AIM:To evaluate to morbidity and mortality differences between 4 underlying heart diseases,myocardial infarction(MI),angina pectoris(Angina),heart failure(HF),and atrial fibrillation(AF),after radical surgery for gastric ***:We retrospectively collected data from 221 patients of a total of 15167 patients who underwent radical gastrectomy and were preoperatively diagnosed with a history of Angina,MI,HF,or AF in 8 ***:We find that the total morbidity rate is significantly higher in the MI group(44%) than the Angina(15.7%),AF(18.8%),and HF(23.1%) groups(P < 0.01).Moreover,we note that the risk for postoperative cardiac problems is higher in patients with a history of HF(23.1%) than patients with a historyof Angina(2.2%),AF(4.3%),or MI(6%; P = 0.01).The HF and MI groups each have 1 case of cardiogenic ***:We conclude that MI patients have a higher risk of morbidity,and HF patients have a higher risk of postoperative cardiac problems than Angina or AF.

＜正＞ This paper describes an inverter-interfaced distributed generation （DG） system that is interconnected with the electric power system （EPS）. This system has two main goals such as to serve the customer with reliable electric power and to improve power quality like harmonics and voltage event. This system is interfaced through the series and the shunt inverters because inverters are effective to control the output voltage and current. The shunt inverter can serve electric power to the customer and keep the harmonic current from flowing into the power system whereas the series inverter can compensate the voltage events such as voltage sag, swell, and interruption. Because DGs are interconnected with the electric power system, DGs should have countermeasures against disturbances from the electric power system. This paper proposes the operating strategy of DGs including parallel operation mode, transition mode, and independent operation mode in consideration for the protection, islanding, and power quality. Using this operating strategy, the site load can be served high quality power.

Gastric inverted hyperplastic polyp(IHP) is a rare gastric polyp characterized by the downward growth of hyperplastic mucosal components into the submucosal layer. Macroscopically, a gastric IHP resembles a subepithelial tumor(SET); as a result, accurately diagnosing gastric IHP is difficult. This issue has clinical significance because gastric IHP can be misdiagnosed as SET or as malignant neoplasm In addition, adenocarcinoma can accompany benign gastric IHP. Although in most cases, gastric IHPs are asymptomatic and are found incidentally, these polyps may cause anemia secondary to chronic bleeding. Here, we report one case involving gastric IHP accompanied by chronic iron deficiency anemia that was successfully managed using endoscopic submucosal dissection.

AIM:To evaluate the incidence and risk factors of Korean tuberculosis(TB) infection in patients with inflammatory bowel disease(IBD) undergoing anti-TNF ***:The data of IBD patients treated with anti-TNFs in 13 tertiary referral hospitals located in the southeastern region of Korea were collected *** failed to show response or were intolerant to conventional treatments,including steroids or *** measures for latent TB infection(LTBI)and the incidence and risk factors ofactive TB infection after treatment with anti-TNFs were ***:Overall,376 IBD patients treated with antiTNF agents were recruited(male 255,mean age of anti-TNF therapy 32.5±13.0 years);277 had Crohn’s disease,99 had ulcerative colitis,294 used infliximab,and 82 used *** anti-TNF treatment,screening tests for LTBI including an interferon gamma release assay or a tuberculin skin test were performed in 82.2%of *** patients(8%)had *** cases of active TB infection including one TB-related mortality occurred during 801 personyears(PY)follow-up(1997.4 cases per 100000 PY)after anti-TNF ***(OR=5.76,95%CI:1.57-21.20,P=0.008)and WBC count<5000 mm3(OR=4.5,95%CI:1.51-13.44,P=0.007)during follow-up were identified as independently associated risk ***:Anti-TNFs significantly increase the risk of TB infection in Korean patients with *** considerable burden of TB and marked immunosuppression might be attributed to this risk.

AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,***: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. ＜正＞The stress-strain relationship between ferrite and martensite phases in the commercial dual-phase DP980 steel was studied by using in-situ neutron diffraction and the crystal plasticity finite element method（CPFEM）.The phase identification method based on the image quality（IQ） of electron backscatter diffraction（EBSD） and a filtering process was used to obtain information concerning individual crystallographic orientations for ferrite and martensite phases in DP980 ***（200） and （211） lattice strains of ferrite and martensite phases were measured along the loading and transverse directions as a function of macroscopic stress using the in-situ neutron diffraction.A CPFEM based on representative volume elements（RVEs） was applied to determine the microscopic hardening parameters for each phase by fitting the measured macroscopic stress and measured（200） and（211） lattice strains.

In this study, large-scale production of microalgal lipid containing high levels of docosahexaenoic acid (DHA) by fermentation of Aurantiochytrium sp. KRS101 was performed. The microalgal strain yielded productivity of docosahex-aenoic acid (DHA) productivity of 1.08 and 1.6 g/L/d by fermentation at 300-L and 5000-L scale stirrer-type bioreactor. The productivity was significantly enhanced upto 5.6 g/L/d by fermentation at 6000-L scale airlift-type bioreactor, probably due to the reduced shearing force. The microalgal lipid could be efficiently recovered by safe extraction methods such as ethanol extraction, hot water extraction or supercritical fluid extraction, promising commercial potential of the microalgal DHA-rich lipid in the food and feed industry.

Infectious agents causing aborted fetus problems in domestic pigs were investigated in this study. More than 10 different infectious agents were known to cause abortion in swine and the major eight viruses among them were inspected. One hundred twelve samples of aborted fetuses from nine provinces in South Korea were collected during April to November, 2013 in this study for the diagnosis of infectious agents causing abortions in pigs. Eight major infection viruses were examined in this study mainly using various diagnostic kits and reverse transcriptase polymerase chain reaction （RT-PCR）. Positive rate of the detection differed from each viruses. In this study, the main focus was the porcine reproductive and respiratory syndrome virus （PRRSV）, which took the second large portion in the positive rate of detection, and then its ORF5 gene was compared with modified live virus （MLV） vaccine strain to figure out the influence of vaccine on disease. Between four positive samples＇ sequence, two of them were 99.9%-100% similar to MLV vaccine strain and two other samples were 88.6%-92.7% similar. Similarity rate of the sequences between the vaccine and virus from aborted fetuses are very crucial, because it implies that abortion in swine can be made due to the usage of vaccine not only by the infection of field virus, and if MLV vaccine actually do have an impact on the infection, usage of the vaccine should be reconsidered.
