background: Venous thromboembolic disease (VTE) is a clinical entity whose two clinical manifestations are deep vein thrombosis (DVT) and pulmonary embolism (PE). It is a frequent and severe disease in Cameroon, thus constituting a significant public health problem. We aimed to describe VTE management in the Yaoundé Emergency Center, in particular the use of thrombolysis. Methods: This was a retrospective study on patients hospitalized at the Yaoundé Emergency Center for DVT and/or PE from January 1, 2015, to December 31, 2020. We collected clinical signs, paraclinical signs, risk factors of VTE, and management methods from each patient. Results: We recruited 106 participants. Dyspnea was the most frequent symptom;PE was the most common form of VTE in eight patients on 10. Obesity and high blood pressure were the main cardiovascular risk factors. The main clinical signs were oedema and pain in the limb for DVT, dyspnea, and tachycardia for PE. Heparinotherapy was the most commonly used management modality. Thrombolysis was performed in 7.5% of participants, especially in the case of hypotension or massive PE. Conclusion: In VTE management, thrombolysis remains the least used therapeutic modality in our context. Heparinotherapy remains the basis of the therapy.

background: Heart failure (HF) is a rising global health problem. Patients with HF tend to use several therapies obtained via different treatment routes to relieve their symptoms. It is rampant in sub-Saharan Africa (SSA), leading to poor health-seeking behaviours and worsened HF health outcomes. We aimed to describe the different therapeutic routes of HF patients from the onset of their first symptom until treatment in a specialised cardiology centre to identify and rebuke harmful therapeutic routes. Materials and Methods: This was a cross-sectional study at the Yaoundé Central Hospital in Cameroon between December 2018 to July 2019. Patients were recruited by consecutive convenient sampling. Adult patients aged above 18 years with confirmed HF were included using the Framingham criteria. Variables relating to socio-demographic and clinical data and the health-seeking behaviours of HF patients were studied. Results: We included 132 patients with a mean age of 62.90 years (62.88% women). Very few patients (0.90%) followed an ideal route;60.71% of subjects had a pseudo-ideal route, 19.64% accessed a specialised facility directly, and 21.42% used an erratic route. At the arrival time in a cardiology unit, 49.24% and 35.61% of our subjects were in NYHA stage III and IV HF compared to 15.15% for stage II. None of them was in Stage I. Conclusion: Most heart failure patients in Cameroon have resorted to non-specialised care, which worsens their clinical presentation. There is an urgent need for health education of HF patients in our context.

background: Cancer increases the incidence of venous thromboembolic disease (VTE), which represents a significant cause of morbidity, mortality, and economic burden in cancer patients. Objective: We aimed to describe the epidemiologic, clinical, and therapeutic pattern of VTE in cancer patients followed-up in two reference hospitals in Cameroon over the past ten years. Methods: This was a cross-sectional retrospective study conducted in the oncology department of the General hospitals of Yaoundé and Douala. We included the medical records of all patients aged 18 years and above who had active cancer with a confirmed diagnosis of VTE from 2010 to 2021. Results: We analysed 408 patients’ medical records. The prevalence of VTE was 7.6%. All those having VTE had solid tumours. There were twenty (64.5%) cases of deep venous thrombosis, five (16.1%) cases of pulmonary embolism, and three (9.7%) cases of both. Poor performance status and chemotherapy were independently associated with the development of VTE. Most of the patients were treated with compression stockings and low molecular weight heparin. Conclusion: VTE prevalence is high among cancer patients in Cameroon. It is most frequent in solid tumours originating from the genitourinary system, the lung, the pancreas, and the brain.

background: Venous thromboembolism (VTE) is a major public health problem due to its increasing frequency, mortality and management cost. This cost may require major financial efforts from patients, especially in developing countries like ours where less than 7% of the population has health insurance. This study aimed to estimate the direct cost of managing VTE in three reference hospitals in Yaoundé. Methods: This was a cross-sectional retrospective study over a three-year period (from January 1st 2018 to December 31 2020) carried out in the Cardiology departments of the Central and General Hospitals, and the Emergency Centre of the city of Yaoundé. All patients managed during the study period for deep vein thrombosis and pulmonary embolism confirmed by venous ultrasound coupled with Doppler and computed tomography pulmonary angiography respectively were included. For each patient, we collected sociodemographic and clinical data as well as data on the cost of consultation, hospital stay, workups and medications. These data were analysed using SPSS version 23.0. Results: A total of 92 patient’s records were analysed. The median age was 60 years [48 - 68] with a sex ratio of 0.53. The median direct cost of management of venous thromboembolism was 766,375 CFAF [536,455 - 1,029,745] or $1415 USD. Management of pulmonary embolism associated with deep vein thrombosis was more costly than isolated pulmonary embolism or deep vein thrombosis. Factors influencing the direct cost of management of venous thromboembolism were: hospital structure (p = 0.015), health insurance (p 0.001), type of pulmonary embolism (p = 0.021), and length of hospital stay (p = 0.001). Conclusion: Management of VTE is a major financial burden for our patients and this burden is influenced by the hospital structure, health insurance, type of pulmonary embolism and length of hospital stay.

background: Heart failure is the commonest cardiovascular complication in type 2 diabetes mellitus. However, subclinical left ventricular dysfunction can’t be detected using 2D echocardiography which is routinely used for cardiac evaluation of diabetic patients. We aimed to determine the prevalence and factors associated to left ventricular global longitudinal strain (LV GLS) impairment in type 2 diabetes Cameroonians patients. b>Methods:b> We conducted a cross-sectional study from January 2019 to June 2019, including type 2 diabetes patients with preserved left ventricle ejection fraction. Clinical and echocardiographic data were collected, and LV GLS was assessed using speckle tracking technique, a value ≤ -16% been considered as normal value. b>Results:b> We recruited 95 patients, with a mean age of 57.4 ± 11.8 years old and median diabetes duration of 5 [2 - 12] years. Echocardiographic evaluation found 56.3% of left ventricle remodelling, 51.6% of left ventricle diastolic dysfunction and mean left ventricle ejection of 63.3% ± 6.6%. Introduction: Infections are significant causes of mortality in sickle cell children in resource-limited countries. This study aimed to determine the clinical profile and bacterial ecology of infections in children with sickle-cell disease in two referral hospitals in Niamey. Patients and methods: A retrospective descriptive study was conducted from January 2018 to July 2020 in two referral hospitals in Niamey. All children aged one (1) to 15 years with sickle cell disease admitted for suspected infection, including at least one bacterial culture, were studied. bacteriological analysis was performed using the appropriate culture media, using bactAlert (Reference 4700003 bTA3D60 bioMérieux). Results: Over 36-months, 350 children with a mean age of 10.9 months were admitted. The sex ratio was 1.2. The SS electrophoretic profile was the most common (93.4%). Immunization status was up to date in 66% of patients. Fever was the common reason for consultation (55.1%). Infection was confirmed in 62 patients (17.7%). The primary diagnoses were bacterial gastroenteritis (24.2%) and urinary tract infection (19.4%). blood cultures were isolated from Salmonella typhi (13.0%) and Escherichia coli (8.7%). Klebsiella spp (7.1%) and Escherichia coli (5.0%) were detected in cytobacteriological examination of urine. Salmonella typhi (23.5%) and Escherichia coli (5.9%) were isolated on coproculture. Conclusion: bacterial ecology appears not different from that usually observed in sickle-cell children. Salmonella and Escherichia coli were predominant.

The feasibility of a sustainable non-autoclaved cellular concrete,based on flax vegetable co-products,for the production of usable specimen in the lightweight construction field,has been investigated *** produced specimen,containing various volume ratios of flax particles with respect to preformulatd Tradical PF70 lime binder of 0,1,and 2,were lightened by creating a porous structure in the matrix through the addition of 0.3%*** powder(able to react with calcium hydroxide from the binder and result in microscopic air-bubbles).Fresh and hardened specimen properties,including hydration,fresh density,porosity,hardened density,compressive and flexural strengths,toughness energy,and dry thermal conductivity at different temperatures,were assessed for varying flax-to-binder *** have shown that the addition of Aluminum powder leads to restrain the setting time delay of binder-based ***,the hardened material displays a significant decrease in specimen density,thereby resulting in a compressive strength level compatible with that required in the cellular construction materials *** also highlighted the ability of added flax to induce a change in the specimen from brittle to ductile ***,a high degree of thermal insulation can be achieved,which makes the cellular specimen based on flax particle suitable as insulated-bearing walls material.

A New Method for Sentiment Analysis Using Contextual Auto-Encoders

Sentiment analysis, a hot research topic, presents new challenges for understanding users＇ opinions and judg-ments expressed online. They aim to classify the subjective texts by assigning them a polarity label. In this paper, weintroduce a novel machine learning framework using auto-encoders network to predict the sentiment polarity label at theword level and the sentence level. Inspired by the dimensionality reduction and the feature extraction capabilities of theauto-encoders, we propose a new model for distributed word vector representation ＂PMI-SA＂ using as input pointwise-mutual-information ＂PMI＂ word vectors. The resulted continuous word vectors are combined to represent a sentence. Anunsupervised sentence embedding method, called Contextual Recursive Auto-Encoders ＂CoRAE＂, is also developed forlearning sentence representation. Indeed, CoRAE follows the basic idea of the recursive auto-encoders to deeply composethe vectors of words constituting the sentence, but without relying on any syntactic parse tree. The CoRAE model consistsin combining recursively each word with its context words （neighbors＇ words： previous and next） by considering the wordorder. A support vector machine classifier with fine-tuning technique is also used to show that our deep compositionalrepresentation model CoRAE improves significantly the accuracy of sentiment analysis task. Experimental results demon-strate that CoRAE remarkably outperforms several competitive baseline methods on two databases, namely, Sanders twittercorpus and Facebook comments corpus. The CoRAE model achieves an efficiency of 83.28% with the Facebook dataset and97.57% with the Sanders dataset.

Assessment of Dry Port Efficiency in Africa Using Data Envelopment Analysis

This work involves the evaluation of dry port competitiveness through analysis of efficiencies for selected dry ports in Africa. Five dry ports were selected and analysis carried out over a period of four years. The dry ports considered were Mojo and Kality in Ethiopia, Mombasa in Kenya, Isaka in Tanzania and Casablanca in Casablanca, Morocco. Data Envelopment Analysis (DEA) was applied for this work. Container throughputs for the various ports under consideration were used as the output variable for the data analysis model, while the number of reach stackers, the number of tractors, the number of forklifts and the size of the dry port were used as the input variables. From the results, the Mombasa dry port was found to be the most efficient with an average score of approximately 1 over the period under consideration. Casablanca was the second efficient dry port with an average score of 0.762, while Isaka was the least efficient with an average score of 0.142. This research is significant since the African countries have embraced the dry port concept, as witnessed in the huge investments in this sector, and would serve to highlight areas that need improvement for the few existing dry port facilities, most of which are undergoing expansion as well as modernization.
