目的分析對於I型呼吸衰竭患者採用經鼻高流量氧療(HFNC)氧療對比無創通氣(NIV)治療的應用價值。方法選取2022年5月~2023年4月在鏡湖醫院急症室使用HFNC及NIV治療的I型呼吸衰竭患者,分析基礎資料、治療結局、呼吸困難評分、舒適度評分。結果HFNC組41例、NIV組26例。治療後的呼吸困難評分(VAS)、呼吸頻率的差異無統計學意義;治療後的氣合指數(PaO_(2)/FiO_(2))在NIV組改善更多,差異有統計學意義。在急症室通過HFNC或NIV治療2小時内的失敗率、入院後續用HFNC或NIV治療6小時内插管率、28天病死率,差異均無統計學意義。HFNC組舒適度評分更高,差異有統計學意義。結論對於I型呼吸衰竭患者,HFNC與NIV均可改善Ⅰ型呼吸衰竭患者生理指標及血氣分析指標,但與NIV比較,患者對HFNC的耐受性更好,更利於患者臨床治療。

Reactive oxygen species(ROS)-associated oxidative stress,inflammation storm,and massive hepatocyte necrosis are the typical manifestations of acute liver failure(ALF),therefore specific therapeutic interventions are essential for the devastating ***,we developed a platform consisting of versatile biomimetic copper oxide nanozymes(Cu NZs)-loaded PLGA nanofibers(Cu NZs@PLGA nanofibers)and decellularized extracellular matrix(dECM)hydrogels for delivery of human adipose-derived mesenchymal stem/stromal cells-derived hepatocyte-like cells(hADMSCs-derived HLCs)(HLCs/Cu NZs@fiber/dECM).Cu NZs@PLGA nanofibers could conspicuously scavenge excessive ROS at the early stage of ALF,and reduce the massive accumulation of pro-inflammatory cytokines,herein efficiently preventing the deterioration of hepatocytes ***,Cu NZs@PLGA nanofibers also exhibited a cytoprotection effect on the transplanted ***,HLCs with hepatic-specific biofunctions and anti-inflammatory activity acted as a promising alternative cell source for ALF *** dECM hydrogels further provided the desirable 3D environment and favorably improved the hepatic functions of *** addition,the pro-angiogenesis activity of Cu NZs@PLGA nanofibers also facilitated the integration of the whole implant with the host ***,HLCs/Cu NZs@fiber/dECM performed excellent synergistic therapeutic efficacy on ALF *** strategy using Cu NZs@PLGA nanofiber-reinforced dECM hydrogels for HLCs in situ delivery is a promising approach for ALF therapy and shows great potential for clinical translation.

Feasibility of gallbladder preservation during robotic left hepatectomy:A retrospective comparative study

Objective:Traditionally,gallbladder is routinely removed during left hepatectomy even if there is no gallbladder ***,adverse consequence after cholecystectomy,though rare,still occa-sionally *** study aims to evaluate the feasibility of gallbladder preservation during robotic left ***:All consecutive robotic left hepatectomy cases between December 2010 and January 2022 in Prince of Wales Hospital,the Chinese University of hong Kong were retrieved from a prospectively collected *** gallbladder was preserved by moving the liver transection line just away from the gallbladder *** were divided into two groups:gallbladder preservation(GBP)and non-gallbladder preservation(NGBP).Operative results and long-term outcomes were compared between these two ***:There were 11 cases in the GBP group and 25 cases in the NGBP *** two groups were comparable in terms of the patient demographics and disease *** was no operative *** was no difference between the two groups in operative time(GBP 270 min *** 332 min,p=0.132),blood loss(GBP 50 mL *** 150 mL,p=0.115)or complication rate(GBP 27.3%*** 24.0%,p>0.999).There was also no difference in 5-year overall *** the GBP group,no patient developed specific symptoms or complications related to the preserved ***-up ultrasound or computed tomography revealed a normal appearance of the preserved gallbladders except in one patient who developed a 3-mm gallbladder *** the other hand,one(4%)patient in the NGBP group developed troublesome diarrhoea after ***:Gallbladder preservation is safe and feasible during robotic left *** preserved gallbladder does not lead to any symptoms,while postcholecystectomy diarrhoea can be avoided.

It is largely unknown whether lower urinary tract symptoms(LUTS)or acute retention of urine(AROU)is linked to shorter life expectancy in *** conducted a multicenter,retrospective database analysis of patients undergoing transurethral resection of prostate(TURP)to study their *** Cox regression analysis and Kaplan-Meier analysis with stratification to age and indication of TURP were *** further performed an age-and sex-matched survival analysis with the general population using data from the Census and Statistics Department of the hong Kong Special Administrative Region(hong Kong,China).From January 2002 to December 2012,3496 patients undergoing TURP were included in our study,with 1764 patients in the LUTS group and 1732 patients in the AROU *** age,ischemic heart disease,cerebrovascular accident,and AROU were risk factors of *** aged80 *** to the general population,younger patients in the LUTS group appeared to have better survival(80 years,survival was similar with the general population in both the LUTS and AROU *** conclusion,AROU at young age was associated with mortality,while early detection and management of LUTS may improve survival.

目的通過對《鏡湖醫學》復刊30年的醫學文獻信息及發文作者信息進行統計與分析,探討《鏡湖醫學》期刊的學術特色和發展趨勢,以及鏡湖醫院各學科在不同時期的發展水平。方法檢索《鏡湖醫學》復刊30年間出版的全部期刊,從每期的題錄中篩選出醫學文獻,採集文獻數量及分類、作者及作者單位等基本信息;採集第一作者的背景數據,包括學歷、畢業年限、工作經歷及專業技術職稱等;運用描述性統計和卡方檢驗等方法,統計和分析年發文量及累積發文量、文獻種類的分布和變遷、作者及發文單位的構成、第一作者不同背景與不同文獻種類發文量的關係,以及核心作者的特徵。結果在1993年~2022年這30年間,《鏡湖醫學》共出版了40期,總計刊登了1516篇各類文章,其中1248篇醫學文獻(中文1216篇,英文32篇)。年發文量從最初的25篇增長到最近的72篇,出版週期由年刊改為半年刊。各類文獻佔比:臨床研究23%,經驗交流34%,病例報告23%,醫學人文6%,綜述11%,技術介紹3%。病例報告類及醫學人文類發文比例呈上升趨勢,綜述類及技術介紹類文章的比例呈下降趨勢。發文量佔總文獻量5%以上的專業科室/部門依次為影像科、兒科、心内科、病理科、骨科、急診及重症醫學科和護理部。署名作者有743人,第一作者476人(院外作者12人)。發文量1篇的作者佔46.1%,發文量2篇的作者佔20.5%;5篇及以上發文量的作者定義為核心作者,共69人,佔14.5%,發文佔全部文獻的42.1%,作者最高發文量為36篇。作者獨立署名251篇(20.1%),合作署名997篇(79.9%);1144篇(91.7%)由單一部門/科室署名,而104篇(8.3%)由多部門/科室合作署名,其中院内部門合作的有64篇,院内外機構合作的有40篇;署名作者6人及以上的有118篇(9.5%)。第一作者的學歷在本科及以上,其發文量明顯高於中專和大專學歷的作者(P=0.000);本科學歷的作者傾向發表臨床研究類型的文章,碩士學歷的作者傾向發表病例報告類型的文章而不傾向發表醫學人文類型的文章,中專及大專學歷的作者傾向發表經驗交流、醫學人文類型的文章,不傾向發臨床研究、病例報告類型的文章。作者的發文年限與發文量呈正相關,發文年限≦5年組與16年~20年組間和≦5年組與≧26年組間比較有明顯差異(P=0.04和P=0.000);發文年限為≦5年的作者傾向發表綜述類型的文章,不傾向發表技術介紹類型的文章;發文年限為6~10年的作者傾向發綜述類型的文章,不傾向發表經驗交流、醫學人文類型的文章;發文年限為11~15年的作者傾向發表病例報告類型的文章,不傾向發表臨床研究、醫學人文、綜述類型的文章;發文年限為16~20年的作者傾向發表經驗交流類型的文章,不傾向發表醫學人文、綜述、技術介紹類型的文章;發文年限為21~25年的作者傾向發表經驗交流類型的文章,不傾向發病例報告類型的文章;發文年限為>26年的作者傾向發表醫學人文類型的文章,不傾向發經驗交流、病例報告類型的文章。有外院工作經歷的作者的發文量明顯高於只在一所醫院工作的作者(P=0.003)。對目前在冊的第一作者的專業技術職稱與發文量及種類分析:技術職稱與發文量呈正相關(P<0.001),正高級職稱人均發文5.5篇,副高級職稱人均發文4.8篇,中級職稱人均發文2.3篇,初級職稱人均發文1.5篇。護理專業發文只佔全部文獻的4.9%。結論《鏡湖醫學》復刊30年來,在一代人的辛勤努力下,編輯出版及發行工作越來越規範,刊載的信息量越來越多,設置的欄目和内容日趨成熟,也體現了鏡湖醫院各學科的發展水平。核心作者群是《鏡湖醫學》復刊30年來持續發展的中堅力量,該期刊的發展前景值得期待。

Pattern of disease recurrence and its implications for postoperative surveillance after curative hepatectomy for hepatocellular carcinoma: experience from a single center

Background: Hepatectomy is a widely accepted curative treatment for hepatocellular carcinoma (HCC). However, the disease frequently recurs after a curative hepatectomy. The objective of this study is to provide a better understanding of the pattern of disease recurrence and the risk factors involved so as to improve the postoperative surveillance. Methods: A retrospective study for all patients receiving hepatectomy for HCC between 2003 and 2014 was performed. Emphasis was made on the timing and pattern of recurrent disease, and type of treatment given. Results: There were 506 patients in the study. Median follow-up was 43.7 months. The 1-, 3-, 5-, 10-year overall and disease free survival were 89.5%, 74.1%, 63.9%, 49.0% and 69.5%, 54.3%, 43.4%, 30.9%respectively. Recurrent disease occurred in 267 patients, 47.2% occurred within 9 months of hepatectomy and 80.1% recurred only in liver. Median survival was shorter for recurrence occurring within 9 months compared with those occurring between 10 months and 2 years postoperatively (36.2 vs. 65.7 months, P3.5 cm and history of rupture were risk factors for recurrence within 9 months. Conclusions: These findings suggest that recurrent diseases are common after curative hepatectomy for HCC and most recurrences occur in the remnant liver. Since almost half of recurrences occurred within first 9 months after hepatectomy, a more stringent postoperative surveillance with target imaging of liver in this period is needed. Early diagnosis of recurrent disease and curative retreatment hopefully can bring about a longer survival.

An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in hong Kong

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted.
