Nephritis Syndrome Tubulo-Intertitial and Uveitis (Tinu Syndrome): About 2 Cases

Introduction: Tubular and interstitial nephropathy syndrome with uveitis or TINU syndrome is a rare association. Patients and methods: We have listed cases in which the clinical-biological picture and histology were consistent with this syndrome. Results: Two cases were retained: These were two female patients whose mean age at diagnosis was 16 years. The inaugural symptoms were bilateral anterior uveitis in two cases. The time to onset of renal signs: was 4 to 2 weeks months. Acute kidney failure was reported in both cases with a mean creatinine of 36.4 mg/l. An average proteinuria of 1.1 g/24 h associated with aseptic leukocyturia in one case/2 and glycosuria normoglycemic in 1 case/2. Non inflammatory syndrome specific with inflammatory anemia is reported in both cases. Kidney puncture biopsy confirmed the diagnosis with nephropathy diffuse polymorphic tubulointerstitial and immunofluorescence negative in both cases. Oral corticosteroid therapy (1 mg/kg per day) was started for 4 to 6 months. Both patients received initial local corticosteroid therapy for uveitis. The outcome was favorable under treatment with remission complete. Kidney function recovered after 6 months with an average serum creatinine of 6 mg/l. Discussion: The incidence of TINU syndrome appears to be underestimated in the literature. It is believed to be an autoimmune disease. The positive diagnosis is based on clinical, biological and histological. An etiological investigation in search of disease systemic is necessary before making the diagnosis. There is no codified treatment. The prognosis is favorable in the majority of cases. Conclusion: Investigation of renal function is necessary for any patient.

近年来,互联网在全球范围内对人们的交流方式产生了巨大影响,新型的合作方式也随之出现。随着开源软件(OSS)的蓬勃发展,身处世界各地的众多开发者都通过互联网参与到了开源软件项目的协作开发中,这些项目同时也成为开发者们分享、存档和交换信息的地方。GitHub作为当前最具代表性的社交编程及代码托管网站,将众多开源软件项目及其开发者都聚集到了一个虚拟的协作平台当中。在这个平台中,不仅开发者之间由于协同工作而存在社交关系,久而久之,各种项目由于共享同样的开发者或者存在互相调用等协作关系也会形成某种形式的社交网络。研究GitHub平台上软件仓库(项目)之间存在的社交关系可能帮助开发者提升协作开发的效率,这是本研究的意义所在。但是,如何从这些软件仓库中提取出有用的信息以组织项目之间的协作,是一个极富挑战性的问题,本文针对这个挑战研究了GitHub上的项目之间可能存在的社交关系。本文提出了3种模型,用于刻画GitHub中项目之间可能存在的3类社交关系(基于开发者的、基于复刻的和基于源代码的社交关系),并分别提出了抽取对应社交关系的3种算法。为了支持本文的研究工作,我们使用包含108718个项目和499485个用户数据的MSR2014数据集构建了基于开发者的和基于复刻的两种项目之间的社交网络。此外,我们基于Apache许可协议用自行开发的程序下载了GitHub上的421个开源项目的代码,解析源代码构建这些项目之间基于源代码的社交网络。之后,本文在两个层面对已构建的3种项目之间的社交网络(PSN)进行了实证分析:(1)从复杂网络的常用度量指标(如规模密度、度分布、直径等)层面分析PSN的性质;(2)从复杂网络结构的层面对3种PSN进行了对比分析。本论文的研究工作可以帮助深入理解OSS项目之间是如何协同配合的,并帮助理解OSS项目之间的相关性。同时也为如何建议开发者组织开源软件的协作工作和探究软件功能演变的因素打下了一定的基础。此外本文的研究工作对形式化开源软件项目的组织协作也有着基奠意义。

Orthogonal frequency division multiplexing （OFDM） is a special form of multi-carrier transmission that uses the policy of divide and rule. In this scheme, a large number of orthogonal, overlapping, narrow band sub-channels （subcarriers） are transmitted in parallel and divide the available transmission bandwidth. This techniqueis originally based on the Fast Fourier Transform of the information data. In order to improve the performance of the OFDM and overcome some limitations, an alternative OFDM approach based on the Wavelet Transform is proposed. In this paper, we study the performance of such systems in additive white Gaussian channel （AWGN）. MATLAB simulations are realized and performance comparisons are presented.

Introduction: Malignant gliomas refer to grade III or IV brain tumors defined according to the World Health Organization (WHO) classification. They are a heterogeneous group of pathologies and represent a serious health problem by their frequency, severity and treatment difficulties. The prognosis of malignant gliomas remains poor despite all the medical advances. Materials and Methods: It is a retrospective study included 20 cases of malignant glial tumors treated at the medical oncology department, Fattouma Bourguiba hospital in Monastir between 2012 and 2016, according to the STUPP protocol. Results: These were 12 men and 8 women with a median age of 43. Clinical signs were not very specific, dominated by intracranial hypertension and deficit signs. Imagery referred to the diagnosis of malignant gliomas in 1st intention. Surgery consisted of a macroscopically complete exeresis in (15%) cases, a partial exeresis in (50%), the rest of the patients had a stereotactic biopsy. Histology found GBM in 16 patients (80%), 2 cases of Grade III anaplastic astrocytoma (10%), 1 case of anaplastic oligodendroglioma (5%), and 1 case of Grade III anaplastic eppendymoma (5%). Use of X-ray contrast allows us to differentiate between two or more adjacent structures on radiographic studies. The X-ray contrast agent can be the one with increase X-ray absorption, like iodine and a barium X-ray contrast agent or the one with decrease X-ray absorption like air and carbon dioxide contrast agent. Each contrast agent possesses different risks and benefits in various ways. Carbon dioxide as an intravascular contrast agent can be used as an alternative intravascular contrast agent and has superior results in some cases. In patients with renal dysfunction or iodinated contrast allergy, the use of Iodinated Contrast Agent poses the risk of considerable morbidity. Similarly, use of Gadolinium is discouraged in subject with severe renal dysfunction. Use of carbon dioxide(CO_2) as an intravascular contrast, offers an alternative in such patients for certain procedures, as it is not nephrotoxic and it does not incite allergic reactions. It is inexpensive, readily available and due to its unique physical properties, it can be used to image a wide variety of vascular beds and chambers. The aim of this paper is to systemically review the current literature to describe the indications, contraindications, adverse effects, instruments, precautions, latest methodologies and data supporting for the use of CO_2 as a contrast agent.

Correlation between Radiological Staging of Chronic Rhinosinusitis and Revision of Endoscopic Sinus Surgery

Introduction: Chronic rhinosinusitis (CRS) is an inflammatory condition affecting the sinonasal mucosa with duration of more than 12 weeks. Multifactorial aetiology contributes to its refractoriness. Functional endoscopic sinus surgery (FESS) is the most common surgical technique preferred in CRS not responding to medical management, showing high success rates. However, some failure has been noted, requiring revision FESS. CT (Computed Tomography) is a diagnostic tool for CRS, and aids in improving surgical outcomes. Combined with the Lund-Mackay scoring system, CT can help stage the CRS, predicting the need for revision surgery. Methods: In our paper, we aimed to study the relationship between severity of CRS using radiological staging and revision of functional endoscopic sinus surgery, assessing outcome after at least 1 year of follow-up. There was a single centre retrospective study involving 150 patients selected by random sampling, to study the correlation between the severity of chronic rhinosinusitis and revision of functional endoscopic sinus surgery (FESS). The study was conducted in the ENT department of King Hamad University Hospital, Bahrain for a time period of 6 months. Records were scanned and scored using Lund-Mackay system. Lund Mackay score of 10 and above was considered severe. The relation between increased severity with Lund Mackay score on the CT scan and the need for revision sinus surgeries was then determined. Results and Conclusion: This study concludes that LMS score in CT scans can be used to predict the outcomes of primary FESS procedure and the need for revision surgery in the management of CRS.

Assessment of the Characteristics of the Municipal Solid Waste Compost in Lebanon

Waste management is crucial due to the fast increase of human population, causing an increase in solid waste generation which if not properly managed causes environmental problems. Around 57% of the wastes generated from homes are made up of green material (fruits, vegetables…). Thus, reusing and recycling green wastes through composting is one way of reducing the waste load to landfills. Composting is the transformation of raw organic materials into organic soil amendments that provide nutrients to crops and enhance the tilth, fertility, and productivity of soils. Aerobic windrow composting system at Sukomi Greensite facility located at Karantina is performed, where materials biodegrade under controlled conditions to produce compost. However, assessment of the quality of the compost is fundamental in order to determine its usages. Thus, regular testing of physical, chemical and biological parameters was performed for adequate monitoring purposes. The basic objective of this study was to determine the characteristics of the Lebanese municipal solid waste compost on a yearly basis and compare these characteristics amongst the years. Hence, each parameter was tested and compared to the BNQ international Canadian standards for proper classification of the compost and adequate identification of its usages. The preliminary data obtained were statistically diagnosed through principal component analysis by Spadv55 software. All the data reflected the normal content value of the studied parameters with minor differences between the years except for year 2007 which demonstrated higher levels of Potassium, Phosphate, Lead and Cadmium. The characteristics of the compost enabled it to be used as a soil amendment on all types of agricultural and landscape commodities at the adequate dosages and proper timing. Life is being developed every day in all of the life aspects. One of the major developing aspects is information technology (IT), and communication technology which makes life easier, faster, and more connected. ICT is evolving fast in Jordan and offering the government to deliver multiple delivery services with different characteristics among E-government services. The Jordanian government has invested heavily in E-government initiatives for the last 10 years to transform from traditional service delivery to more effective and efficient service to deliver high-quality customer-centric and performance-driven services to E-government stakeholders. However the global rank of E-government readiness as well as regional rank of Jordan is still in low rank according to the global countries but it is still quite according to the Arab countries. This research provides a trend analysis to find the trends (positive or negative) in the UN E-government indicators in Jordan and provides an overview to the E-government in Jordan where the researchers analyze the development of E-government in Jordan by introducing a general framework for the E-government through discussing the past, present status and the future plans for E-government in Jordan to get better service to their recipients and to improve overall progress of Jordan achievements compared with regional and global countries.
