Inverted Meckel's diverticulum preoperatively diagnosed using double-balloon enteroscopy

An inverted Meckel’s diverticulum is a rare gastrointestinal congenital anomaly that is difficult to diagnose prior to surgery and presents with anemia, abdominal pain, or intussusception. Here, we report the case of 57-year-old men with an inverted Meckel’s diverticulum, who was preoperatively diagnosed using double-balloon enteroscopy. He had repeatedly experienced epigastric pain for 2 mo. Ultrasonography and computed tomography showed intestinal wall thickening in the pelvis. Double-balloon enteroscopy via the anal route was performed for further examination, which demonstrated an approximately 8-cm, sausage-shaped, submucosal tumor located approximately 80 cm proximal to the ileocecal valve. A small depressed erosion was observed at the tip of this lesion. Forceps biopsy revealed heterotopic gastric mucosa. Thus, the patient was diagnosed with an inverted Meckel’s diverticulum, and single-incision laparoscopic surgery was performed. This case suggests that an inverted Meckel’s diverticulum should be considered as a differential diagnosis for a submucosal tumor in the ileum. Balloon-assisted enteroscopy with forceps biopsy facilitate a precise diagnosis of this condition.

Genetic Polymorphisms and Toxicities of First-Line Antituberculosis Drugs: Systematic Review of the Literature

Introduction: Polymorphisms are the main genetic factors associated with toxicities of antituberculosis drugs. This literature review summarizes the polymorphisms of the genes that code for the enzymes of the metabolism of antituberculosis drugs and their transmembrane transporters. Some mechanisms of drug-associated toxicities and strategies for their management have also been described in this review. Methods: The bibliographic searches were exclusively carried out in PubMed, over a period of ten years (2010-2020). The search terms were the words “toxicity + antituberculosis drug + one or two word(s) among the following: polymorphism, genetics, mutation, SNP, HLA or haplotype”. Publications in English or French, relating to the various toxicities associated with first-line anti-tuberculosis drugs (Rifampicin, Isoniazid, Ethambutol and Pyrazinamide) administered to patients with pulmonary tuberculosis, extrapulmonary tuberculosis or co-infected with TB/HIV were included in this review. Duplicates, in vitro, in silico or drug-induced toxicity studies other than antituberculosis drugs and genetic mutations of Mycobacteria strains were not included. Results: The studies selected and included were case reports, cohort studies, original research, systematic reviews and meta-analyses on human subjects of different ethnic origins. Hepatotoxicity is the most common toxicity associated with NAT2, CYP2E1, GSTM1 and GSTT1 polymorphisms in patients on antituberculosis drugs. Other forms of toxicity, less frequent, occurring in certain patients under concomitant treatment with nonsteroidal anti-inflammatory drugs (NSAIDs), antiretrovirals (ARVs), antibiotics or antiepileptics have also been identified. Conclusion: The genetic polymorphisms associated with the toxicities of antituberculosis drugs concern both the main enzymes of the metabolic pathways (NAT2, CYP2E1, GST) and the transmembrane transporters (SLCO1B1 and ABCB1). Development of Powder Metallurgy （PM） Compacted Cu-TaC Electrodes for EDM

The main aim of this paper is to investigate the properties of Cu-TaC electrodes produced by Powder Metallurgy （PM） method. The design of Experiment （DOE） method was used to plan the investigation. Two different compositions of the powders （Cu-TaC with 30 and 55 % wt TaC） were used. The major properties which determine suitability of electrodes for Electro Discharge Machining （EDM） are electrical conductivity, therrnal conductivity and to some extent density. These properties were measured for the green compacted electrodes, analyzed and compared with their sintered counterparts. This is the initial stage to determine the suitability or otherwise of the compacted electrodes. The results showed that the compacted electrodes in green form can be suitable for EDM, since the electrical conductivities are very high （94.96-189.92Ω^-1m^-1）. The thermal conductivity is good （29.70-33.20W/m K）. The density ranges between 6.13 and 9.80 g/cm3. The sintered electrodes were found to be unsuitable at the specified conditions, because they became non-conductive electrically after sintering. Current efforts are geared towards improving these properties for the sintered ones and also determining their optimum levels.

Processing and Properties of Porous (K,Na)NbO_3-based Lead-free Piezoceramics

Porous piezoceramics are of interest for applications such as low frequency hydrophones. Porous(K,Na,Li)NbO_3-BaZrO_3-(Bi,Na)TiO_3 lead-free piezoceramics having vertical morphotropic phase boundary composition were fabricated by adding 30 μm-diameter acrylic resins. The ceramic powders were synthesized by a conventional solid-state reaction method and then mixed with the resins. Volume ratios of ceramics to resin were 90:10, 80:20, 70:30, and 60:40. After burning out the resins and binder, the specimens were sintered at 1170–1190 ℃ for 5 h. The porous specimen with 30 vol% acrylic resin had longitudinal acoustic velocity of 4518 m/s, acoustic impedance of 16.44 Mkg/(m^2·s), and hydrostatic figure of merit of 405×10^(-15)m^2/N. The hydrostatic figure of merit was about 4 times of that of monolithic ceramics. Compared with monolithic ceramics, the porous specimen had a better transducer sensitivity for underwater applications.

Usefulness of a novel slim type Flush Knife-BT over conventional Flush Knife-BT in esophageal endoscopic submucosal dissection

To investigated the usefulness of a novel slim type ball-tipped FlushKnife (FlushKnife-BTS) over ball-tipped FlushKnife (FlushKnife-BT) in functional experiments and clinical *** order to evaluate the functionality of FlushKnife-BTS, water aspiration speed, resistance to knife insertion through the scope, and waterjet flushing speed were compared between FlushKnife-BTS and BT. In clinical practice, esophageal endoscopic submucosal dissection (ESD) performed using FlushKnife-BTS or BT by an experienced endoscopist between October 2015 and January 2016 were retrospectively reviewed. The treatment speed and frequency of removing and reinserting the knife to aspirate fluid and air during ESD sessions were *** experiments revealed that water aspiration speed by the endoscope equipped with a 2.8-mm working channel with FlushKnife-BTS was 7.7-fold faster than that with conventional FlushKnife-BT. Resistance to knife insertion inside the scope with a 2.8-mm working channel was reduced by 40% with FlushKnife-BTS. The waterjet flushing speed was faster with the use of FlushKnife-BT. In clinical practice, a comparison of 6 and 7 ESD using FlushKnife-BT and BTS, respectively, revealed that the median treatment speed was 25.5 mm2/min (range 19.6-30.3) in the BT group and 44.2 mm2/min (range 15.5-55.4) in the BTS group (P = 0.0633). However, the median treatment speed was significantly faster with FlushKnife-BTS when the resection size was larger than 1000 m2 (n = 4, median 24.2 mm2/min, range 19.6-27.7 vs n = 4, median 47.4 mm2/min, range 44.2-55.4, P = 0.0209). The frequency of knife replacement was less in the BTS group (median 1.76 times in one hour, range 0-5.45) than in the BT group (7.02 times in one hour, range 4.23-15) (P = 0.0065).CONCLUSIONOur results indicate that FlushKnife-BTS enhances the performance of ESD, particularly for large lesions, by improving air and fluid aspirat

In this paper, we have obtained inequalities in the reverse direction for the above class of polynomials. Besides, extensions of some Turan-type inequalities for the polar derivative of polynomials have been considered.

研究者注意到,在塞内加尔,英语作为第二语言学习的课程仍然受到传统教学方法的影响,传统教学方法注重语言能力,而不利于交际能力的培养。本研究旨在调查新推的交际语言教学法,了解其对塞内加尔坦巴昆达地区几所学校学生21世纪技能发展的潜在影响。开展本研究,很大程度上是为了强调交际语言教学法对学习者21世纪技能发展的影响以及教师在实施过程中面临的挑战,并试图提出解决问题的方案。本研究采用了混合的调查方式,既向教师分发了包括选择题和开放式陈述题的问卷,同时又对学生进行了半结构化访谈,以更好地了解与研究问题相关的问题。最后,研究者意识到我们仍然需要花费更多的努力,以使得交际语言教学法变得有效,最终使学习者能够取得更好的结果。研究对教学做出了一些提议,也提出了一些建议,以帮助决策者制定新的政策来扭转当前趋势。

职业健康和安全是所有工人的一项全球人权,不论其属于正式或非正式部门,也不论其属于发达国家或发展中国家。它只是呼吁为所有人创造一个安全、健康的工作环境。几百年来,国际劳工组织和世界卫生组织等伙伴以及非政府组织和民间社会组织一直通过有关职业健康和安全的行政、政策和法律文书,支持工人的基本权利。在全球范围内,农业部门是低技能弱势工人阶级（如妇女）的主要雇主,特别是在加纳这样的发展中国家。本论文以Ashiaman市政装配区和Sekondi-Takoradi都市装配区的手工棕榈仁油加工集团为重点,以提升棕榈仁油加工者的健康和安全为目标,开展了非正式农业加工中小型企业的职业健康和安全管理研究。论文采用实地调查,问卷、访谈等形式采集了相关数据,应用Microsoft Excel和Mintab软件对数据进行了分析。分析了手工棕榈仁油加工者的现有工作条件、棕榈仁油加工者的健康和安全状况,阐述了生产中有效的健康和安全实践以及城市农业加工中心集合区域具有独特的属性。研究发现:手工棕榈仁加工企业吸引了教育水平低的妇女、移民以及由于工作对体力的需求而处于生命黄金时期的妇女。这些手工棕榈仁加工群体在日常加工活动中的健康和安全管理实践总体水平很差,加工商忽视劳动者的日常活动中的健康和安全措施。劳动群体严重依赖于多年的加工经验而忽视了基本的人员防护装备。负责职业健康和安全条例和教育的受委托的国家机构在加纳大型非正规部门的这一分部门中代表性很差。基于上述研究,论文提出了在非正式部门建立和教育关于基本职业卫生和安全管理与相关知识培训的建议,同时建议加纳议会通过的一项国家政策,以便在加纳建立一种卫生和安全文化。

Mineralogical characterization and leaching behavior of Nigerian ilmenite ore

The characterization and H2SO4 leaching behavior of a Nigerian ilmenite ore following mechanical activation and alkali roasting were investigated. The effects of NaOH/ore ratio, H2SO4 concentration, leaching and roasting temperature on the Ti recovery from the milled ore were examined. The results show that mechanical activation significantly enhances the dissolution of ilmenite ore. Under the leaching conditions of 90 °C, 60% （v/v） H2SO4 and 4 h, about 72% Ti extraction was obtained from a milled ore roasted at 850 °C with 60% NaOH. X-ray diffraction （XRD） phase analysis of the roasted mass, water treated residue and leach residue supports the reaction mechanism and experimental results.
