Triple negative breast cancer(TNBC) accounts for 15%-20% of all breast cancer, and is still defined as what it is not. Currently, TNBC is the only type of breast cancer for which there are no approved targeted therapies and maximum tolerated dose chemotherapy with taxanes and anthracycline-containing regimens is still the standard of care in both the neoadjuvant and adjuvant settings. In the last years, metronomic chemotherapy(MC) is being explored as an alternative to improve outcomes in TNBC. In the neoadjuvant setting, purely metronomic and hybrid approaches have been developed with the objective of increasing complete pathologic response(p CR) and prolonging disease free survival. These regimens proved to be very effective achieving pC R rates between 47%-60%, but at the cost of great toxicity. In the adjuvant setting, MC is used to intensify adjuvant chemotherapy and, more promisingly, as maintenance therapy for high-risk patients, especially those with no pC R after neoadjuvant chemotherapy. Considering the dismal prognosis of TNBC, any strategy that potentially improves outcomes, specially being the oral agents broadly available and inexpensive, should be considered and certainly warrants further exploration. Finally, the benefit of MC needs to be validated in properly designed clinical trials were the selection of the population is the key.

Background:Beginning with the concepts of stress developed by Selye,an approach to stress and pain management,known as neuro-emotional technique(NET),has been *** is a treatment approach based on the principle that the stressor effects of dormant and/or current unresolved issues or trauma are what determine one’s bodily *** responses are relatively personalized to the conditioned,experiential and emotional reality of the ***:To determine the effect of NET on patients with chronic low back pain(ClBP)over ***,setting,participants,and interventions:In a randomized,double-blinded,placebo-controlled study conducted in a single clinic,NET or control treatments were given twice weekly for 4 weeks in a population of 112 *** outcome measures:Outcome measures,including Oswestry Disability Index,Quadruple Visual Analogue Scale,the psychoneuroimmunology markers of blood serum levels of C-reactive protein,tumour necrosis factor-a,interleukin-1(Il-1),Il-6,and Il-10,and 10 dimensions of the Short Form Health Survey scale,were assessed at baseline and at 1,3 and 6 months following the intervention ***:Compared to placebo,NET produced clinical and statistical significance(Plines of virtually all physiological,pain and disability markers,accompanied by gains in quality-of-life indicators at 0(baseline),1,3 and 6 *** of the percentages of patients whose 5 biomarkers lay outside the normative range were achieved at 1,3 and 6 months by NET but not control ***:A randomized,controlled trial of ClBP patients indicated that 8 NET interventions,compared to placebo,produced clinically and statistically significant reductions in pain,disability and inflammatory biomarkers,and improvements in quality-of-life measures.

AIM To investigate the survival impact of clinicopathological factors, including pathological complete response(p CR) and tumor-infiltrating lymphocytes(s TIl) levels according to subtypes, in breast cancer(BC) patients who received neo-adjuvant chemotherapy(NAC).METHODS We evaluated 435 BC patients who presented and received NAC at the Instituto Nacional de Enfermedades Neoplasicas from 2003 to 2014. s TIl was analyzed as the proportion of tumor stroma occupied by lymphocytes, and was prospectively evaluated on hematoxylin and eosin-stained sections of the preN AC core biopsy. p CR was considered in the absence of infiltrating cancer cells in primary tumor and axillary lymph nodes. Analysis of statistical association between clinical pathological features, s TIl, p CR and survival were carried out using *** Median age was 49 years(range 24-84 years) and the most frequent clinical stage was ⅢB(58.3%). luminal A, luminal B, HER2-enriched and(triple-negative) TN phenotype was found in 24.6%, 37.9%, 17.7% and 19.8%, respectively. p CR was observed in 11% and median percentage of s TIl was 40%(2%-95%) in the whole population. p CR was associated to Ct1-2(P = 0.045) and to high s TIl(P = 0.029) in the whole population. It is difficult to obtain in vivo digestion kinetics data of high protein ingredients using chickens. Collecting kinetics data requires repeated sampling of digesta from the small intestine during the digestion process,which is not easily accomplished due to the anatomical structure of chicken digestive tract. An in vitro technique is proposed for measuring the digestion kinetics of protein sources fed to chickens. The method has a 30 min gastric and 3 h intestinal phase. Five hundred milligram crude protein(CP)equivalent of each meal sample(CP = % N × 6.25) was digested with pepsin(28,260 units) in 50 m l polyethylene centrifuge tubes for 30 min in a shaking water bath(150 strokes/min; 30 mm stroke length)at 41C. The 6.5 ml pancreatin was selected as the enzyme concentration for the intestinal phase, during which time 500 m l aliquots were collected at 0, 15, 30, 45, 60, 90, 120, 150, 180 and 240 min. Samples were diluted 1:820 with HCl and sodium acetate buffer, and then mixed with ninhydrin reagent(2:1) at100 ± 2C for 15 min and spectrometric readings taken at 568 nm. To validate the assay, 5 replications of soybean meal(SBM), corn gluten meal(CGM), corn distillers dried grains with solubles(CDDGS), porcine meal(PCM), fish meal(FM) and casein(CA) were digested. The digestion data were modeled with PROC NlIN procedure, and the intra coefficient of variation(CV) assessed using PROC MEANS of SAS 9.4. The digestion values at 180 min were SBM 95 ± 4, FM 93 ± 3, PCM 68 ± 4, CGM 82 ± 3 and CDDGS 70 ± *** CV for SBM, CGM, CDDGS, PCM and FM were 5%, 5%, 12%, 10% and 2%, respectively. The estimated fractional digestion rates for SBM, CGM, CDDGS, FM and PCM were 0.023, 0.013, 0.009, 0.024 and 0.013,respectively. In conclusion, the proposed in vitro technique estimated the rate and extent of the digestion of CP for the meals with low intra CV.

Electronic commerce is impacting almost all commercial activities. The resulting emerging commercial activities bring with them many new modeling and optimization problems. This survey reviews pioneering works in this new area, covering topics in advertising strategy, web page design, automatic pricing, auction methods, brokerage strategy, and customer behavior analysis. Mathematical models for problems in these areas and their solution algorithms are discussed. In addition to presenting and commenting on these works, we also discuss possible extensions and related problems. The objective of this survey is to encourage more researchers to pay attention to this emerging area.

Coronavirus disease 2019(COVID-19)is a highly infectious disease which emerged into a global *** it primarily causes respiratory symptoms for affected patients,COVID-19 was shown to have multi-organ *** liver enzymes appear to be commonly observed during the course of COVID-19,and there have been numerous reports of liver injury secondary to COVID-19 *** has been established that patients with pre-existing chronic liver disease(ClD)are more likely to have poorer outcomes following COVID-19 infection compared to those without ***-morbidities such as diabetes,hypertension,obesity,cardiovascular and chronic kidney disease frequently co-exist in individuals living with ClD,and a substantial population may also live with some degree of *** mechanisms of how COVID-19 induces liver injury have been *** syndrome(HRS)is the occurrence of kidney dysfunction in patients with severe ClD/fulminant liver failure in the absence of another identifiable cause,and is usually a marker of severe decompensated liver *** reports of HRS following acute COVID-19 infection have been presented,although the risk factors and pathophysiological mechanisms leading to HRS in COVID-19 infection or following COVID-19 treatment remain largely unestablished due to the relative lack and novelty of published *** discussing the management of HRS in highdependency care and intensive care contexts is only *** this article,we provide an overview on the speculative pathophysiological me-chanisms of COVID-19 induced HRS and propose strategies for clinical diagnosis and management to optimize outcomes in this scenario.
