Cost effectiveness of nonpharmacological prevention programs for diabetes:A systematic review of trial-based studies

Trial-based economic value of prevention programs for diabetes is inexplicit.We aimed to review the cost-effectiveness of nonpharmacological interventions to prevent type-2 diabetes mellitus(T2DM)for high-risk people.Six electronic databases were searched up to March 2022.Studies assessing both the cost and health outcomes of nonpharmacological interventions for people at high-risk of T2DM were included.The quality of the study was assessed by the Consolidated health Economic Evaluation Reporting Standards 2022 checklist.The primary outcome for synthesis was incremental cost-effectiveness ratios(ICER)for quality-adjusted life years(QALYs),and costs were standardized in 2022 US dollars.Narrative synthesis was performed,considering different types and delivery methods of interventions.Sixteen studies included five based on the US diabetes prevention program(DPP),six on non-DPP-based lifestyle interventions,four on health education,and one on screening plus lifestyle intervention.Compared with usual care,lifestyle interventions showed higher potential of cost-effectiveness than educational interventions.Among lifestyle interventions,DPP-based programs were less cost-effective(median of ICERs:$27,077/QALY)than non-DPP-based programs(median of ICERs:$1395/QALY)from healthcare perspectives,but with larger decreases in diabetes incidence.Besides,the cost-effectiveness of interventions was more possibly realized through the combination of different delivery methods.Different interventions to prevent T2DM in high-risk populations are both cost-effective and feasible in various settings.Nevertheless,economic evidence from low-and middle-income countries is still lacking,and interventions delivered by trained laypersons and combined with peer support sessions or mobile technologies could be potentially a cost-effective solution in such settings with limited resources.

Anti-obesity effects of fucoidan from Sargassum thunbergii in adipocytes and high fat diet induced obese mice through inhibiting adipogenic specific transcription factor

Studying the soil stoichiometric ratios and the influencing factors of different altitude gradients in mountain ecosystems in arid zones is significant in revealing the soil nutrient status and maintaining the sustainable development of mountain ecosystems. In this paper, 13 sample plots were established along the elevation gradient in the mountains of the southern Ili River Valley, and soil in the depth of 0-20 cm was studied to investigate nutrient stoichiometric ratios and their influencing factors. The results showed that: (1) Soil acidity gradually increased with elevation. Organic carbon and total nitrogen contents are significantly higher in the middle and high mountain belts than in the low mountain belts, while total phosphorus and total potassium contents are opposite. (2) Due to altitudinal differences, the carbon-to-nitrogen ratio is significantly higher in the mid-alpine belt than in the low and alpine belts; carbon and phosphorus ratios are significantly higher in the middle and high mountain belts than in the middle and low mountain belts; carbon and potassium ratios are significantly higher in the middle and high mountain belts than in the low mountain belts; nitrogen-phosphorus ratios are higher in the alpine zone, but there is no major difference between the transition zone and the low mountain zone; nitrogen-potassium ratios are much higher in the alpine zone, whereas there is no significant difference between the mid-alpine zone and the transition zone; the differences of phosphorus/potassium ratios between altitudinal gradients are ignorable. (3) The dominant factors affecting the stoichiometric ratios of soil physicochemical indicators are soil organic carbon (62.4%), total phosphorus (15.9%), and total carbon (13.7%); while the major environmental factors are mean annual soil temperature (27.1%), vegetation index (11.2%), elevation (12.3%), and mean annual atmospheric temperature (3.4%). The prevalence of obesity has increased and is a health concern worldwide.Due to the concerns regarding synthetic anti-obesity treatments,nowadays natural products become a trend.Previous studies proved that there is a potential to use marine algae as anti-obesity agents.Therefore,in this study,the lipid inhibitory effect of crude polysaccharide of amyloglucosidase-assisted hydrolysate from Sargassum thunbergii(STAC)and its fucoidan fractions(STAFs)on 3T3-L1 cells and high-fat diet(hFD)-induced obese mice were investigated.According to the results,the STAF3,showed the highest xylose content and exhibited significant inhibitory effects on lipid accumulation by downregulating adipogenic and lipogenic proteins in 3T3-L1 cells.Furthermore,oral supplementation with STAC significantly declined gain in body weight and fat weight,and serum lipid contents in an hFD-induced obesity mouse model.Structural and chemical characterizations demonstrated that puritied STAF3 has consistent surface morphology and small particle size,with similar structural characteristics as commercial fucoidan.Together,these results indicate that STAC and purified STAF3 from Sargassum thunbergia is a potent source to develop as ananti-obesity agents or functional food products to counter obesity.

Optical tweezers have proved to be a powerful tool with a wide range of applications.The gradient force plays a vital role in the stable optical trapping of nano-objects.The scalar method is convenient and effective for analyzing the gradient force in traditional optical trapping.however,when the third-order nonlinear effect of the nano-object is stimulated,the scalar method cannot adequately present the optical response of the metal nanoparticle to the external optical field.here,we propose a theoretical model to interpret the nonlinear gradient force using the vector method.By combining the optical Kerr effect,the polarizability vector of the metallic nanoparticle is derived.A quantitative analysis is obtained for the gradient force as well as for the optical potential well.The vector method yields better agreement with reported experimental observations.We suggest that this method could lead to a deeper understanding of the physics relevant to nonlinear optical trapping and binding phenomena.

Background Recent infection with SARS‑CoV-2 in children has been associated with multisystem inflammatory syndrome in children(MIS-C).SARS‑CoV-2 has undergone different mutations.Few publications exist about specific variants and their correlation with the severity of MIS-C.Methods This was a single-center,retrospective study including all patients admitted with MIS-C at Rady Children’s hospital-San Diego between May 2020 and March 2022.Local epidemiologic data,including viral genomic information,were obtained from public records.Demographics,clinical presentation,laboratory values,and outcomes were obtained from electronic medical records.Results The analysis included 104 pediatric patients.Four MIS-C waves were identified.Circulating variants in San Diego during the first wave included clades 20A to C.During the second wave,there were variants from clades 20A to C,20G,21C(Epsilon),20I(Alpha),and 20J(Gamma).The third wave had Delta strains(clades 21A,21I,and 21J),and the fourth had Omicron variants(clades 21K,21L,and 22C).MIS-C presented with similar symptoms and laboratory findings across all waves.More patients were admitted to the pediatric intensive care unit(PICU)(74%)and required inotropic support(63%)during the second wave.None of the patients required mechanical circulatory support,and only two required invasive ventilatory support.There was no mortality.Conclusions The various strains of SARS-CoV-2 triggered MIS-C with differing severities,with the second wave having a more severe clinical course.Whether the differences in disease severity across variants were due to changes in the virus or other factors remains unknown.

Foam rolling and stretching do not provide superior acute flexibility and stiffness improvements compared to any other warm-up intervention: A systematic review with meta-analysis

Background:Acute improvement in range of motion(ROM)is a widely reported effect of stretching and foam rolling,which is commonly explained by changes in pain threshold and/or musculotendinous stiffness.Interestingly,these effects were also reported in response to various other active and passive interventions that induce responses such as enhanced muscle temperature.Therefore,we hypothesized that acute ROM enhancements could be induced by a wide variety of interventions other than stretching or foam rolling that promote an increase in muscle temperature.Methods:After a systematic search in PubMed,Web of Science,and SPORTDiscus databases,38 studies comparing the effects of stretching and foam rolling with several other interventions on ROM and passive properties were included.These studies had 1134 participants in total,and the data analysis resulted in 140 effect sizes(ESs).ES calculations were performed using robust variance estimation model with R-package.Results:Study quality of the included studies was classified as fair(PEDro score=4.58)with low to moderate certainty of evidence.Results showed no significant differences in ROM(ES=0.01,p=0.88),stiffness(ES=0.09,p=0.67),or passive peak torque(ES=-0.30,p=0.14)between stretching or foam rolling and the other identified activities.Funnel plots revealed no publication bias.Conclusion:Based on current literature,our results challenge the established view on stretching and foam rolling as a recommended component of warm-up programs.The lack of significant difference between interventions suggests there is no need to emphasize stretching or foam rolling to induce acute ROM,passive peak torque increases,or stiffness reductions.

The insensitivity of semiconductors to visible and infrared light is a key constraint on the utilization of light energy in photocatalytic reactions.Constructing photocatalysts with full-spectrum absorption through surface engineering is an effective approach to fully harnessing light energy in semiconductor materials.herein,a novel stable Ti_(3)C_(2)T_(x)MXene/CdS heterojunction catalyst is obtained by in-situ epitaxial growth of two-dimensional(2D)CdS nanosheets on 2D MXene interface via a solvothermal method.The exceptional light absorption properties of MXene confer outstanding full-spectrum driven photocat-alytic hydrogen evolution capability upon the heterogeneous catalyst.The unique 2D/2D structure effectively mitigated the recombination of photogenerated carriers,enhancing the photocatalytic performance of the catalyst.Moreover,the composite catalyst exhibits a significantly higher surface temperature of 80.4℃under visible light irradiation at an intensity of 0.1 W/cm^(2),which is 1.84 times higher than that of CdS.Under irradiation of visible and near infrared light,the composite catalyst with photothermal ef-fect demonstrates a remarkable hydrogen evolution rate of 65.4 mmol g^(-1)h^(-1),which is 7.2 times higher than that of CdS catalyst.This study introduces a novel approach for constructing full-spectrum absorption catalysts and expands the application of the photothermal effect in photocatalytic hydrogen evolution research.

Side lobe level reduction(SLL)of antenna arrays significantly enhances the signal-to-interference ratio and improves the quality of service(QOS)in recent and future wireless communication systems starting from 5G up to 7G.Furthermore,it improves the array gain and directivity,increasing the detection range and angular resolution of radar systems.This study proposes two highly efficient SLL reduction techniques.These techniques are based on the hybridization between either the single convolution or the double convolution algorithms and the genetic algorithm(GA)to develop the Conv/GA andDConv/GA,respectively.The convolution process determines the element’s excitations while the GA optimizes the element spacing.For M elements linear antenna array(LAA),the convolution of the excitation coefficients vector by itself provides a new vector of excitations of length N=(2M−1).This new vector is divided into three different sets of excitations including the odd excitations,even excitations,and middle excitations of lengths M,M−1,andM,respectively.When the same element spacing as the original LAA is used,it is noticed that the odd and even excitations provide a much lower SLL than that of the LAA but with amuch wider half-power beamwidth(hPBW).While the middle excitations give the same hPBWas the original LAA with a relatively higher SLL.Tomitigate the increased hPBWof the odd and even excitations,the element spacing is optimized using the GA.Thereby,the synthesized arrays have the same hPBW as the original LAA with a two-fold reduction in the SLL.Furthermore,for extreme SLL reduction,the DConv/GA is introduced.In this technique,the same procedure of the aforementioned Conv/GA technique is performed on the resultant even and odd excitation vectors.It provides a relatively wider hPBWthan the original LAA with about quad-fold reduction in the SLL.

This paper presents the development and experimental utilization of a synchronized off-harmonic laser system designed as a probe for ultra-intense laser±plasma interaction experiments. The system exhibits a novel seed-generation design,allowing for a variable pulse duration spanning over more than three orders of magnitude, from 3.45 picoseconds to 10 nanoseconds. This makes it suitable for various plasma diagnostics and visualization techniques. In a side-view configuration, the laser was employed for interferometry and streaked shadowgraphy of a laser-induced plasma while successfully suppressing the self-emission background of the laser±plasma interaction, resulting in a signal-to-self-emission ratio of 110 for this setup. These properties enable the probe to yield valuable insights into the plasma dynamics and interactions at the PhELIX facility and to be deployed at various laser facilities due to its easy-to-implement design.
