Prevalence of clonorchiasis in patients with gastrointestinal disease: A Korean nationwide multicenter survey

Background: Leptomeningeal seeding （LMS） is a devastating complication of malignancy. Among several pathogenesis, surgical spillage has been one of the iatrogenic factors for the development of LMS. We retrospectively analyzed various surgical factors affecting the development of LMS. Patients and Methods: We identified 294 patients with brain metastasis who had been treated using surgery between 2001 and 2009. We classified the tumor location into subcortical, pial, epipial, juxtaventricular and ventricular. The description of surgical resection was divided into piecemeal and en-bloc and the risk of using cavitron ultrasonic aspirator （CUSA） were investigated. Results: A total 44 of the 294 patients （15%） developed LMS after undergoing index procedure. The median time to LMS development was 6months （range, 1.0-42.0）. Among the 294 surgical patients, seven out of 101 en-bloc removal patients （6.9%） developed LMS, whereas 37 out of 193 piecemeal removal patients （19.2%） developed LMS, and the incidence of LMS was significantly different between these two surgical resection modes （p = 0.005）.The adjusted hazard ratio （HR） for patients whose tumors were resected by piecemeal compared to en-bloc was 3.12 （95% CI, 1.4-7.0;p = 0.006）. The incidence of LMS was significantly higher （p ＜ 0.001） in patients whose tumors were resected by CUSA, and the HR of using CUSA was 4.18 （95% CI, 1.9-9.4;p = 0.001）. The location of tumor at high-risk of CSF exposure （epipial and ventricular） conferred increased risk of LMS after the treatment compared to low-risk group with the HR 2.52 （95% CI, 1.4-4.7;p = 0.004）. On the other hand, the location of tumor at low-risk of CSF exposure （subcortical） showed significantly reduced risk of LSM compared to the other locations （HR=0.27;95% CI 0.12-0.65;p-value = 0.003）. Conclusion: The piecemeal resection mode and the use of CUSA were found to increase risk of LMS in this retrospective study. Therefore, the piecemeal resection using CUSA should be

AIM： To investigate prevalence of C/onorchis sinensis in patients with gastrointestinal symptoms, and the relation of the infection to hepatobiliary diseases in 26 hospitals in Korea. METHODS： Consecutive patients who had been admitted to the Division of Gastroenterology with gastrointestinal symptoms were enrolled from March to April 2005. Of those who had been diagnosed with clonorchiasis, epidemiology and correlation between infection and hepatobiliary diseases were surveyed by questionnaire. RESULTS： Of 3080 patients with gastrointestinal diseases, 396 （12.9%） had clonorchiasis and 1140 patients （37.2%） had a history of eating raw freshwater fish. Of those with a history of raw freshwater fish ingestion, 238 （20.9%） patients had clonorchiasis. Cholangiocarcinoma was more prevalent in C. sinensis-infected patients than nonnfected patients [34/396 （8.6%） vs 145/2684 （5.4%）, P = 0.015]. Cholangiocarcinoma and clonorchiasis showed statistically significant positive cross-relation （P = 0.008）. Choledocholithiasis, cholecystolithiasis, cholangitis, hepatocellular carcinoma, and biliary pancreatitis did not correlate with clonorchiasis. CONCLUSION： Infection rate of clonorchiasis was still high in patients with gastrointestinal diseases in Korea, and has not decreased very much during the last two decades. Cholangiocarcinoma was related to clonorchiasis, which suggested an etiological role for the parasite.

近年来,科学技术发展日新月异,每天都会出现各种新型研究领域,众多研究成果被发表与公布。为此,来自各个国家的组织机构做了巨大的努力要为自己的机构带来外部与内部的变化。事实上,对于很多机构来说,研究的意义在于通过分析研究成果得到有意义的结论。因此,本文通过对具体机制下的专利标题(发明名称)进行分析从而提出一种演化模型。首先,我们根据关键词的特性对标题的关键词进行分类,然后定义专利的相互关系。我们建议演化模型的相互关系建立在时间轴基础上,并且可以适用于实际数据。通过应用于实际数据的模型可预测出未来专利的关键词。

AIM:To evaluate the incidence and risk factors of Korean tuberculosis(TB) infection in patients with inflammatory bowel disease(IBD) undergoing anti-TNF ***:The data of IBD patients treated with anti-TNFs in 13 tertiary referral hospitals located in the southeastern region of Korea were collected *** failed to show response or were intolerant to conventional treatments,including steroids or *** measures for latent TB infection(LTBI)and the incidence and risk factors ofactive TB infection after treatment with anti-TNFs were ***:Overall,376 IBD patients treated with antiTNF agents were recruited(male 255,mean age of anti-TNF therapy 32.5±13.0 years);277 had Crohn’s disease,99 had ulcerative colitis,294 used infliximab,and 82 used *** anti-TNF treatment,screening tests for LTBI including an interferon gamma release assay or a tuberculin skin test were performed in 82.2%of *** patients(8%)had *** cases of active TB infection including one TB-related mortality occurred during 801 personyears(PY)follow-up(1997.4 cases per 100000 PY)after anti-TNF ***(OR=5.76,95%CI:1.57-21.20,P=0.008)and WBC count<5000 mm3(OR=4.5,95%CI:1.51-13.44,P=0.007)during follow-up were identified as independently associated risk ***:Anti-TNFs significantly increase the risk of TB infection in Korean patients with *** considerable burden of TB and marked immunosuppression might be attributed to this risk.

We report the electrical detection of captured gases through measurement of the quantum tunneling characteristics of gas-mediated molecular junctions formed across *** gas-sensing nanogap device consists of a pair of vertically stacked gold electrodes separated by an insulating 6 nm spacer(~1.5 nm of sputteredα-Si and~4.5 nm ALD SiO2),which is notched~10 nm into the stack between the gold *** exposed gold surface is functionalized with a self-assembled monolayer(SAM)of conjugated thiol linker *** the device is exposed to a target gas(1,5-diaminopentane),the SAM layer electrostatically captures the target gas molecules,forming a molecular bridge across the *** gas capture lowers the barrier potential for electron tunneling across the notched edge region,from~5 eV to~0.9 eV and establishes additional conducting paths for charge transport between the gold electrodes,leading to a substantial decrease in junction *** demonstrated an output resistance change of>108 times upon exposure to 80 ppm diamine target gas as well as ultralow standby power consumption of<15 pW,confirming electron tunneling through molecular bridges for ultralow-power gas sensing.
