Neoadjuvant hormone therapy for patients with high-risk prostate cancer:a systematic review and meta-analysis

This study aimed to identify the pathological outcomes and survival benefits of neoadjuvant hormone therapy(NHT)combined with radical prostatectomy(RP)and radiotherapy(RT)administered to patients with high-risk prostate cancer(HRPCa).We searched PubMed,Embase,and the Cochrane Library for studies comparing NHT plus RP or RT with RP or RT alone,administered to patients with *** used a random-effects model to compute risk estimates with 95%confidence intervals(CIs)and quantified heterogeneity using the I2 *** and sensitivity analyses were performed to identify potential sources of *** selected 16 *** before RP significantly decreased lymph node involvement(risk ratio[RR]=0.69,95%CI:0.56–0.87)and increased the rates of pathological downstaging(RR=2.62,95%CI:1.22–5.61)and organ-confinement(RR=2.24,95%CI:1.54–3.25),but did not improve overall survival and biochemical progression-free survival(bPFS).The administration of NHT before RT to patients with HRPCa was associated with significant benefits for cancer-specific survival(hazard ratio[HR]=0.51,95%CI:0.39–0.68),disease-free survival(HR=0.51,95%CI:0.44–0.60),and bPFS(HR=0.54,95%CI:0.46–0.64).Short-term NHT combined with RT administered to patients with HRPCa conferred significant *** the advantage of local control was observed when NHT was administered before RP,there was no significant survival benefit associated with ***,short-term NHT combined with RT is recommended for implementation in standard clinical practice but not for patients who undergo RP.

Study on Inclusions in Large Sapphire Optical Crystal Grown by SAPMAC Method

The sapphire （Al2O3） single crystal is a kind of excellent infrared transmission window materials. A large-sized sapphire （Ф225 mm×205 mm, 27.5 kg） was grown by SAPMAC method （sapphire growth technique with micro-pulling and shoulder-expanding at cooled center）. Several kinds of inclusion in the large sapphire crystal were investigated by means of an optical microscopy （OM）, scanning electron microscopy （SEM） and electron probe microanalysis （EPMA）. The experimental results show that most inclusions are consisted of solid metallic and non-metallic particles as well as gas pores caused by the impurity of alumina as the raw material, the thermal dissociation of aluminum oxide melt and the reaction of the melt to the crucible material （Mo） at high temperatures. It is also found that in different crystal regions the inclusions are of varied sizes, morphology and chemical compositions. Finally, the measures to reduce and eliminate the inclusions are proposed to improve the crystal quality.

Development and validation of a nomogram including lymphocyte-to-monocyte ratio for initial prostate biopsy:a double-center retrospective study

Here,we developed a prostate cancer(PCa)risk nomogram including lymphocyte-to-monocyte ratio(LMR)for initial prostate biopsy,and internal and external validation were further conducted.A prediction model was developed on a training *** risk factors with P4%and<99%.The nomogram predicting PCa risk in patients with PSA 4-10 ng ml^(-1)also displayed good calibration and discrimination performance(C-index:0.734,95%Cl:0.708-0.760).This nomogram incorporating age,PSA,digital rectal examination,abnormal imaging signals,PSA density,and LMR could be used to facilitate individual PCa risk prediction in initial prostate biopsy.

Serum lipid profiles： novel biomarkers predicting advanced prostate cancer in patients receiving radical prostatectomy

This study aimed to evaluate the role of serum lipid profiles as novel biomarkers in predicting pathological characteristics of prostate cancer （PCa）. We retrospectively analyzed 322 consecutive patients with clinically localized PCa receiving radical prostatectomy （RP） and extended pelvic lymphadenectomy. Unconditional logistic regression was used to estimate the prostatectomy Gleason score （pGS）, pathological stage and lymph node involvement （LNI） in RP specimens. Preoperative prostate-specific antigen （PSA） levels, biopsy GS （bGS）, and preoperative tumor, node, metastasis staging were used as basic variables to predict postoperative pathological characteristics. Preoperative serum lipid profiles were introduced as potential predictors. A receiver operating characteristic （ROC） curve was used to determine predictive efficacy. Significant differences in pathological characteristics were observed among patients with normal and abnormal total cholesterol （TC）, triglyceride （TG）, and low-density lipoprotein （LDL） levels, with the exception of pGS in the TG group. Multivariable regression analysis revealed that the odds ratio for high levels of TC for LNI compared with normal TC levels was 6.386 （95% confidence interval [Cl] 1.510-27.010）, 3.270 （95% CI. 1.470-7.278） for high levels of TG for pT3-4 disease, and 2.670 （95% Ch 1.134-6.287） for high levels of LDL for pGS. The area under the ROC curve of the models with dyslipidemia was larger than that in models without dyslipidemia, in predicting pathological characteristics. Abnormal TC, TG, and LDL levels are significantly associated with postoperative pathological status in PCa patients. Together with preoperative PSA levels, bGS, and clinical stage, dyslipidemia is more accurate in predicting pathological characteristics.

Numerical investigation and modeling of sweep effects on inlet flow field of axial compressor cascades

Swept blades are widely utilized in transonic compressors/fans and provide high load,high through-flow,high efficiency,and adequate stall ***,there is limited quantitative research on the mechanism of the effect of swept blades on the flow field,resulting in a lack of direct quantitative guidance for the design and analysis of swept blades in fans/*** better understand this mechanism,this study employs a reduced-dimensional force equilibrium method to analyze more than 1500 swept cascades *** verify that circumferential fluctuation terms are responsible for inducing radial migration in the inlet airflow field of the swept blade,resulting in variations in the incidence angle and consequently leading to changes in the characteristics of the swept ***,a combination of simple functions and machine learning is utilized to model the circumferential fluctuation terms and quantify the sweep *** prediction accuracy of the model is high,with coefficient of determination greater than 0.95 on the test *** the model is applied in a meridional flow analysis program,the calculation accuracy of the program for the incidence angle is improved by 0.4°and 0.6°at the design and off-design conditions respectively,compensating for the program’s original ***,the model can also provide quantitative guidance for the design of swept blades,thereby reducing the number of design iterations and improving design efficiency.

Application of the revised Tumour Node Metastasis （TNM） staging system of clear cell renal cell carcinoma in eastern China： advantages and limitations

This study was designed to evaluate whether the revised 2010 Tumour Node Metastasis （TNM） staging system could lead to a more accurate prediction of the prognosis of renal cell carcinoma （RCC） patients. A total of 1216 patients who had undergone radical nephrectomy or partial nephrectomy for RCC from 2003 to 2011 were enrolled. All of the patients had pathologically confirmed clear cell RCC （ccRCC）. All cases were staged by both the 2002 and 2010 TNM staging systems after pathological review, and survival data were collected. Univariate and multivariate Cox regression models were used to evaluate cancer-specific survival （CSS） and progression-free survival （PFS） after surgery. Continuous variables, such as age and tumour diameter, were calculated as mean values and standard deviations （s.d.） or as median values. Survival was calculated by the Kaplan-Meier method, and the log-rank test assessed differences between groups. Statistically significant differences in CSS and PFS were noted among patients in T3 subgroups using the new 2010 staging system. Therefore, the revised 2010 TNM staging system can lead to a more accurate prediction of the prognosis of ccRCC patients. However, when using the revised 2010 staging system, we found that more than 92% of patients （288/313） with T3 tumours were staged in the T3a subgroup, and their survival data were not significantly different from those of patients with T2b tumours. In addition, T2 subclassification failed to independently predict survival in RCC patients.
