OBJECTIVES Elderly patients show a higher incidence of ischemic and bleeding events after percutaneous transluminal coron-ary intervention(PCI).We sought to investigate outcomes in elderly patients treated with antithrombotic strategy guided by bleeding and ischemic risks after revascularization with last generation everolimus-eluting stent(EES).mEThODS Prospective multicenter registry including patients over 75 years revascularized with EES and antithrombotic ther-apy guided by clinical presentation,PCI complexity and PRECISE DAPT ***-primary safety endpoints were:(1)composite of cardiac death,myocardial infarction and stent thrombosis and;(2)bleeding(BARC 2-5).Primary efficacy endpoint was target lesion revascularization.A matched group of patients revascularized with current drug-eluting stents and no such tailored antith-rombotic therapy was used as *** Finally,1064 patients were included in SIERRA-75 cohort,80.8±4.2 years,36.6%women,71%acute coronary syn-dromes(ACS)and 53.6%complex ***-primary safety endpoint of major adverse cardiovascular events was met in 6.2%,co-primary safety endpoint of bleeding in 7.8%and primary efficacy endpoint of TKLR in 1.5%.The multivariable adjusted model showed no significant association of the prescribed short/long dual antiplatelet therapy(DAPT)durations with any endpoint suggesting a well tailored *** stent thrombosis reported in the subgroup with 1-3 months DAPT *** compared to control group,bleeding BARC 2-5 was significantly lower in SIERRA-75 group(7.4%vs.10.2%,P=0.04)as well as the net safety-efficacy endpoint(14.3%vs.18.5%,P=0.02).CONCLUSIONS In elderly population,the application of this risks-adjusted antithrombotic protocol after revascularization with last generation EES seems to be associated with an improved prognosis in terms of ischemic and bleeding outcomes.

A pathophysiology-based approach to the management ot early priapism

Priapism is a rare condition that involves persistent penile erection for greater than 4 h. Distinct variants exist, each with unique characteristics. Ischemic priapism is a painful medical emergency that may occur as a result of veno-occlusion leading to hypoxia and tissue death. Recurrent bouts of ischemic priapism, or stuttering priapism, require treatment for individual attacks as well as long-term prevention. Non-ischemic priapism is associated with trauma and may be managed conservatively. Recent advances into the pathophysiology of priapism have allowed the development of treatment algorithms that specifically target the mechanisms involved. In this review, we outline the basics of smooth muscle contraction and describe how derangement of these pathways results in priapism. A pathophysiological approach to the treatment of priapism is proposed with duration-based algorithms presented to assist in management.

Improving wood volume predictions in dry tropical forest in the semi-arid Brazil

The volumetric variability of dry tropical forests in Brazil and the scarcity of studies on the subject show the need for the development of techniques that make it possible to obtain adequate and accurate wood volume *** this study,we analyzed a database of thinning trees from a forest management plan in the Contendas de SincoráNational Forest,southwestern Bahia State,*** data set included a total of 300 trees with a trunk diameter ranging from 5 to 52 ***,validation and statistical selection of four volumetric models were *** to the difference in height values for the same diameter and the low correlation between both variables,we do not suggest models which only use the diameter at breast height(DBh)variable as a predictor because they accommodate the largest estimation *** comparing the best single entry model(hohenald-Krenn)with the Spurr model(best fit model),it is noted that the exclusion of height as a predictor causes the values of 136.44 and 0.93 for Akaike information criterion(AIC)and adjusted determination coefficient(R2 adj),which are poorer than the second best model(Schumacher-hall).Regarding the minimum sample size,errors in estimation(root mean square error(RmSE)and bias)of the best model decrease as the sample size increases,especially when a larger number of trees with DBh≥15.0 cm are randomly *** sampling by diameter class produces smaller volume prediction errors than random sampling,especially when considering all *** summary,the Spurr and Schumacher-hall models perform *** models suggest that the total variance explained in the estimates is not less than 95%,producing reliable forecasts of the total volume with *** estimates indicate that the bias around the average is not greater than 7%.Our results support the decision to use regression methods to build models and estimate their parameters,seeking stratification strategies in diameter classes for the sample trees.

Trace elements migrating from tailings to rock varnish laminated sediments in an old mining region from Nelson,Nevada,USA

The dispersion and influence of soluble and particulate trace elements(TEs) present in the materials from abandoned mines is an imminent problem on terrestrial and aquatic *** goal of this research is to provide a broader view of such transport,assessing particle size distribution and TE interactions(e.g.,scavenging) with mine tailings and locally derived sediments transported by water in arid *** grains from wash sediment were collected from a dry ephemeral wash in Nelson,Nevada where the Techatticup mine and mill worked between 1850 and *** were evaluated to determine the association and behavior of certain metals and metalloids including aluminum,arsenic,barium,cadmium,copper,chromium,iron,manganese,lead,selenium,vanadium,and zinc to rock varnish laminated *** show that some metals and metalloids were more concentrated on larger particles whereas others indicated the opposite *** example,As was greatest on silt fractions and least on coarse fractions,while Se was detected only on ***,Cu,Fe,mn,Pb,and Zn concentrations all increased with decreasing particle size(silt> sand> coarse),whereas Al,Ba,and V showed the opposite trend(silt he enrichment factor(EF) of Cu,Zn,Pb,As,and Se were lower in mine tailing sediment treated with wash than those obtained for source *** in treated wash sediment samples,the high EFs of Ba,Cr,V,and mn suggested a possible greater affinity of these metals to the Al,Fe,and mn oxides from the RV *** data found in this study show that transport of contaminated sediments in arid regions could lead to the enrichment of some TEs in downgradient areas from source materials.

Corrosion behavior of hypoeutectic Al-Cu alloys in h_2SO_4 and NaCl solutions

The aim of this work was to evaluate the electrochemical behaviour of hypoeutectic Al-Cu alloys immersed in two different solutions containing sulphate and chloride ions, respectively. The influence of Al2Cu associated to the dendritic arm spacing on the general corrosion resistance of such alloys is analysed. The typical microstructural pattern was examined by using scanning electron microscope. The corrosion tests were performed in both 0.5 m sulphuric acid and 0.5 m NaCl solutions at 25℃ by using an electrochemical impedance spectroscopy （EIS） technique and potentiodynamic polarization curves. Equivalent circuits by using the ZView software, were also used to provide quantitative support for the discussions. It was found that as the Cu content increased （i.e., increasing the Al2Cu fraction）, a higher susceptibility to the corrosion action in the NaCl solution is detected. In contrast, the tests carried out in the h2SO4 solution resulted in similar corrosion,rates for the three different hypoeutectic alloys.

Gastric and small intestinal lipid digestion kinetics as affected by the gradual addition of lipases and bile salts

We studied the effect of evolving from static in vitro digestion conditions towards the gradual addition of essential digestive compounds for lipid *** oil-in-water emulsions were considered:a low(5%w/w)and high(20%w/w)triolein-based emulsion with identical surfactant-to-oil ratio(0.2).Emulsions were subjected to in vitro static digestion conditions or gradual gastric lipase addition,gradual pancreatic lipase addition,and/or gradual bile salt *** these three latter cases,similar amounts of gastric/pancreatic lipase and/or bile salts were provided as in the static case,however divided over 4 doses added during the first 30 min of each digestive *** the low-lipid emulsion,gradually adding lipases and bile salts did not significantly affect lipolysis *** can be related to the sufficient amounts of digestive compounds present even in the smallest initial *** the high-lipid emulsion,the gradual addition of bile salts significantly reduced the lipolysis *** salts are essential to remove lipid digestion products from the interface and thus allow the continuation of the lipid digestion process at the ***,the lipolysis extent after 2 h of small intestinal phase was not significantly influenced by the digestion *** is again explained by the simple nature of the emulsions studied and the excess of lipase even in the smallest initial ***,this work showed that evolving towards more(semi-)dynamic digestion conditions can impact(lipid)digestion kinetics,even for relatively simple food compositions,and is of interest to obtain more physiological relevant digestion kinetics.

Electro-acupuncture to prevent prolonged postoperative ileus:A randomized clinical trial

AIm:To examine whether acupuncture can prevent prolonged postoperative ileus(PPOI)after intraperitoneal surgery for colon cancer. mEThODS:Ninety patients were recruited from the Fudan University Cancer hospital,Shanghai,China. After surgery,patients were randomized to receive acupuncture(once daily,starting on postoperative day 1, for up to six consecutive days)or usual *** was defined as an inability to pass flatus or have a bowel movement by 96 h after *** main outcomes were time to first flatus,time to first bowel movement, and *** outcomes were quality of life(QOL)measures,including pain, nausea,insomnia,abdominal distension/fullness,and sense of well-being. RESULTS:No significant differences in PPOI on day 4 (P=0.71)or QOL measures were found between the *** were also no group differences when the data were analyzed by examining those whose PPOI had resolved by day 5(P=0.69)or day 6(P= 0.88).No adverse events related to acupuncture were reported. CONCLUSION:Acupuncture did not prevent PPOI andwas not useful for treating PPOI once it had developed in this population.

LABORATORY EXPERImENTS ON NAVIGATION-INDUCED BED ShEAR STRESSESAND SEDImENT RESUSPENSION

motivated by the need to assess the environmental impact of navigation in the upper mississippi River basin. a method was developed for computing *** in unsteady flows similar to the flows induced by navigation traffic. The method accounts for statistical differences between bed shear stress distributions of steady and unsteady flows. Unsteady pulse flow tests performed in a laboratory flume indicate that the stochastic method works well for predicting entrainment, but not significantly better than a similar deterministic method. Temporal and spatial distributions of bed shear stress were also measured beneath a 1:25 scale model of a 3 barge wide by 5 barge long barge tow for a variety of flow and operating conditions. For one set of flow and operating conditions (PDF test) a large number of realizations were gathered so that statistical properties of the bed shear stress below the propeller axis could be observed. As in the laboratory tests, differences between stochastic and deterministic entrainment predictions were small for the PDF barge tow test. Consequently, deterministic calculations of total sediment entrainment are given for the remaining barge tow tests. Finally. it was found that although the stochastic model presented herein does not offer a large improvement over existing deterministic models, fluctuations in bed shear stress do play an important role in the suspension of sediment, and the timing of sediment entrainment in unsteady flows is more closely related to shear stress fluctuations than it is to the ensemble averaged shear stress.

We present a new design of a plasmonic Luneburg lens made from a gradient-index metasurface that was constructed with an array of nanometer-sized holes in a dielectric thin film. The fabricated structure consists of a planar lens with a diameter of 8.7 μm composed of a rectangular array of holes with a periodicity of 300 nm. The experimental characterization includes leakage-radiation microscopy imaging in the direction and frequency space. The former allows for characterization of the point spread function and phase distribution,whereas the latter grants access to qualitative measurements of the effective mode indices inside the plasmonic lens. The experimental results presented here are in good agreement with the expected average performance predicted by the numerical calculations. Nevertheless, the robustness of the characterization techniques presented here is also exploited to determine deviations from the design parameters.

Community-based forest management agreement in the country is a needed instrument in attaining sustainability of mangrove ***,there is no assurance that the system implemented in the mangrove forest management is ***,evaluating the mangrove management sustainability of the local tribe is a viable avenue for the appropriate *** this study,the sustainability of the mangrove management system of the Tagbanua tribe in ***,Puerto Princesa City,Palawan was *** study utilized various criteria with relevant indicators of sustainable mangrove forest management in assessing the mangrove forest management *** group discussions were conducted to identify the relevant sustainable mangrove forest management C&I and *** indicator was rated using the formulated verifiers in the form of the rating *** household interviews,FGD,KII,mangrove assessment,and secondary data analysis,this study also used a mathematical model on the Sustainability Index for Individual Criteria(SIIC)to evaluate the scores for individual criteria and the Overall Sustainability Index(OSI)of the *** a result,there are a total of seven relevant criteria,and 35 relevant indicators for mangrove management in Barangay *** on the individual rating of seven criteria,the overall rating of the sustainable mangrove management system is 1.80,which implies a fairly sustainable mangrove management ***,the computed overall sustainability index is 0.26,which is fairly or moderately *** criterion has strengths and weaknesses and needs to be improved to have a highly sustainable mangrove management system.
