Intrahepatic cholangiocarcinoma: Introducing the preoperative prediction score based on preoperative imaging

Background: Intrahepatic cholangiocarcinoma(ICC) still has a poor long-term outcome, even after complete resection. We investigated different parameters gathered in preoperative imaging and analyzed their influence on resectability, recurrence, and survival. Methods: All patients who underwent exploration due to ICC between January 2008 and June 2018 were analyzed retrospectively. Kaplan-Meier model, log-rank test and Cox regression were used. Results: Out of 184 patients, 135(73.4%) underwent curative intended resection. Median overall survival(OS) was 22.2 months with a consecutive 1-, 3-and 5-year OS of 73%, 29%, and 17%. Median recurrencefree survival(RFS) was 9.3 months with a consecutive 1-, 3-and 5-year RFS of 36%, 15%, and 11%. Site of tumor, parenchymal localization, tumor configuration/dissemination, and estimated tumor volume had significant influence on resectability. Univariate analyses showed that site of tumor, tumor configuration/dissemination, number of nodules, and estimated tumor volume had predictive values for OS and RFS. Together with tumor size the preoperative prediction(POP) score was created showing significance for OS and RFS(all P<0.001). In multivariate analysis, POP score(HR=1.779;95% CI: 1.26 8-2.4 95;P=0.001), T stage(HR=1.255;95% CI: 1.040-1.514;P=0.018) and N stage(HR=1.334;95% CI: 1.081-1.645;P=0.007) were the independent predictors for OS. For RFS, POP score(HR=1.733;95% CI: 1.30 0-2.311;P<0.0 01) and M stage(HR=3.036;95% CI: 1.376-6.697;P=0.006) were the independent predictors. Conclusions: The POP score showed to have a highly significant influence on OS and RFS. The score is easy to assess through preoperative imaging. For patients in the high risk group at least staging laparoscopy or preoperative chemotherapy should be evaluated, because they showed equal outcome compared to the irresectable group.

Space decomposition based parallelization solutions for the combined finiteediscrete element method in 2D

The combined finiteediscrete element method （FDEM） belongs to a family of methods of computationalmechanics of discontinua. The method is suitable for problems of discontinua, where particles aredeformable and can fracture or fragment. The applications of FDEM have spread over a number of disciplinesincluding rock mechanics, where problems like mining, mineral processing or rock blasting canbe solved by employing FDEM. In this work, a novel approach for the parallelization of two-dimensional（2D） FDEM aiming at clusters and desktop computers is developed. Dynamic domain decompositionbased parallelization solvers covering all aspects of FDEM have been developed. These have beenimplemented into the open source Y2D software package and have been tested on a PC cluster. Theoverall performance and scalability of the parallel code have been studied using numerical examples. Influence of structural depth of laser-patterned steel surfaces on the solid lubricity of carbon nanoparticle coatings

Carbon nanoparticle coatings on laser-patterned stainless-steel surfaces present a solid lubrication system where the pattern's recessions act as lubricant-retaining *** study investigates the influence of the structural depth of line patterns coated with multi-walled carbon nanotubes(CNTs)and carbon onions(COs)on their respective potential to reduce friction and *** laser interference patterning(DLIP)with a pulse duration of 12 ps is used to create line patterns with three different structural depths at a periodicity of 3.5μm on AISI 304 steel ***,electrophoretic deposition(EPD)is applied to form homogeneous carbon nanoparticle coatings on the patterned *** ball-on-disc experiments are conducted on the as-described surfaces with an alumina counter body at a load of 100 *** results show that the shallower the coated structure,the lower its coefficient of friction(COF),regardless of the particle ***,with a minimum of just below 0.20,CNTs reach lower COF values than COs over most of the testing *** resulting wear tracks are characterized by scanning electron microscopy,transmission electron microscopy,and energy-dispersive X-ray *** friction testing,the CNTs remain in contact,and the immediate proximity,whereas the CO coating is largely *** of structural depth,no oxidation occurs on CNT-coated surfaces,whereas minor oxidation is detected on CO-coated wear tracks.

Comparison of short- with long-term regeneration results after digital nerve reconstruction with musclein-vein conduits

Muscle-in-vein conduits are used alternatively to nerve grafts for bridging nerve defects. The purpose of this study was to examine short- and long-term regeneration results after digital nerve reconstruction with muscle-in-vein conduits. Static and moving two-point discriminations and Semmes-Weinstein Monofilaments were used to evaluate sensory recovery 6–12 months and 14–35 months after repair of digital nerves with muscle-in-vein in 7 cases. Both follow-ups were performed after clinical signs of progressing regeneration disappeared. In 4 of 7 cases, a further recovery of both two-point discriminations and in another case of only the static two-point discrimination of 1–3 mm could be found between the short-term and long-term follow-up examination. Moreover, a late recovery of both two-point discriminations was demonstrated in another case. Four of 7 cases showed a sensory improvement by one Semmes-Weinstein Monofilaments. This pilot study suggests that sensory recovery still takes place even when clinical signs of progressing regeneration disappear.

Correlation of leaking microaneurysms with retinal thickening in diabetic retinopathy

Introduction:Bats are threatened by agricultural intensification,and although bat ecology in agricultural landscapes is in the focus of current research,the effects of interacting spatiotemporal factors on species-specific bat activity above farmland remain *** aim was to identify spatiotemporal factors and their interactions relevant for the activity of bat species above conventionally managed arable ***:We repeatedly monitored relative bat activity above open arable fields in Germany using acoustic *** used site-related biotic and abiotic factors and landscape characteristics across five spatial scales,their combinations,and interactions to identify those factors which best explain variation in bat ***:Numerous interactions between landscape characteristics and the insect abundance affected bat activity above *** instance,Pipistrellus pipistrellus became more active with increasing insect abundance,but only above fields with a low proportion of woody vegetation cover in the ***,the level of bat activity in summer depended on landscape *** example,the activity of Pipistrellus nathusii was relatively low in summer above fields that were surrounded by vegetation patches with a high degree of edge complexity(e.g.,hedgerow).However,the activity remained at a relatively high level and did not differ between seasons above fields that were surrounded by vegetation patches with a low degree of edge complexity(e.g.,roundly shaped forest patch).Conclusions:Our results revealed that landscape characteristics and their interactions with insect abundance affected bat activity above conventionally managed fields and highlighted the opportunistic foraging behavior of *** improve the conditions for bats in agricultural landscapes,we recommend re-establishing landscape heterogeneity to protect aquatic habitats and to increase arthropod availability.

The main application of hydrocyclones in most industry sectors is the separation of particles from *** thickening,however,the separation properties of the cyclone should be adjustable,which requires a different operating ***,we investigated an add-on thickening module,consisting of a conical plug and a secondary vortex finder in the underflow of the *** scale experiments are carried out to investigate the function of the concept of annular gap modification by the use of the thickening *** the second step,we transferred this concept to industrial scale for field tests at a municipal wastewater treatment *** the field tests,geometrical properties(annular gap modification)and operational parameters(volume flow rate,sludge type)were *** results show that thickening in terms of increasing the total solids content of sludge is possible with the modified *** the field tests,a thickening factor(total solids content of thickened sludge related to feed sludge)of 1.31(average)was reached for digested ***,a hydrocyclone could be a promising addition in wastewater treatment plants for dewatering of sludge.

This paper discusses the reduction of background noise in an industrial environment to extend *** the Industry 4.0 era,the mass development of voice control(speech recognition)in various industrial applications is possible,especially as related to augmented reality(such as hands-free control via voice commands).As Industry 4.0 relies heavily on radiofrequency technologies,some brief insight into this problem is provided,including the Internet of things(IoT)and 5G *** study was carried out in cooperation with the industrial partner Brose CZ spol.s.r.o.,where sound recordings were made to produce a *** experimental environment comprised three workplaces with background noise above 100 dB,consisting of a laser/magnetic welder and a press.A virtual device was developed from a given dataset in order to test selected commands from a commercial speech recognizer from *** tested a hybrid algorithm for noise reduction and its impact on voice command recognition *** virtual devices,the study was carried out on large speakers with 20 participants(10 men and 10 women).The experiments included a large number of repetitions(100 times for each command under different noise conditions).Statistical results confirmed the efficiency of the tested *** welding environment efficiency was 27%before applied filtering,76%using the least mean square(LMS)algorithm,and 79%using LMS+independent component analysis(ICA).Magnetic welding environment efficiency was 24%before applied filtering,70%with LMS,and 75%with LMS+*** workplace environment efficiency showed no success before applied filtering,was 52%with LMS,and was 54%with LMS+ICA.

Comparison of p38 MAPK, soluble endoglin and endothelin-1 level in severe preeclampsia and HELLP syndrome patients

Objective: To compare the levels of p38 mitogen-activated protein kinase (MAPK), soluble endoglin and endothelin-1 in the serum of patients with severe preeclampsia, hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome and normal pregnancies. Methods: This study was an observational analytic cross-sectional study performed at Wahidin Sudirohusodo Hospital, Makassar, Indonesia, in the period of 5th February 2016 to 20th January 2017. P38 MAPK, soluble endoglin and endothelin-1 levels of patients with normal pregnancies, severe preeclampsia and HELLP syndrome were measured by enzyme-linked immunoabsorbentassay technique, using kits of human soluble endoglin, endothelin-1 and p38 MAPK, Quantikine immunoassay: R&D System Inc. Results: Level of serum p38 MAPK in HELLP syndrome group was higher than in severe preeclampsia and normal pregnancy groups. Soluble endoglin and endothelin-1 levels in pregnancies with severe preeclampsia and HELLP syndrome were higher than normal pregnancy but there was no significant difference between these two groups (P>0.05). Levels of p38 MAPK, soluble endoglin and endothelin-1 also had a positive linear correlation with systolic and diastolic blood pressures (P<0.05). Conclusions: P38 MAPK in serum may be a marker for evidence of the severe hypoxia and its application may be considered for the diagnosis of HELLP syndrome.
