OC,particularly in BRCAMut and platinum-sensitive OC.

Offshore system safety and operational challenges in harsh Arctic operations

Offshore oil and gas drilling operations are going to remote and harsh arctic environments with demands forheightened safety and resilience of operational facilities. The remote and harsh environment is characterized byextreme waves, wind, storms, currents, ice, and fog that hinder drilling operations and cause structural failuresof critical offshore infrastructures. The risk, safety, reliability, and integrity challenges in harsh environment operations are critically high, and a comprehensive understanding of these factors will aid operations and protectthe investment. The dynamics, environmental constraints, and the associated risk of the critical offshore infrastructures for safe design, installation, and operations are reviewed to identify the current state of *** paper introduces a systematic review of harsh environment characterization by exploring the metoceanphenomena prevalent in harsh environments and their effects on the floating offshore structures performanceand supporting systems. The dynamics of the floating systems are described by their six degrees of freedom andtheir associated risk scenarios. The systematic methodology further explores the qualitative, quantitative, andconsequences modeling techniques for risk analysis of floating offshore systems in a harsh environment. Whilepresenting the current state of knowledge, the study also emphasizes a way forward for sustainable offshore operations. The study shows that the current state of knowledge is inexhaustive and will require further researchto develop a design that minimizes interruption during remote harsh offshore operations. Resilient innovation,IoT and digitalization provide opportunities to fill some of the challenges of remote Arctic offshore operations.

seroprevalence of Helicobacter pylori infection among students of a Nigerian University

Objective:To investigate the seroprevalence of Helicobacter pylori(***) infection among the undergraduate students of Nasarawa state University,Keffi,***:A total of 200 serum samples were collected from undergraduate students of Nasarawa state University, Keffi,and 100μL of each serum was aseptically transferred to the specimen well of an *** antigen kit(Clinotech UsA).The 2 distinctive red lines apearing in the control and test regions of the kit after 10 minufes indicated positive ***:Of the 200 students sampled, 108(54%) were *** of seroprevalence of *** revealed the correlation between infection prevalence and *** infection prevalence was 45.5%among students aged 18-20,rose to the peak of 85.7%adults aged 31-40,dropped to 66.7%among those 41-50 years old,and continuously went down to 28.6%in the 51-year-old and above *** was a statistically significant difference(using Chi-square) with respects to gender,age and type of infection(symptomatic or asymptomatic seropositive infection)(Ps).Conclucions: Community Health Personnel should be aware of this microorganism as a potential cause of illness in ***,the mode of transmission and possible means of controlling the bacterial infection among students or a community is of public health concern and requires further study.

In this editorial,we comment on the article by Zhang et al entitled Development of a machine learning-based model for predicting the risk of early postoperative recurrence of hepatocellular *** carcinoma(HCC),which is characterized by high incidence and mortality rates,remains a major global health challenge primarily due to the critical issue of postoperative *** recurrence,defined as recurrence that occurs within 2 years posttreatment,is linked to the hidden spread of the primary tumor and significantly impacts patient *** predictive factors,including both patient-and treatment-related factors,have limited predictive ability with respect to HCC *** integration of machine learning algorithms is fueled by the exponential growth of computational power and has revolutionized HCC *** study by Zhang et al demonstrated the use of a groundbreaking preoperative prediction model for early postoperative HCC *** persist,including sample size constraints,issues with handling data,and the need for further validation and *** study emphasizes the need for collaborative efforts,multicenter studies and comparative analyses to validate and refine the *** these challenges and exploring innovative approaches,such as multi-omics integration,will enhance personalized oncology *** study marks a significant stride toward precise,efficient,and personalized oncology practices,thus offering hope for improved patient outcomes in the field of HCC treatment.

本文介绍1例激素受体阳性、人表皮生长因子受体-2(human epidermal growth factor receptor-2,HER2)阴性晚期乳腺癌多线内分泌治疗的多学科诊疗过程。该病例有较长的无瘤生存期(10年),初始复发转移肿瘤分布范围广,涉及两肺多发、多处骨骼及颈部锁骨上等区域多发淋巴结转移,经锁骨上转移淋巴结穿刺病理证实,免疫组织化学检测仍为雌激素受体(estrogen receptor,ER)强阳性/HER2阴性,但是孕激素受体(progesterone receptor,PR)仅小区阳性。经过多学科讨论后患者依次一线接受高剂量氟维司群(500 mg,肌注,28 d/次,首月第14天负荷剂量)内分泌治疗,二线选择哌柏西利(125 mg/d,服21 d,休7 d)+来曲唑(2. 5 mg/d),三线接受哌柏西利+氟维司群(同前),其2次进展仅表现为新发淋巴结转移或胸腔积液,并未发现内脏新发病灶或者病灶增大。其一、二线内分泌治疗的无进展生存期(progression free survival,PFs)均为12个月,三线内分泌治疗保留哌柏西利,换用一线的内分泌治疗药物(氟维司群)。临床上,还有全身化疗和局部放疗等选择。由于患者自发病以来一直拒绝接受化疗,术后辅助仅选择阿那曲唑(1 mg/d)辅助内分泌治疗(5年),解救治疗期间同样要求仅选择内分泌治疗。鉴于患者的意愿和肿瘤特征与演变、分子检测与监测,结合MDT专家意见,连续选择内分泌治疗作为解救治疗,取得较长的疾病控制实践,患者维持较好的生活质量。

With the mandate of worldwide carbon neutralization,pursuing comfortable living environment while consuming less energy is an enticing and unavoidable *** composite aerogels with super thermal insulation and high sunlight reflection are developed for energy-efficient buildings.A solvent-assisted freeze-casting strategy is used to produce boron nitride nanosheet/polyvinyl alcohol(BNNs/PVA)composite aerogels with a tailored alignment channel *** effects of acetone and BNNs fillers on microstructures and multifunctional properties of aerogels are *** acetone in the PVA suspension enlarges the cell walls to suppress the shrinkage,giving rise to a lower density and a higher porosity,accompanied with much diminished heat conduction throughout the whole *** addition of BNNs fillers creates whiskers in place of disconnected transverse ligaments between adjacent cell walls,further ameliorating the thermal insulation transverse to the cell wall *** resultant BNNs/PVA aerogel delivers an ultralow thermal conductivity of 23.5 mW m^(−1) K^(−1) in the transverse *** superinsulating aerogel presents both an infrared stealthy capability and a high solar reflectance of 93.8%over the whole sunlight wave-length,far outperforming commercial expanded polystyrene foams with reflective *** anisotropic BNNs/PVA composite aerogel presents great potential for application in energy-saving buildings.
