Prediction of Functional Outcome in Post-Hospital Brain Inj.ry Rehabilitation

Post-hospital brain inj.ry rehabilitation programs are afforded limited time to reduce chronic disability resulting from acquired brain inj.ry. Therefore, the purpose of this study was to identify deficit areas resulting from acquired brain inj.ry that have the greatest impact on functional outcomes to enable greater efficiency in rehabilitation programming. Study participants were 1717 persons with acquired brain inj.ry treated in residential post-hospital rehabilitation programs. Participants were assessed at admission and discharge on the MPAI-4. Functional status at discharge was evaluated based on T-scores derived from MPAI-4 discharge participation index items: Initiation, self-care and residence. The data base of 1717 was randomly divided into two subsets. Items from admission Abilities Index and select person variables were entered into stepwise multiple regression on subset one and then in a hierarchical multiple regression on subset two. Rash analysis demonstrated satisfactory construct validity and internal consistency of admission and discharge MPAI-4 evaluations (Person reliability > 0.90, Item reliability = 0.99). Both regression analyses revealed that Mobility and Novel Problem Solving accounted for 40% of the variance in functional outcome, p j.ry results in a myriad of cognitive and physical impairments. Of the many possible deficits, the greatest gains in overall functional outcomes may be achieved by allocating additional treatments aimed at reducing disability in mobility and novel problem solving.

Traumatic Brain Inj.ry and Cerebral Vascular Accident: Application of Rasch Analysis to Examine Differences in Disability and Outcome in Post-Hospital Rehabilitation

The purpose of this study was to demonstrate an application of Rasch analysis to identify differences in disability profiles resulting from traumatic brain inj.ry (TBI) and cerebral vascular accident (CVA) and to examine outcome differences between the two groups following post-hospital residential rehabilitation. Participant data were collected from 32 facilities in 16 states. From 2990 neurologically impaired individuals with consecutive admissions from 2011 through 2017, 874 met inclusion criteria: TBI (n = 687) or CVA (n = 187), 18 years or older, minimum length of stay of one month, and maximum chronicity of 1 year. Participants were evaluated at admission and discharge on the Mayo Portland Adaptability Inventory-Version 4 (MPAI-4). Rasch analysis was performed to establish item reliability, construct validity and item difficulty. A Repeated Measures Multivariate Analysis of Covariance (RM MANCOVA) determined group differences and improvement from admission and discharge. Rasch Analysis demonstrated satisfactory construct validity and internal consistency (Person reliability > 0.90, Item reliability > 0.98 for admission and discharge MPAI-4s). Both groups showed significant improvement on the MPAI-4 (p 0.0005). The TBI group was more impaired on the adj.stment scale at both admission and discharge (p 0.001). Rasch analysis identified two distinct impairment patterns. CVA participants exhibited deficits characteristic of focal impairment while the TBI group presented with deficits reflective of diffuse impairment. Rehabilitation was shown to be beneficial in reducing disability following neurologic inj.ry in both groups. Importantly, Rasch Analysis accurately produced unique disability profiles that differentiated the treatment groups. This unique statistical technique offers a promising prescriptive hierarchical model for guiding neurological rehabilitation treatment.

Impact of Chronicity on Outcomes Following Post-Hospital Residential Brain Inj.ry Rehabilitation: Application of Multivariate Statistics and Rasch Analysis

This study evaluated the impact of chronicity (onset of inj.ry to admission in-terval) on three domains of functional outcomes for a large group of traumatic brain inj.red (TBI) survivors. Subj.cts included 528 TBI adults who were treated in post-hospital residential rehabilitation centers. Subj.cts were assigned to one of three chronicity groups: 1) Early Interval (EI), 2.00 - 8.00 months n = 245, 2) Mid Interval (MI), 8.01 - 24.00 months n = 129, and (3) Late Interval (LI), 24.01 months and greater n = 154. Functional status was assessed with the MPAI-4. RM MANCOVA was applied to evaluate differences among groups from admission to discharge. Rasch analysis demonstrated satisfactory construct validity and internal consistency (Person reliability = 0.90 - 0.94, Item reliability = 0.99) for the admission and discharge MPAI-4s. Controlling for LOS and age, the RM MANCOVA revealed that each chronicity group showed significant improvement in MPAI-4 abilities, adj.stment, and participation indices from admission to discharge (p < 0.001). Improvement observed from admission to discharge was the greatest among the EI group (p < 0.001). This study demonstrated the utility of multivariate statistical approaches for understanding the complexities of TBI treatment outcomes. As measured across three domains of functioning, rehabilitation was effective in reducing disability for participants in each chronicity group. Of the three groups, EI participants presented as the most disabled at admission but also made the greatest gains when assessed at discharge.

Where Is Phase Velocity in Minkowski Space?

In the special theory of relativity, massive particles can travel at neither the speed of light c nor faster. Meanwhile, since the photon was quantized, many have thought of it as a point particle. How pointed? The idea could be a mathematical device or physical simplification. By contrast, the preceding notion of wave-group duality has two velocities: a group velocity v g and a phase velocity v p . In light v p = v g = c; but it follows from special relativity that, in massive particles, v p > c . The phase velocity is the product of the two best measured variables, and so their product constitutes internal motion that travels, verifiably, faster than light. How does v p then appear in Minkowski space? For light , the spatio-temporal Lorentz invariant metric is s 2 = c 2 t 2 − x 2 − y 2 − z 2 , the same in whatever frame it is viewed. The space is divided into 3 parts: firstly a cone, symmetric about the vertical axis ct > 0 that represents the world line of a stationary particle while the conical surface at s = 0 represents the locus for light rays that travel at the speed of light c . Since no real thing travels faster than the speed of light c, the surface is also a horizon for what can be seen by an observer starting from the origin at time t = 0. Secondly, an inverted cone represents, equivalently, time past. Thirdly, outside the cones, inaccessible space. The phase velocity v p , group velocity v g and speed of light are all equal in free space, v p = v g = c, constant. By contrast, for particles, where causality is due to particle interactions having rest mass m o > 0, we have to employ the Klein-gordon equation with s 2 = c 2 t 2 − x 2 − y 2 − z 2 + m o 2 c 2 . Now special relativity requires a complication: v p . v g = c 2 where v g c. In the volume outside the cones, causality due to light interactions cannot extend beyond the cones. Background:Newly diagnosed breast cancer patients experience symptoms that may affect their quality of life,treatment outcomes,and *** and managing breast cancer-related symptoms soon after diagnosis is *** purpose of this study was to investigate the associations between health-related fitness(HRF)and patient-reported symptoms in newly diagnosed breast cancer ***:This study utilized baseline data from the Alberta Moving Beyond Breast Cancer Cohort Study that were collected within 90 days of *** measures included peak cardiopulmonary fitness(peak volume of oxygen consumption(VO_(2peak))),maximal muscular strength and endurance,flexibility,and body *** measures included depression,sleep quality,and *** multivariable logistic regression was performed for ***:Of 1458 participants,51.5%reported poor sleep quality,26.5%reported significant fatigue,and 10.4%reported moderate *** multivariable-adj.sted models,lower relative VO_(2peak)was independently associated with a greater likelihood of all symptom measures,including moderate depression(p<0.001),poor sleep quality(p=0.009),significant fatigue(p=0.008),any symptom(p<0.001),and multiple symptoms(p<0.001).VO_(2peak)demonstrated threshold associations with all symptom measures such that all 3 lower quartiles exhibited similar elevated risk compared to the highest *** strength of the threshold associations varied by the symptom measure with odds ratios ranging from-1.5 for poor sleep quality to-3.0 for moderate depression and multiple ***,lower relative upper body muscular endurance was also independently associated with fatigue in a dose-response manner(p=0.001),and higher body weight was independently associated with poor sleep quality in an inverted U pattern(p=0.021).Conclusion:Relative VO_(2peak)appears to be a critical HRF component associated with multiple patient-reported symptoms in newly diag

Axonal regeneration following surgical nerve repair is slow and often incomplete,resulting in poor functional recovery which sometimes contributes to lifelong ***,there are no FDA-approved therapies available to promote nerve *** accelerates axonal regeneration,but systemic side effects presently outweigh its potential benefits for peripheral nerve *** authors describe herein a biodegradable polyurethane-based drug delivery system for the sustained local release of tacrolimus at the nerve repair site,with suitable properties for scalable production and clinical application,aiming to promote nerve regeneration and functional recovery with minimal systemic drug *** is encapsulated into co-axially electrospun polycarbonate-urethane nanofibers to generate an implantable nerve wrap that releases therapeutic doses of bioactive tacrolimus over 31 *** and drug loading are adj.stable for applications in small and large caliber nerves,and the wrap degrades within 120 days into biocompatible *** released from the nerve wrap promotes axon elongation in vitro and accelerates nerve regeneration and functional recovery in preclinical nerve repair models while off-target systemic drug exposure is reduced by 80%compared with systemic *** its surgical suitability and preclinical efficacy and safety,this system may provide a readily translatable approach to support axonal regeneration and recovery in patients undergoing nerve surgery.

Fluorescence Enhancement Ratio Dropdown at Low Fluences during Femtosecond Double Pulse Laser Ablation

In the study of double pulse ablation of materials （silicon and copper）, a dropdown of double pulse to single pulse fluorescence signal enhancement at low fluences is observed. The dropdown is analysed with a simple theoretical one-dimensional heat diffusion model and verified by fluorescence time constants change as a function of fluence. The dropdown is explained as a result of liquid-solid mixture layer at the liquid and solid boundary. The effect of the layer becomes important at low fluences.

Utilization of Logistical Regression to the Modified Sine-gordon Model in the MST Experiment

In this paper, a logistical regression statistical analysis (LR) is presented for a set of variables used in experimental measurements in reversed field pinch (RFP) machines, commonly known as “slinky mode” (SM), observed to travel around the torus in Madison Symmetric Torus (MST). The LR analysis is used to utilize the modified Sine-gordon dynamic equation model to predict with high confidence whether the slinky mode will lock or not lock when compared to the experimentally measured motion of the slinky mode. It is observed that under certain conditions, the slinky mode “locks” at or near the intersection of poloidal and/or toroidal gaps in MST. However, locked mode cease to travel around the torus;while unlocked mode keeps traveling without a change in the energy, making it hard to determine an exact set of conditions to predict locking/unlocking behaviour. The significant key model parameters determined by LR analysis are shown to improve the Sine-gordon model’s ability to determine the locking/unlocking of magnetohydrodyamic (MHD) modes. The LR analysis of measured variables provides high confidence in anticipating locking versus unlocking of slinky mode proven by relational comparisons between simulations and the experimentally measured motion of the slinky mode in MST.

Chronic disease and the link to physical activity

Chronic diseases have become a focal point of public health worldwide with estimates of trillions of dollars in annual health care cost and causing more than 36 million deaths a year. Lifestyle factors such as physical inactivity are heavily correlated with the development of many chronic diseases. New strategies for primary and secondary disease prevention are desperately needed to aid in blunting the negative economic and social impact of these diseases. Physical activity （PA） and exercise are now considered principal interventions for use in primary and secondary prevention of chronic diseases. Currently, more emphasis in primary prevention of disease is necessary to reduce disease risk in youth and adults; however with chronic disease prevalence so high, similar emphasis is also necessary for secondary prevention in those children and adults already inflicted with chronic diseases. Conditions such as cardiovascular disease, type 2 diabetes, obesity, and cancer are drastically improved when PA and exercise are part of a medical management plan. In addition, the national PA guidelines in conj.nction with PA promotion tools like Exercise is MedicineTM are needed to promote increased PA and exercise levels worldwide.
