To the Editor:Extraovarian primary peritoneal carcinoma(EOPPC)is an uncommon malignancy with many similarities to epithelial ovarian carcinoma in histological,clinical,and etiological aspects[1].This phenomenon is explained by their common embryonal origin,in which both develop from the coelomic epithelium in the early embryological *** their similarities,the incidence of EOPPC is significantly lower than that of epithelial ovarian carcinoma(6.78 cases per million vs.120.5 cases per million)[1].

Different brain tumors of distinct histology can co-exist in the setting of phakomatoses or as a complication of radiotherapy. In the absence of these predisposing factors, this phenomenon is uncommon. When the lesions are in close proximity they are described as collision tumors and are extremely rare. A 58-year-old woman presented with persistent headache and cognitive decline for three months. Magnetic resonance imaging revealed a tumor arising from the atrium of the left lateral ventricle with heterogeneous contrast enhancement. This intraventricular lesion was adjacent to another extensive infiltrating tumor of the basal cisterns. Operative findings revealed a vascular ventricular tumor and gross total resection was achieved. An adjacent avascular basal cistern tumor with a pearly white sheen was encountered and partial excision was performed. The histopathological diagnosis was central neurocytoma and epidermoid tumor. There is only one documented description of a central neurocytoma co-existing with a tumor of different pathology. To our knowledge, this is the first reported collision tumor case involving central neurocytoma. Since the incidence of both lesions co-existing juxtaposed is extremely low, a chronic oncogenetic inflammatory process stimulated by the epidermoid tumor to the subventricular region is suggested. Other mechanisms for tumor collision are discussed and we suggest a classification system for this rare association to reflect their pathogenesis.

Due to the easy coarsening caused by poor thermal stability,the verified annealing-induced hardening in nanograined metals can only maintain at a relatively low-temperature *** this study,a nanolam-inated(CrCoNi)_(97.4)Al_(0.8)Ti_(1.8)medium-entropy alloy with an average lamellae thickness of∼20 nm embedded by thinner nanotwins was fabricated by severe cold rolling to achieve superior thermal *** with the conventional nanotwinned CrCoNi with nanotwins inside ultra-fined grains,the hier-archical nanolaminated-nanotwinned(CrCoNi)_(97.4)Al_(0.8)Ti_(1.8) exhibits a significant annealing-induced hard-ening effect,i.e.,hardness increasing from∼250 HV in the original specimen to∼500 HV in the cold-rolled status and finally∼630 HV after annealing at 600℃for 1 *** microstructure characterizations reveal that the reduced dislocation density and formation of L1_(2)ordered domain are mainly responsible for such hardening effect,which is facilitated by the effectively suppressed coarsening with annealing temperature,i.e.,slow detwinning process and well-retained low-angle nanolamellar *** coarsening mechanisms from the cold-rolled nanolamellae to the fully recrystallized micro-equiaxed structures under the annealing temperatures ranging from 400 to 800℃ were also elucidated by atomic observations.

This paper presents a novel observer model that integrates quantum mechanics, relativity, idealism, and the simulation hypothesis to explain the quantum nature of the universe. The model posits a central server transmitting multi-media frames to create observer-dependent realities. Key aspects include deriving frame rates, defining quantum reality, and establishing hierarchical observer structures. The model’s impact on quantum information theory and philosophical interpretations of reality are examined, with detailed discussions on information loss and recursive frame transmission in the appendices.

Background: Transarterial chemoembolization(TACE) is the most commonly used adjuvant therapy for hepatocellular carcinoma(HCC) after curative resection. Responses to TACE are variable due to tumor and patient heterogeneity. We had previously demonstrated that expression of Granulin-epithelin precursor(GEP) and ATP-dependent binding cassette(ABC)B5 in liver cancer stem cells was associated with chemoresistance. The present study aimed to evaluate the association between GEP/ABCB5 expression and response to adjuvant TACE after curative resection for HCC. Methods: Patients received adjuvant TACE after curative resection for HCC and patients received curative resection alone were identified from a prospectively collected database. Clinical samples were retrieved for biomarker analysis. Patients were categorized into 3 risk groups according to their GEP/ABCB5 status for survival analysis: low(GEP-/ABCB5-), intermediate(either GEP +/ABCB5-or GEP-/ABCB5 +) and high(GEP +/ABCB5 +). Early recurrence(recurrence within 2 years after resection) and disease-free survival were analyzed. Results: Clinical samples from 44 patients who had followed-up for more than 2 years were retrieved for further biomarker analysis. Among them, 18 received adjuvant TACE and 26 received surgery alone. Patients with adjuvant TACE in the intermediate risk group was associated with significantly better overall survival and 2-year disease-free survival than those who had surgery alone( P = 0.036 and P = 0.011, respectively). Adjuvant TACE did not offer any significant differences in the early recurrence rate, 2-year disease-free survival and overall survival for patients in low and high risk groups. Conclusions: Adjuvant TACE can only provide survival benefits for patients in the intermediate risk group(either GEP +/ABCB5-or GEP-/ABCB5 +). A larger clinical study is warranted to confirm its role in patient selection for adjuvant TACE.

Choledochal cysts are congenital anomalies of the biliary ducts, characterized by cystic dilatation of the *** diagnosis of this anomaly using ultrasonography (US) has been well documented. Magnetic resonance imaging (MRI) has recently become an important complement to US in prenatal diagnosis of fetal anomalies. We herein report a patient in whom at 24 wk' gestation US suggested a right upper quadrant abdominal cyst and in whom at 26 wk' gestation MRI more clearly delineated the cyst and its surrounding structures and suggested a choledochal cyst, which was confirmed at postnatal surgery and histopathology.

AIM:To investigate the safety and diagnostic yield of colonoscopy in Chinese children in whom the procedure is not often ***:We conducted a retrospective review of all colonoscopies in consecutive children who underwent their fi rst diagnostic colonoscopy from Jan 2003 to ***:Seventy-nine children (48 boys,31 girls;mean age 9.2 ± 4.2 years) were identified and reviewed with a total of 82 colonoscopies *** caecal and ileal intubation rates were 97.6% and 75.6% *** patients (50.6%) had a positive diagnosis made in colonoscopy and that included colonic polyps (23),Crohn's disease (12),ulcerative colitis (1),and miscellaneous causes (4).80% of polyps were in the rectosigmoid *** but one were juvenile hamartomatous *** exceptionwas an adenomatous *** mean ages for children with inflammatory bowel disease (IBD) and polyps were 11.3 and 4.3 years *** was no procedure-related ***:Colonoscopy is a safe procedure in our Chinese *** increasing diagnosis of IBD in recent decades may reflect a rising incidence of the disease in our children.

Aging is considered an irreversible biological process and also a major risk factor for a spectrum of geriatricdiseases. Advanced age-related decline in physiological functions, such as neurodegeneration, development ofcardiovascular disease, endocrine and metabolic dysfunction, and neoplastic transformation, has become thefocus in aging research. Natural aging is not regarded as a programmed process. however, accelerated agingdue to inherited genetic defects in patients of progeria is programmed and resembles many aspects ofnatural aging. Among several premature aging syndromes, Werner syndrome (WS) and Hutchinson–Gilfordprogeria syndrome (HGPS) are two broadly investigated diseases. In this review, we discuss how stem cellaging in WS helps us understand the biology of aging. We also discuss briefly how the altered epigeneticlandscape in aged cells can be reversed to a “juvenile” state. Lastly, we explore the potential application ofthe latest genomic editing technique for stem cell-based therapy and regenerative medicine in the context of aging.

Reperfusion therapy is the preferred treatment for ischemic stroke,but is hindered by its short treatment window,especially in patients with diabetes whose reperfusion after prolonged ischemia is often accompanied by exacerbated *** mechanisms underlying exacerbated hemorrhage are not fully *** study aimed to identify this mechanism by inducing prolonged 2-hour transient intraluminal middle cerebral artery occlusion in diabetic Ins2Akita/+mice to mimic patients with diabetes undergoing delayed mechanical *** results showed that at as early as 2 hours after reperfusion,Ins2Akita/+mice exhibited rapid development of neurological deficits,increased infarct and hemorrhagic transformation,together with exacerbated down-regulation of tight-junction protein ZO-1 and upregulation of blood-brain barrier-disrupting matrix metallopeptidase 2 and matrix metallopeptidase 9 when compared with normoglycemic Ins2+/+*** indicated that diabetes led to the rapid compromise of vessel integrity immediately after reperfusion,and consequently earlier death and further aggravation of hemorrhagic transformation 22 hours after *** observation was associated with earlier and stronger up-regulation of pro-angiogenic vascular endothelial growth factor(VEGF)and its downstream phospho-Erk1/2 at 2 hours after reperfusion,which was suggestive of premature angiogenesis induced by early VEGF up-regulation,resulting in rapid vessel disintegration in diabetic *** reticulum stress-related pro-apoptotic C/EBP homologous protein was overexpressed in challenged Ins2Akita/+mice,which suggests that the exacerbated VEGF up-regulation may be caused by overwhelming endoplasmic reticulum stress under diabetic *** conclusion,the results mimicked complications in patients with diabetes undergoing delayed mechanical thrombectomy,and diabetes-induced accelerated VEGF up-regulation is likely to underlie exacerbated hemorrhagic transformat

Spermatogenesis is a tightly regulated process of development for the generation of mature spermatozoa. Mammalian spermatogenesis occurs in the convoluted seminiferous tubules of the testes, where spermatogenic stem cells （SSCs） reside on sustentacular Sertoli cells of somatic origin, forming a germinal epithelial structure. Spermato genesis is accomplished in a complicated, timely controlled differentiation of SSCs into premeiotic primary spermatocytes, followed by formation of postmeiotic round spermatids and their subsequent maturation into spermatozoa.
