Preoperative lymph node staging of oral cancer with FDG-PET

Purpose: To evaluate the diagnostic accuracy of FDG-PET relative to CT for detecting metastatic cervical lymph nodes in patients with oral squamous cell carcinoma (OSCC), and to ascertain the factors that affect this accuracy。Methods: A total of 1076 lymph nodes obtained from 35 neck dissections in 26 OSCC patients who preoperatively underwent both FDG-PET and CT were retrospectively analyzed。For pathological metastatic lymph nodes, the lymph node size (short-axis diameter), the ratio of intranodal tumor deposits, and the size of intranodal tumor deposits (maximum diameter of metastatic foci in each lymph node) were histologically recorded。Results: Forty-six lymph nodes from 23 neck sides were pathologically diagnosed metastases。The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of FDG-PET evaluated individually per neck side were 74, 92, 80, 94 and 65%, respectively, whereas those of CT were 78, 58, 71, 78 and 58%, respectively。FDG-PET detected 100% of metastatic lymph nodes > 10 mm, intranodal tumor deposits -> 9 mm, and intranodal tumor deposits with a ratio > 75%, whereas no nodes or tumor deposits smaller than 5 mm were detected。The spatial resolution limitations of FDG-PET were responsible for 16 of 20 (80%) false-negative PET results in lymph nodes。Conclusion: FDG-PET is a useful tool for preoperative evaluation of the neck because it accurately detects metastatic lymph nodes≥10 mm and has fewer false positive cases than CT。The high specificity of FDG-PET for lymph node metastases may play an important role in avoiding unnecessary neck dissection。

Clinical Evaluation of Moisturizers with Physiological Analysis of Stratum Corneum TARC and TSLP

To verify the usefulness of moisturizers, a multi-center study was conducted on patients with atopic dermatitis who visited 3 university hospitals in Japan between November 2009 and March 2012. Thirty-seven patients with dry skin and stable symptoms who were receiving topical and oral treatments were given moisturizers for 8 weeks. The moisturizers contained components such as glycerin and seawater minerals in 3 forms: lotion, emulsion, and cream. The moisturizers were chosen by physicians based on the degree of dryness of the patients’ skin. In addition to observing the skin condition of the entire body, high-frequency conductance as a parameter of skin surface hydration and transepidermal water loss (TEWL) in the buccal region and the medial side of the forearm were measured, and those of the back were used for comparison. Furthermore, stratum corneum levels of thymic stromal lymphopoietin (TSLP) and thymus and activation-regulated chemokine (TARC), both of which were objective parameters for atopic dermatitis skin lesion severity, were analyzed using the tape stripping method. At the beginning of the study, TSLP showed a correlation with skin symptoms (dryness, itching) and high-frequency conductance, whereas TARC showed a correlation with skin symptoms (erythema, dryness, itching) and TEWL. In addition, a correlation was noted among TSLP, TARC, and itching. At the end of the study, erythema, dryness, itching, TEWL,TSLP, and TARC were significantly reduced, whereas high-frequency conductance was significantly increased. The moisturizers clearly improved the dry skin symptoms of these patients with atopic dermatitis and improved the physiology of their sensitive and damaged skin. These findings also support the involvement of cytokines/chemokines in the pathogenesis of atopic dermatitis lesions.

Maternal Depression and Mother-to-Infant Bonding: The Association of Delivery Mode, General Health and Stress Markers

Aim: The aim of the present study was to examine associations among maternal stress level, general health, mother to infant bonding, maternal depression level, and mode of delivery. Methods: Mothers who delivered a single baby at term were recruited with a total 435 mothers participating in the study. Outcome measures: Data were collected 6 months after delivery using General Health Questionnaire (GHQ-28), Mother-to-Infant Bonding Scale (MIBS), Edinburgh Postnatal Depression Scale (EPDS) as well as socio-demographic and medical information at 6 months. Additionally, salivary cortisol and chromogranin levels were determined. Results: The proportion of breast-feeding mothers in the vaginal delivery group (51.4%) was significantly higher than that of the cesarean section group (24%). GHQ-28 scores were significantly related to EPDS scores and MIBS scores (P Conclusion: Maternal general health is associated with maternal psychological health. These results suggest that the support of maternal health is important to maintain maternal psychological status and bonding to infants. Because EPDS scores were significantly worse in the planned cesarean section group (4.2 ± 3.3), careful management is needed of mothers who deliver by planned cesarean section.

AIM: To determine the efficacy of Mac-2 binding protein (Mac-2bp) for diagnosis of chronic ***: Fifty-nine healthy volunteers (HV), 162 patients with chronic pancreatitis (CP), and 94 patients with pancreatic ductal adenocarcinoma (PDAC) were enrolled in this study. We measured serum Mac-2bp using our developed enzyme-linked immunosorbent assay kit. Additional biochemical variables were measured using an automated analyzer (including aminotransferase, alanine aminotransferase, γ-glutamyltransferase, alkaline phosphatase, triglyceride, C-reactive protein, and amylase levels) or chemiluminescent enzyme immunoassay (carbohydrate antigen 19-9 and carcinoembryonic antigen). The ability of Mac-2bp to predict CP diagnosis accurately was assessed using receiver operating characteristic (ROC) ***: Serum Mac-2bp levels were significantly increased in CP patients compared to HV (P < 0.0001) and PDAC patients (P < 0.0001). Area under the ROC curve values of Mac-2bp for the discrimination of CP from HV and PDAC were 0.727 and 0.784, respectively. Multivariate analyses demonstrated that serum Mac-2bp levels were independent determinants for CP diagnosis from HV and PDAC patients. Immunohistological staining showed that Mac-2bp was expressed faintly in the pancreas tissues of both CP and PDAC patients. Serum aspartate aminotransferase, alanine aminotransferase, γ-glutamyltransferase, alkaline phosphatase, and triglyceride levels were significantly higher in patients with CP or PDAC. Serum Mac-2bp levels were highly correlated with protein levels of alanine aminotransferase, γ-glutamyltransferase, and C-reactive protein, but not amylase, suggesting that the damaged liver produces ***: Measurement of serum Mac-2bp may be a novel and useful biomarker for CP diagnosis as well as liver fibrosis in the general population.

An experimental high resolution VTR for home use,which can record the wide band video signal such as the NTSC-TV for broadcasting or EDTV (Extended Definition TV System) recommended by Hitachi,has recently been developed. Such wide band video signal is recorded on only one recording channel without any bandwidth compression techniques by the combination of the Co-doped γFeO tape and newly-developed ferrite heads. In this paper,the design policy to realize the wide band recording and the construction and performances of the experimental 1/2 inch home use VTR are introduced.

Clinical Evaluation of Novel Natural Polysaccharides Sacran as a Skincare Material for Atopic Dermatitis Patients

Atopic dermatitis is a skin disease characterized by inflammation, pruritus, and chronic or relapsing eczematous lesions. Recently, ampholytic polysaccharide sacran has attracted a particular focus of attention as a novel biomaterial. In the present study, we investigated the effect of sacran solution on atopic dermatitis in the clinical study. Almost all of the average scores for atopic dermatitis symptoms of each patient treated with sacran solutions were improved. In addition, the scores of sleep disorder and itching were also significantly ameliorated by the sacran treatment for 4 weeks, compared with those of initial states. In immatured dermal skin model stimulated with 2,4-dinitrofluorobenzene (DNFB), the sacran treatment markedly down-regulated inflammatory cytokine and chemokine mRNA levels such as MCP-1, TNF-α, IL-1β, and IL-6 mRNAs, compared with that of DNFB alone. Furthermore, a sacran solution significantly suppressed the mRNA expression of TNF-α and COX-2 in RAW264.7 cells, a murine macrophage-like cell line, stimulated with phorbol-12-myristate-13-acetate (PMA). In RBL-2H3 cells, a rat basophilic leukemia cell line, a sacran solution significantly lowered β-hexosaminidase release, indicating the suppression of allergic response. These results suggest that a sacran solution may have the potential to improve atopic dermatitis through the impairment of production of inflammatory cytokine and chemokine mRNA.

A Study of Mallory-Weiss Syndrome Secondary to Upper Gastrointestinal Bleeding

Background: In Mallory-Weiss Syndrome (MWS), vomiting causes the mucous membrane and submucosa near the esophagogastric mucosal junction to tear and bleed. Thus, MWS can arise after heavy drinking and as a complication of endoscopic upper gastrointestinal examinations or procedures. However, there has been no report of MWS secondary to upper gastrointestinal bleeding thus far. Case Subjects: Of 79 MWS cases over a 10-year period from 2002 to 2011, we identified and studied 6 cases, in which MWS was probably caused by another lesion causing upper gastrointestinal bleeding. Results: There were 2 cases, each of gastric ulcers, duodenal ulcers, and varicose veins. In 3 cases, MWS was discovered during treatment of the primary lesion and was simultaneously treated. In the other 3 cases, patients were treated for MWS according to endoscopic diagnosis, and the primary lesion was overlooked;in these cases, the primary lesion was identified and treated after the bleeding recurred. One patient with cirrhosis died of hepatic failure. Conclusion: Care should be taken when dealing with MWS because it can occur as a result of vomiting caused by hematemesis or as a complication of endoscopic examination and treatment. When assessing MWS, other hemorrhagic lesions may be overlooked.

The East Japan Great Earthquake and Three Cases of Near-Drowning with Tsunami Due to the Earthquake

We encountered the Great East Japan Earthquake on March 11, 2011. The magnitude of the earthquake is 9.0 and it is one of the greatest from A.D. 1900 to now. The earthquake developed large tsunami and many people living on the pacific coast of east Japan died from lung disorder caused by near drowning with tsunami. We also encountered three cases of lung disorders caused by near drowning. All three were females, and two of them were old elderly. All segments of both lungs were involved in all the three patients, necessitating ICU admission and endotracheal intubation and mechanical ventilation. All three died within three weeks. In at least two cases, misswallowing of oil was suspected from the features noted at the time of the detection. Many bacteria were detected from the phlegm of two cases.

Steroid Sulfatase Inhibitor Reduces Proliferation of Ishikawa Endometrial Cancer Cells in Co-Culture Systems

Objectives: Estrogens significantly contribute toward the growth and development of endometrial cancers. Two principal pathways have been implicated in the final steps of estrogen synthesis: the steroid sulfatase (STS) and aromatase pathways. In this study, we aimed to evaluate the possible effects of tumor-stromal interactions on local estrogen biosynthesis in endometrial cancer. We also assessed the biological effects of inhibitors of steroid sulfatase and aromatase in the co-culture system compared with usual monocultures. Methods/Materials: We isolated stromal cells from endometrial cancer patients to examine local biosynthesis of estrogens and tumor-stromal interactions. Next we examined the effects of steroid sulfatase inhibitor and aromatase inhibitor in monoculture of endometrial cancer cell line (Ishikawa) and in a co-culture system involving an Ishikawa cells and stromal cells. Results: Estrogen receptor and steroid sulfatase mRNA levels in cancer cells were significantly higher in the co-cultures compared with the monocultures of endometrial cancer cells. Estradiol and androstenediol concentrations were also significantly higher in the co-cultured cells. Proliferation of the cancer cells was significantly increased through the steroid sulfatase pathway, which metabolizes androgens, estrone sulfate, and estradiol sulfate as its substrates. However, its proliferation was significantly decreased by the treatment of steroid sulfatase or aromatase inhibitors. The significant growth inhibition by the steroid sulfatase and aromatase inhibitors were also observed in the co-culture system. Conclusions: We evaluated the effects of STS inhibitor and aromatase inhibitors on the proliferation of estrogen-dependent endometrial cancer cells. Considering that intratumoral estrogen metabolism plays an important role, our co-culture systems provide an environment similar to that of the tumor in living patients in terms of metabolism and synthesis of intratumoral estrogens.
