研究背景:超过20%的患者在大手术后发生围术期并发症,耗费了大量医疗资源和医保费用。因此,改善预后的重点应包括及时识别和并发症管理。胰腺切除术作为一种复杂手术,其并发症发生率和死亡率均较高。早期识别和处理并发症可以改善患者预后。研究方法:本研究是一项开放标签、全国性、阶梯楔形聚类随机试验,纳入所有接受胰腺切除术的患者,无针对中心或患者的排除标准。所有17家进行胰腺手术的中心均被随机分配到常规术后管理(对照组)和根据多模式、多学科算法早期识别和微创处理术后并发症组(干预组)。本研究开发了一个智能手机软件,该软件结合了该算法包括临床和生化标志物的日常评估,确定何时进行腹部CT、穿刺引流、抗感染和拔除腹部引流管。研究终点为需要侵入性干预的出血、器官衰竭以及术后90天内死亡。算法开发的源数据来自既往研究报道和国家数据库。

Mechanisms and anisotropy of serrated flow in Mg-Gd single crystals

Serrated flow has been primarily studied at the macron scale,yet the length and times scales at which the solute-meditated dislocation pinning and de-pinning processes that underlie the phenomenon occur are largely inaccessible by macroscopic ***,direct insights into the dominant slip systems in the serrated flow regime,which is particularly critical in Mg alloys given their high plastic anisotropy,requires the use of small-scale testing methods such as ***,in this work,a combination of microcompression and TEM based EDS/STEM measurements have used to critically study the temperature and strain rate dependences in single crystals of pure Mg and a Mg-Gd alloy oriented for twinning,basal-,prismatic-,and *** results provide compelling evidence that the solute drag mechanism underlie serrated flow in the alloy;they also show that serrated flow in Mg alloys is markedly *** anisotropy is caused by differences between the Burgers vector for slip/twinning,and between the impurity diffusivity along/perpendicular to the basal plane.

A Force-Based Grid Manipulator for ALE Calculations in a Lobe Pump

In this paper, a time-dependant calculation of flow in a lobe pump is presented. Calculations are performed using the arbitrary Lagrangean Eulerean (ALE) method. A grid manipulator is needed to move the nodes between time steps. The used grid manipulator is based on the pseudo-force idea. This means that each node is fictitiously connected with its 8 neighbours via fictitious springs. The equilibrium of the resulting pseudo spring forces defines the altered position of the nodes. The grid manipulator was coupled with a commercial flow solver and the whole was tested on the flow through a three-lobe lobe pump. Results were obtained for a rotational speed of 460 rpm and incompressible silicon oil as fluid.

VLES Modelling with the Renormalization Group

In a Very-Large-Eddy Simulation (VLES), the filterwidth-wavenumber can be outside the inertial range, and simple subgrid models have to be replaced by more complicated (''RANS-like'') models which can describe the transport of the biggest eddies. One could approach this by using a RANS model in these regions, and modify the lengthscale in the model for the LES-regions. The problem with these approaches is that these models are specifically calibrated for RANS computations, and therefore not suitable to describe inertial range quantities. We investigated the construction of subgfid viscosity and transport equations without any calibrated constants, but these are calculated directly form the Navier-Stokes equation by means of a Renormalization Group (RG) procedure. This leads to filterwidth dependent transport equations and effective viscosity with the right limiting behaviour (DNS and RANS limits).

Efficiency of the motion amplification device with viscous dampers and its application in high-rise buildings

After nearly a decade of application and investigation, a motion amplification device with viscous dampers for energy dissipation has been recognized as an effective solution to mitigate wind or seismic excitation, especially for stiff structural systems. As a result of compensation of amplified motion, it has been proved that the efficiency of viscous damper largely depends on the motion amplification device configuration, particularly for device stiflhess. In this paper, a ＂scissor-jack＂ type of motion amplification device, called a ＂toggle brace damper＂ system, is studied. It is demonstrated that the efficiency of such a device reflected by its amplification factor is not merely a function of its geometric configuration, but is highly dependent on the support elements＇ stiffness as well, similar to the mechanism of a leverage arm. accordingly, a mathematical model in terms of complex modulus of the viscous damper with consideration of the support brace＇s stiffness is established. The results indicate that the efficiency of the motion amplification device with viscous dampers significantly depends on the stiffness of the support elements. Other parameters, such as toggle brace configuration and damping values of the viscous damper, are studied and compared. As an application example, numerical analyses are conducted to study the dynamic performance of a 39-story office tower installed with toggle brace dampers constructed on soft soil in a reclaimed area, under a combined effect of the vortex shedding of an adjacent existing 52-story building and earthquakes. The results show that viscous dampers with a motion amplification system using a properly designed toggle brace device proved to be an effective solution to alleviate the external excitations.

A new cavitating model by using bubble size distribution based on mass of bubbles is proposed. Liquid phase is treated with Eulerian framework as a mixture containing minute cavitating bubbles. Vapor phase consists of various sizes of minute vapor bubbles, which is distributed to classes based on their mass. The change of bubble number density for each class was solved by considering the change of bubble mass due to phase change as well as generation of new bubbles due to heterogeneous nucleation. In this method the mass of bubbles is treated as an independent variable, in other word, a new coordinate, and dependant variables are solved in Eulerian framework for spatial coordinates and bubble-mass coordinate. The present method is applied to a cavitating flow in a convergent-divergent nozzle, and the two-phase flow with bubble size distribution and phase change was successfully predicted.

THREE-DIMENSIONAL AXISYMMETRIC FLOW IN PERFECTLY PLASTIC METALS

A method is presented for solving the three-dimensional axisymmet- ric field equations for a perfectly plastic material which obeys the von-Mises yield criterion and the Levy-Mises flow law. The method is used for the particular case in which a small axisymmetric perturbed flow is superposed on a uniform flow without flow reversal taking place. The method then leads to solving a fourth order differen- tial equation for the velocity potential. The special case of a thick cylindrical shell under compressive flow is examined. The solution so obtained, being derived from the three dimensional theory, includes a correct treatment of transverse shear distor- tion. A preferred mode of instability is identified having a wave-length in reasonable agreement with that obtained experimentally by other workers.

Hemostatic agents for prehospital hemorrhage control: a narrative review

Hemorrhage is the leading cause of preventable death in combat trauma and the secondary cause of death in civilian trauma.A significant number of deaths due to hemorrhage occur before and in the first hour after hospital arrival.A literature search was performed through PubMed,Scopus,and Institute of Scientific Information databases for English language articles using terms relating to hemostatic agents,prehospital,battlefield or combat dressings,and prehospital hemostatic resuscitation,followed by cross-reference *** were screened to determine relevance and whether appropriate further review of the original articles was *** on these findings,this paper provides a review of a variety of hemostatic agents ranging from clinically approved products for human use to newly developed concepts with great potential for use in prehospital *** hemostatic agents can be administered either systemically or locally to stop bleeding through different mechanisms of *** of current hemostatic products and further directions for prehospital hemorrhage control are also discussed.
