Valorisation of industrial hemp (Cannabis sativa L.) residues and cheese whey into volatile fatty acids for single cell protein production

The production of single cell protein(SCP)using lignocellulosic materials stands out as a promising route in the circular bioeconomy ***,multiple steps are necessary for lignocellulosics-to-SCP processes,involving chemical pretreatments and specific aerobic *** there are no studies that investigated the SCP production from lignocellulosics by using only biological processes and microbial biomass able to work both anaerobically and *** this view,the valorisation of industrial hemp(Cannabis sativa L.)biomass residues(HBRs),specifically hurds and a mix of leaves and inflorescences,combined with cheese whey(CW)was investigated through a semi-continuous acidogenic co-fermentation process(co-AF).The aim of this study was to maximise HBRs conversion into VFAs to be further used as carbon-rich substrates for SCP *** process conditions were tested by either removing CW or increasing the amount of HBRs in terms of VS(i.e.,two and four times)to evaluate the performance of the co-AF *** HBRs resulted in a proportional increase in VFA production up to 3115 mg HAc L^(-1),with experimental production nearly 40%higher than theoretical *** synergy between HBRs and CW was demonstrated,proving the latter as essential to improve the biodegradability of the *** produced VFAs were subsequently tested as substrates for SCP synthesis in batch aerobic tests.A biomass concentration of 2.43 g TSS L^(-1) was achieved with a C/N ratio of 5.0 and a pH of 9.0 after two days of aerobic fermentation,reaching a protein content of 42%(g protein per g TSS).These results demonstrate the overall feasibility of the VFA-mediated HBR-to-SCP valorisation process.

Very late bioresorbable scaffold thrombosis and reoccurrence of dissection two years later chronic total occlusion recanalization of the left anterior descending artery

We describe the case of a patient presenting with STsegment elevation myocardial infarction due to very late scaffold thrombosis. The patient was already admitted for an elective percutaneous recanalization of a chronically occluded left anterior descending artery(LAD). The procedure was performed according the subintimal tracking and re-entry(STAR) technique with 4 bioresorbable vascular scaffolds implantation. However, even though the coronary flow was preserved at the end of the procedure, the dissected segment was only partially sealed at the distal segment of the LAD. After 18 mo of regular assumption, dual antiplatelet therapy was discontinued for 10 mo before his presentation at the emergency room. This is the first reported case of a very late scaffold thrombosis after coronary chronic total occlusion(CTO) recanalization performed according to the STAR technique. This case raises concerns about the risk of very late scaffold thrombosis after complex CTO revascularization.

Drug-Eluting Stent for the Treatment of Early Fistula Failure

Introduction: Morbidity and mortality in chronic haemodialysis patients is related to dialysis efficiency. Several complications may occur with vascular access, usually associated with a stenosis. This is known to occur frequently in the venous outflow. Stenosis of the arterial side is not as frequently discussed, but it is also likely to compromise fistula function. Traditionally, surgical and percutaneous interventions have been used to treat failing fistulas, but the employment of drug-eluting stents for the treatment of failing fistulas due to the arterial stenosis has been described rarely. Methods: A 65-year-old male patient referred to our ambulatory because of hand ischemia during haemodialysis treatments only few days after radial-cephalic fistula creation. After physical and echo-color-doppler examination, angiography was performed and percutaneous intervention was proposed. After the positioning of a guiding catheter, the lesion was crossed with a 0.014” guide wire followed by direct drug-eluting stent implantation. Results: Final angiogram showed a good result and a preserved flow through the fistula. Six months later the patient was asymptomatic and the fistula was still working. Conclusion: Although further prospective studies are necessary, percutaneous transluminal angioplasty (PTA) with drug-eluting stents implantation could be considered a safe and effective technique for the treatment of arteriovenous fistulas stenosis.

Early diagnosis of Parkinson’s disease(PD):how we can improve the therapeutic approach:PD is a neurodegenerative disease characterized by motor dysfunctions(tremor,rigidity,bradykinesia and impaired posture/balance)elicited by selective depletion of dopaminergic(DA)neurons in substantia nigra pars *** neuron loss is associated with neuronal inclusions of the phosphorylatedα-synuclein protein called Lewy body(Shults,2006).Although the underlying neurodegenerative process is not affected,the management of PD patients has been revolutionized with the introduction of levodopa and DA drugs in the routine therapy,which ensures initial symptomatic relief of motor functions through the DA supply in the nigrostriatal *** drugs are currently the best option for treating PD,although their chronic use is associated with progressive dopamine resistance and loss of effectiveness in the recovery of motor *** therapeutic strategies,including agonists of DA receptors,monoamine oxide B inhibitors,and even deep brain stimulation techniques have been developed to overcome these clinical ***,these therapeutic approaches cannot restore PD-compromised functions,as irreversible DA neurodegeneration has occurred in substantia nigra pars compacta when first motor symptoms *** nigrostriatal system is traditionally considered as the first region affected by neuronal impairment in Parkinsonisms;however,α-synuclein aggregation appears in a pre-motor stage of the disease in the enteric nervous system(ENS).

Testicular parenchymal abnormalities in Klinefelter syndrome： a question of cancer？ Examination of 40 consecutive patients

Klinefelter syndrome （KS） is a hypergonadotropic hypogonadism characterized by a 47, XXY karyotype. The risk of testicular cancer in KS is of interest in relation to theories about testicular cancer etiology generally; nevertheless it seems to be low. We evaluated the need for imaging and serum tumor markers for testicular cancer screening in KS. Participants were 40 consecutive KSpatients, enrolled from December 2009 to January 2013. Lactate dehydrogenase （LDH）, alpha-fetoprotein （AFP）, and beta-human chorionic gonadotrophin subunit （^-HCG） serum levels assays and testicular ultrasound （US） with color Doppler, were carried out at study entry, after 6 months and every year for 3 years. Abdominal magnetic resonance （MR） was performed in KS when testicular US showed micro-calcifications, testicular nodules and cysts. Nearly 62% of the KS had regular testicular echotexture, 37.5% showed an irregular echotexture and 17.5% had micro-calcifications and cysts. Eighty seven percent of KS had a regular vascular pattern, 12.5% varicocele, 12.5% nodules 〈1 cm, but none had nodules 〉1 cm. MR ruled out the diagnosis of cancer in all KS with testicular micro calcifications, nodules and cysts. No significant variations in LDH, AFP, and ^-HCG levels and in US pattern have been detected during follow-up. We compared serum tumor markers and US pattern between KS with and without cryptorchidism and no statistical differences were found. We did not find testicular cancer in KS, and testicular US, tumor markers and MR were, in selected cases, useful tools for correctly discriminating benign from malignant lesions.

S100B protein in the gut:The evidence for enteroglial-sustained intestinal inflammation

Glial cells in the gut represent the morphological and functional equivalent of astrocytes and microglia in the central nervous system （CNS）. In recent years, the role of enteric glial cells （EGCs） has extended from that of simple nutritive support for enteric neurons to that of being pivotal participants in the regulation of inflammatory events in the gut. Similar to the CNS astrocytes, the EGCs physiologically express the SIOOB protein that exerts either trophic or toxic effects depending on its concentration in the extracellular milieu. In the CNS, SIOOB overexpression is responsible for the initiation of a gliotic reaction by the release of pro-inflammatory mediators, which may have a deleterious effect on neighboring cells. SlOOB-mediated pro-inflammatory effects are not limited to the brain： SIOOB overexpression is associated with the onset and maintenance of inflammation in the human gut too. In this review we describe the major features of EGCs and SIOOB protein occurring in intestinal inflammation deriving from such.

The occurrence of leiomyoma of the rectum is uncommon. Most of these lesions are clinically silent and are found incidentally during laparotomy or endoscopic procedures for unrelated conditions. Symptomatic leiomyomas of the rectum are encountered less frequently, with only sporadic reports in the literature. We describe a case of a leiomyoma of the rectum presenting as recurrent lower gastrointestinal hemorrhage and secondary anemia.

Inflammatory bowel disease(IBD)includes Crohn’s disease(CD),ulcerative colitis and unclassified *** commonly involves the terminal ileum and colon but at the time of diagnosis it can be confined to the small bowel(SB)in about 30%of the patients,especially in the young *** of isolated SB-CD can be challenging and objective evaluation of the SB mucosa is essential in differentiating CD from other enteropathies to achieve therapeutic decisions and to plan the *** introduction of cross-sectional imaging techniques and capsule endoscopy(CE)have significantly expanded the ability to diagnose SB diseases providing a non-invasive test for the visualization of the entire SB *** main CE limitations are the low specificity,the lack of therapeutic capabilities and the impossibility to take *** assisted enteroscopy(DAE)enables histological confirmation when traditional endoscopy,capsule endoscopy and cross-sectional imaging are inconclusive and also allows therapeutic interventions such as balloon stricture dilation,intralesional steroid injection,capsule retrieval and more recently stent *** the current review we will discuss technical aspect,indications and safety profile of DAE in children and adults with IBD.

Background Breast conserving surgery along with adjuvant radiotherapy is effective in terms of local control and survival for early- stage breast cancer （1）. External beam radiotherapy （EBRT） following breast conserving surgery has been shown to improve survival by preventing local recurrence, in the Early Breast Cancer Trialists＇ Collaborative Group meta-analysis （2）. Standard radiotherapy typically requires numerous fractions over a 3-5 week period and is performed weeks or months after surgery or chemotherapy.
