A Modified Enhanced Recovery after Surgery (ERAS): Use and Surgical Outcome in Breast Cancer Patients

Background: ERAS protocols are perioperative interventions aimed at reducing postoperative complications, length of hospital stay (LOS) and early return to normal activities. This has improved outcome in many surgical specialties, including breast surgery. We present the surgical outcome of breast cancer (BC) patients treated over a 12-month period following the principle of ERAS protocols and highlight the underpinning evidence. Methods: A retrospective analysis of all BC patients diagnosed and had breast cancer surgery over 12 months. Data collected included patient’s demographics, type of surgery, LOS, other perioperative care and significant postoperative complications. Excluded were patients with bilateral cancer surgeries, diagnostic excision, margin clearance or breast reconstruction. Results: There were 621 BC diagnosed including 5 male and 12 bilateral female BC. The ages ranged from 25 to 93 years. Excluding bilateral BC, 351 patients (70.2 %) had breast conserving surgery (BCS) while 149 (29.8%) patients had mastectomy as index cancer surgery. Sixteen (4.5%) of the women who initially underwent BCS subsequently had a completion mastectomy. The overall rate of successful BCS was 335/500 (67%). 441 (85.5%) of patients were discharged same or next day. 12 (7.2%) cases of postoperative haematoma, 6 cases of wound infection and a case of seroma requiring surgical/radiological drainage recorded. Conclusion: ERAS protocol in BC surgery is associated with decreased LOS and low complication rate. Delayed discharges are mostly due to adverse social factors and medical comorbidity rather than post-operative surgical complications.

Estimation of actual carbon dioxide removal in burned forests using satellite data:A case study in South Korea

With the increasing impact of climate change,carbon emissions and removals have become major *** are major carbon pools,and forest fires are an essential part of the carbon *** study introduces a model for estimating the detailed actual CO_(2)removal in burned forests using burn severity and tree *** CO_(2)removal was estimated from empirical yield tables without using the standard carbon removal provided by the national *** primary CO_(2)calculation method followed the guidelines of the International Panel on Climate *** burn severity was mapped using Sentinel-2 multispectral instrument data,and the survivability of each forest type was estimated according to burn *** survivability was applied to the pre-fire CO_(2)removal of each forest to estimate post-fire CO_(2)*** our case study,the burned forest area was 1,034 ha,and the CO_(2)removal before the fire was 8,615.3t/*** the fire,removal decreased by 81.2%to 1,618.4 t/*** particular,the decrease in coniferous forests was high,more than 86%.The lack of survivability data on burned trees was a major limitation of our *** accumulating field monitoring data of post-fire forests will be necessary for future research and could serve as a reference for devising immediate countermeasures against forest fires.

Heat of absorption of CO_2 in aqueous N,N-diethylethanolamine+ N-methyl-1,3-propanediamine solutions at 313 K

Differential heat of absorption of CO_2 in aqueous solutions of N,N-diethylethanolamine(DEEA) and activated DEEA solutions up to a total concentration of 2 mol·L^(-1) was measured as a function of CO_2 loading at 313.15 K using a reaction calorimeter. In order to analyze the performance of N-methyl-1,3-propanediamine(MAPA)as an activator, DEEA blended solutions containing 0.05, 0.1 and 0.2 mol·L^(-1) MAPA were studied. The heat of CO_2 absorption in single DEEA solutions was unaffected by changing the DEEA concentration in the range of(0.5–2) mol·L^(-1). On the other hand, increasing the concentration of MAPA in aqueous amine mixtures of(DEEA + MAPA) raised the heat of absorption.

Oscillating-grid turbulence at large strokes: Revisiting the equation of Hopfinger and Toly

A quasi-isotropic, quasi-homogeneous turbulence generated by an oscillating-grid, spatially decays according to power law of u ∝ Z-nM , where u is the root mean square （rms） horizontal velocity, Z is the vertical distance from the grid and nu = 1. However, the findings of Nokes and Yi indicate that as the stroke of oscillation increases, the power law n u= 1 and does not follow the established decay law equation of Hopfinger. This paper investigates the characteristics of the turbulence that are generated using larger strokes S/M = 1.6 and 2 and compares with that obtained using a S/M = 0.8, which is the stroke used when the equation was developed. Measurements of the grid-generated turbulence in a water tank were taken using particle image velocimetry （PIV）. The results showed that the homogeneity occurred at distance beyond 2.5 mesh spacings away from the grid midplane, independent of the stroke and the frequency of oscillation. Within this region, the turbulent kinetic energy distribution was quasi-homogeneous, and the secondary mean flow is negligible. The statistical characteristics of the measured turbulence confirmed that although nu decreases as stroke increases, the grid-turbulence generated at S/M = 1.6 and 2 obeys the universal decay law.

Strong internal resonance in a nonlinear, asymmetric microbeam resonator

Exploiting nonlinear characteristics in micro/nanosystems has been a subject of increasing interest in the last *** others,vigorous intermodal coupling through internal resonance(IR)has drawn much attention because it can suggest new strategies to steer energy within a micro/nanomechanical ***,a challenge in utilizing IR in practical applications is imposing the required frequency commensurability between vibrational modes of a nonlinear micro/***,we experimentally and analytically investigate the 1:2 and 2:1 IR in a clamped–clamped beam resonator to provide insights into the detailed mechanism of *** is demonstrated that the intermodal coupling between the second and third flexural modes in an asymmetric structure(e.g.,nonprismatic beam)provides an optimal condition to easily implement a strong IR with high energy transfer to the internally resonated *** this case,the quadratic coupling between these flexural modes,originating from the stretching effect,is the dominant nonlinear mechanism over other types of geometric *** design strategies proposed in this paper can be integrated into a typical micro/nanoelectromechanical system(M/NEMS)via a simple modification of the geometric parameters of resonators,and thus,we expect this study to stimulate further research and boost paradigm-shifting applications exploring the various benefits of IR in micro/nanosystems.

Nanomechanical Characterization of Bone Quality Depending on Tissue Age via Bimodal Atomic Force Microscopy

Modulators affecting the immune dialogue between human immune and colon cancer cells

The link between chronic inflammation and colorectal cancer has been well established. The events proceeding along tumorigenesis are complicated and involve cells activated at the cancer microenvironment, tumor infiltrating polymorphonuclears, immune cells including lymphocyte subtypes and peripheral blood mononuclear cells(PBMC), as well as tumor-associated macrophages. The immune cells generate inflammatory cytokines, several of them playing a crucial role in tumorigenesis. Additional factors, such as gene expression regulated by cytokines, assembling of tumor growth- and transforming factors, accelerated angiogenesis, delayed apoptosis, contribute all to initiation, development and migration of tumor cells. Oxygen radical species originating from the inflammatory area promote cell mutation and cancer proliferation. Tumor cells may over-express pro-inflammatory mediators that in turn activate immune cells for inflammatory cytokines production. Consequently, an immune dialogue emerges between immune and cancer cells orchestrated through a number of activated molecular pathways. Cytokines, encompassing migration inhibitory factor, transforming growth factor beta 1, tumor necrosisfactor-α, Interleukin(IL)-6, IL-10, IL-12, IL-17, IL-23 have been reported to be involved in human cancer development. Some cytokines, namely IL-5, IL-6, IL-10, IL-22 and growth factors promote tumor development and metastasis, and inhibit apoptosis via activation of signal transducer activator transcription-3 transcription factor. Colon cancer environment comprises mesenchymal, endothelial and immune cells. 3D X-ray microscopy with a CsPbBr3 nanowire scintillator

X-ray microscopy is an essential imaging method in many scientific fields,which can be extended to three-dimensional(3D)using ***,metal halide perovskite(MHP)nanomaterials have become a promising candidate for X-ray scintillators,due to their high light yield,high spatial resolution,and easy *** requires many projections and therefore scintillators with excellent *** is challenging for MHPs,which often suffer from fast degradation under X-ray irradiation and ambient ***,we demonstrate that MHP scintillators of CsPbBr3 nanowires(diameter:60 nm,length:5–9μm)grown in anodized aluminum oxide(CsPbBr3 NW/AAO)have sufficient stability for X-ray micro-tomography.A tomogram was taken with a Cu X-ray source over 41 h(dose 4.2 Gyair).During this period the scintillator brightness fluctuated less than 5%,which enabled a successful reconstruction.A long-term study with 2 weeks of continuous X-ray exposure(37.5 Gyair)showed less than 14%fluctuations in brightness and no long-term degradation,despite variations in the ambient relative humidity from 7.4%RH to 34.2%*** resolution was stable at(180±20)lp·mm−1,i.e.,about 2.8 *** demonstrates that CsPbBr3 NW/AAO scintillators are promising candidates for high resolution X-ray imaging detectors.

Chemical interaction motivated structure design of layered metal carbonate hydroxide/MXene composites for fast and durable lithium ion storage

Rational architecture design has turned out to be an effective strategy in improving the electrochemical performance of electrode materials for ***,an elaborate structure that could simultaneously endow active materials with promoted reaction reversibility,accelerated kinetic and restricted volume change still remains a huge ***,a novel chemical interaction motivated structure design strategy has been proposed,and a chemically bonded Co(CO_(3))_(0.5)OH·0.11 H_(2)O@MXene(CoCH@MXene)layered-composite was fabricated for the first *** such a composite,the chemical interaction between Co^(2+)and MXene drives the growth of smaller-sized CoCH crystals and the subsequent formation of interwoven CoCH wires sandwiched in-between MXene *** unique layered structure not only encourages charge transfer for faster reaction dynamics,but buffers the volume change of CoCH during lithiation-delithiation process,owing to the confined crystal growth between conductive MXene layers with the help of chemical ***,the sandwiched interwoven CoCH wires also prevent the stacking of MXene layers,further conducive to the electrochemical performance of the *** a result,the as-prepared CoCH@MXene anode demonstrates a high reversible capacity(903.1 mAh g^(-1)at 100 mA g^(-1))and excellent cycling stability(maintains 733.6 mAh g^(-1)at1000 mA g^(-1)after 500 cycles)for lithium ion *** work highlights a novel concept of layerby-layer chemical interaction motivated architecture design for futuristic high performance electrode materials in energy storage systems.
