Assessment of the correlation between serum prolidase and alpha-fetoprotein levels in patients with hepatocellular carcinoma

AIM: To determine the predictive value of increased prolidase activity that reflects increased collagen turnover in patients with hepatocellular carcinoma(HCC).METHODS: Sixty-eight patients with HCC(mean age of 69.1 ± 10.1), 31 cirrhosis patients(mean age of59.3 ± 6.3) and 33 healthy volunteers(mean age of51.4 ± 12.6) were enrolled in this study. Univariate and multivariate analysis were used to evaluate the association of serum α-fetoprotein(AFP) values with HCC clinicopathological features, such as tumor size,number and presence of vascular and macrovascular invasion. The patients with HCC were divided into groups according to tumor size, number and presence of vascular invasion(diameters; ≤ 3 cm, 3-5 cmand ≥ 5 cm, number; 1, 2 and ≥ 3, macrovascular invasion; yes/no). Barcelona-clinic liver cancer(BCLC)criteria were used to stage HCC patients. Serum samples for measurement of prolidase and alphafetoprotein levels were kept at-80 ℃ until *** levels were measured spectrophotometrically and AFP concentrations were determined by a chemiluminescence immunometric commercial diagnostic ***: In patients with HCC, prolidase and AFP values were evaluated according to tumor size, number,presence of macrovascular invasion and BCLC staging classification. Prolidase values were significantly higher in patients with HCC compared with controls(P <0.001). Prolidase levels were significantly associated with tumor size and number(P < 0.001, P = 0.002,respectively). Prolidase levels also differed in patients in terms of BCLC staging classification(P < 0.001).Furthermore the prolidase levels in HCC patients showed a significant difference compared with patients with cirrhosis(P < 0.001). In HCC patients grouped according to tumor size, number and BCLC staging classification, AFP values differed separately(P = 0.032,P = 0.038, P = 0.015, respectively). 目的:通过深度增强成像光断层扫描(EDI-OCT)技术以探究体温如何影响脉络膜厚度。方法:前瞻性研究。通过EDI-OCT检测9∶00~17∶00中心凹(SF-CT),中心凹鼻侧500μm(N-CT)和中心凹颞侧500μm(T-CT)正常人(41例)的脉络膜厚度(CT)。使用非接触式红外测温仪逐时检测体温(BT),以评估CT和BT日间变化的相关性。结果:SF-CT值在9∶00和13∶00(P=0.021),9∶00和14∶00(P=0.012),9∶00和16∶00(P=0.048),及9∶00和17∶00(P=0.002)之间存在显著性差异。N-CT值在8h内(均P>0.05)无显著性变化,9∶00和13∶00(P=0.004)则存在明显差异。结论:CT与BT从9∶00~17∶00之间逐时变化无显著性差异。

Comparison of Fractional Flow Reserve-Guided Revascularization Strategies in Isolated Proximal Left Anterior Descending Coronary Artery Disease

The data about FFR-guided revascularization in isolated proximal LAD disease are limited and studies comparing long-term outcomes of FFR-guided PCI versus FFR-guided CABG in single-vessel proximal LAD disease are lacking. We aimed to assess the 4-year long-term safety and effectiveness of fractional flow reserve (FFR)-guided percutaneous coronary intervention (PCI) and FFR-guided coronary artery bypass graft surgery (CABG) for the treatment of proximal left anterior descending (LAD) lesions. The study included 129 patients with functionally significant (FFR ≤ 0.80) isolated proximal LAD stenosis (PCI, 88 patients vs. CABG, 41). Clinical endpoints were assessed by Kaplan-Meier method and compared by the log-rank test. At a mean follow-up time of 47 ± 12 months, a higher incidence of myocardial infarction in the PCI group (PCI: 32% vs. CABG: 15%;p = 0.003) and a higher incidence of stroke in the CABG group (CABG: 3 (7%) vs. PCI 0 (0%);p = 0.031) were observed. However, there were no significant differences in the primary composite endpoint, death and target vessel revascularization between PCI and CABG groups. The PCI and CABG in isolated proximal LAD lesions yielded similar long-term outcomes regarding the primary composite clinical endpoints. However, stroke was more frequent in the CABG group than in the PCI group.

Hypoxia and cytokines regulate carbonic anhydrase 9 expression in hepatocellular carcinoma cells in vitro

AIM: To study the expression of carbonic anhydrase(CA) 9 in human hepatocellular carcinoma(HCC) ***: We studied CA9 protein, CA9 m RNA and hypoxia-inducible factor-1 alpha(HIF-1α) protein levels in Hep3 B cells exposed in different parallel approaches. In one of these approaches, HCC cells were exposed to extreme in vitro hypoxia(24 h 0.1% O2) without or with interleukin(IL)-1, IL-6, tumor necrosis factoralpha(TNF-α) and transforming growth factor-beta(TGF-β) stimulation for the same hypoxic exposure time or exposed to normoxic oxygenation conditions without or with cytokine ***: The tumour cell line analysed showed a strong hypoxic CA9 m RNA expression pattern in response to prolonged severe hypoxia with cell-line specific patterns and a marked induction of CA9 protein in response to severe hypoxia. These results were paralleled by the results for HIF-1α protein under identical oxygenation conditions with a similar expression tendency to that displayed during the CA9 protein expression experimental series. Continuous stimulation with the cytokines, IL-1, IL-6, TNF-α and TGF-β, under normoxic conditions significantly increased the carbonic anhydrase 9 expression level at both the protein and m RNA level, almost doubling the CA9 m RNA and CA9 and HIF-1α protein expression levels found under hypoxia. The findings from these experiments indicated that hypoxia is a positive regulator of CA9 expression in HCC, and the four signal transduction pathways, IL-1, IL-6, TNF-α and TGF-β, positively influence CA9 expression under both normoxic and hypoxic ***: These findings may potentially be considered in the design of anti- cancer therapeutic approaches involving hypoxia-induced or cytokine stimulatory effects on expression. 背景在此项研究中，我们评估了应用苯妥英钠预防癫痫的患儿行开颅手术中输注丙泊酚对其肝、胰酶和酸碱平衡的影响。方法在这项前瞻性的临床研究中，我们测定了30例4—12岁患儿的血清谷草转氨酶（AST）、谷丙转氨酶（ALT）、γ-谷氨酰转肽酶（GGT）、碱性磷酸酶（ALP）、胰淀粉酶、胰脂肪酶及三酰甘油水平。患儿接受丙泊酚麻醉并应用苯妥英钠预防癫痫。以前已用苯妥英钠的患儿则继续用药。未曾用药者口服苯妥英钠5mg·kg^-1·d^-1。经医院批准于术前1天，术后第1、3、5、7天测定患儿血清中AST、ALT、GGT、ALP、胆红素、胰淀粉酶、胰脂肪酶、三酰甘油水平。分别于气管插管后、术中2小时和术中4小时、拔管以及拔管后1、2、6、12小时采集动脉血气样本。结果术后血清AST、ALT、GGT、ALP、胰淀粉酶、胰脂肪酶、三酰甘油水平显著高于术前基础值，且于术后第1天达峰值，并在1周内恢复正常。拔管后碱剩余与基础值相比显著降低，但仍在正常范围内，并于术后6小时恢复到基础值水平。患儿并未表现出肝炎或胰腺炎的临床症状或体征。胆红素水平正常。患儿在术后4—6个月内未发生肝胰相关的并发症。结论尽管开颅术患儿肝、胰酶在术后轻微上升，但丙泊酚全麻维持未对其酸碱平衡或肝、胰酶产生临床意义的显著影响。

学术讲座作为高等院校学生获取知识的主要途径之一,对学生而言具有不可或缺的重要性。为了确保学生能够充分吸收讲座内容,做笔记成为了一项至关重要的技能。事实上,记笔记的能力以及笔记的质量与学生的学习成绩之间存在着密切的关联。因此,在EAP课程中,记笔记教学被视为一个不可或缺的组成部分,旨在培养学生良好的笔记习惯,从而助力他们取得更好的学术成绩。本研究旨在构建一个基于体裁的教学框架(Genre-Based Instruction,GBI),用于指导笔记教学的实践。为实现这一目标,本研究对八个跨学科讲座进行了深入分析。首先,我们依据Bouziri(2020)所建立的框架,对修辞功能进行了明确界定。随后,我们采纳了?del(2010)与Hyland(2005)共同提出的元话语框架,并据此确定了体现组织结构的元话语要素。经过深入研究,我们发现了修辞功能与元话语要素之间存在紧密的关联。也就是说,某些元话语项目可以作为使用某些修辞功能的信号。因此,记笔记的GBI框架是根据修辞功能与元话语要素之间的关联而构建的。

A candidate identification questionnaire (CIQ) was tested to determine its predictive value for patient-reported satisfaction in patients switched from once-weekly or once-daily treatment with a bisphosphonate to once-monthly dosing. This was a prospective, open-label, multicenter international study in patients with postmenopausal osteoporosis who had been receiving once-daily or once-weekly alendronate or risendronate for at least 3 months. Patients completed a CIQ, then commenced 150 mg monthly ibandronate for 6 months. Patients completed the Osteoporosis Patient Satisfaction Questionnaire (OPSAT-QTM) at baseline for 6 months. Scores were converted to composite satisfaction scores (CSS, scale 0-100). Totally 677 patients completed a CIQ, 645 were enrolled in the treatment phase and comprised the intent-to-treat (ITT) population, and 630 completed the study. In the ITT population, 68.1% patients answered “yes” to one or more CIQ questions. OPSAT-Q scores increased for the convenience, quality of life and overall satisfaction domains (p scores for the side effects domains were significant (p < 0.001) in the CIQ “yes” group, but not for the degree of bother (decrease in mean of 0.1 points, p = 0.50) or duration (no change, p = 0.84) of non-gastrointestinal side effects. Of 638 patients who completed the preference questionnaire, 93.0% of patients preferred the once-monthly dosing schedule and 563 patients (90.7%) found it more convenient. The most common adverse events were dyspepsia (1.9%), nausea (1.1%), and upper abdominal pain (0.9%). Patients are likely to prefer treatment with monthly ibandronate to a weekly or monthly bisphosphonate irrespective of their stated preference before switching treatment.

地图识读能力、解决问题的反思性思维能力,二者之间有何关系,自然教育对二者有何影响,以此为研究目标,根据探索性序列混合研究设计,以36名七年级学生为研究样本,使用地图识读量表、解决问题的反思性思维能力量表,并设计半结构式访谈表、活动手册、学生工作表收集数据。定量研究结果显示,自然教育之下,学生的地图识读能力、解决问题的反思性思维能力都有了明显提高,并且两种能力之间存在很强的相关性。对参与学生进行的访谈也证实了以上研究发现。基于以上发现,研究人员建议教师投入更多时间进行自然教育,并在课程中普及自然教育。
