The effects of EMLA cream on pain responses of preschoolers

The aim of present study was to assess the effects of a eutectic mixture of topical anesthetics (EMLA cream) on pain responses of preschoolers during venipuncture. The pain responses were reported by children, nurses, and the children’s mothers. The pre- and post-test-designed study, which included a non-equivalent control group, was carried out in a pediatric unit at E University Hospital in South Korea. Pain responses during venipuncture were measured by the self-reporting of the children using the Face Pain Rating Scale (FPRS), the Procedure Behavior Checklist by nurses (PBCL), the Visual Analog Scale (VAS) by mothers, in addition to measurements of the children’s pulse rate and level of oxygen saturation. EMLA cream was effective in decreasing the pain responses of the children as assessed by FPRS, PBCL, and VAS scores. However, pulse rate and level of oxygen saturation of the children during venipuncture were not significantly different between the two groups. EMLA cream could be a useful option for reducing pain in preschool children during invasive procedures.

Wall boiling and bubble population balance equations combined with a two-fluid model are employed to predict boiling two-phase flow in an inclined channel with downward- facing heated wall. For vertical flow, these models have been highlighted by the mechanistic formulation of bubble nucleation on a vertical heated wall by the bubble sliding and lift-off formulations. For downward-facing heated wall in an inclined channel, however, the bubble nucleation and departure behaviors are affected by the large bubbles near the heated wall as vapor accumulates near the upper wall of the inclined channel. In order to observe the boiling behavior on the inclined, downward-facing heated wall, a visualization experiment was carried out with a 100 mm x 100 mm of the cross section, one meter long rectangular channel, inclined by 10 o from the horizontal plane. The size of the heated wall was 50 mm by 750 mm and the heat flux was provided by the Joule heating of the heated wall using the DC electrical current. High speed video images showed the bubble nucleation and growth, sweeping of these bubbles by a large bubble sliding near the wall. For these circumstances of wall boiling and two-phase flow in the inclined channel, the existing wall boiling model encompassing bubble growth and sliding were modified by considering the influence of large bubbles near the heated wall. Using the model developed, boiling two-phase flow in a 3 m long, inclined channel with downward-facing heated wall was analyzed.

AIM:To evaluate the feasibility of 3-Tesla magnetic resonance elastography(MRE)for hepatic fibrosis and to compare that with diffusion-weighted imaging(DWI)and gadoxetic acid-enhanced magnetic resonance(MR)***:Forty-two patients were included in the *** MRE,mean stiffness values were measured on the elastograms in *** apparent diffusion coefficient(ADC)of the liver was measured using *** gadoxetic acid enhanced MR,the contrast enhancement index(CEI)was calculated as signal intensity(SI)post/SIpre,where SIpost is liver-to-muscle SI ratio on hepatobiliary phase images and SIpre is that on nonenhanced *** between aspartate aminotransferase to the platelet ratio index(APRI)and three MR parameters was *** MR parameter was compared between a hepatic fibrosis(HF)group and non-hepatic fibrosis(n HF)***:Liver stiffness showed strong positive correlation with APRI[Spearman correlation coeffiecient(r)=0.773,P<0.0001],while ADC and CEI showed weak or prominent negative correlation(r=-0.28 and-0.321,respectively).In the HF group,only liver stiffness showed strong correlation with APRI(r=0.731,P<0.0001).Liver stiffness,ADC,and APRI were significantly different between the HF group and n HF ***:MRE at 3-Tesla could be a feasible method for the assessment of hepatic fibrosis.

对硅锰钛脱氧钢炉冷条件下的微观结构和夹杂物进行研究。运用带X射线能谱分析（EDS）的场发射扫描电镜（FE-SEM）对夹杂物的成分和形貌进行分析。热力学计算所得夹杂物的种类、成分结果和实验所得结果一致。在硅锰钛脱氧钢中主要有两类夹杂物:一类是锰钛氧化物夹杂,另一类是MnS夹杂物带有TiN析出相。本文通过系统地实验研究,阐述了晶内铁紊体的形成机理。由于在锰钛氧化物夹杂周围的基体中形成的贫锰区增加了化学驱动力,促使晶内铁素体以锰钛氧化物夹杂为核心形核。TiN和晶内铁素体有很低的晶格错合度,所以MnS表面析出的TiN相通过降低表面能促进晶内铁素体的形核。

Transgastric endoscopic necrosectomy has been recently introduced as the effective and alternative management of infected pancreatic necrosis and pancreatic abscess. However,up to 40% of patients who undergo endoscopic necrosectomy may need an additional percutaneous approach for subsequent peripancreatic fluid collection or non-resolution of pancreatic necrosis. This percutaneous approach may lead to persistent pancreatocutaneous fistula,which remains a serious problem and usually requires prolonged hospitalization,or even open-abdominal surgery. We describe the first case of pancreatocutaneous fistula and concomitant abdominal wall defect following transgastric endoscopic necrosectomy and percutaneous drainage,which were endoscopically closed with fibrin glue injection via the necrotic cavity.

＜正＞These days,the basic maintenance strategies are being shifted from a periodic maintenance to a predictive and condition-based maintenance based on an on-line condition monitoring of deteriorated components,equipment and *** are several on-going efforts to monitor and detect the deteriorated components and the disturbed processes[1,2,3].Their efforts are to achieve an optimum safety and economy for an operation and *** information of the symptom signals emitted from the deteriorated components will provide confidence in an understanding of the condition of the underlying process disturbance. In order to increase the confidence level of the understanding on the condition of the underlying processes and components,KAERI I＆*** Research team has developed an alarm root cause tracking system（ACTs）,an alarm and diagnosis-integrated operator support system（ADIOS）,and a dynamic alarm system（DAS）[2,3]. An alarm processing and presentation system,LogACTs（Logic Alarm Cause Tracking system） of their researches is developed and installed into the main control room（MCR） of the Wolsong nuclear power plant（NPP） unit *** especially,provides the capabilities for a diagnostics and prognostics evaluation of a process disturbance that will detect component degradation prior to a component failure or a process shutdown.
