Emerging evidence and perspectives have pointed towards the heart playing an important role in hepatorenal syndrome(HRS),outside of conventional understanding that liver cirrhosis is traditionally considered the sole origin of a cascade of pathophysiological mechanisms directly affecting the kidneys in this *** the absence of established heart disease,cirrhotic cardiomyopathy may occur more frequently in those with liver cirrhosis and kidney *** is a specific form of cardiac dysfunction characterized by blunted contractile responsiveness to stress stimuli and altered diastolic relaxation with electrophysiological *** the clinical description of these potential cardiac-related complications of the liver,the role of the heart has traditionally been an overlooked aspect of circulatory dysfunction in *** from a physiological sense,temporality(prior onset)of cardiorenal interactions in HRS and positive effects stemming from portosystemic shunting demonstrated an important role of the heart in the development and progression of kidney dysfunction in cirrhotic *** this review,we discuss current concepts surrounding how the heart may influence the development and progression of HRS,and the role of systemic inflammation and endothelial dysfunction causing circulatory dysfunction within this *** temporality of heart and kidney dysfunction in HRS will be *** a subgroup of patients who receive portosystemic shunting,the dynamics of cardiorenal interactions following treatment is *** research to determine the unknowns in this topic is anticipated,hopefully to further clarify the intricacies surrounding the liver-heart-kidney connection and improve strategies for management.

Development of an in vitro protein digestibility assay mimicking the chicken digestive tract

It is difficult to obtain in vivo digestion kinetics data of high protein ingredients using chickens. Collecting kinetics data requires repeated sampling of digesta from the small intestine during the digestion process,which is not easily accomplished due to the anatomical structure of chicken digestive tract. An in vitro technique is proposed for measuring the digestion kinetics of protein sources fed to chickens. The method has a 30 min gastric and 3 h intestinal phase. Five hundred milligram crude protein(CP)equivalent of each meal sample(CP = % N × 6.25) was digested with pepsin(28,260 units) in 50 m l polyethylene centrifuge tubes for 30 min in a shaking water bath(150 strokes/min; 30 mm stroke length)at 41C. The 6.5 ml pancreatin was selected as the enzyme concentration for the intestinal phase, during which time 500 m l aliquots were collected at 0, 15, 30, 45, 60, 90, 120, 150, 180 and 240 min. Samples were diluted 1:820 with HCl and sodium acetate buffer, and then mixed with ninhydrin reagent(2:1) at100 ± 2C for 15 min and spectrometric readings taken at 568 nm. To validate the assay, 5 replications of soybean meal(SBM), corn gluten meal(CGM), corn distillers dried grains with solubles(CDDGS), porcine meal(PCM), fish meal(FM) and casein(CA) were digested. The digestion data were modeled with PROC NlIN procedure, and the intra coefficient of variation(CV) assessed using PROC MEANS of SAS 9.4. The digestion values at 180 min were SBM 95 ± 4, FM 93 ± 3, PCM 68 ± 4, CGM 82 ± 3 and CDDGS 70 ± *** CV for SBM, CGM, CDDGS, PCM and FM were 5%, 5%, 12%, 10% and 2%, respectively. The estimated fractional digestion rates for SBM, CGM, CDDGS, FM and PCM were 0.023, 0.013, 0.009, 0.024 and 0.013,respectively. In conclusion, the proposed in vitro technique estimated the rate and extent of the digestion of CP for the meals with low intra CV.

Electronic commerce is impacting almost all commercial activities. The resulting emerging commercial activities bring with them many new modeling and optimization problems. This survey reviews pioneering works in this new area, covering topics in advertising strategy, web page design, automatic pricing, auction methods, brokerage strategy, and customer behavior analysis. Mathematical models for problems in these areas and their solution algorithms are discussed. In addition to presenting and commenting on these works, we also discuss possible extensions and related problems. The objective of this survey is to encourage more researchers to pay attention to this emerging area.

Coronavirus disease 2019(COVID-19)is a highly infectious disease which emerged into a global *** it primarily causes respiratory symptoms for affected patients,COVID-19 was shown to have multi-organ *** liver enzymes appear to be commonly observed during the course of COVID-19,and there have been numerous reports of liver injury secondary to COVID-19 *** has been established that patients with pre-existing chronic liver disease(ClD)are more likely to have poorer outcomes following COVID-19 infection compared to those without ***-morbidities such as diabetes,hypertension,obesity,cardiovascular and chronic kidney disease frequently co-exist in individuals living with ClD,and a substantial population may also live with some degree of *** mechanisms of how COVID-19 induces liver injury have been *** syndrome(HRS)is the occurrence of kidney dysfunction in patients with severe ClD/fulminant liver failure in the absence of another identifiable cause,and is usually a marker of severe decompensated liver *** reports of HRS following acute COVID-19 infection have been presented,although the risk factors and pathophysiological mechanisms leading to HRS in COVID-19 infection or following COVID-19 treatment remain largely unestablished due to the relative lack and novelty of published *** discussing the management of HRS in highdependency care and intensive care contexts is only *** this article,we provide an overview on the speculative pathophysiological me-chanisms of COVID-19 induced HRS and propose strategies for clinical diagnosis and management to optimize outcomes in this scenario.

A TWO-PHASE APPROACH TO FUZZY SYSTEM IDENTIFICATION

A two-phase approach to fuzzy system identification is proposed. The first phase produces a baseline design to identify a prototype fuzzy system for a target system from a collection of input-output data pairs. It uses two easily implemented clustering techniques: the subtractive clustering method and the fuzzy c-means (FCM) clustering algorithm. The second phase (fine tuning) is executed to adjust the parameters identified in the baseline design. This phase uses the steepest descent and recursive least-squares estimation methods. The proposed approach is validated by applying it to both a function approximation type of problem and a classification type of problem. An analysis of the learning behavior of the proposed approach for the two test problems is conducted for further confirmation.

Thrapeutic equivalence in the treatment of hypertension:Can lercanidipine and nifedipine GITS be considered to be interchangeable?

AIM: To undertake a review of the evidence that nifedipine GITS and lercanidipine are therapeutically equivalent in the management of essential ***: A systematic review of the published literature was prompted by the findings of two meta-analyses which indicated that there was a lower incidence of peripheral(ankle) oedema with lercanidipine. However,neither meta-analysis gave detailed attention to comparative antihypertensive efficacy or cardiovascular protection. Accordingly,a systematic,detailed and critical review was undertaken of individual published papers. The review started with those studies incorporated into the 2 meta-analyses and then all other salient and directly relevant papers identified through the following search criteria: all randomized controlled trials in which the therapeutic profile and antihypertensive effects of lercanidipine were directly compared with those of nifedipine GITS(in hypertensive patients). The searchstrategy was focused on the reports of clinical trials of lercanidipine vs nifedipine GITS,which were identified through a systematic search of PubMed(from 1966 to October 2012),Embase(from 1980 to October 2012) and the Cochrane library(from 1 October 2008 to end October 2013). The search combined terms related to lercanidipine vs nifedipine GITS(including MeSH search using calcium antagonists,calcium channel blockers and dihydropyridines).RESUlTS: With regard to blood pressure(BP) control and the consistency of BP control throughout 24-h,there is limited published evidence. However,two studies using 24 h ambulatory blood pressure monitoring clearly identified the dose-dependency of BP lowering with lercanidipine and its variably sustained 24-h efficacy. In contrast,there is evidence of a consistent antihypertensive effect throughout 24 h with nifedipine GITS.
