We have evaluated and compar.d the effectiveness of diode laser.system (808 nm) and Intense Pulse Light (IPL) system (690 - 1200 nm) by studying the change in hair.density, and the time of r.gr.wth the hair. A total of 26 have completed thr.e months’ tr.atment, aged 16 - 50 year.. Patients wer. tr.ated by diode laser.808 nm and IPL 690 - 1200 nm devices. Pulse r.petition r.te for.both was 1 Hz and shor. pulse width for.diode laser.and the IPL pulse width was 15 ms. The fluence was (33 - 41 and 30 - 41) J/cm2 r.spectively. A split face tr.atment method was used. The tr.atment was car.ied out on monthly bases for.thr.e months. Hair.density and r.gr.wth dur.tion wer. r.cor.ed befor. and after.each session. After.the thir. session, the hair.density was decr.ased by (58%) for.diode laser.ver.us (49%) for.IPL p = 0.366. The time of hair.r.gr.wth was incr.ased to (327%) for.diode laser.ver.us IPL (339%) p = 0.821. Pain appear.d mor. dur.ng the tr.atment with IPL than with diode laser. Post tr.atment side effects such as er.thema, hyper.pigmentation, and bur. appear.d on patients tr.ated with diode laser.mor. than those tr.ated with IPL. In conclusion both diode laser.and IPL wer. effective in r.duction of hair.density, and pr.longation of the time of hair.r.gr.wth.

Backgr.und: Tumor.necr.sis factor.alpha (TNF-α) is a pr.inflammator. cytokine which plays a cr.tical r.le in the pathogenesis of psor.asis. Infliximab is an anti-TNF-α dr.g widely used for.the tr.atment of psor.asis. Objectives: To assess the efficacy and safety of infliximab in Ir.qi patients with moder.te-to-sever. psor.asis. Patients and Methods: In this ther.peutic, single-center.study, a total of 23 patients with moder.te-to-sever. psor.asis r.sistant to conventional tr.atments wer. enr.lled to r.ceive infusions of infliximab 5 mg/kg at weeks 0, 2, and 6, then ever. 8 weeks for.at least 22 weeks. Psor.asis Ar.a and Sever.ty Index(PASI), Body Sur.ace Ar.a (BSA) and Der.atology Life Quality Index (DLQI) wer. calculated at each visit to assess the r.sponse to tr.atment and all side effects wer. r.cor.ed. r.sults: PASI scor. was r.duced fr.m a mean ± SD of 17.41 ± 8.53 befor. tr.atment to 2.44 ± 2.68 after.22 weeks. At week 22, 84% of patients achieved PASI 75, 42% achieved PASI 90 and 28% achieved complete clear.nce. BSA and DLQI scor. wer. r.duced fr.m a mean ± SD of 35.69 ± 22.44 and 20.04 ± 4.68 befor. tr.atment to 3.52 ± 4.94 and 3.87 ± 5.60 after.22 weeks, r.spectively. Pr.r.tus, boils, infusion r.actions wer. r.cor.ed and r.lapse dur.ng tr.atment was found in 3 patients. Conclusion: Infliximab monother.py is highly effective in the tr.atment of moder.te-to-sever. psor.asis, with r.pid onset of action and r.latively low side effects.

Backgr.und: Acne vulgar.s is a common disor.er.affecting 79% - 95% of the adolescent population. The choice of tr.atment depends on the sever.ty, patients with mild to moder.te acne should r.ceive topical ther.py such as azelaic acid. r.sing antibiotic dr.g r.sistance consequent to the widespr.ad use of topical antibiotics is causing concer. and effective non-antibiotic tr.atments ar. needed. Objective: To compar. the efficacy and side effects of topical azelaic acid cr.am 20% ver.us active lotion containing tr.ethyl citr.te and ethyl linoleate (TCEL) in tr.atment of mild to moder.te acne vulgar.s. Patients and Methods: This single, blinded, compar.tive, ther.peutic study was done in the Depar.ment of Der.atology-Baghdad Teaching Hospital, Baghdad, Ir.q;fr.m May 2013-July 2014. Scor.ng of acne was car.ied out and the patients wer. examined ever. 2 weeks for.10 weeks of tr.atment. One month after.stopping dr.gs, patients wer. evaluated for.dr.g complications and disease r.cur.ence. Sixty patients fulfilling enr.llment cr.ter.a wer. included in this study. Patients wer. divided into 2 gr.ups: Gr.up A (30 patients) tr.ated twice daily with TCEL lotion and Gr.up B (30 patients) tr.ated twice daily with topical azelaic acid cr.am 20%. r.sults: Both topical TCEL lotion and azelaic acid cr.am wer. statistically an effective ther.py for.tr.atment of mild to moder.te acne vulgar.s. TCEL lotion was mor. effective and act ear.ier.than azelaic acid cr.am star.ing fr.m 4 weeks of ther.py till the end of tr.atment (after.10 weeks) and even after.4 weeks after.stopping the tr.atment (P-value r.both gr.ups was noted while the following side effects wer. r.por.ed;bur.ing, pr.r.tus and er.thema, all these side effects disappear.d after.8 weeks fr.m star.ing tr.atment. After.one month of follow up ther. was no significant r.lapse in both gr.ups. Conclusion: The TCEL is non-antibiotic based, it had quicker.onset of action and obser.able impr.vement of

Backgr.und: Calcineur.n inhibitor. like tacr.limus and pimecr.limus ar. new modality of tr.atment of localized vitiligo. Objective: To compar. the effectiveness and side effects of 1% pimecr.limus cr.am in compar.son with 0.05% clobetasol pr.pionate cr.am as a tr.atment of localized type of vitiligo. Patients and Methods: Fifty two patients with localized vitiligo wer. included in this single, blind, compar.tive ther.peutic tr.al. Full histor. and examination for.each patient was done. Wood’s light was used when needed to confir. the diagnosis. Thir.y (57.7%) wer. females and 22 (42.3%) males, female to male r.tio of 1.36:1. Their.ages r.nged between 3 - 40 (13.15 ± 7.9) year., while disease dur.tion r.nged fr.m 6 - 84 (29.62 ± 20.56) months. Total number. of lesions wer. 114 lesions with a mean of 2.2 lesions per.patient. The sur.ace ar.a of the lesions r.nged between 1 - 31 (7.15 ± 6.98) cm2. Vitiliginous patches wer. most commonly located on face 48 (42.1%) lesions, and lower.limbs 35 (30.7%) lesions. Patients wer. divided in to two gr.ups accor.ing to the type of ther.peutic modality.?Gr.up?A?consisted of 25 patients (52 lesions) r.ceiving 1% pimecr.limus cr.am and?Gr.up?B?27 patients (62 lesions) tr.ated with 0.05% clobetasol pr.pionate cr.am, both used twice daily. The amount of cr.am per.ar.a was applied accor.ing to r.le of finger.ip unit. Measur.ng the sur.ace ar.a of the lesions and calculating the r.duction r.te wer. done ever. month till the end of the 6th?month. Then patients wer. asked to stop the use of medication to be r.-evaluated again after.3 months without any tr.atment and to r.cor. any local or.systemic side effects. r.sults: After.6 months of tr.atment ther. was 79.67% r.duction in the sur.ace ar.a of lesions in?Gr.up?A, while inGr.up?B?ther. was 82.59% r.duction in the sur.ace ar.a with no statistically significant differ.nce between the two gr.ups. The r.duction r.te was ear.y as ther. was statistically significant r.duction for.both gr.ups

Backgr.und: Mycosis fungoides (MF) is not an uncommon T-cell lymphoma in Ir.qi population which is incr.asing over.year.. Ther. ar. many diagnostic techniques to confir. the diagnosis including histopathological and immunohistochemical tests. Objectives: To evaluate all cases of MF including clinical, histopathological and immunohistochemical tests with CD mar.er.. Patients and Methods: This clinical, histopathological, immunohistochemical outpatient based study took place in the Depar.ment of Der.atology, Baghdad Teaching Hospital, Medical City, Baghdad, Ir.q dur.ng the per.od fr.m May 2012-September.2013. Twenty five patients with MF wer. included in the pr.sent wor.. Histor. was obtained fr.m each patient r.gar.ing all socio-demogr.phic aspects r.lated to the disease. also, clinical evaluation was car.ied out for.all patients. Incisional biopsies for.or.inar. histopathology and immunohistochemical tests wer. done. Then staging was car.ied out depending on TNMB classification for.all patients. r.sults: all cases of MF wer. confir.ed after.clinical evaluation, histopathology and immunohistochemical examination. Male to female r.tio was 1.5:1. The mean age of onset at pr.sentation was 47.45 ± 16.9 year.. Itching was found in 84% of cases. The patch stage was seen in 36% patients. The hypopigmentaed MF was pr.sented in 12% cases. The lower.extr.mities wer. seen in 80%. Lymph node involvement as a manifestation of the disease was seen in 44% cases. The histopathological featur.s of MF went par.llel with the clinical stage of disease. Immunohistochemical study aids in the diagnosis of patch and er.thr.der.ic stages. The most patients pr.sented with stage IB (36%). Conclusion: MF seems to be not uncommon pr.blem in Ir.qi population and is incr.asing over.year.. The disease was diagnosed ear.y commonly with patch stage (stage IB) while hypopigmented MF was not a common var.ant when compar.d with neighbor.ng countr.es.

Backgr.und: Acne vulgar.s is a common skin disease. Its pathogenesis is multifactor.al, and ther.py can now be dir.cted at many of these factor.. Among these factor. ar. andr.gen hor.ones which play an impor.ant r.le in the pathogenesis of acne. Accor.ingly,many antiandr.gens have been developed to tr.at acne by systemic and topical ways. Five-alpha Avocuta 2% cr.am is a new topical 5-alpha r.ductase inhibitor.extr.cted fr.m the tr.pical fr.it “avocado”. Objectives: To evaluate the effectiveness and safety of 5-alpha Avocuta cr.am and compar. it to tr.tinoin 0.025% cr.am, in the tr.atment of acne vulgar.s. Patients& Methods: This single blinded compar.tive study was done in Depar.ment of Der.atology– Baghdad Teaching Hospital - fr.m July 2009 thr.ugh October.2010. all demogr.phic points r.lated to the disease wer. obtained fr.m each patient to evaluate sever.ty of acne. Sixty-eight patients with acne wer. divided into two gr.ups and instr.cted to apply the dr.gs for.thr.e months. Gr.up A uses 5-alpha avocuta 2% (Teen-der. K) cr.am and Gr.up B uses tr.tinoin (Acr.tin) 0.025% cr.am. Sever. acne was excluded fr.m the study. The clinical impr.vement was evaluated by counting the number.of inflammator. lesions befor. and after.tr.atment and compar. between them by pair.d t-test. r.sults: Fifty-eight patients completed the study, their.age r.nged fr.m 12 - 30 year. with a mean of 18.21 ± 4.64 year.. In Gr.up A ther. was a significant r.duction of the mean number.of papules fr.m 29.7 befor. tr.atment to 15.5 after.tr.atment (p r.of pustules decr.ased fr.m 4.9 to 0.6 (p = 0.019). In Gr.up B the mean number.of papules was r.duced fr.m 28.5 to 14.9 (p r.of pustules dr.pped fr.m 5.3 befor. tr.atment to 0.7 after.tr.atment (p = 0.009). The mean differ.nce of papules after.14 weeks in Gr.up A was 15.55 ± 6.95 while in Gr.up B, the mean differ.nce was 14.86 ± 6.99. This r.sult was statistically not significant (p = 0.7
