Involvement of the Genetic Diversity of HIV-1 in the Virological Treatment Failure of First Line Antiretroviral in Kinshasa

Background: Genetic diversity of human immunodeficiency virus affects the treatment and the emergence of resistance. Some subtypes would develop resistance more frequently than others. The aim of this study is to determine the rate of virological treatment failure and the involvement of genetic diversity and different mutations in this failure in Kinshasa. Methods: Of the 153 Antiretroviral-naive patients who were included in the cohort, 138 patients have been received for the appointment of the 6th month. Clinical parameters were recorded on individual patient charts. The determination of Viral Load (VL) was done at the Laboratory of Molecular Biology. Clinical and biological parameters of the 6th month were compared with those taken at baseline of the cohort to determine the evolution of patients under treatment. Results: At the consultation of the 6th month, 138 patients (90.2%) had returned out of the 153 included. Eighty-one (58.7%) patients were women and 57 (41.3%) men. The age of patients is between 18 and 65 with an average of 37 years. Ten deaths (6.5%) and 5 (3.3%) lost have been reported. One hundred twenty-five patients (90.5%) were in clinical stage 3 and 13 (9.5%) in clinical stage 4. The median CD4 T cells is 560 cells mm3. The median VLs of patients was 0.90 log10 RNA copies/ml. Of the 34 patients in virological failure, 8 (23.5%) are minimal failure, 23 (67.7%) in moderate failure and 3 (8.8%) in severe failure. According to the Pearson’s test, VLs at 6th months were highly correlated with that of inclusion, with V75 and K70 mutations for NRTIs, with V108 mutation for NNRTI well as the virological failure of treatment. Conclusion: Our results confirmed the hypothesis that high Viral Load at the start of the treatment is a poor prognosis for the development of therapy. Transmitted mutations are involved in treatment failure.

Background:This study is aimed at describing the prevalence of and risk factors associated with early post-operative complications after Crohn’s disease-related intestinal ***:This was a retrospective analysis of data from the PRACTICROHN *** Crohn’s disease patients who underwent ileocolonic resection with ileocolonic anastomosis between January 2007 and December 2010 were *** complications evaluated included death,ileus,anastomotic leak,abscess,wound infection,catheter-related infection,digestive bleeding and other extra-abdominal infections that occurred in the 30 days after ***:A total of 364 patients(median age at surgery 38 years and 50%men)were *** for surgery was:stricturing disease(46.4%),penetrating disease(31.3%),penetrating and stricturing disease(14.0%)or resistance to medical treatment(5.8%).Early complications were recorded in 100(27.5%)patients,with wound infection,intra-abdominal abscess and anastomotic leakage being the most frequent *** hospitalization duration was 16 days for patients with complications vs.9 days without complications(P<0.001).Complications were more common among patients with penetrating disease(36/114,31.6%)and those refractory to treatment(9/21,42.9%)compared with stricturing disease(45/169,26.6%)or stricturingtpenetrating disease(6/51,11.8%)(P=0.040).The rate of complications was higher among patients with diagnosis made at the time of surgery(15/31,48.4%)compared with the rest(85/331,25.7%)(P=0.013).Medication received at the time of surgery did not affect the rate of ***:Almost a quarter of patients developed early complications after intestinal *** disease and urgent surgery were associated with an increased risk of complications.

Low-Dose Misoprostol versus Vaginal-Insert Dinoprostone for At-Term Labor Induction: A Prospective Cohort Study

Introduction: Given the pharmacological change taking place in Spanish delivery rooms, we designed this study to compare the effectiveness and safety of misoprostol (25 mcg/4h) to 10 mg dinoprostone in its slow-releasing vaginalinsert form to induce at-term labor. Methods: In a prospective cohort study (2013-2015), 401 at-term pregnant women referred for induction (Bishop score ≤ 6), who gave birth at the Rey Juan Carlos Hospital in Móstoles (Madrid, Spain), of whom 241 participants were induced with misoprostol and 160 with dinoprostone. Patients were recruited by non-probabilistic sampling of consecutive cases as the inductions in the Obstetrics Block during the study period were indicated. Results: The cesarean section rate was 14% for the misoprostol group and 19% for the dinoprostone group [RR = 1.38 (95%CI: 0.89 - 2.13), p = 0.145]. No differences were found in the vaginal birth rate in <24 h from starting induction [44% vs. 53%, RR = 1.08 (0.90 - 1.29), p = 0.401], nor in the vaginal birth rate in <12 h, requiring oxytocin or meconium fluid, or for maternal-neonatal complications deriving from the method employed. The percentage of cesarean sections due to failed induction was lower in the misoprostol group (15% vs. 39%, p = 0.028). The time between induction and giving birth was shorter for the dinoprostone group (20 h vs. 23.4 h, p = 0.018). Conclusion: Inducing delivery by low-dose misoprostol offers similar effectiveness and safety to induction by dinoprostone in its slow-releasing vaginal-insert form. However, our sample is limited, and few studies are available on the subject. Further analyses to draw solid conclusions are required.

Menarcheal Age and Nutritional Status of Secondary School Adolescent Girls in Nnewi, South-East Nigeria

Background: Menarche is an important indicator of pubertal development in adolescent girl. Age at menarche has declined over the years attributable to many factors. Objective: To determine the age at menarche of secondary school adolescent girls and its relationship with anthropometric values, nutritional status, social class of parents, and category of school. Subjects and Methods: Cross-sectional study of 311 secondary school adolescent girls (10-19 years) from two secondary schools (boarding school-Maria Regina secondary school Nnewi and Anglican Girls Secondary School Nnewi-day school) in Nnewi North LGA of Anambra State, Southeastern Nigeria, between May and June 2018. Semi-structured questionnaires were used to extract their data on sociodemographic characteristics and age at menarche. Height and weight were measured using WHO recommended methods. Body mass index (BMI) was determined using the Quetelet index (kg/m2). The relationship between menarche and anthropometric measurement was determined. Results: Of the 313 participants, 212 (68.2%) were post menarcheal, while 99 (31.8%) were pre-menarcheal. The mean age at menarche was 12.79 ± 1.20 years. Early menarcheal age was associated with significantly higher anthropometric values and BMI. Boarding-students had significantly lower menarcheal age (12.69 ± 1.26) compared to the day students (12.86 ± 1.15);students with parents of the high social class had lower menarcheal age (12.56 ± 1.51), compar

Protective Effect of Phenolic-Rich Extracts from Different Parts of <i>Opuntia joconostle</i>Fruit against Carbon Tetrachloride-Induced Oxidative Stress in Mice

Opuntia joconostle fruit is a rich source of biocompounds such as polyphenols including gallic, vanilic, 4-hidroxybenzoic, cafeic, and syringic acids, catechin, epicatechin, rutin, and vanillin, besides betalains. The objective of this research was to evaluate the effect of supplementation polyphenols-rich extracts from different parts of Opuntia joconostle against carbon tetrachloride-induced oxidative stress in a mouse model. The animals were treated orally with polyphenols-rich extracts at 50, 100 and 200 mg/kg BW for 30 consecutive days. On day 30th the mice received carbon tetrachloride (CCl4) as hepatoxic agent. Biochemical evaluations were carried out 24 h after induction of the oxidative stress. Data showed that methanolic extracts from different parts of Opuntia joconostle exerting protective effect against the CCl4-induced oxidative stress in mice. Histology examination revealed that the damage decreased in groups treated with polyphenols-rich extracts compared to the group that did not receive any treatment. Opuntia joconostle fruit contains many phenolic compounds, flavonoids and betalains. The protective effect of extracts may be related to the phenolic composition and also by a counteraction with other compounds, such as betalains and flavonoids that increase their antioxidant effect.

Ten years of lung cancer in a single center: gender, histology, stage and survival

Aim:The aim was to describe,in a prospective manner,the clinical,histopathological and epidemiological characteristics of lung cancer patients who attended as outpatients at the Lluís Alcanyís,Xàtiva Medical Oncology Hospital,València,Spain from January 2004 to July *** also analyzed survival and compared our data with that reported in the ***:Clinical and demographic characteristics were analyzed for the entire series and trends were compared by year of *** in epidemiology were examined and ***:There were 701 patients(91.4%were men,mean age 67.6).Main histology was squamous cell carcinoma(41.5%).Squamous cell carcinoma prevailed in men(45.5%)and adenocarcinoma(ADC)in women(60.3%).The percentage of men with lung cancer and of patients with squamous cell carcinoma was higher than in the reported worldwide data and remained throughout the 10 years *** survival was low,with<10%survivors at 5 *** of disease remained the main prognostic factor for ***:Squamous cell carcinoma continues to be the most frequent histological type in our *** and smoking is associated with lung carcinoma while ADC more often occurs in *** the time,our epidemiological and histological patterns have not changed,possibly in relation to maintenance of smoking habits.

Design of a Mobile Photovoltaic Module System for Demonstration and Experimentation

In Mexico, owing to solar radiation conditions, there is great potential for PV （photovoltaic） systems. Besides, since 2007, the solar electricity interconnection agreement became official which allowed for an important growth of the photovoltaic industry. Due to the important development of the PV industry, there is a need for engineers trained to design, install, and evaluate PV systems The UAEMEX （autonomous university of the state of Mexico） offers a new bachelor degree of engineering program which dedicated to the development of sustainable energy systems. This kind of programs requires special equipment for experimentation and practice Specialists at UAEMEX also detected the need for a system to evaluate and demonstrate the application of PV modules, but they also found that most of the systems on the market are very expensive and unable to evaluate different arrangements. The goal of this paper is to present the design of a mobile system to evaluate PV modules and arrangements. This system is useful, not only for demonstration practices, but also for experimentation with different materials and types of PV panels. The design allows for two or four PV modules to be assembled in different positions and inclinations. The prototype is equipped with a data acquisition system that will be used to obtain the PV module performance curves. It will also be used to evaluate the atmospheric, interference and shadow effects on these modules. This prototype will be used to complement the practice learning of solar PV systems, but also to demonstrate the use of PV modules. The prototype is being design and constructed by a group of mechanical and electronic engineering students and this fact will be useful to meet the needs of basic science, math, and engineering teaching objectives in an integrated and hands-on way project. Clival Salivary Heterotopia (Salivary Choristoma): A Rare Radiopathogical Lesión

Objective: To describe the radiological and pathological presentation of a salivary heterotopia in clivus. The lesion was detected incidentally in a 57-year-old woman. Material and Methods: Radiologic imaging (CT scan and MRI) and histological picture obtained from a clival lesion. Results: Radiologically the lesion presented as a well-limited lytic lesion with a diameter of 1.5 cm and accompanied by a cerebrospinal fluid secondary fistula. Histological examination revealed non-pathological salivary gland tissue. Conclusion: The differential diagnosis of this rare entity mandates a histological examination of the lesion.

Myxoid Meningioma:An Example of a Rare Brain Tumour of Difficult Diagnosis

We present a case of a Myxoid Meningioma noticed in a 43-year-old woman. The patient, who had no noteworthy medical antecedents, showed repeated focal epileptic crisis. Imaging studies (MR and CT) showed a solitary and well delimited tumour located in the frontral lobe. Once extirpated, the histological study revealed a neoplasm of myxoid and benign appearance, with elongated and uniform cells, with no atypias and a very low mitotic index. Immunohistochemisty studies (positivity for Vimentin and Epithelial Membrane Antigen, and negativity for the other markers) as well as physical and radiological examination of the patient (absence of tumours in any other location), suggested the diagnosis of a Myxoid Meningioma, which was confirmed by electron microscopy (by the presence of desmosomes). We also revise the characteristics of Myxoid Meningiomas, an absolutely exceptional primary tumour of the central nervous system.
