To the Editor： To date, epilepsy is one of the most common neurological disorders globally, affecting more than 50 million people worldwide. It is debilitating and represents a large health-care and economic burden, especially when treatment may often be expensive and not necessarily effective. It has been suggested that the burden of lifetime epilepsy has been significantly increasing in China in recent decades, The International League Against Epilepsy has defined epilepsy in 2014 as a neurological disorder with at least two unprovoked seizures occurring more than 24 h apart.

BACKGROUND： Acupuncture has been used as a treatment for cognitive impairment. OBJECTIVE： This review assesses clinical evidence for or against acupuncture as a treatment for cognitive impairment. This review also discusses the proposed mechanism（s） that could link acupuncture to improved cognitive function. METhoDS： We searched the literature using PolyUone search from its inception to January 2013, with full text available and language limited to English. Levels of evidence were examined using Oxford Centre for Evidence-based Medicine-Levels of Evidence （March, 2009）. RESULTS： Twelve studies met the inclusion criteria： 3 human studies and 9 animal studies. Levels of evidence ranged from level lb to level 5. CONCLUSION： Most animal studies demonstrated a positive effect of acupuncture on cognitive impairment. however, the results of human studies were inconsistent. Further high-quality human studies with greater statistical power are needed to determine the effectiveness of acupuncture and an optimal protocol.

BACKGROUND & OBJECTIVE: Little is known about the relative efficacies of percutaneous coronary intervention (PCI) with drug-eluting stents (DES) and bare-metal stents (BMS) in elderly patients。 The objective of this study was to evaluate the clinical outcome for geriatric patients who received either DES or BMS。 METhoDS: From January 2002 to October 2005, 199 consecutive Chinese geriatric patients (≥ 75 years old) underwent PCI with coronary DES or BMS implantation at our institution。 We analyzed the major clinical end points that included all-cause mortality, cardiovascular death, myocardial infarction, target lesion revascularization (TLR), stent thrombosis, and bleeding complications。 RESULTS: The three-year cumulative rates of all-cause mortality, cardiovascular death, and myocardial infarction were significantly lower in the DES group (6.3%,3.6%,5.4%) compared with the BMS group (16.2%,11.5%,14.9%; P hout increased risk of TLR, stent thrombosis, or bleeding complications at three years in this group of Chinese geriatric patients。

Impact of small-for-size liver grafts on medium-term and long-term graft survival in living donor liver transplantation: A meta-analysis

BACKGROUND Small-for-size grafts (SFSGs) in living donor liver transplantation (LDLT) could optimize donor postoperative outcomes and also expand the potential donor pool. Evidence on whether SFSGs would affect medium-term and long-term recipient graft survival is lacking. AIM To evaluate the impact of small-for-size liver grafts on medium-term and longterm graft survival in adult to adult LDLT. METhoDS A systematic review and meta-analysis were performed by searching eligible studies published before January 24, 2019 on PubMed, EMBASE, and Web of Science databases. The primary outcomes were 3-year and 5-year graft survival. Incidence of small-for-size syndrome and short term mortality were also extracted. RESULTS This meta-analysis is reported according to the guidelines of the PRISMA 2009 Statement. Seven retrospective observational studies with a total of 1821 LDLT recipients were included in the meta-analysis. SFSG is associated with significantly poorer medium-term graft survival. The pooled odds ratio for 3-year graft survival was 1.58 [95% confidence interval 1.10-2.29, P = 0.014]. On the other hand, pooled results of the studies showed that SFSG had no significant discriminatory effect on 5-year graft survival with an odds ratio of 1.31 (95% confidence interval 0.87-1.97, P = 0.199). Furthermore, incidence of small-for-size syndrome detected in recipients of SFSG ranged from 0-11.4% in the included studies. CONCLUSION SFSG is associated with inferior medium-term but not long-term graft survival. Comparable long-term graft survival based on liver graft size shows that smaller grafts could be accepted for LDLT with appropriate flow modulatory measures. Close follow-up for graft function is warranted within 3 years after liver transplantation.

The mechanisms underlying rootzone-localized responses to salinity during early stages of barley development remain *** this study,we performed the analyses of multi-root-omes(transcriptomes,metabolomes,and lipidomes)of a domesticated barley cultivar(Clipper)and a landrace(Sahara)that maintain and restrict seedling root growth under salt stress,*** generalized linear models were designed to determine differentially expressed genes(DEGs)and abundant metabolites(DAMs)specific to salt treatments,genotypes,or rootzones(meristematic Z1,elongation Z2,and maturation Z3).Based on pathway over-representation of the DEGs and DAMs,phenylpropanoid biosynthesis is the most statistically enriched biological pathway among all salinity responses *** with histological evidence,an intense salt-induced lignin impregnation was found only at stelic cell wall of Clipper Z2,compared with a unique elevation of suberin deposition across Sahara *** suggests two differential salt-induced modulations of apoplastic flow between the *** on the global correlation network of the DEGs and DAMs,callose deposition that potentially adjusted symplastic flow in roots was almost independent of salinity in rootzones of Clipper,and was markedly decreased in *** together,we propose two distinctive salt tolerance mechanisms in Clipper(growth-sustaining)and Sahara(salt-shielding),providing important clues for improving crop plasticity to cope with deteriorating global soil salinization.
