BACKGROUNd:This study aimed to review bicycle-related injuries during the COVId-19 pandemic to assist with reinforcement or implementation of new policies for injury ***:This is a retrospective descriptive analysis of injuries sustained during cycling for patients 18 years old and above who presented to Singapore General Hospital from January to June *** records were reviewed and *** analyses were used to summarize patient characteristics,and differences in characteristics subgrouped by triage acuity and discharge status were ***:The study included 272 patients with a mean age of 43 years and a male predominance(71.7%).Most presented without referrals(88.2%)and were not conveyed by ambulances(70.6%).Based on acuity category,there were 24(8.8%)Priority 1(P1)patients with 7 trauma activations,174(64.0%)and 74(27.2%)P2 and P3 patients *** most common injuries were fractures(34.2%),followed by superficial abrasion/contusion(29.4%)and laceration/wound(19.1%).Thirteen(4.8%)patients experienced head injury and 85 patients(31.3%)were documented to be wearing a *** majority occurred on the roads as traffic accidents(32.7%).Forty-two patients(15.4%)were admitted with a mean length of stay of 4.1 d and 17(6.3%)undergone surgical *** of 214(78.7%)discharged patients,no re-attendances or mortality were *** the subgroup analysis,higher acuity patients were generally older,with higher proportions of head injuries leading to ***:Our study highlights significant morbidities in bicycle-related *** is also a high proportion of fractures in the young healthy male *** prevention is paramount and we propose emphasizing helmet use and road user safety.

Robotic Knee Tracking Control to Mimic the Intact Human Knee Profile Based on Actor-Critic Reinforcement Learning

We address a state-of-the-art reinforcement learning(RL)control approach to automatically configure robotic pros-thesis impedance parameters to enable end-to-end,continuous locomotion intended for transfemoral amputee ***,our actor-critic based RL provides tracking control of a robotic knee prosthesis to mimic the intact knee *** is a significant advance from our previous RL based automatic tuning of prosthesis control parameters which have centered on regulation control with a designer prescribed robotic knee profile as the *** addition to presenting the tracking control algorithm based on direct heuristic dynamic programming(dHdP),we provide a control performance guarantee including the case of constrained *** show that our proposed tracking control possesses several important properties,such as weight convergence of the learning networks,Bellman(sub)optimality of the cost-to-go value function and control input,and practical stability of the human-robot *** further provide a systematic simulation of the proposed tracking control using a realistic human-robot system simulator,the OpenSim,to emulate how the dHdP enables level ground walking,walking on different terrains and at different *** results show that our proposed dHdP based tracking control is not only theoretically suitable,but also practically useful.

Effect of lignite amendment on carbon and nitrogen mineralization from raw and composted manure during incubation with soil

The application of animal manure as a source of plant nutrients requires the determination of the amount and pattern of nutrient mineralization from manure.A laboratory incubation study was conducted to investigate the influence of lignite amendment and lignite type on carbon(C)and nitrogen(N)mineralization in raw(feedstock)and composted cattle manure following application to soil at 30 and 60 t ha^(-1).The mineralization of C and N was determined by measuring changes in CO_(2) evolution and mineral N(NH_(4)^(+)-N+NO_(3)^(-)-N)over 40 *** results showed that lignite amendment suppressed the amount of manure C mineralized in both feedstock and compost,with the effect being more pronounced in the *** the 40-d incubation,the percentage of applied C mineralized was 26.4%-27.8%and 16.3%-21.4%in unamended and lignite-amended feedstocks,*** corresponding C mineralized in the composts was 12.4%-14.1%and 3.5%-6.5%.Lignite had no significant effect on the net N mineralized in compost(4.8%-6.7%and2.5%-7.8%in unamended and lignite-amended composts,respectively).Lignite had either no effect or increased the net N mineralized in feedstock(from3.2%-8.7%without lignite to 10.4%-13.5%)depending on the type of lignite *** study suggests that using lignite-amended manure,especially when composted,has the potential to build up soil organic C without limiting the availability of mineral N.

Intrauterine insemination with donor sperm:only the number of motile spermatozoa inseminated influences both pregnancy and live-birth rates

Intrauterine insemination with donor sperm(IUI-d)is an assisted reproductive technology(ART)offered to couples with definitive male infertility or risk of genetic disease ***,we sought to evaluate our practice in IUI-d and identify factors that influenced the success *** performed a retrospective,single-center study of all IUI-d procedures performed at Lille University Medical Center(Lille,France)between January 1,2007,and december 31,*** and multivariate analyses with a mixed logistic model were used to identify factors associated with clinical pregnancies and live *** included 322 couples and 1179 IUI-d *** clinical pregnancy rate was 23.5%,and the live birth rate was 18.9%per *** a multivariate analysis,the women’s age was negatively associated with the live birth *** number of motile spermatozoa inseminated was the only factor associated with both clinical pregnancies and live births,with a chosen threshold of 0.75 *** clinical pregnancy and live birth rates were,respectively,17.3%and 13.0%below the number of motile spermatozoa inseminated threshold and 25.9%and 21.0%at or above the threshold(all P=0.005).The number of motile spermatozoa inseminated was the only factor that significantly influenced both pregnancies and live-birth rates after ***,below a threshold of 0.75 million motile spermatozoa inseminated,those rates were significantly *** of this number of motile spermatozoa inseminated threshold may help centers to allocate donations more effectively while maintaining reasonable waiting times for patients.

Background Children with rare diseases experience challenges at home and school and frequently require multi-disciplinary *** aimed to determine health service utilization by Australian children with rare diseases and barriers to access-ing *** Parents completed an online survey on health professional and emergency department(Ed)presentations,hospi-talization,and barriers to accessing *** barriers to service access included residential location(city,regional,remote)and child health-related functioning,determined using a validated,parent-completed measure-of-function *** Parents of 462 children with over 240 rare diseases completed the *** with the general population,these children were more likely to be hospitalized[odds ratio(OR)=17.25,95%confidence interval(CI)=15.50-19.20]and present to the Ed(OR=4.15,95%CI=3.68-4.68)or a family physician(OR=4.14,95%CI=3.72-4.60).Child functional impairment was nil/mild(31%),moderate(48%)or severe(22%).Compared to children with nil/mild impair-ment,those with severe impairment were more likely to be hospitalized(OR=13.39,95%CI=7.65-23.44)and present to the Ed(OR=11.16,95%CI=6.46-19.27).Most children(75%)lived in major cities,but children from regional(OR=2.78,95%CI=1.72-4.55)and remote areas(OR=9.09,95%CI=3.03-25.00)experienced significantly more barriers to healthcare access than children from major *** included distance to travel,out-of-pocket costs,and lack of specialist medical and other health *** Children with rare diseases,especially those with severe functional impairment have an enormous impact on health services,and better integrated multidisciplinary services with patient-centered care are *** must be improved for children living in rural and remote settings.
