Classic polyarteritis nodosa(PAN) that targets mediumsized muscular arteries and microscopic polyangiitis(MPA),characterized by inflammation of small-caliber vessels and the presence of circulating myeloperoxidase anti-neutrophil cytoplasmic antibodies(MPO-ANCA),are distinct clinicopathological entities of systemic vasculitis.A 66-year-old woman presented with fever,cholestasis and positive *** examination showed a pancreatic mass compressing the bile ***,we performed *** examination revealed that necrotizing vasculitis predominantly affecting the medium-sized vessels,spared arterioles or capillaries in the pancreas,a finding consistent with ***,renal biopsy revealed small-caliber vasculitis and glomerulonephritis,supporting *** initial manifestation of a pancreatic mass associated with vasculitis has only been reported in 7 *** diagnosis is challenging because no reliable clinico-radiological findings have been *** should be aware of such cases and early diagnosis followed by immunosuppression is *** findings may reflect a polyangiitis overlap syndrome coexisting between pancreatic PAN and renal MPA.

We encountered 12 elderly patients with lichen sclerosus (LS), a relatively high percentage of whom were living with their families. There is a tendency to assume that elderly people living alone or older facility users with paralysis are more likely to require social care, but we note that elderly people living with their families are also likely to develop LS.

The patient was a 62-year-old female with bladder carcinoma. TUR-BT was performed in March 2010, and the pathological finding was UC, G2 > G1, pTa. However, the cancer recurred in the urinary bladder 3 months after TUR-BT. Radical cystectomy was performed, and the disease was considered to have been cured, but metastasis developed in the skin, lung, liver, and bone several months after surgery. Chemotherapy was ineffective, and the patient died. On pathological examination at the time of radical cystectomy, the lesion was sarcomatoid carcinoma of the urinary bladder. Although the prognosis associated with this carcinoma is known to be poor, the possibility of underestimation on preoperative staging cannot be ruled out from the cause, in addition to the insufficiency of the current therapeutic strategy.

Consensus of primary care in acute pancreatitis in Japan

The incidence of acute pancreatitis in Japan is increasing and ranges from 187 to 347 cases per million populations. Case fatality was 0.2% for mild to moderate, and 9.0% for severe acute pancreatitis in Japan in 2003. Experts in pancreatitis in Japan made this document focusing on the practical aspects in the early management of patients with acute pancreatitis. The correct diagnosis of acute pancreatitis and severity stratification should be made in all patients using the criteria for the diagnosis of acute pancreatitis and the multifactor scoring system proposed by the Research Committee of Intractable Diseases of the Pancreas as early as possible. All patients diagnosed with acute pancreatitis should be managed in the hospital. Monitoring of blood pressure, pulse and respiratory rate, body temperature, hourly urinary volume, and blood oxygen saturation level is essential in the management of such patients. Early vigorous intravenous hydration is of foremost importance to stabilize circulatory dynamics. Adequate pain relief with opiates is also important. In severe acute pancreatitis, prophylactic intravenous administration of antibiotics at an early stage is recommended. Administration of protease inhibitors should be initiated as soon as thediagnosis of acute pancreatitis is confirmed. A combination of enteral feeding with parenteral nutrition from early stage is recommended if there are no clear signs and symptoms of ileus and gastrointestinal bleeding. Patients with severe acute pancreatitis should be transferred to ICU as early as possible to perform special measures such as continuous regional arterial infusion of protease inhibitors and antibiotics, and continuous hemodiafiltration. The Japanese Government covers medical care expense for severe acute pancreatitis as one of the projects of Research on Measures for Intractable Diseases.

A Literature Review on the State of High School Sex Education Implemented by School Nurses

Objective: In recent years, increases in sexually transmitted infections, cervical cancer caused by HPV, and abortions due to unwanted pregnancy among those in their 20s have become serious issues that threaten fertility. This study aims to identify issues in need of attention and the difficulties experienced by school nurses in teaching sex education in high schools preemptively before these students become sexually active in order to promote responsible sexual behaviors for the prevention of STIs. Method: ICHUSHI Web Ver. 5 and CiNii were queried for literature published between 2000 and May 2022. Search terms were “yōgokyōyu” [school nurses], “seikyōiku” [sex education], “seikansenshō” [sexually transmitted infections/diseases], and “kōkōsei” [high school students]. In addition, we limited the search to Japanese literature only, due to differences in cultural background and the roles of school nurses. Results & Observations: Sex education is taught by multiple faculty members and implemented into multiple subjects such as health & physical education and home economics. There are differences in the content taught by school nurses and other faculty. As specialists in health and hygienics, school nurses demonstrate high awareness and positive attitudes toward sex education. Additionally, while they play a central role in sex education, they face difficulties due to having no position in school health plans, differences in awareness among faculty, busy schedules, difficulty securing time, and failure to coordinate with other faculty. It is necessary to consider school-wide policies that systematize sex education as a continuous subject. Furthermore, while they have many opportunities to undertake consulting duties on sexuality, there is uncertainty among school nurses on their ability to meet student needs. Therefore, the development of teaching materials and opportunities for school nurses to improve their teaching skills is absolutely essential.

＜正＞Cs2TCNQ3 crystallizes into a columnar structure consisting of a periodic stack of TCNQ0/TCNQ/TCNCT, resulting in a semiconductor with the lowest optical energy gap of 0.3 eV [1]. The radicals TCNQ are dimerized, so that the uppermost valence band is formed by the π* orbit of spin singlet. Since the on-site repulsion energy U ≈ 1.2 eV of the π* electrons is higher than the optical energy gap, Cs2TCNQ3 may be regarded as a charge-transfer semiconductor of the 1/3-filled Hubbard system. It has been reported that this material undergoes the phase transition to a metallic state at around 3 GPa [2], although the optical energy gap remains unclosed [1]. In the present work, to obtain information about the mechanism of this semiconductor-to-metal transition, we measure the pressure dependence of the infrared molecular vibration. Figure 1 shows the absorption spectra in the C-CN stretching region at several pressures. The features at 1205 and 1211 cm-1 at the ambient pressure arise from neutral and radical molecules, respectively, whereas the one at 1183 cm-1 is the electron-molecular-vibration （e-mv） coupled mode. The former two features show a blue-shift with increasing pressure, while increasing their energy spacing. At around 3.6 GPa they abruptly displace toward each other about half the way of the energy spacing, showing that the π* electrons delocalize significantly from radical to neutral molecules. Evidently this is related to the high electric conductivity at high pressures. It is noteworthy that in the high-pressure phase radical-like and neutral-like molecules still coexist spectroscopically, and thus the π* electrons are not completely delocalized. The frequency, on the other hand, of the e-mv mode remains almost unchanged at low pressures. The e-mv mode disappears above 3.6 GPa, in coincidence with the disappearance of the electronic band S1, which is the counterpart of the e-mv mode [1].

A monolithic lens-window-prism（LWP） device, made of lithium fluoride（LiF） or magnesium fluoride（MgF2）,was proposed. When either of the devices was fixed onto one end of a gas cell filled with Xe, it becomes a"wedge-crystal"-like device and was used to convert a 1 MHz femtosecond 347 nm laser to its third harmonic radiation at 10.7 eV. This led to an improved beam profile and a more compact and less lossy configuration.A stable output power of ～11 μW was demonstrated for 2 h using LiF-LWP. In addition, MgF2-LWP was also verified for its practicability at 10.7 eV.
