Improvement in social skills in patients with schizophrenia following 6 months of day-care treatment

Using role-playing tests simulating real-life situations, we investigated improvement in the social skills, especially information processing, of patients with schizophrenia in response to day-care in psychiatry and social skills training. From 20 pair-matched schizophrenia patients, 10 were assigned to a day-care/SST group and 10 to a control group undergoing usual treatment. After 6 months, sending and receiving skills were assessed by means of role-playing tests, psychiatric symptoms, social function, and cognitive function. The day-care/SST group showed a significant improvement and a large effect size in sending skills. Receiving skills also improved, showing a medium effect size, but the difference from the control group was not statistically significant. No statistically significant changes were observed in the other measures. That no changes occurred in social function, psychiatric symptoms, or cognitive function leads to two conclusions: 1) cognition targeted, specialized training and comprehension of cognitive remediation, psychiatric rehabilitation, and social skill development are necessary for improvement in other domains, especially symptoms and social functions;and 2) improvement resulting from day-care in psychiatry and SST alone is limited. In addition, further investigations over a longer observation period and a greater sample size are needed.

Topical nitrate drip infusion using cystic duct tube for retained bile duct stone:A six patients case series

A retained bile duct stone after operation for cholelithiasis still occurs and causes symptoms such as biliary colic and obstructive jaundice.An endoscopic retrograde cholangiopancreatography with endoscopic sphincterotomy(EST),followed by stone extraction,are usually an effective treatment for this condition.However,these procedures are associated with severe complications including pancreatitis,bleeding,and duodenal perforation.Nitrates such as glyceryl trinitrate(GTN) and isosorbide dinitrate(ISDN) are known to relax the sphincter of Oddi.In 6 cases in which a retained stone was detected following cholecystectomy,topical nitrate drip infusion via cystic duct tube(C-tube) was carried out.Retained stones of 2-3 mm diameter and no dilated common bile duct in 3 patients were removed by drip infusion of 50 mg GTN or 10 mg ISDN,which was the regular dose of intravenous injection.Three other cases failed,and EST in 2 cases and endoscopic biliary balloon dilatation in 1 case were performed.One patient developed an adverse event of nausea.Severe complications were not observed.We consider the topical nitrate drip infusion via C-tube to be old but safe,easy,and inexpensive procedure for retained bile duct stone following cholecystectomy,inasmuch as removal rate was about 50% in our cases.

＜正＞Twenty-one patients with primary high-grade advanced gastric cancer in whom the presence of stage ? was con- firmed by preoperative diagnostic imaging,were treated with a wide-spectrum regimen,designated as FEPMTX therapy to establish an effective salvage chemotherapy.FEPMTX therapy consisted of 5-fluorouracil（5-FU）and the triple biochemical modulators;methotrexate（MTX）,calcium leucovorin（LV）,cisplatin（CDDP）,and in addition of epiru- bicin（EPI）.The schedule comprised 3 days continuous administration of 5-FU（350 mg/m2/day）and MTX（35 mg/ m2）on day 1,LV（30 mg/m2）on the day 2,LV（30 mg/m2）and CDDP（30 mg/m2）and EPI（20 mg/m2）on day 3.The treatment was performed two or more courses with 2 weeks interval in principle.Eleven partial responses（PR）, five no changes（NC）and five progressive diseases（PD）were obtained,and the response rate was 52%.Ten patients （PR 7,NC 2,PD 1）were received gastrectomy（resectability rate 48%）.The survival in all cases resulted in median survival of 184 days and one-year survival rate of 23%.While there is no significant difference insurvival between the cases with or without the resection of the primary lesion,the survival of responders was significantly longer than that of non-responders（median survival time;responder 356days,non-responder 152days）.Adverse effects such as myelo- suppression and anorexia and fatigue sometimes occurred,but were mild and all the patients well tolerated.FEPMTX therapy is thought to be an effective regimen for neoadjuvant chemotherapy with longer survival and little toxic for patients with stage?gastric cancer.
