Role of Vaginal Progesterone in Prevention of Preterm Labor in Women with Previous History of One or More Previous Preterm Births

Objective: To evaluate whether prophylactic administration of 200 mg vaginal progesterone can reduce the incidence of preterm birth in women with documented history of preterm birth setting: Obstetrics and Gynecology Department, Zagazig University Hospital, Egypt. Methods: Ninety patients with previous history of preterm birth prior to 37 week presenting with singleton pregnancy between 20 - 24 weeks were randomly allocated to receive either the progesterone 200 mg vaginal suppository or no treatment. Results: The incidence of preterm labor before 37 weeks of gestation was significantly lower in the study group than in the control group (22.2% vs. 53.3%) especially in earlier gestational ages. While, the mean birth weight was significantly higher in the study group than in control group (2872.67 ± 565.76 gm vs. 2487.78 ± 742.40 gm). The neonatal morbidities and mortality associated with preterm labor were significantly lower in the study group than in the control group as shown by lower incidence of neonatal RDs (13.3% vs. 31.1%;P = 0.043) and lower incidence of the need for NICU admission (15.6% vs. 35.5%;P = 0.03). Conclusion: Administration of prophylactic vaginal progesterone (200 mg, daily) can significantly reduce the rate of preterm birth before 37, 32 and 28 wks of gestation among women with previous spontaneous preterm birth. In addition, the rates of RDs and admission to NICU were significantly decreased among infants of women assigned to progesterone treatment. Also, there was an additional benefit of vaginal progesterone for prevention of preterm birth in women who had prior spontaneous preterm birth and cervical length 25 mm.

strong>Background:strong> Terminal QRS distortion and fragmentation (fQRS) with elevated myeloperoxidase (MPO) were linked to poor cardiovascular outcomes in acute coronary syndrome. We aimed to investigate these parameters in early prediction of coronary artery disease severity based on SYNTAX score and in-hospital adverse events in STEMI patients. Methods: A total of 215 patients with first STEMI admitted for primary PCI were included in the study. They were divided according to the admission ECG into group I with QRS distortion or fQRS, group II with combined QRS distortion and fQRS, and group III without QRS distortion or fQRS. Myeloperoxidase and troponin I levels, ST resolution ratio, left ventricular EF%, and severity of coronary artery lesions using SYNTAX risk score were measured. Results: MPO level, SYNTAX score, and in-hospital mortality were higher in group I and II and were higher in group II compared to group I. By regression analysis, QRS distortion, fQRS, and MPO > 412 ng/ml were independent predictors of both CAD severity and in-hospital mortality. DM was an independent predictor of CAD severity (OR: 2.851, P 0.012) while high SYNTAX score was an independent predictor of in-hospital mortality (OR: 6.113, P 0.001). Adding MPO level to any QRS configuration pattern increased predictive value for the detection of CAD severity that was more evident in the combined QRS distortion and fragmentation. Conclusion: span style="font-family:Verdana;">Terminal QRs distortion, fragmentation, or combined QRs distortion and fragmentation have a significant value in predicting in-hospital adverse events and CAD severity

stability of Production Planning Problem with Fuzzy Parameters

The traditional production planning model based upon the famous linear programming formulation has been well known in the literature. The consideration of uncertainty in manufacturing systems supposes a great advance. Models for production planning which do not recognize the uncertainty can be expected to generate inferior planning decisions as compared to models that explicitly account the uncertainty. This paper deals with production planning problem with fuzzy parameters in both of the objective function and constraints. We have a planning problem to maximize revenues net of the production inventory and lost sales cost. The existing results concerning the qualitative and quantitative analysis of basic notions in parametric production planning problem with fuzzy parameters. These notions are the set of feasible parameters, the solvability set and the stability set of the first kind.

Contribution of Geophysical Prospecting (1D) to Characterize a Precambrian Basement Aquifers Agadez Region (North, Niger)

In the A?r massif, and more particularly in the Iférouane area, the basement is flush. Thus, the essential of water resources is contained in the discontinuous aquifers of basement. Indeed, the weathered and cracked basement is an excellent reservoir of groundwater. The objective of this study is to determine the areas favorable to establishment of productive drilling in the village of Ebourkoum. A methodological approach based on the combination of remote sensing and geophysics methods was implemented. The fracturing map developed highlighted three main accidents of directions N0° - N10°, N30° - N50° and N130° - N140° in the said locality. The geophysical investigation mainly electrical prospecting has allowed to locate structures with high hydraulic potential below alluvium and alterites. subsequently, drilling will be carried out in the areas retained.

Gem-Diphosphonates： The Motif of Diverse Biological and Medicinal Importance

The introduction of gem-diphosphonates （known as bisphosphonates, BPs） in oncology has dramatically changed the management of patients with metastatic bone disease. In this manuscript, we thoroughly scrutinize the available body of clinical trials supporting the use of bisphosphonates in this setting and review new and ongoing research. Additionally, we summarize the data showing the benefits of BP-use in the prevention of treatment-induced bone loss and the intriguing emerging evidence on the antitumor potential of some of these agents when used in the adjuvant setting. Finally, we address the need for a careful consideration of potential benefits of BPs therapy and the risk for osteonecrosis, a recently recognized late-toxicity of their use.

New Periodic solitary Wave solutions for a Variable-Coefficient Gardner Equation from Fluid Dynamics and Plasma Physics

The Gardner equation with a variable-coefficient from fluid dynamics and plasma physics is investigated. Different kinds of solutions including breather-type soliton and two soliton solutions are obtained using bilinear method and extended homoclinic test approach. The proposed method can also be applied to solve other types of higher dimensional integrable and non-integrable systems.

Temperature Trend on Makkah, saudi Arabia

The main objective of this study is to investigate temperature trend and distribution over 29 years period (1985 to 2013) in Makkah, saudi Arabia, the holiest city for all Muslims. Monthly mean, maximum, and minimum temperature levels and their trends are investigated using Regression analysis and Theil-sen nonparametric test. Also, trends in deviations from the reference period (1985-2013) are analyzed. The results showed that the number of hot days and nights increased annually by 1.5966 and 1.832, respectively, while the number of cold nights decreased annually by 0.4054 nights. Both Regression analysis and Theil-sen test demonstrated positive trends in mean, minimum and maximum temperature levels. Trends are determined for various seasons and months of the year. The annual mean of daily mean, maximum and minimum temperature increased by 0.0398°C, 0.0552°C, 0.0398°C per year, respectively. The minimum value of monthly mean temperature (Tmmean = 23.98°C) was found in January, whereas the maximum value of the mean temperature (Tmmean= 35.95°C) was found in July. Maximum value of monthly mean of daily maximum temperature (Tmmax = 43.88°C) was found in June and minimum (30.54°C) in January. The monthly mean of the daily minimum temperature (Tmmin) varied between a minimum of 18.82°C in January and a maximum of 29.59°C in August. From the above analysis it can be concluded that Makkah is suffering from a considerable warming temperature trend which is confirmed by the Theil-sen non-parametric test and there is potentially an increasing medical risk from heat waves that will be more intense. This requires specific attention toward: the energy demands for extra cooling, water resources, draughts, and medical preparedness by the decision makers in order to minimize these risks to residents, pilgrims who gather annually to perform hajj rituals and other visitors.

This paper investigates the fine structure of the Gabor frame generated by the B-spline Bsub>3sub>. In other words, one extends the known part of the Gabor frame set for the 3-spline with the construction of the compactly supported dual windows. The frame set of the function Bsub>3sub> is the subset of all parameters (a,b) ∈Rsup>2sup>+ sub>for which the time-frequency shifts of Bsub>3sub> along aZ × bZ form a Gabor frame for Lsup>2sup>(R).

Efficacy of Different Fungicides against Mango Anthracnose in senegalese soudanian Agroclimate

Anthracnose is the one of the most devastating diseases of mango produced in the southern part of senegal. The field trials against post harvest rot due to anthracnose took place in three orchards over two production seasons, in 2009 and 2010. Three systemic fungicides (thiophanate methyl, azoxystrobin and myclobutanyl) and one contact fungicide (mancozeb) were tested for their efficacy. The first treatments in 2009 took place in the rainy season, while 20% of mangoes on trees exhibited anthracnose lesions. At ripening stage, 100% of fruits from non treated control mango trees were affected by anthracnose and got rotten, while between 73.2% and 80% of mangoes ripened free of disease when treated with thiophanate methyl. With the treatment with azoxystrobin, between 46.6% and 60% of fruits were not infected. Treatment with myclobutanyl was less effective. This level of effectiveness was clearly improved in 2010 by preventive spraying, days ahead of onset of the rainy season, before the appearance of anthracnose symptoms on fruits. The level of effectiveness was higher respectively for thiophanate methyl (between 96% and 100% of fruits not infected) and azoxystrobin (between 84% and 96% of fruits not infected).
