Frozen shoulder-A prospective randomized clinical trial

AIM To compare the results of arthroscopic capsular release with intra-articular steroid injections in patients of frozen *** Fifty-six patients with frozen shoulder were randomised to one of two treatment groups: Group 1, complete 360 degree arthroscopic capsular release and group 2, intra-articular corticosteroid injection(40 mg methyl prednisolone acetate). Both groups were put on active and passive range of motion exercises following the intervention. The outcome parameters were visual analogue scale(VAS) score for pain, range of motion and Constant score which were measured at baseline, 4, 8, 12, 16 and 20 wk after *** All the parameters improved in both the groups. The mean VAS score improved significantly more in the group 1 as compared to group 2 at 8 wk. This greater improvement was maintained at 20 wk with P value of 0.007 at 8 wk, 0.006 at 12 wk, 0.006 at 16 wk and 0.019 at 20 wk. The Constant score showed a more significant improvement in group 1 compared to group 2 at 4 wk, which was again maintained at 20 wk with P value of 0.01 at 4, 8, 12 and 16 wk. The gain in abduction movement was statistically significantly more in arthroscopy group with P value of 0.001 at 4, 8, 12, 16 wk and 0.005 at 20 wk. The gain in external rotation was statistically significantly more in arthroscopy group with P value of 0.007 at 4 wk, 0.001 at 8, 12, and 16 wk and 0.003 at 20 wk. There was no statistically significant difference in extension and internal rotation between the two groups at any time. CONCLUSION Arthroscopic capsular release provides subjective and objective improvement earlier than intra-articular steroid injection.

Delayed migration of K-wire into popliteal fossa used for tension band wiring of patellar fracture

Breakage of K-wires and stainless steel wires which are used for fracture fixation is not uncommon, but migration is rare. We report a case of migration of bro- ken K-wire used for patella tension band wiring to the popliteal fossa. The broken hardware was removed surgically. We would like to suggest that K-wire and wire fixation used for treatment ofpatellar fractures can migrate into the posterior compartment of the knee and cause clini-cal symptoms. Close clinical and radiological follow-up af- ter internal fixation to identify the presence of hardware breakage or movement and removal of wires once fracture has united can avert such complications.

Dear editor Treatment emergent neuroendocrine prostate cancer(tNEPC)is most commonly observed after development of resistance to androgen deprivation therapy(ADT)and is associated with rapid progression and widespread metastases with survival less than 1 year from diagnosis[1].Management of this disease is mainly through cytotoxic chemotherapy and there is no published evidence of treating the primary prostatic lesion in this stage of the disease,unlike that of localized prostate cancer or castrate sensitive metastatic prostate cancer[2].In this letter,we report the only known case of t-NEPC who presented with an initial diagnosis of bladder urothelial carcinoma and was treated by early radical surgery and chemotherapy,which led to longterm disease control and preservation of quality of life.

Competition of Electron Capture and Beta Decay Rates in Explosive Astrophysical Scenario of Type II Supernova

Stellar weak interaction processes play a significant role during the supernova explosion condition after collapse leading to the formation of neutron star. In dynamic events like core-collapse supernovae the high entropy wind scenario arises from considerations of the newly born proto-neutron star. Here, the late neutrinos interact with matter of the outermost neutron star layers leading to moderately neutron rich ejecta. We study the electron capture and beta decay rates of Co and Cd isotopes at various temperature and density conditions in an astrophysical environment and found that the beta decay rates are much higher than the corresponding electron capture rates at all the conditions.

Brodie＇s abscess of medial distal femoral condyle after thorn prick： rare clinical presentation

Thorn prick injuries are generally conceded frivolous and rarely demand medical attention. Howbeit deep seated injuries are well described in the literature. We presented a case of thorn prick injury to the knee that manifested as Brodie＇s abscess of the medial distal femoral condyle and synovitis. Magnetic resonance imaging （MILl） and ultrasonography could only construe the affliction but not spot the thorn. Arthrotomy was undertaken for explora- tion and debridement. Empirical therapy with initial systemicCefotaxime and subsequent Ofloxacin worked well after the surgery. Deep seated injuries by a thorn prick may take a protracted and torpid course. Thorough curettage of the bony lesion and debridement are vital for proper management.

Energetic Gamma Radiation from Blazar and Its Effect on Intergalactic Medium

Blazars are subclass of AGN （active galactic nuclei） with relativistic jet that is pointing in the general direction of the earth. The energy flux in the jet is about （10^44-10^47） erg/s. The γ-rays in the energy range 10 GeV-10 TeV emanating from blazar are examined and their effect on the IGM （intergalactic medium） is studied. The γ-ray emission is considered as due to inverse Compton scattering of relativistic electrons. There appears to be a gamma ray flux from blazar above 10 GeV having a line intensity of 1.5 × 10^-4 cm^-2 s^-1 sr^-1. The IGM is measured to large distances and it is found to be clumpy. It is found that at higher redshifl the density of IGM increases. The cosmological density parameter measured is found to lie within the suggested limit of 0.03 at redshift z = 3 for background intensity of radiation 10^-24 W·m^-2·Hz^-1·sr^-1. The iso-ΩIGM curve shows a kink in its profile which might be significant at the particular value of z equal to 2. Some more detailed studies are required for understanding the distribution of intergalactic medium at the unstable point at z = 2 as shown in inverse Compton radiation.

Efficacy and safety outcomes with remdesivir in COVID-19 patients:A meta-analysis

BACKGROUND Remdesivir is a broad-spectrum antiviral drug having in vitro activity against severe acute respiratory syndrome coronavirus 2 and is currently being used on a compassionate basis outside of clinical *** To analyze the efficacy and safety of remdesivir compared with other interventions in coronavirus disease 2019(COVID-19)*** We searched online databases to include randomized controlled trials evaluating the efficacy and safety of remdesivir compared with other interventions in COVID-19 *** summarized efficacy and safety data as risk ratios(RRs)with 95%confidence interval(CI)and used Mantel-Haenszel fixed or randomeffect *** estimated the number needed to treat(NNT)to cause one additional *** used the GRADE approach to assess the quality of the evidence for all outcome *** We included four randomized controlled *** observed no significant difference in mortality(RR:0.83;95%CI:0.57–1.20;I2=59%)and rate of ventilation(RR:0.69;95%CI:0.41-1.18;I2=77%)between remdesivir-and placebo-treated *** showed higher rates of clinical recovery than placebo(RR:1.10;95%CI:1.04–1.16;I2=0%;NNT:14.3).We observed no difference in overall adverse events between remdesivir-and placebo-treated patients(RR:1.05;95%CI:0.86–1.27;I2=77%).We observed less risk of serious adverse events(RR:0.75;95%CI:0.63–0.89;I2=0%)in remdesivir-than placebo-treated *** GRADE approach suggested moderate quality of evidence for all efficacy and safety *** We observed limited clinical benefit of remdesivir over placebo in the treatment of *** findings could be biased because of the small number of trials.

Generation of nanomaterials by reactive laser-synthesis in liquid

Nanomaterials with tailored structures and surface chemistry are in high demand, as these materials play increasingly important roles in biology, catalysis, energy storage, and manufacturing. Their heightened demand has attracted attention towards the development of synthesis routes, particularly, laser-synthesis techniques. These efforts drove the refinement of laser ablation in liquid(lal) and related methods over the past two decades and have led to the emergence of reactive laser-synthesis techniques that exploit these methods’ characteristic, non-equilibrium conditions. Reactive laser-synthesis approaches foster unique chemical reactions that enable the formation of composite products like multimetallic nanoparticles, supported nanostructures, and complex minerals. This review will examine emerging reactive laser-synthesis methods in the context of established methods like *** focus will be on the chemical reactions initiated within the laser plasma, with the goal of understanding how these reactions lead to the formation of unique nanomaterials. We will provide the first systematic review of laser reaction in liquid(LRL) in the literature, and bring a focus to the chemical reaction mechanisms in lal and reactive-lal techniques that have not yet been emphasized in reviews. Discussion of the current challenges and future investigative opportunities into reactive laser-synthesis will impart guidance for researchers interested in designing reactive laser-synthesis approaches to novel nanomaterial production.

The Zika virus(ZIKV)and dengue virus(DENV)flaviviruses exhibit similar replicative processes but have distinct clinical outcomes.A systematic understanding of virus–host protein–protein interaction networks can reveal cellular pathways critical to viral replication and disease *** we employed three independent systems biology approaches toward this ***,protein array analysis of direct interactions between individual ZIKV/DENV viral proteins and20,240 human proteins revealed multiple conserved cellular pathways and protein complexes,including proteasome ***,an RNAi screen of 10,415 druggable genes identified the host proteins required for ZIKV infection and uncovered that proteasome proteins were crucial in this ***,high-throughput screening of 6016 bioactive compounds for ZIKV inhibition yielded 134 effective compounds,including six proteasome inhibitors that suppress both ZIKV and DENV *** analyses of these orthogonal datasets pinpoint proteasomes as critical host machinery for ZIKV/DENV *** study provides multi-omics datasets for further studies of flavivirus–host interactions,disease pathogenesis,and new drug targets.

Modified vaccinia virus Ankara(MVA) is an approved smallpox vaccine and a promising vaccine vector for other pathogens as well as for cancer therapeutics with more than 200 current or completed clinical *** was derived by passaging the parental Ankara vac-cine virus hundreds of times in chick embryo fibroblasts during which it lost the ability to rep-licate in human and most other mammalian *** this replication deficiency is an important safety feature,the genetic basis of the host restriction is not ***,an unbiased human genome-wide RNAi screen in human A549 cells revealed that the zinc-finger antiviral protein(ZAP),previously shown to inhibit certain RNA viruses,is a host restriction factor for MVA,a DNA *** studies demonstrated enhanced MVA replication in several human cell lines following knockdown of ***,CRISPR-Cas9 knockout of ZAP in human A549 cells increased MVA replication and spread by more than one log but had no effect on a non-attenuated strain of vaccinia *** intact viral C16 protein,which had been disrupted in MVA,antagonized ZAP by binding and sequester-ing the protein in cytoplasmic punctate *** aimed at exploring the mecha-nism by which ZAP restricts MVA replication in the absence of C16 showed that knockout of ZAP had no discernible effect on viral DNA or individual mRNA or protein species as deter-mined by droplet digital polymerase chain reaction,deep RNA sequencing and mass spec-trometry,***,inactivation of ZAP reduced the number of aberrant,dense,spherical particles that typically form in MVA-infected human cells,suggesting that ZAP has a novel role in interfering with a late step in the assembly of infectious MVA virions in the absence of the C16 protein.
