Postoperative bleeding in patients on antithrombotic therapy after gastric endoscopic submucosal dissection

To investigated the relationship between postoperative bleeding following gastric endoscopic submucosal dissection (ESD) and individual antithrombotic agents. METHODSA total of 2488 gastric neoplasms in 2148 consecutive patients treated between May 2001 and June 2016 were studied. The antithrombotic agents were categorized into antiplatelet agents, anticoagulants, and other antithrombotic agents, and we included combination therapies [e.g., dual antiplatelet therapy (DAPT)]. The risk factors associated with post-ESD bleeding, namely, antithrombotic agents overall, individual antithrombotic agents, withdrawal or continuation of antithrombotic agents, and bleeding onset period (during the first six days or thereafter), were analyzed using univariate and multivariate analyses. RESULTSThe en bloc resection and complete curative resection rates were 99.2% and 91.9%, respectively. Postoperative bleeding occurred in 5.1% cases. Bleeding occurred in 10.3% of the patients administered antithrombotic agents. Being male (P = 0.007), specimen size (P < 0.001), and antithrombotic agent used (P < 0.001) were independent risk factors for postoperative bleeding. Heparin bridging therapy (HBT) (P = 0.002) and DAPT/multidrug combinations (P < 0.001) were independent risk factors associated with postoperative bleeding. The bleeding rate of the antithrombotic agent continuation group was significantly higher than that of the withdrawal group (P < 0.01). Bleeding within postoperative day (POD) 6 was significantly higher in warfarin (P = 0.015), and bleeding after POD 7 was significantly higher in DAPT/multidrug combinations (P = 0.007). No thromboembolic events were reported. CONCLUSIONWe must closely monitor patients administered HBT and DAPT/multidrug combinations after gastric ESD, particularly those administered multidrug combinations after discharge.

A chain event of the 2016 Kumamoto earthquakes caused considerable geotechnical damage related to liquefaction in many places around Kumamoto plain. Many low-rise houses and traditional Japanese style houses, which were constructed on shallow foundation, suffered differential settlement and tilting due to liquefaction. To mitigate the building damages due to the liquefaction, a new countermeasure method of jet grout grid form with a horizontal slab is introduced in this study. The effectiveness of the proposed technique was evaluated through physical modelling and numerical modelling. As a part of the physical modelling, a set of 1 g shaking table tests for unimproved case and improved case were performed, in which the mitigation effects of the grid form with a horizontal reinforcing slab were examined based on the acceleration, excess pore water pressure ratio as well as ground settlement. The addition of surface acoustic wave(SAW)technologies to microfluidics has greatly advanced lab-on-a-chip applications due to their unique and powerful attributes,including high-precision manipulation,versatility,integrability,biocompatibility,contactless nature,and rapid ***,the development of SAW microfluidic devices is limited by complex and time-consuming micro/nanofabrication techniques and access to cleanroom facilities for multistep photolithography and vacuum-based *** simplify the fabrication of SAW microfluidic devices with customizable dimensions and functions,we utilized the additive manufacturing technique of aerosol jet *** successfully fabricated customized SAW microfluidic devices of varying materials,including silver nanowires,graphene,and poly(3,4-ethylenedioxythiophene)polystyrene sulfonate(PEDOT:PSS).To characterize and compare the acoustic actuation performance of these aerosol jet printed SAW microfluidic devices with their cleanroom-fabricated counterparts,the wave displacements and resonant frequencies of the different fabricated devices were directly measured through scanning laser Doppler ***,to exhibit the capability of the aerosol jet printed devices for lab-on-a-chip applications,we successfully conducted acoustic streaming and particle concentration ***,we demonstrated a novel solution-based,direct-write,single-step,cleanroom-free additive manufacturing technique to rapidly develop SAW microfluidic devices that shows viability for applications in the fields of biology,chemistry,engineering,and medicine.

Case of a tumor comprising gastric cancer and duodenal neuroendocrine tumor

The present report describes a rare case of a tumor composed of early gastric cancer and a duodenal neuroendocrine tumor(NET). A 78-year-old woman underwent esophagogastroduodenoscopy at a local institution for screening of the upper gastrointestinal tract which revealed a protruded tumor through the pyloric ring from the pyloric antrum. The tumor was too large to treat at the facility; consequently, she was referred to our hospital for further management. Esophagogastroduodenoscopy with tumor biopsy of the lesion revealed the diagnosis of early gastric cancer. Endoscopic submucosal dissection was performed with sufficient free margins in both vertical and horizontal directions. Histopathological findings showed NET confined to the submucosal layer and covered by welldifferentiated adenocarcinoma. Immunohistochemical stainings showed that the two lesions existed continuously. While the possibility of a collision cancer was considered, it was suggested that the two lesions existed continuously. Finally, the tumor was diagnosed as gastric cancer composed of duodenal NET G1, with a lymphatic invasion of NET component.

Fractal image analysis and bruise damage evaluation of impact damage in guava

Impact bruise damage and quality of‘Gim Ju’guava were investigated for different drop heights and number of drops using fractal image *** the impact test,a stainless-steel metal ball(250 g)was dropped on fruit from three drop heights(0,0.3,0.6 m)either once or five *** quality was evaluated for impact energy,bruise area(BA),bruise volume(BV),bruise susceptibility,bruise score and pulp color(L*,a*,b*and C values).The fractal dimension(FD)value using fractal image analysis was analyzed at the bruise *** showed that five drops(0.3 m)with a high impact energy(3678.75 J)and a single drop(0.6 m)with a low impact energy(1471.50 J)exhibited no significant in BA,BV,bruise score as well as all color values(L*,a*,b*and C).While the FD value of a single drop from 0.6 m had a higher FD value than that of five drops from 0.3 *** is indicated that FD exhibited a better performance to classify impact bruising level of guava than BA,BV and color *** FD value gradually decreased with increase of storage time and bruise *** correlation coefficient(r)values of FD(r=−0.794 and−0.745)between BA and BV were more significant than those L*(r=−0.660 and−0.615)and a*(r=0.579 and 0.473).The coefficient of determination(R^(2))of the polynomial equation in bruised fruit(R^(2)=0.85 to 0.99)was greater than the control(no bruise)(R^(2)=0.80).A higher R^(2)val(0.88 and 0.92)was exhibited at five ***,FD analysis showed greater potential than color measurement to assess bruise impact damage in guava.

A unified multi-view multi-person tracking framework

Despite significant developments in 3D multi-view multi-person (3D MM) tracking, current frameworks separately target footprint tracking, or pose tracking. Frameworks designed for the former cannot be used for the latter, because they directly obtain 3D positions on the ground plane via a homography projection, which is inapplicable to 3D poses above the ground. In contrast, frameworks designed for pose tracking generally isolate multi-view and multi-frame associations and may not be sufficiently robust for footprint tracking, which utilizes fewer key points than pose tracking, weakening multi-view association cues in a single frame. This study presents a unified multi-view multi-person tracking framework to bridge the gap between footprint tracking and pose tracking. Without additional modifications, the framework can adopt monocular 2D bounding boxes and 2D poses as its input to produce robust 3D trajectories for multiple persons. Importantly, multi-frame and multi-view information are jointly employed to improve association and triangulation. Our framework is shown to provide state-of-the-art performance on the Campus and Shelf datasets for 3D pose tracking, with comparable results on the WILDTRACK and MMPTRACK datasets for 3D footprint tracking.

Cr-coated zirconium alloy was prepared by pulsed laser deposition(PLD)for the application of accident-tolerant fuel cladding in light water *** microstructural characteristics of the Cr coating and its evolution with temperature were investigated using grazing incidence X-ray diff raction and in situ heating transmission electron microscopy(TEM).Results show that the microstructure of the laser-deposited Cr coatings consists mainly of fine and non-specific shaped nano-crystals in the inner layer and columnar crystals in the outer *** recrystallization of the Cr-coating layer starts at 300–400℃ to release the high strain introduced by PLD,and the grain coalescence starts at temperatures>400°*** annealing,the(110)-texture gradually intensifi es because of its high reticular density and low close-packed ***,in situ heating TEM observation shows the presence of cavities on the Cr–Zr interface,which may result from the interdiff usion and/or the transformation from amorphous to crystalline.
