Humoral Response and Tolerance of Vaccination against SARS-CoV-2 in Adults Senegalese Patients Undergoing Hemodialysis: A multicenter Prospective Study

Introduction: Following the COVID-19 pandemic, vaccination has been proposed in several countries as the main preventive measure despite very limited data, particularly in dialysis patients. We conducted this study to assess the immunological response to vaccination in Senegalese hemodialysis patients. Patients and methods: We conducted a prospective study, in two dialysis centers in Dakar from march 30th to August 30th, 2021 including patients on hemodialysis for >6 months, vaccinated against SARS-CoV-2 according to the vaccination schedule recommended by WHO. A vaccine response was considered positive when seroconversion was observed after one dose of vaccine. The clinical efficacy of immunization was defined as the absence of new COVID-19 infection in patients who received a complete vaccination. Results: Among the 81 patients included in the study, 7.4% had anti-Spike Igm antibodies before their first vaccination. Seroprevalence of Igm antibodies was 38.3% one month after the first vaccine dose (at m1) and 8.6% one month after the second dose (at m4). Anti-Spike IgG antibodies were present in 40.3% of patients before vaccination, in 90.1% at m1, and in 59.7% at m4. Among patients previously infected with SARS-CoV-2, 10.2% had Igm antibodies at m0, 31.6% at m1, and 10.5% at m4 post-vaccination. Similarly, seroprevalences of IgG antibodies in this subgroup were 31.5%, 61.3%, and 50.0% respectively at m0, m1, and m4 post-vaccination. A comparison of seroconversion rates between m0 and m4 showed significant differences only for IgG in COVID-19 naive patients. mean duration in dialysis and the existence of previous COVID-19 infection were associated with patients’ vaccinal response after the two doses. Age, gender and the use of immunosuppressive treatment did not influence post-vaccinal antibody production. Conclusion: Vaccination against COVID-19 in Senegalese hemodialysis patients induced a low seroconversion rate but it was well tolerated. m

Contribution of <sup>99m</sup>Tc-DPD Scintigraphy in the Diagnosis of Cardiac Amyloidosis in Black Africans

Cardiac amyloidosis presents a picture of hypertrophic cardiomyopathy with heart failure with preserved ejection fraction. It is largely underdiagnosed, especially in black Africans, and therefore falls under the category of heart disease classified as idiopathic. Light chain amyloidosis (AL) is mainly found in Caucasian subjects and the mutant variant of transthyretin (TTRm) in negroid subjects. Numerous studies have shown that ATTRm was found predominantly in black American and black British patients. In African countries the entity of idiopathic heart failure is quite important because of lack of diagnosis, ETT, mRI and immuno-histochemistry are expensive or not available. We can probably assume that the proportion of cardiac amyloidosis is quite important in black Africans. The question is if 99mTc-DPD really easy to perform, can probably help to investigate in the nuclear medicine department in Africa. No large-scale study has been able to demonstrate the prevalence or not of cardiac amyloidosis in black-African subjects and by extension reduce this nosological entity of idiopathic heart disease. The 99mTc-DPD scintigraphy using Perrugini’s visual sore allows localization and classification of amyloid damage. The mechanism of binding of 99mTc-DPD to amyloid fibril deposits is not well known, its binding to TTR-type (mutated or wild type) amyloidosis is greater than the AL variant. In the diagnostic algorithm, endomyocardial biopsy is the gold standard but remains invasive, ETT with the strain allows a presumptive diagnosis and remains an operative examination dependent and is not reproducible. Cardiac mRI allows some localization of amyloid deposits but still remains less sensitive than scintigraphy. In addition, performing the whole-body mRI is very restrictive (time, antenna change and cost). Water/Oil Pickering Emulsion Stabilized by magnesium Oxide Particles: A Potential System with Two Active Substances (Paracetamol and Griseofulvin)

Pickering emulsions are systems without surfactants, stabilized by solid particles. These emulsions are experiencing a renewed interest, on the one hand, because it is preferable to limit the use of synthetic surfactants for toxicological and environmental reasons and, on the other hand, the need to make new formulations in order to control the drug release patterns by encapsulation or controlled release. Thus, we were interested in the formulation and evaluation of W/O Pickering emulsions stabilized by particles of magnesium oxide with paracetamol in the internal phase and griseofulvin in the external phase. The Bancroft rule served as a model for the formulation. The emulsification was carried out by progressively adding an aqueous phase dispersed in an oil-dispersing phase using a turbo rotor stator mixer. The stability of these emulsions was studied using several parameters (droplet size, pH, viscosity, conductivity ...) and the qualitative and quantitative analysis of the active ingredients by UV-visible spectrophotometry. The results obtained showed that the dye test and the conductivity measurement confirmed the W/O nature of these emulsions. Some parameters such as droplet size, pH and viscosity were strongly influenced by the amounts of magnesium oxide particles and the two active ingredients used. The qualitative and quantitative analysis of the active ingredients confirmed the presence of griseofulvin in the oil phase and paracetamol in the aqueous phase. Thus, we have succeeded in developing a stable W/O Pickering emulsion with magnesium oxide particles. In addition, we were able to incorporate paracetamol into the dispersed phase and griseofulvin into the dispersing phase of the emulsion.

Purpose: Bacterial resistance to antibiotics has become a global public health problem. Enterobacteriaceae ESBL is among the most incriminated in this emergence which reduces the therapeutic possibilities. Thus, the objective of this study is to determine the prevalence of the extended-spectrum beta- lactamase (ESBL) producing Enterobacteriaceae at ZNH and their antibiotic susceptibility profile. materials and methods: This is a prospective study carried out over 5 months in all hospitalized and non-hospitalized patients in whom a culture was taken for the diagnosis of an infection. The search for ESBL is done by the double disc diffusion method. Results: In total, 21 out of 45 of our strains are ESBL-producing, i.e. a frequency of 46.7%. The mean age is 41.62 (±22.90) with extremes of 2.6 - 78 years. The distribution of ESBL producing species showed a predominance of E. coli with 66.7% followed by K. pneumoniae and K. oxytoca each 9.5%. All ESBL strains were resistant to Amoxicillin, Cefalotin, Pipiracillin, Piperacillin + tazobactam, Ticarcillin, Ticarcillin + clavulanic acid. Resistance to C3G and Aztreonam was each 95.5%, to Amoxicillin/clavulanic acid 9.1%. All strains were sensitive to imipenem. E. coli strains showed resistance: 85.7% to ciprofloxacin, 50% to Amikacin, 57.1% to Gentamicin. For K. pneumoniae, it is 66.7% for Gentamicin and Ciprofloxacin. Conclusion: Our study reports a high prevalence of ESBL at the HNZ. This must be taken into account in order to monitor this phenomenon which constitutes a public health problem. The study also reports sensitivity to Amoxicillin/clavulanic acid and Aminoside which can be an alternative.

The aim of this study was to evaluate the level of protection of employees who are exposed to radiation in a level III hospital establishment. It was a descriptive cross-sectional survey of six months’ duration, involving eight level III Hospitals (Aristide Le Dantec, Fann, Hoggy, Hear, Abass Ndao, Pikine, Touba, and Thiès) in Senegal. Sixty-one of the one hundred questionnaires were recovered (overall response rate of 61%). The population of the study was mainly female (54.1%). The average age was 38.57 with extremes ranging from 23 to 65 years old. In the places where ionizing radiation sources are handled, only at the Aristide Le Dantec Hospital did we find a “competent person in radiation protection”. This explained the lack of a classification of employees and work areas. Forty out of sixty-one (73.77%) had no knowledge of the basic principles of radiation protection (justification, optimisation, dose limitation) and had not ever taken radiation holidays. For radiovigilance, exposure time limits to ionizing radiation concerned only 29/61 or 47.54% of the study population. The inverse square law of distance was known by only 40 workers, of whom 15 had no compliance. We found the presence of dosifilms in only 7/61 or 11.47% of the workers. On the other hand, the use of lead aprons was well established and concerned 57/61 workers,m> i.e.m>, 93.44%. In sum, ionizing radiation causes adverse health effects. The absence of a good radiation protection culture in Senegal requires the presence of at least 4 to 5 competent persons in radiation protection for quality training of workers in radiobiology, radiopathology and radiation protection.

Current Status and Possibilities for the Development of the Cattle Breeding in the Primorsko-goranska County, Croatia

At the beginning of 2007 the milk producers from the Primorsko-goranska County, Croatia were surveyed through the questionnaire in order to determine the current status and to highlight the obstacles and the possibilities of development and improvement of the modern cattle breeding in the observed county. The questionnaire spanned 2001-2006 period and all together 260 families were surveyed. The survey aimed to get as much as possible useful information＇s that would indicate the problems concerning the cattle breeding in this area, and based on these results to give certain guidelines for the improvement of current status. Older age of milk producers, significant number of family farms with small number of cattle and difficult implementation of selective measures due to the small number of cows that were included in milk recording were determined as the main obstacles for cattle breeding development in the surveyed county. It is emphasized that development of cattle breeding in this area should be based upon the increased number of family farms with larger herds. To achieve this it is necessary to retain existing cattle fund, and then gradually increase the number of cattle in the mentioned county through the import of breeding heifers of dairy and combined breeds from neighboring European countries with highly developed cattle breeding. Besides that, it is also necessary to create conditions for the production of organic （ecological） products, which represents the future of agriculture, livestock production, bearing in mind that demanding European market has recognized the value of organic agricultural products that were produced through environment friendly production, which Primorsko-goranska County, due to its significant natural resources, could easily assure.
