AIM: To examine an association between the cytotoxic T-lymphocyte antigen 4 (CTLA4) gene that plays a role in downregulation of T-cell activation and inflammatory bowel disease consisting of ulcerative colitis (UC) and Crohn's disease (CD) in the ***: We studied 108 patients with UC, 79 patients with CD, and 200 sex-matched healthy controls, with respect to three single nucleotide polymorphisms (SNPs)in CTLA4, such as C-318T in the promoter region, A+49G in exon 1 and G+6230A in the 3' untranslated region (3'-UTR) by a PCR-restriction fragment length polymorphism method, and to an (AT)n repeat polymorphism in 3'-UTR by fragment analysis with fluorescence-labeling on denaturing sequence gels. Frequency of alleles and genotypes and their distribution were compared statistically between patients and controls and among subgroups of patients, using χ2 and Fisher exact ***: The frequency of 'A/A' genotype at the G+6230A SNP site was statistically lower in UC patients than in controls (3.7% vs 11.0%, P= 0.047, odds ratio (OR) = 0.311). Moreover, the frequency of'G/G' genotype at the A+49G SNP site was significantly higher in CD patients with fistula (48.6%) than those without it (26.2%)(P = 0.0388, OR=2.67).CONCLUSION: The results suggest that CTLA4 located at 2q33 is a determinant of UC and responsible for fistula formation in CD in the Japanese.

Chayote [Sechium edule (Jacq.) Sw.] is an economically important species in Latin America;however, there are very few reports available regarding its genetic diversity. Out of 11 microsatellite markers isolated, 10 loci provided 1 to 7 alleles per locus in a set of Mexican chayote accessions. Observed and expected heterozygosities for each locus ranged from 0.00 to 0.85 and 0.00 to 0.73, respectively. The overall genetic diversity detected by microsatellites was compared with that detected by P450-based analogue markers, a genome-wide dominant marker. Genetic diversity values obtained by the newly designed microsatellite markers were almost equal to the value estimated by PBA markers, but genetic distances calculated by both marker systems were not significantly correlated. Additional microsatellite markers, which could detect more polymorphisms, may be necessary to analyze the genetic diversity and structure of Mexican chayote collections.

A Phase II Study of Erlotinib in Patients with Previously Treated Non-Small Cell Lung Cancer

Background: Erlotinib has been reported to be effective for the treatment of non-small cell lung cancer (NSCLC). To evaluate the efficacy and safety of erlotinib under conditions similar to daily clinical practice, a phase II trial was conducted in Japanese patients with previously treated NSCLC. Methods: The eligibility criteria were stage IIIB/IV NSCLC, a performance status (PS) of 0 - 2, and previous treatment with 1 - 2 non-EGFR-TKI regimens. Patients received erlotinib (150 mg/day) orally until disease progression or intolerable toxicity occurred. The primary endpoint was the objective response rate (ORR). In addition, the disease control rate (DCR), progression-free survival (PFS), overall survival (OS), safety, and EGFR gene mutation status were evaluated. Results: Thirty-eight patients were enrolled, and 37 patients were evaluated. The median age was 69 years (range, 50 - 80 years). Patient characteristics were as follows: 26 were male and 11 were female;12 had a PS of 0, 20 had a PS of 1, and 5 had a PS of 2;and 26 had adenocarcinoma, and 11 had non-adenocarcinoma histology. The ORR and DCR were 21.6% (95% confidence interval [CI], 11.4% - 37.2%) and 54.1% (95% CI, 35.9% - 66.6%), respectively. Twenty-seven patients could be evaluated for EGFR gene status (12, mutated;15, wild-type). The ORR for EGFR-mutated patients was 41.7%, while that for patients with wild-type EGFR was 13.3%. The median PFS was evaluated as 4.4 months (95% CI, 2.2 - 10.7 months). The median OS was 14.9 months (95% CI, 9.2 months - not reached). Common adverse events were tolerable skin toxicities, diarrhea, and stomatitis. In addition, interstitial lung disease occurred in 8.1% of patients. Conclusion: As efficacy and safety were similar to previous studies, erlotinib was found to be effective for Japanese patients with previously treated NSCLC in clinical practice.

AIM: To investigate the frequency and distribution of N-acetyltransferase 2 (NAT2) and uridine 5′-diphosphate (UDP)-glucuronosyltransferase 1A7 (UGT1A7) genes in patients with ulcerative colitis (UC) and Crohn's disease (CD).METHODS: Frequencies and distributions of NAT2 and UGT1A7SNPs as well as their haplotypes were investigated in 95 patients with UC, 60 patients with CD, and 200gender-matched, unrelated, healthy, control volunteers by PCR-restriction fragment length polymorphism (RFLP),PCR-denaturing high-performance liquid chromatography (DHPLC), and direct DNA ***: Multiple logistic regression analysis revealed that the frequency of haplotype, NAT2*7B, significantly increased in CD patients, compared to that in controls (P= 0.0130, OR = 2.802, 95%CI = 1.243-6.316). However,there was no association between NAT2 haplotypes and UC, or between any UGT1A7haplotypes and inflammatory bowel disease (IBD).CONCLUSION: It is likely that the NAT2 gene is one of the determinants for CD in Japanese. Alternatively, a new CD determinant may exist in the 8p22 region, whereNAT2is located.

We present a female patient with continuous melena, diagnosed with rectal variceal bleeding. She had a history of esophageal varices, which were treated with endoscopic therapy. Five years after the treatment of esophageal varices, continuous melena occurred. Since colonoscopy showed that the melena was caused by giant rectal varices, we thought that they were not suitable to receive endoscopic treatment. We chose the modified percutaneous transhepatic obliteration with sclerosant, which is one of the interventional radiology techniques but a new clinical procedure for rectal varices. After the patient received this therapy, her condition of rectal varices was markedly improved.

人们不断要求汽车传动系具有最佳的操纵性和油耗 ,可满足这个要求的方法之一是 CVT(无级变速器 )。已经有报告表明 V型钢带推块 CVT改善燃油消耗大于 1 5% [1 ],已经生产了 70 0 ,0 0 0台以上的 CVT在实际应用中取得了很好的成果 ,近 1 0年来 CVT已经缓慢地走向世界。但到目前为止 ,市场上所有的这种汽车都是小型的 ,排量低于 1 30 0 cc。最近有报告表明大功率的 V型钢带已在 3.3L带篷货车和赛车上进行试验 ,但对于大功率发动机的 CVT仍未投放市场。所几年来 ,汽车已经对大功率 CVT提出需要 ,满足这种要求的牵引传动 CVT已有可能。在牵引传动中 ,动力是由两滚动体间存在的弹性流体润滑油膜 (EHL)的剪切力来传递的。牵引传动的性能在很大程度上取决于弹流接触液体的流变特性。 1 960年 [3]已经形成了该理论。大家知道 ,Hewko[4]在 1 960年制成的复曲面变速器是用于汽车的牵引传动 CVT的一个实例 ,Kraus[5]进行了车辆试验。 Lowenthal[6]和 Coy[7]完成了基础研究。由 Monsanto开发了牵引油 ,但 CVT仍未进入商业性的实际使用。其原因如下 :1 .因为在牵引接触区的高压和高温 ,目前牵引传动对滚零件的材料的可靠性不够。2 .牵引油不能满足汽车的所有工况。3.不具有能支承高速和大的轴向负荷的轴承。1
