Objective:To investigate the modulatory effects of bitter gourd extract on the insulin signaling pathway in the liver and skeletal muscle tissues of diabetic ***:The ethanolic extract of bitter gourd was prepared and its contents of total polyphenols and flavonoids were assayed.A neonatal streptozotocin-induced diabetic rat model was established and the diabetic rats were assigned into different groups and were treated with different doses of bitter gourd extract(100,200,400,or 600 mg/kg)or with glibenclamide(0.1 mg/kg)for 30 *** blood glucose,insulin,and lipid profile were evaluated and the insulin signaling pathway in the liver and skeletal muscle of rats was *** correlations between homeostasis model assessment(HOMA)and the components of insulin signaling pathway were also ***:Different doses of bitter gourd extract significantly ameliorated fasting blood glucose level and HOMA index for insulin ***,bitter gourd extract increased serum insulin and improved disrupted serum lipid *** levels of insulin receptor substrate-1(IRS-1),p-insulin receptorβ(p-IR-β),protein kinase C(PKC),GLUT2,and GLUT4 were improved by treatment with bitter gourd *** best results were obtained with 400 mg/kg dose of the extract,the effect of which was equivalent to that of *** in the bitter gourd treated rats was negatively correlated with p-IR-β,IRS-1 and PKC in hepatic and skeletal *** was also negatively correlated with skeletal muscle ***:Bitter gourd extract improves glucose homeostasis and lipid profile in diabetic rats via enhancement of insulin secretion and ***,bitter gourd can be used as a potential pharmacological agent for the treatment of type 2 diabetes mellitus.

Micropunch grafting for healing of refractory chronic venous leg ulcers

Background: Chronic venous ulcers are a serious problem for both patients and physicians. The CEAP classification(clinical manifestations(C), etiologic factors(E), anatomic distribution of disease(A), and underlying pathophysiologic findings(P)) for chronic venous disorders(CVD) was developed in 1994. Published papers on CVD use all or part of the CEAP. Micropunch grafts, which are a variation of skin grafts, have been used with great success in plastic surgery for both esthetic and reconstructive purposes. This study aimed to assess the outcomes of micropunch grafting in the treatment of chronic refractory venous ***: Patients with chronic venous ulcers who did not respond to conservative treatment for more than 6months were included in the study. All patients underwent ulcer coverage by micropunch skin grafts using a micrometer. The patients were discharged on the same day as the surgery. Micropunch skin grafts were manually counted per square centimeter intraoperatively and during follow-up using Dermlite DermatoschopeTM II Pro *** satisfaction was assessed by using a ***: Twenty patients underwent the surgery. Most of the patients were middle-aged males. The ulcers were predominantly small-and medium-sized. The mean operation time was 167.00 ± 86.01 min. After 6 months, the survival rate of the planted micrografts was 84%. Three ulcers had graft loss and two patients had an infection at the ulcer site, which was managed conservatively. The patients were followed-up for 14 ***: Micropunch grafting is a useful and convenient method for the treatment of difficult venous ulcers and can be performed on an outpatient basis.

A large subset of corneal pathologies involves the formation of new vessels(neovascularization), leading to compromised visual acuity. This article aims to review the clinical causes and presentations of corneal neovascularization(CNV) by examining the mechanisms behind common CNV-related corneal pathologies, with a particular focus on herpes simplex stromal keratitis,contact lenses-induced keratitis and CNV secondary to keratoplasty. Moreover, we reviewed CNV in the context of different types of corneal transplantation and keratoprosthesis, and summarized the most relevant treatment available so far.

Parkinson’s disease(PD),one of whose symptoms is dysphonia,is a prevalent neurodegenerative *** use of outdated diagnosis techniques,which yield inaccurate and unreliable results,continues to represent an obstacle in early-stage detection and diagnosis for clinical professionals in the medical *** solve this issue,the study proposes using machine learning and deep learning models to analyze processed speech signals of patients’voice *** of these processed speech signals were obtained and experimented on by random forest and logistic regression *** were highly successful,with 90%accuracy produced by the random forest classifier and 81.5%by the logistic regression ***,a deep neural network was implemented to investigate if such variation in method could add to the *** proved to be effective,as the neural network yielded an accuracy of nearly 92%.Such results suggest that it is possible to accurately diagnose early-stage PD through merely testing patients’*** research calls for a revolutionary diagnostic approach in decision support systems,and is the first step in a market-wide implementation of healthcare software dedicated to the aid of clinicians in early diagnosis of PD.

The effect of diet supplemented with vegetable oils and/or monensin on the vaccenic acid production in continuous culture fermenters

Studies have shown that supplementing ruminant diets with vegetable oils modulated the rumen biohydrogenation and increased polyunsaturated fatty acid in their products. These positive values are often accompanied by a marginal loss of supplemented unsaturated fatty acids and rise in the concentrations of saturated fatty acids. This study were carried out mainly to investigate the effect of supplementing diets with sunflower oil, olive oil with or without monensin on the production and accumulation of vaccenic acid(VA) in continuous culture fermenters as a long term in vitro rumen simulation *** dual-flow continuous culture fermenters were used in an 8 replication experiment lasted 10 days each(first 7 days for adaptation and last 3 days for samples collection). Supplementing diets with plant oils and monensin in the present experiment increased VA and conjugated linoleic acids(P > 0.05) in ruminal cultures. The results suggest that supplementing diets with both olive oil and sunflower oil and monensin increased VA accumulation compared to plant oils supplemented alone without affecting the rumen dry matter and organic matter digestibility.

Occult HCV Infection (OCI) Diagnosis in Cirrhotic and Non-cirrhotic Naive Patients by Intra-PBMC Nested Viral RNA PCR

Background and Aims:Occult HCV infections(OCIs)include IgG antibody seronegative cryptogenic(COCIs),as well as seropositive secondary na(i)ve(SNOCIs)and experienced(SEOCIs)*** used peripheral-blood-mononuclear-cell(PBMC)-PCR to evaluate COCIs and SNOCIs prevalence,serum HCV spontaneous disappearance(SCSD)in na(i)ve cirrhotics and non-cirrhotics,intra-PBMC HCV-RNA strands in relation to cirrhosis density in na(i)ve non-viremia cases,and HCV-RNA seroconversion after 1 year of solitary naive intra-PBMC ***:The anti-HCV IgG antibodypositive na(i)ve-patients(n=785)were classified into viremic(n=673)and non-viremic[n=112,including non-cirrhotics(n=55)and cirrhotics(n=57)],and 62 controls without evidence of *** and post-HCV non-viremia cases(n=62+112=174)were submitted to hepatic FibroscanElastography *** subjects(n=847)were screened for intra-PBMC HCV-RNA sense and antisense strands by ***:Na(i)ve-OCI cases(4.84%)that were diagnosed by PBMC-PCR significantly raised the total numbers of HCV-infection to 714(p=0.01).The percent positivity of SNOCIs(34.82%)was significantly higher than for asymptomatic-COCIs(3.125%,p=0.0001).Comparing PBMC-PCR with single-step-reverse-transcription(SRT)-PCR for identification of SCSD in na(i)ve IgG antibody-positive nonviremia patients(n=112)revealed a decline in SCSD prevalence by PBMC-PCR(from 14.27%to 9.3%),regardless of presence of hepatic cirrhosis(p=0.03).SCSD was found to be higher by PBMC-PCR in non-cirrhotics compared to cirrhotics(p=0.0001),with an insignificant difference when using SRT-PCR(p=0.45).Intra-PBMC HCV-RNA infection was significantly more frequent in cirrhotics compared to both non-cirrhotics and controls(p<0.0005).An increased hepatic fibrosis density was recognized in intra-PBMC HCV-RNA infection with sense(p=0.0001)or antisense strand(p=0.003).HCV-RNA seroconversion was associated with intra-PBMC infection when both sense and antisense strands were dete

The Value of Continuous Noninvasive Hemoglobin Monitoring in Intraoperative Blood Transfusion Practice during abdominal Cancer Surgery

Introduction: Patients undergoing major oncological surgery may suffer from severe bleeding. Sometimes, it is difficult to anesthesiologist to take decision about timing of administration blood products to such patients. The aim of this study is to evaluate the use of continuous noninvasive hemoglobin monitoring as a guide for blood transfusion practice. Methods: One hundred patients undergoing elective abdominal cancer surgeries were randomly allocated into two groups, Group I (n = 50): laboratory Hb was obtained at baseline (immediate preoperative), intraoperative (when to suggest transfusion triggering value) and immediate postoperative. Group II (n = 50): The probe of Masimo for SpHb monitoring was applied immediately after induction of anesthesia at the index finger. Laboratory Hb was obtained at baseline (immediate preoperative), intraoperative (when to suggest transfusion triggering value) and immediate postoperative. Results: A number of transfused units of RBC were significantly lower in SpHb group than in control group (p value 0.05). Conclusion: SpHb monitoring had clinically acceptable absolute and trend accuracy. SpHb monitoring altered transfusion decision making and resulted in decreased RBC utilization and decreased RBC costs while facilitating earlier transfusions when indicated.

Reliable Analytic Strategy to Correlate the Morphological and Cytological Parameters on Lupinus termis L, against Fusarium oxysporum Infection

Many analytic strategies have emerged to estimate plant responses to Fusarium wilt. The demand for fast and reliable method （diagnosis, prediction） to determine isolate strength accurately is not established yet. Early determination of pathogen strength helps in plant medication. The aim of this study was to develop a faster strategy and method for early determination of fungal isolates strength in correlation to plant response. Till now, the scientists have no consensus on the most correlated parameters that could express wilt precisely. In this study, 30 isolates of Fusarium oxysporum isolated from Lupinus termis L. were used to provide an explicit image about the real strength of Fusarium isolates and its impact on the plant. Wilting percentage ranged from 26.67% to 93.33% of the infected plants depending on isolate virulence. Some of cellular, morphological and physical measurements were conducted on 8 out of 30 isolates, including root （length, fresh weight （FW） and dry weight （DW））, nodules （water content （WC）, FW, DW）, stem （height, WC, FW, DW）, total leaves/plant （WC, FW, DW） and the fourth leaf （WC, FW, DW, leaf area, epidermal cell area, epidermal cell number, succulence）. Hierarchical clustering was used to determine the variance between the isolates. Detrended correspondence analysis （DCA） and canonical correspondence analysis （CCA） were used to determine the most important growth parameters that could express wilting accurately. The CCA results showed that most of the measured parameters on the fourth leaf, except for leaf epidermal cell number, were highly and positively correlated to wilt. That makes these specific parameters valuable and sensitive for any changes in isolates strength. Accordingly, a mathematical model was created to be helpful in the quick determination of isolate strength and precise medication.

HCV Therapy Follow-up Fractionation (CTF2) by Intra-PBMC Nested RNA PCR Recognizes Early Virologic Response and Relapse

Background and Aims:Sustained virologic response is evaluated by single-step reverse transcription(SRT)PCR as-say,which assesses hepatitis C virus(HCV)clearance from plasma but not from tissues such as peripheral blood mono-nuclear cells(PBMCs).Persistence of HCV RNA in PBMCs beyond end of treatment(EOT)is associated with *** goal was to measure intra-PBMC HCV RNA levels during oral antiviral therapy according to the HCV therapy follow-up fractionation(CTF2)***:Compen-sated chronic HCV patients(n=278 SRT-PCR positive)were scheduled to receive oral antiviral *** were followed-up by SRT and intra-PBMCs HCV RNA PCR at the end of the 2nd,6th,10th,14th,18th and 24th weeks to evaluate virus clearance from plasma and PBMCs,*** CTF2 protocol evaluated SRT and PBMC PCR status at each follow-up point for determining therapy continuation or inter-ruption to address cost ***:All patients tested negative by SRT PCR after therapy for 2 ***-cation of the CTF2 protocol revealed:a)increasing HCV clear-ance rate from 75.9%at the end of 10th week to 90.3%at the end of 24th week(p<0.00001);b)faster clearance of HCV from plasma compared to PBMCs at each point of follow-up until the 18th week(p<0.05);c)higher viral elimination rates diagnosed by PBMC HCV RNA PCR(?)compared to PBMC HCV RNA PCR(+)from the 6th to 24th week of treatment(p<0.0001);d)higher over-time increase curve of combined plasma and PBMC HCV RNA determined negativity compared to the decline in positivity curves by PBMC PCR at the 6th-18th week compared to the 24th week(p<0.01)—these results validated treatment continuation;and e)solitary evaluation of EOT sustained HCV infection and relapses by PBMC HCV RNA(p<0.001).Conclusions:Early elimination of serum and tissue(PBMC)HCV infection by oral antiviral therapy can be achieved and evaluated during a cost-effective CTF2 pro-tocol application.
