Background:skin and soft tissue infections are *** and Aspergillosis are the most common cutaneous fungal *** alum(Aluminium Potassium sulphate)is a natural mineral with antifungal and antibacterial ***:This study aimed to confirm Yemeni alum’s antifungal effectiveness against selected fungal strains and develop useful topical ***:Using different alum concentrations,we prepared twenty-three formulations,including four non-adjusted aqueous solutions,eight adjusted pH aqueous solutions,eight glycerite solutions,and three Oil/Water cream ***,we examined the antifungal activity against the suggested *** animal skin irritation test evaluated the local skin sensitivity reactions that might happen upon using the formulations we pre-pared on the intact animal’s ***,this study also included six weeks of stability testing to estimate the expected shelf life(t90)of the selected ***:Alum exerted antifungal effect against Candida albicans,Aspergillus niger,and Aspergillus *** animal’s skin was not irritated by any of the formulations *** 5%alum aqueous solution and the 10%alum Oil/Water cream exhibited promising results among all tested *** approximate formulation’s shelf-lives(t90)were 2 and 1.52 years with alum content(%)ranging between 105.75%to 106.02%,***:Based on the results we found,10%alum Oil/Water cream is a promising formulation for large-scale manufacture.

The wall shear stress （Wss） that a moving fluid exerts on a surface affects many processes including those relating to vascular function. Wss plays an important role in normal physiology （e.g. angiogenesis） and affects the microvasculature＇s primary function of molecular transport. Points of fluctuating Wss show abnormalities in a number of diseases; however, there is no established technique for measuring Wss directly in physiological systems. All current methods rely on estimates obtained from measured velocity gradients in bulk flow data. In this work, we report a nanosensor that can directly measure Wss in microfluidic chambers with sub-micron spatial resolution by using a specific type of virus, the bacteriophage M13, which has been fluorescently labeled and anchored to a surface. It is demonstrated that the nanosensor can be calibrated and adapted for biological tissue, revealing Wss in micro-domains of cells that cannot be calculated accurately from bulk flow measurements. This method lends itself to a platform applicable to many applications in biology and microfluidics.

Thoracoscopic internal mammary lymph nodes dissection: a staging tool for internal mammary lymph nodes in breast cancer

Objective： The aim of our study was to investigate the feasibility and safety of thoracoscopic internal mammary lymphadenectomy as a method to refine and thereby improve nodal staging in breast cancer. Methods： During the period from June 2004 to May 2007, 50 patients with operable breast cancer underwent modified radical mastectomy （MRM） or breast conserving surgery （BCs）, followed by thoracoscopic internal mammary lymphadenectomy, using 3 ports through the skin incision of the MRM or the BCs. Metal clips were used to mark precise site of lymphadenectomy. Results： of total number of 50 patients, the mean age of patients was 44 years （range, 27-60 years）. 40 （80%） had medio-central tumor, 10 （20%） had lateral tumor. 35 （70%） had clinically involved axillary nodes. 16 out of 50 patients received neo-adjuvant CTH. 44 patients underwent MRM and 6 patients underwent BCs. No intra-operative complications occurred. Atelectasis was the only postoperative complication that was encountered, which occurred in 12 cases, and was treated conservatively. The average chest drainage period was 1.2 day （range, 1-2 days）. The total number of IMN metastasis was 18 patients （36%）. The risk of IMN metastasis was higher; in younger patients （P = 0.03）, in medio-central tumors （P = 0.03）, in bigger tumors （P = 0.05）, with heavier metastasis of axillary LNs （P = 0.001）. But a correlation with the histological pattern of the lry tumor didn＇t exist （P = 1）. Knowing the IMN status helped in proper staging of patients, 7 patients showed evident stage migration after adding the IMN analysis to that of primary tumor and axillary LN. During the follow up period （the median, 22 months; range, 7 to 42 months）, no patient had pleural dissemination or port-site metastasis. Conclusion： Thoracoscopic IMN lymphadenectomy is a safe procedure, which can be done serious additional complications or cosmetic compromise. And allow proper nodal staging, which allow proper treatment planning.

Hospital admissions for anaphylaxis in Istanbul, Turkey

Background There are very limited data characterizing the epidemiology of anaphylaxis from low- and middle-income country settings. We aimed to estimate the frequency of anaphylaxis admissions to hospitals in *** We obtained data from all 45 hospitals in Istanbul over a 12-month period and used ICD-10 codes to extract data on those admitted with a recorded primary diagnosis of anaphylaxis. Because of concerns about possible under-coding, we undertook an additional analysis to identify patients admitted with two or more clinical codes for symptoms and/or signs suggestive of, but not coded as having, *** A total of 114 cases (79 people with anaphylaxis codes and 35 with symptoms and signs suggestive of anaphylaxis) were identified, giving an overall estimate of 1.95 cases per 100 000 *** The novel two-stage identification approach employed suggests significant under-recording of anaphylaxis in those admitted to hospitals in Istanbul.

Background: Combined spinal-epidural block (CsEB) has aroused increasing interest, as it combines the reliability of a spinal block and the flexibility of an epidural block (EB). We have conducted a comparative investigation of the maternal and fetal effects of CsEB and of EB administered for Cesarean section. Methods: Eighty pregnant women at term were randomized into two groups. Women in the CsEB group (N = 40) were each given 1.5- 1.8 mL 0.5% hyperbaric bupivacaine intrathecally, followed by 10 mL 0.25% bupivacaine and 50 μ g fentanyl through the epidural catheter 10 min later. Women in the EB group (N = 40) received 14- 16 mL 0.5% bupivacaine and 100 μ g fentanyl. The quality and side effects of surgical anesthesia and the hemodynamic parameters, Apgar scores, and postoperative duration of pain were compared between the two groups. Results: The time for the block to reach the T- 4 level differed significantly between the two groups (8.02± 3.4 versus 18.34± 4.6; P sEB group achieved complete motor blockade (Bromage score 3), and it was reached earlier than in the EB group (P scle relaxation and motor block were better in the CsEB group than in the EB group (P scores were 7 or more in almost all newborns in both groups. There were no significant differences between the groups in the incidences of adverse effects such as hypotension or nausea and vomiting, but the patients in the EB group experienced more shivering (P stoperative pain was significantly shorter in the CsEB group. Conclusion: We decided that CsEB, and more specifically spinal anesthesia with supporting epidural anesthesia, has greater efficacy and fewer side effects than EB when administered for Cesarean section.

特发性肾病综合征是最常见的儿童肾小球疾病,其特征是大量蛋白尿、低蛋白血症和(或)伴有水肿,全球每年儿童患病率为(1.15~16.90)每10万例。约有85%~90%的患者在接受糖皮质激素治疗的4~6周内可达到蛋白尿完全缓解,表现为激素敏感型肾病综合征(ssNs)。约70%~80%的ssNs患者在随访期间至少复发1次,其中多达50%的患者出现频繁复发或依赖糖皮质激素以维持蛋白尿缓解。为延长复发间隔时间,激素治疗的用药剂量和用药时长始终是热点问题之一,且患者出现激素相关并发症的概率较高。临床已应用多种激素助减药物(免疫抑制药物),但在疾病治疗过程中这些药物的选择和用药时机存在明显差异。因此,需要国际循证临床实践建议(CPRs)来指导临床实践、减少实践差异。国际儿科肾脏病学会(IPNA)召集专家小组通过对12个临床相关PICO(研究对象、干预措施、对照、结果/结局)问题进行系统的文献回顾,制定了关于儿童ssNs诊断和治疗的建议和分级。对治疗转归提出了新的定义,有助于指导治疗改变,并为重要的研究问题提供了参考。
