Beta-blockers and 1-year clinical outcomes in hospitalized heart failure patients with atrial fibrillation

OBJECTIVE To assess the association between beta-blockers and 1-year clinical outcomes in heart failure(HF)patients with atrial fibrillation(AF),and further explore this association that differs by left ventricular ejection fraction(LVEF)*** We enrolled hospitalized HF patients with AF from China Patient-centered Evaluative Assessment of Cardiac Events Prospective Heart Failure *** proportional hazard regression models were employed to calculate hazard ratio of *** primary outcome was all-cause *** Among 1762 HF patients with AF(756 women[41.4%]),1041(56%)received beta-blockers at discharge and 1272(72.2%)had an LVEF>40%.During one year follow up,all-cause death occurred in 305(17.3%),cardiovascular death occurred in203 patients(11.5%),and rehospitalizations for HF occurred in 622 patients(35.2%).After adjusting for demographic characteristics,social economic status,smoking status,medical history,anthropometric characteristics,and medications used at discharge,the use of beta-blockers at discharge was not associated with all-cause death[hazard ratio(HR):0.86;95%Confidence Interval(CI):0.65-1.12;P=0.256],cardiovascular death(HR:0.76,95%CI:0.52-1.11;P=0.160),or the composite outcome of all-cause death and HF rehospitalization(HR:0.97,95%CI:0.82-1.14;P=0.687)in the entire *** were no significant interactions between use of beta-blockers at discharge and LVEF with respect to all-cause death,cardiovascular death,or composite *** the adjusted models,the use of beta-blockers at discharge was not associated with all-cause death,cardiovascular death,or composite outcome across the different levels of LVEF:reduced(<40%),mid-range(40%-49%),or preserved LVEF(≥50%).CONCLUSION Among HF patients with AF,the use of beta-blockers at discharge was not associated with 1-year clinical outcomes,regardless of LVEF.

The MCrAlY coating has been potential candidate for the parts applied in friction and corrosion conditions, and CNTs (carbon nanotubes) are expected to improve the service performance of coatings owing to high lubrication and low chemical reactivity. In this work, a systematic investigation on the tribological and corrosion properties of CoCrAlYTaSiC-xCNTs coatings deposited by laser melting was analyzed. Results showed that the coatings had good-quality without typical metallurgical defects. The CNTs addition homogenized and refined the microstructure of coating, and also improved the tribological and corrosion properties. As the CNTs content changed from 0 to 4 wt%, the wear rate of coating decreased from 16.23 × 10^(-3) to 7.58 × 10^(-3) mg m^(−1), the jcorr of coating decreased from 4.13 × 10^(-4) to 1.23 × 10^(-4) A cm^(−2), and the Rct values increased from 12.69 to 25.07 Ω cm2.

Stem cell transplantation and low-energy shock-wave therapy （LESWT） have emerged as potential and effective treatment protocols for diabetic erectile dysfunction. During the tracking of transplanted stem cells in diabetic erectile dysfunction models, the number of visible stem cells was rather low and decreased quickly. LESWT could recruit endogenous stem cells to the cavernous body and improve the microenvironment in diabetic cavernous tissue. Thus, we deduced that LESWT might benefit transplanted stem cell survival and improve the effects of stem cell transplantation. In this research, 42 streptozotocin-induced diabetic rats were randomized into four groups： the diabetic group （n = 6）, the LESWT group （n = 6）, the bone marrow-derived mesenchymal stem cell （BMSC） transplantation group （n = 15）, and the combination of LESWT and BMSC transplantation group （n = 15）. One and three days after BMSC transplantation, three rats were randomly chosen to observe the survival numbers of BMSCs in the cavernous body. Four weeks after BMSC transplantation, the following parameters were assessed： the surviving number of transplanted BMSCs in the cavernous tissue, erectile function, real-time polymerase chain reaction, and penile immunohistochemical assessment. Our research found that LESWT favored the survival of transplanted BMSCs in the cavernous body, which might be related to increased stromal cell-derived factor-1 expression and the enhancement of angiogenesis in the diabetic cavernous tissue. The combination of LESWT and BMSC transplantation could improve the erectile function of diabetic erectile function rats more effectively than LESWT or BMSC transplantation performed alone.

Background Transvenous lead placement is the standard approach for left ventricular （LV） pacing in cardiac resynchronization ther- apy （CRT）, while the open chest access epicardial lead placement is currently the most frequently used second choice. Our study aimed to compare the ventricular electromechanical synchronicity in patients with heart failure after CRT with these two different LV pacing tech- niques. Methods We enrolled 33 consecutive patients with refractory heart failure secondly to dilated cardiomyopathy who were eligible for CRT in this study. Nineteen patients received transvenous （TV group） while 14 received open chest （OP group） LV lead pacing. Intraand inter-ventricular electromechanical synchronicity was assessed by tissue Doppler imaging （TDI） before and one year after CRT procedure. Results Before CRT procedure, the mean QRS-duration, maximum time difference to systolic peak velocity among 12 left ventricle segments （LV Ts-12）, standard deviation of time difference to systolic peak velocity of 12 left ventricle segments （LV Ts-SD）, and inter-ventficular mechanical delay （IVMD） in OP and TV group were 166 ± 17 ms and 170 ± 21 ms, 391 ±42 ms and 397 ± 36 ms, 144 ± 30 ms and 148 ± 22 ms, 58 ± 25 ms and 60 ± 36 ms, respectively （all P 〉 0.05）. At one year after the CRT, the mean QRS-duration, LV Ts-12, LV Ts-SD, and IVMD in TV and OP group were 128 ± 14 ms and 141 ± 22 ms （P = 0.031）, 136 ± 37 ms and 294 ± 119 ms （P = 0.023）, 50± 22 ms and 96 ± 34 ms （P = 0.015）, 27 ± 11 ms and 27 ± 26 ms （P = 0.86）, respectively. The LV lead implantation procedure time was 53.4±16.3 rain for OP group and 136 ± 35.1 min for TV group （P = 0.016）. The mean LV pacing threshold increased significantly from 1.7 ± 0.6 V/0.5 ms to 2.3 ± 1.6 V/0.5 ms （P 〈 0.05） in TV group while it remained stable in the OP group.
