Predicting adverse maternal and perinatal outcome after threatened miscarriage

Objectives: Threatened miscarriage is the most common complication of pregnancy, occurring in 20% - 25% of ongoing pregnancies. The purpose is to study maternal and perinatal outcome in women with threatened miscarriage. Methods: A prospective study was conducted over a period of 20 months in the Department of Obstetrics and Gynaecology, Pannadhay Mahila Chikitsalaya, a tertiary care center in Udaipur, India. It included a cohort of pregnant patients with a history of threatened miscarriage during the first twenty weeks of pregnancy. They were registered, followed up prospectively at antenatal clinics and delivered in the same hospital. For the purpose of comparison, another group of booked patients (controls) without a history of threatened miscarriage were recruited, similarly scanned and followed throughout pregnancy and labour. All women were matched for age, parity, social class, BMI and gestational age at booking. Discrete variables were taken as counts (or frequencies) and were evaluated by Chi-square test. Continuous variables with normal distribution were presented as mean ± standard deviation (sD) and were compared by unpaired student’s t test. Data were fed into a Microsoft Excel worksheet and were analyzed by using the sPss ver.17 (sPss Inc., Chicago, IL, UsA). p-value > 0.05 was considered statistically significant. Results: Out of 62 cases, 13 (21%) patients spontaneously aborted after diagnosis of threatened miscarriage. Women with threatened miscarriage had a significantly higher incidence of low lying placenta (p = 0.02) when compared with those without firsttrimester bleeding. They were more likely to experience PROM (p = 0.02), preterm delivery (p = 0.02) and to have babies with low Birthweight (p = 0.03). Conclusion: Pregnancies complicated by threatened miscarriage are at a higher risk for obstetric complications.

Effect of Projectile Break-Up Threshold Energy on Incomplete Fusion at Energy ≈ 4 - 7 MeV/Nucleon

Complete and incomplete fusion cross sections have been measured for the 20Ne + 51V system to investigate the influence of projectile break-up at or above the barrier energies. The excitation function of the individual reaction channel measured using off-line γ-ray spectroscopy has been compared with those estimated theoretically using the statistical model code PACE4. A significant fraction of incomplete fusion has been found in the production of residues involving α-particle in the exit channel. In order to understand the energy dependence of incomplete fusion reactions, incomplete fusion fraction has been deduced, which has also been compared with the earlier data available for 12C and 16O projectiles to look for the projectile structure dependence.

Plasmid vector based generation of transgenic mesenchymal stem cells with stable expression of reporter gene in caprine

The production of cells capable of expressing gene(s) of interest is important for a variety of applications in biomedicine and biotechnology, including gene therapy and a novel method of stem cell therapy in the various diseases. Achieving high levels of transgene expression for the longer period of time, without adversely affecting cell viability and differentiation capacity of the cells, is crucial. In the present study, we investigated the efficiency of plasmid vector for the production of transgenic cMsCs and examined any functional change of cells after transfection. To do so first we have collected bone marrows from the adult goats and cultured them for isolation of mesenchymal stem cells (cBM-MsCs). These cells were characterized using MsC specific markers including differentiation into osteocytes and adipocytes. Transfection with plasmid vector did not adversely affect cBM-MsCs morphology, viability or differentiation potential, and transgene expression levels were unaffected beyond passage 12th. The results indicated that we have been able to generate transgenic caprine MsC (tcBM-MsC) and transfection of cBM-MsCs using plasmid vector resulted in very high and stable transfection efficiency. This finding may have considerable significance in improving the efficacy of MsC-based therapies and their tracking in animal model.

Investigation of lnter and Intra-fractional Uncertainties in Lung IMRT Delivery

Changes in the tumour position due to inter and intra-fractional motion introduce uncertainty in IMRT delivery. In this work, we experimentally evaluated the effect of inter and intra-fractional motion uncertainties in the IMRT delivery. A lung patient was planned with rive field dynamic IMRT for a total dose of60 Gy in 30 fractions using 6 MV photon beam at a dose rate of 400 MU/min. The plan was delivered on varian CL2100 CD linear accelerator with millennium 120 MLC for a single fraction and measured using IMRT Matri XX placed on Quasar motion platform, aligned with respect to isocentre for planar as well as point dose measurements. This measurement was summated for 30 fractions and taken as a reference. 30 measurements were performed for each inter-fractional, intra-fractional and combined effect. The setup errors(Mean+sD) used to simulate the inter-fractional displacements were RL:-0.10+0.27 mm, sI: 0.17+0.45 mm, AP:-0.04+0.38 mm and Rot: 0.02+0.86 degree. The intra-fractional motion was simulated using the motion platform parallel to the MLC leaf motion, for an amplitude of 1 cm and a period of 4 s. The planner fluence of inter, intra-fractional motion and combination of both was analyzed against the reference using gamma criteria of 3%/3 mm. similarly, the point dose measurements were also compared. The maximum deviation in point dose during a single fraction was-3.8%, 3.1% and-5.9% for inter-fractional, intra-fractional and combined respectively. The same resulted in deviation of-0.4%,-0.3% and 0.1% respectively when summated for 30 *** percentage of pixels failing the gamma criteria during a single fraction was28.0%, 16.4% and 28.5% for inter-fractional, intra-fractional and combined respectively. The same revealed 11.4%, 12.4% and 19.2% respectively when summated. Though the point dose deviations were nullified over 30 fractions, the planner fluence variation was observed to be considerable.

studies on the Internal Ballistics of Composite solid Rocket Propellants Incorporating Nano-structured Catalysts

This paper deals with the analysis of burn rate using various catalysts of Iron Oxide and determining which gives the higher burn rate with low pressure variation. The Ammonium Perchlorate (AP) was obtained and ground into fine powder with the particle size ranging from 63 to 125 μm. The propellant strands were prepared with proportions by mixing AP with the binder (Hydroxyl Terminated Polybutadiene), the catalyst (Iron Oxide), curing agent (Isophorone diisocyanate) and the plasticizer (Dioctyladipate). The prepared propellant mixture was cured at around 63 deg C to get various propellant strands. The first strand was prepared with the absence of a catalyst to set an initial base of comparison with other Iron Oxide catalysts, namely, Flower shaped, Micro and Nano, based on the size of the particles. The combustion process was carried out in a strand burner, which was in turn connected to a data acquisition system. The obtained output was analysed in the form of graphs. The burn rate was achieved by calculating the slope of the graph i.e . by calculating the difference between the highest and the lowest peak of the graph and dividing the total time by the answer. The experiment was repeated with the different catalyst types, as mentioned above, at different pressures. It was observed that the Nano shaped Iron Oxide exhibits better burning characteristics when compared to the rest with the pressure index of 0.792. In this paper, the various experiments carried out along with their procedures are explained in detail. The results obtained and the techniques used are also elaborately described in this paper.

Background and study aims: Endoscopic biliary drainage is an established mode of treatment for acute cholangitis. We compared the safety and efficacy of nasobiliary drain (NBD) placement and stent placement for biliary drainage in patients with acute cholangitis. Patients and methods: We recruited a total of 150 patients with severe cholangitis who required endoscopic biliary drainage. Patients were randomized to have either a 7-Fr NBD or a 7-Fr straight flap stent placed during endoscopy. Outcome measures included complications related to endoscopic retrograde cholangiopancreatography (ERCP) and the clinical outco- me. Results: Of the 150 patients, 75 were randomized to the NBD group and 75 to the stent group. The most common causes of biliary obstruction were common bile duct stones (n = 102) and biliopancreatic malignancies (n = 37). The site of the biliary obstruction was predominantly found to be the lower part of common bile duct in both the NBD group (n = 58) and the stent group (n = 59). Indications for biliary drainage were: a fever of > 100.4°F (n = 140), hypotension (n = 23), peritonism (n = 40), impaired consciousness (n = 29), and failure to improve with conservative management (n = 45). Biliary drainage was achieved in 147 patients. Abdominal pain, fever, jaundice, hypotension, peritonism and altered sensorium improved after a median period of 2 days in both groups. Leukocyte counts became normal after a median time of 7 days in the NBD group and 6 days in the stent group. There were no ERCP-related complications. There were no instances of displacement or kinking of an NBD, occlusion of an NBD or stent, or of stent migration. Four patients died (two in the NBD group and two in the stent group) as a result of uncontrolled cholangitis after 1, 2, 4, and 6 days of biliary drainage. The success rates of biliary drainage in cholangitis were not affected by the type of endoprosthesis used (72/74 for NBD patients vs. 71/73 for stent patients), the etiology o
