Postphalloplasty urinary function test:an observational study of novel outcome instrument to capture urinary dysfunction and quality of life after phalloplasty

Due to growing social acceptance,there has been an increasing number of gender-affirmation surgeries performed in North *** research in this patient population focuses on surgical outcomes and advancing ***,little work has been done to study functional *** better evaluate urinary dysfunction in the postphalloplasty trans men patient population,our group developed a novel patient-reported outcome instrument-the postphalloplasty urinary function test(PP UFT)and protocol to measure postvoid urethral volume(PVUR),and we present our preliminary *** conducted a cross-sectional pilot study in a cohort of 15 adult trans men who had undergone phalloplasty with urethral lengthening surgery between 2018 and *** had stable urinary function via the neophallus at the time of *** filled out the PP UFT and were asked to record their PVUR as per our *** average PP UFT score was 8.9 out of 40 and the average quality-of-life(QOL)score was *** dribbling constituted the major complaint and on average comprised 63.2%of the reported PP UFT *** average PVUR was 2.2 ml(range:0.5-5.6 ml).There was a positive correlation between higher PP UFT and worse-reported quality of life(P<0.01;R?=0.4).Current questionnaires accepted in cis-male urology have limitations for accurately capturing urinary dysfunction in this specific patient *** combination of PP UFT and PVUR measurement offers potential for quantifying urinary function and quality of life in patients who undergo *** studies will validate these instruments.

Dental primary afferent(DPA)neurons and proprioceptive mesencephalic trigeminal nucleus(MTN)neurons,located in the trigeminal ganglion and the brainstem,respectively,are essential for controlling masticatory *** extensive transcriptomic studies on various somatosensory neurons,there is still a lack of knowledge about the molecular identities of these populations due to technical challenges in their circuit-validated ***,we employed high-depth single-cell RNA sequencing(scRNA-seq)in combination with retrograde tracing in mice to identify intrinsic transcriptional features of DPA and MTN *** transcriptome analysis revealed five major types of DPA neurons with cell type-specific gene enrichment,some of which exhibit unique mechano-nociceptive properties capable of transmitting nociception in response to innocuous mechanical stimuli in the ***,we discovered cellular heterogeneity within MTN neurons that potentially contribute to their responsiveness to mechanical stretch in the masseter muscle ***,DPA and MTN neurons represented sensory compartments with distinct molecular profiles characterized by various ion channels,receptors,neuropeptides,and ***,our study provides new biological insights regarding the highly specialized mechanosensory functions of DPA and MTN neurons in pain and proprioception.

Case report:simultaneous femtosecond laser astigmatic keratotomy and toric intraocular lens implantation in femtosecond laser-assisted cataract surgery in a patient with surgically induced high corneal astigmatism

Background:Femtosecond laser astigmatic keratotomy(FSAK)and toric intraocular lens(IOL)implantation have been studied individually for comparison to treat astigmatism at cataract *** report a case of surgically induced high corneal astigmatism by laser thermal keratoplasty(LTK)in a patient with cataract who was successfully treated with simultaneous combination of FSAK and toric IOL implantation with femtosecond laser-assisted cataract surgery(FLACS).This is the first report of both procedures combined simultaneously,with or without history of *** Description:A 68-year-old male presented with a history of LTK with two enhancements each eye in 2004,with subsequent surgically induced high corneal astigmatism,and with age-related nuclear cataract of both *** master demonstrated+7.71 diopters of astigmatism at 163 degree right eye and+3.29 diopters of astigmatism at 4 degree left *** extensive discussion of the risks and benefits,the patient agreed to undergo FLACS with FSAK with two 61 degrees of relaxation incisions(RIs)and toric IOL(Alcon SN6AT9)right eye;FLACS with toric IOL(Alcon SN6AT7)alone left *** 2-year follow-up,uncorrected visual acuity was 20/30 right eye,20/25 left *** best corrected visual acuity was 20/25(+0.25+1.00 axis 21)right eye and 20/20(plano+0.25 axis 90)left eye;his best corrected near visual acuity was J1+with add+2.50 diopters right eye and left ***:Patients with age-related cataract and LTK induced high corneal astigmatism can hardly be sufficiently treated with FSAK or toric IOL alone at the time of cataract *** effective way is to combine large FSAK and toric IOL of the highest cylindrical power of T9,in our case,simultaneously,which can achieve an excellent long term visual outcome.

Building Momentum to Realign Incentives to Support Open Science

The COVID-19 pandemic highlights the urgent need to strengthen global scientific collaboration,and to ensure the fundamental right to universal access to scientific progress and its *** Science(OS)is central to achieving these *** aims to make science accessible,transparent,and effective by providing barrier-free access to scientific publications,data,and infrastructures,along with open software,Open Educational Resources,and open *** also promotes public trust in science at a time when it has never been more important to do *** the past decade,momentum towards the widespread adoption of OS practices has been primarily driven by declarations(e.g.,DORA,the Leiden Manifesto).These serve an important role,but for OS to truly take root,researchers also must be fully incentivized and rewarded for its *** requires research funders and academic leaders to take the lead in collaborating,with researchers in designing,and implementing new incentive structures,and to actively work to socialize these throughout the research *** US National Academies of Science,Engineering,and Medicine(NASEM)Roundtable on Aligning Research Incentives for OS is one such *** paper examines the strategy behind convening the Roundtable,its current participant makeup,focus,and *** also explores how this approach might be expanded and adapted throughout the global OS community.

Scattering and diffraction of plane P-waves in a 2-D elastic half-space Ⅱ：shallow arbitrary shaped canyon

Scattering and Diffraction of elastic in-plane P-and SV-waves by a surface topography such as an elastic canyon at the surface of a half-space is a classical problem which has been studied by earthquake engineers and strong-motion seismologists for over forty years. The case of out-of-plane SH waves on the same elastic canyon that is semi-circular in shape on the half-space surface is the first such problem that was solved by analytic closed form solutions over forty years ago by Trifunac. The corresponding case of in-plane P-and SV-waves on the same circular canyon is a much more complicated problem because, the in-plane P-and SV-scattered waves have different wave speeds and together they must have zero normal and shear stresses at the half-space surface. It is not until recently in 2014 that analytic solution for such problem is found by the author in the work of Lee and Liu. This paper uses the technique of Lee and Liu of defining these stress-free scattered waves to solve the problem of the scattered and diffraction of these in-plane waves on an almost-circular surface canyon that is arbitrary in shape.

Small duct primary sclerosing cholangitis: A discrete variant or a bridge to large duct disease, a practical review

The natural history,associations with inflammatory bowel disease(IBD),and long-term outcomes of large duct primary sclerosing cholangitis(ldPSC)have been well *** duct primary sclerosing cholangitis(sdPSC)is a much less common and relatively more benign *** natural history of sdPSC has been difficult to characterize given the limited number of studies in the literature especially with regards to the subset of patients who progress to large duct *** has been unclear whether sdPSC represented a subset of ldPSC,an earlier staging of ldPSC,or a completely separate and distinct entity of its *** associations between sdPSC and IBD have been established with suspicion that concurrent sdPSC-IBD may be a key prognostic factor in determining which patients are at risk of progression to *** is known regarding the discrete circumstances that predisposes some patients with sdPSC to progress to *** has been suspected that progression to large biliary duct involvement subjects this subset of patients to potentially developing lifethreatening *** the authors conducted a thorough review of the published sdPSC literature using Pubmed searches and cross-referencing to compile all accessible studies regarding cohorts of sdPSC patients in order better characterize the subset of sdPSC patients who progress to ldPSC and the associated outcomes.

BACKGROUND There are indications that viral myocarditis,demand ischemia,and renin-angio-tensin-aldosterone system pathway activation play essential roles in troponin elevation in coronavirus disease 2019(COVID-19)*** medications and steroids are used to treat viral myocarditis,but their effect in patients with elevated troponin,possibly from myocarditis,has not been *** To evaluate the effect of dexamethasone,remdesivir,and angiotensin-converting enzyme(ACE)inhibitors(ACEI)on mortality in COVID-19 patients with elevated *** Our retrospective observational study involved 1788 COVID-19 patients at seven hospitals in Southern California,United *** did a backward selection Cox multivariate regression analysis to determine predictors of mortality in our study ***,we did a Kaplan Meier survival analysis in the subset of patients with elevated troponin,comparing survival in patients that received dexamethasone,remdesivir,and ACEI with those that did *** The mean age was 66 years(range 20-110),troponin elevation was noted in 11.5%of the patients,and 29.9%*** patients'age[hazard ratio(HR)=1.02,P<0.001],intensive care unit admission(HR=5.07,P<0.001),and ventilator use(HR=0.68,P=0.02)were significantly associated with *** the subset of patients with elevated troponin,there was no statistically significant difference in survival in those that received remdesivir(0.07),dexamethasone(P=0.63),or ACEI(P=0.8)and those that did *** Although elevated troponin in COVID-19 patients has been associated with viral myocarditis and ACE II receptors,conventional viral myocarditis treatment,including antiviral and steroids,and ACEI did not show any effect on mortality in these patients.

Bile acid signaling and biliary functions

This review focuses on various components of bile acid signaling in relation to cholangiocytes. Their roles as targets for potential therapies for cholangiopathies are also explored. While many factors are involved in these complex signaling pathways, this review emphasizes the roles of transmembrane G protein coupled receptor(TGR5), farnesoid X receptor(FXR), ursodeoxycholic acid(UDCA) and the bicarbonate umbrella. Following a general background on cholangiocytes and bile acids,we will expand the review and include sections that are most recently known(within 5–7 years) regarding the field of bile acid signaling and cholangiocyte function. These findings all demonstrate that bile acids influence biliary functions which can, in turn, regulate the cholangiocyte response during pathological events.
