Therapeutic temperature modulation is associated with pulmonary complications in patients with severe traumatic brain injury

AIM: To examine complications associated with the use of therapeutic temperature modulation(mild hypothermia and normothermia) in patients with severe traumatic brain injury(TBI). MeTHODS: One hundred and fourteen charts were reviewed. Inclusion criteria were: severe TBI with Glasgow Coma Scale(GCS) ensive care unit(ICU) stay > 24 h and non-penetrating TBI. Patients were divided into two cohorts: the treatment group received therapeutic temperature modulation(TTM) with continuous surface cooling and indwelling bladder temperature probes. The control group received standard treatment with intermittent acetaminophen for fever. Information regarding complications during the time in the ICU was collected as follows: Pneumonia was identified using a combination of clinical and laboratory data. Pulmonary embolism, pneumothorax and deep venous thrombosis were identified based onimaging results. Cardiac arrhythmias and renal failure were extracted from the clinical documentation. acute respiratory distress syndrome and acute lung injury were determined based on chest imaging and arterial blood gas results. A logistic regression was conducted to predict hospital mortality and a multiple regression was used to assess number and type of clinical complications. ReSULTS: One hundred and fourteen patients were included in the analysis(mean age = 41.4, SD = 19.1, 93 males), admitted to the Jackson Memorial Hospital Neuroscience ICU and Ryder Trauma Center(mean GCS = 4.67, range 3-9), were identified and included in the analysis. Method of injury included motor vehicle accident(n = 29), motor cycle crash(n = 220), blunt head trauma(n = 212), fall(n = 229), pedestrian hit by car(n = 216), and gunshot wound to the head(n = 27). ethnicity was primarily Caucasian(n = 260), as well as Hispanic(n = 227) and African American(n = 223); four patients had unknown ethnicity. BACKGROUND Dyslipidemias are metabolic abnormalities associated with chronic diseases caused by genetic and environmental *** Mexican population displays regional differences according to ethnicity with an impact on the type of *** To define the main dyslipidemias,the frequency of lipid-related risk alleles,and their association with hyperlipidemic states among different ethnic groups in West *** In a retrospective study,1324 adults were selected to compare dyslipidemias and lipid-related gene ***,clinical,and laboratory data were collected.A subgroup of 196 normal weight subjects without impaired glucose was selected for the association *** was determined by allelic discrimination *** Hypercholesterolemia was the most prevalent dyslipidemia(42.3%).The frequency of the risk alleles associated with hypoalphalipoproteinemia(ABCA1)and hypercholesterolemia(APOe,LDLR)was higher in the Native Americans(P=0.047).In contrast,the Mestizos with european ancestry showed a higher frequency of the risk alleles for hypertriglyceridemia(APOe2,MTTP)(P=0.045).In normal weight Mestizo subjects,the APOB TT and LDLR GG genotypes were associated risk factors for hypercholesterolemia(OR=5.33,95%CI:1.537-18.502,P=0.008 and OR=3.90,95%CI:1.042-14.583,P=0.043,respectively),and displayed an increase in low-density lipoprotein cholesterol levels(APOB:β=40.39,95%CI:14.415-66.366,P=0.004;LDLR:β=20.77,95%CI:5.763-35.784,P=0.007).CONCLUSION Gene polymorphisms and dyslipidemias showed a differential *** primary health care strategies are required to mitigate their prevalence considering the genetic and environmental features which could have important implications for personalized medicine within the new era of precision medicine.

The RXTe/PCA 35-Day X-Ray Lightcurve of Hercules X-1

The archival Rossi X-ray Timing explorer (RXTe) Proportional Counter Array (PCA) observations of HZ Her/Her X-1 are analyzed here. The time periods for Anomalous Low States are refined. The 35-day X-ray lightcurve of Her X-1 is produced using 35-day phases determined with RXTe All-Sky Monitor (ASM) and Swift Burst Alert Telescope (BAT) data, with adjustments based on the RXTe/PCA data. As a result, we constructed the best-phased 35-day lightcurve of Her X-1 with the highest sensitivity. The RXTe/PCA data are separated into 8 different substates of the 35-day cycle: Main High (MH) turn-on, MH, MH decline, Low State (LS) 1, Short High (SH) turn-on, SH, SH decline, and LS2. Orbital lightcurves are created for each substate, and they are compared. eclipse data are extracted for each substate to determine the changes in eclipse shape with 35-day phase. A scattering corona model is fit to the eclipses for the different 35-day substates: this shows that the corona is present with similar intensity at all 35-day phases. We search for previously reported dips without absorption but find none.

estimation of Contraction Coefficient of Gorlin equation for Assessment of Aortic Valve Area in Aortic Stenosis

Background: The Gorlin equation is the reference method for the assessment of aortic valve area in aortic stenosis and is calculated using a constant, called the coefficient of contraction, which is empirically assumed to be 1. This coefficient is the ratio of effective aortic area to anatomic aortic area, and a value of 1 indicates that both are the same. The purpose of this study was to estimate the actual coefficient of contraction in patients with aortic stenosis and to evaluate its impact on aortic area as calculated by the Gorlin equation. Methods: We studied 17 patients with moderate to severe aortic stenosis. effective aortic area was calculated using the continuity equation. Anatomic aortic area was obtained by planimetry with transesophageal echocardiography. Aortic valve area by the Gorlin equation was calculated from echocardiography data. The coefficient of contraction was derived as above. Results: The coefficient of contraction was inversely related to the pressure recovery. effective area was correlated with anatomic area (r = 0.86, P 2). Aortic area by the Gorlin equation was not correlated with anatomic area, but the correlation became significant when the Gorlin equation was corrected for coefficient of contraction and pressure recovery. Conclusions: Using a coefficient of contraction of 1 in the Gorlin equation gives a poor correlation with anatomic area. Using the calculated coefficient of contraction for each patient and the mean gradient for pressure recovery improves the correlation with anatomic area. These facts could be taken in account when Gorlin equation is considered as the reference method.
