Objective:To examine the effect of upright and ambulant positions versus lying down during the active first stage of labor on birth outcomes for nulliparous ***:This is a randomized controlled clinical trial conducted at the delivery department of Damanhour Educational Institute,El Behira Governorate,*** study sample involved 150 parturient women equally divided into intervention and control groups using randomization block *** researchers used four tools for data collection:Demographic data interview schedule,World Health Organization Partograph,Apgar’s score,to evaluate neonatal outcomes,and visual analogue pain intensity *** the study group,the parturient women were encouraged to assume one of the upright positions or ambulating around the bed so as to maintain the pelvis in vertical plane as far as possible for 20-25 min for every 1 *** control group received routine hospital care,which includes lying down in *** SPSS 23.0 was used to analyze the ***:Significant differences(P0.05)were observed between both groups in term of emergency cesarean birth rates,oxytocin use,and neonatal ***:This study proves that upright and ambulant positions significantly enhance uterine contractility,cervical dilatation,and fetal head descent and reduce the first stage duration.

Gas-phase thermolysis of 1-ethoxycarbonyl-benzotriazole under static, FVP and microwaves condition yielded N^1 and NZ-ethylbenzotriazole, 2-ethoxy-1,3-benzooxazole, biphenylene and oxazolidin-2-one. On the other hand, direct condensation and pyrolysis of naphtho[1,8-de][1,2,3]triazine with ethylisocyanate, phenylisocyanate and their isothiocayanates, benzoylisothiocyanate and thiourea at 150-160 ℃ or under microwaves irradiation produced the corresponding naphthopyrimidin-2-one derivatives.

Low-Dose Involved-Field Radiotherapy in Relapsed Low-Grade Non-Hodgkin's Lymphoma in Elderly Patients (Mansoura University Experience)

Purpose: To assess the response rate, duration of response and prognostic factors affecting response after low-dose involved-field radiotherapy in patients with relapsed low-grade B-cell non-Hodgkin lymphoma. Patients and Methods: Forty-four patients were included. Patients were treated with a total dose of 4 Gy (2 × 2 Gy) using 6 - 15 Mv photon or electron beam. Results: most patients were above age of 60 years (59%) with male predominance. Follicular lymphoma was the most common pathological type;bulky disease (>5 cm) was presented in 61.4%. Patients who received only 2 regimens were 63.7% and 31.8% had >2 involved sites. No treatment related toxicity was observed. The overall response rate was 88.7%;complete response was reached in 59.1% and stable disease in 6.8%, progressive disease in 4.5%. Median time to local progression was 33 months (95% CI 23.70 - 42.29);2-year local progression free survival was 78%. Response rate was found to be dependent on age, number of involved sites and lymph node size but independent on sex, pathological type, number of prior regimens, LDH level and time since diagnosis. Conclusion: Short-course-low dose palliative radiotherapy (2 × 2 Gy) affords an attractive option for treatment of relapsed low-grade non-Hodgkin’s lymphoma due to high response rates. However, these results had to be confirmed in a larger number of patients.

One of the significant health issues in the world is the prevalence of ulcerative colitis(UC).UC is a chronic disorder that mainly affects the colon,beginning with the rectum,and can progress from asymptomatic mild inflammation to extensive inflammation of the entire *** the underlying molecular mechanisms of UC pathogenesis emphasizes the need for innovative therapeutic approaches based on identifying molecular ***,in response to cellular injury,the NLR family pyrin domain containing 3(NLRP3)inflammasome is a crucial part of the inflammation and immunological reaction by promoting caspase-1 activation and the release of interleukin-1β.This review discusses the mechanisms of NLRP3 inflammasome activation by various signals and its regulation and impact on UC.

Executive Functions Assessment in Adult Patients with Idiopathic Epilepsy

Objective: Cognitive impairments are common complaints among people with epilepsy with its occurrence to emanate a great topic in the course of the illness, so our study aimed to examine the executive functions in adult patients with idiopathic epilepsy. Methods: Forty consecutive adult patients with idiopathic epilepsy (either generalized or focal) with age range from 18 - 45 years old, IQ > 85, treated with either monotherapy or polytherapy, matched with forty healthy adult volunteers by age, sex and educational level. They were examined in executive functions tests: cognitive flexibility and set shifting (Wisconsin Card sorting Test), Planning (Tower of London), response inhibition (Continuous Performance Test) and working memory (verbal and visuospatial). Results: Patients with epilepsy showed deficits in all executive functions tests, with no difference between patients with generalized and focal epilepsy except for correct response time mean in CPT. There was significant positive correlation between frequency of seizures and mean total time, mean number of extra movies in TOL, in verbal working memory (digit back word), and omission errors in CPT;at the same time there was significant negative correlation between target accuracy rate and frequency of seizures in CPT, and in visuo-spatial part of working memory (Corsi Block-Tapping Test). Conclusion: Adult patients with idiopathic epilepsy had executive dysfunctions than healthy control, with no difference between generalized and focal epilepsy except for correct response time mean in CPT;the increase of the frequency of seizure is associated with impairment in planning, attention and working memory (either verbal or viuso-spatial).

Early Detection of Thyroid Gland Disorders for Students in the Faculty of Applied Medical Sciences

In this study 70 male students were participated to determine the thyroid disorder through ultrasonography. Thyroid scan was done in 70 students prospectively with no indicative of thyroid disease (age of 19 - 23 yrs). Thyroid scan for students who participated the study took place in the department of Faculty of Applied Medical Sciences, King Abdulaziz University by using an IU 22 Philips ultrasound machine with a 5 - 12 MHz linear transducer. Among the total number of the subjects, 26% was found with abnormal ultrasound findings, 17% of them with cystic nodule, while solid and mixed nodule represented 4% for each. The high rate of abnormal findings of thyroid gland in the study suggested that screening using ultrasound scan was useful in detecting early thyroid disorders.

New series of thiazolopyranes 4, 5, 7-10, 14, 18-20 and thiazolopyranopyrimidine derivatives 6, 11, 12, 13, 15-17 bearing a sulfonamide moiety were synthesized. The structures design of these compounds complies with general pharmacophore of the sulfonamide compounds that act as carbonic anhydrase (CA) inhibitor this play a role in their anticancer activity. The structure of the newly synthesized compounds was established by microanalysis IR, H1NMR and mass spectral data. All new synthesized compounds showed considerable cytotoxic activity against breast carcinoma cell line [MCF7] compared with doxorubicin as positive control. This compounds act as radiosensitizer upon combination with γ-radiation.

Accuracy of Ultrasound to Determine Gestational Age in Third Trimester

Rapid and accurate determination of gestational age (GA) may be vital to the appropriate care of the critically ill pregnant patient and improve obstetric care through allowing the optimal timing of necessary interventions and the avoidance of unnecessary ones. Ultrasound scans are considered to be the most cost-effective, accurate and safe methods for measurement of various fetal parts in pregnant women. The aim of this research is to explore the accuracy of ultrasound in determining gestational age of fetus in third trimesters. Data collected for all pregnant women referred to the Maternity & Children’s Hospital in Jeddah. Only women with single live fetus were included in this study. Women who participated in the study were selected on following criteria: Regular menstrual cycles, known date of last menstrual period and previous live normal neonates in multipara. All scans were performed by a single ultrasonologist on one ultrasound machine. From collected data, it was found that out of 53 (100%) patients, 44 (84.62%) pregnant woman have different gestational age from US and last menstrual period (LMP). From this study we can conclude that the main method to follow fetus growth in third trimester not biparietal diameter (BPD) measurement only. The BPD in third trimester is not reliable and be useless when the patient pass 30 weeks and the BPD has to be side with other measurements when we take it in later trimesters to emphasize the normal growth of fetus and avoid wrong measurement of ultrasound.

Langerhans cell histiocytosis is a rare disease involving clonal proliferation of langerhans cells seen in children and young adults. Clinical presentation is variable, ranging from a single location in the bone to severe multivisceral involvement. Moreover, spinal involvement causing myelopathy is even rare and unusual. We report a rare case of adult Langerhans cell histiocytosis in the dorsal spine causing a spinal cord compression associated with a pulmonary process treated by surgery, radiotherapy and systemic therapy with good evolution.
