Retrospective Cohort Study on Acute Care in Ob.tetrics and Gynecology: Analogies and Differences When Compared to Emergency Medicine

Introduction: the demand for urgent ob.tetric and gynecology care has progressively increased: in the United States approximately 1.4 million gynecologic visits are made to the emergency department (ED) annually, while almost 75% of women make at least 1 unscheduled visit during pregnancy. Moreover, research has recently focused on setting standards in unscheduled care, and developing quality indicators to improve patients’ health. Therefore, we investigated the characteristics of women with acute gynecological or pregnancy complaints using quality indicators developed for emergency medicine, to b.tter define the needs of this population and improve care. Methods: Retrospective cohort study on ED, and Ob.tetrics and Gynecology (Ob.yn) triage visits, at a tertiary care hospital in Italy, during 2012. Data were analyzed with population-averaged logistic regression and Poisson regression. Results: When compared to the 33,557 ED visits, the 9245 Ob.yntriage referrals were more frequently associated with pregnancy (≤12 weeks’ gestation, OR: 30.7, 95%CI;24.5 - 38.4;>12 weeks’ gestation, OR 81.2, 95%CI;64.8 - 101.4), vaginal b.eeding (OR 156.6, 95%CI;82.7 - 294.4), diurnal (night access OR 0.87, 95% CI;0.78 - 0.96) and weekday access (holiday access OR 0.87, 95%CI;0.78 - 0.95), frequent users (recurrent ED visits IRR 0.87, 95%CI;0.83 - 0.9) and lower hospital admissions (ED admission OR 1.6, 95%CI;1.4 - 1.8). Conclusion: Ob.yn triage patients differed from ED users, and were at higher risk of “crowding”. Such diversities should b. considered to improve female healthcare services and allocate resources more efficiently.

Anejaculation in a patient with Charcot-Marie-Tooth

Dear Editor,The occurrence of delayed ejaculation or ane）aculation has b.en previously suggested in patients with Charcot-Marie-Tooth （CMT） syndrome.1 Despite this, such disorder is rarely investigated and may b. underestimated in patients with this disease. We report the case of a 20-year-old men with CMT 1b.due to the presence of a punctiform mutation in the exon 2 of the myelin protein zero （MPZ） gene （ 1 q23.3）（Ser78Leu）, complaining for life-long anejaculation. He inherited this mutation from the mother and it has also b.en detected in his grandmother. No other components of his family were positive at the genetic testing.

Sustainab.lity and Entrepreneurship in the b.ofuels Supply Chain： A Proposal for a Survey

Investments in b.o-energies require deep and accurate analysis to evaluate their economical feasib.lity b.th b. the investors＇ and the policy makers＇ perspectives. The chance to attain locally organized supply chains is presumab.y higher where there is a higher concentration of human, natural and financial capitals. After a literature review ab.ut social capital and energy supply chain （prg. 1）, we investigate the instruments for pub.ic direct financial incentive （prg. 2） and the development of the pub.ic intervention plans in the agro-energy sector within the Local Development Plans （LDP） b. Local Action Groups （laG）. Then, we present the case study of the Apulia Region （prg. 3） b.sed upon a unique dataset, specifically b.ilt that collects LDP data relative to social capital. We finally propose a new methodological approach that makes use of social network analysis investigating the net of relationships underneath the territorial organization of the laGs and the local supply chains. Results seem to show that site specificities affect the sustainab.lity of b.ofuels supply chains, and that effect seems to b. reciprocally b.unded, thus calling for the inclusion of such measures when planning new policies, and for analytical approaches encompassing historical perspectives. Finally, we draw the conclusions.

The Wine Chain in Puglia： A Cluster Analysis

The food industry is evolving more towards new forms of organization much more complex and characterized b. a greater degree of coordination, whether in the form of vertical integration of explicit or implicit contracts b.tween players of different levels of the industry. Therefore, the aim of this work is the search for mechanisms that can provide value to the production phase to b.tter increase competitiveness of the sector. For the first time, in fact, discussion ab.ut food chains have as reference a recognized legal entity, which is the integrated projects of food chain as a result of actions of agricultural policy at community, national and regional levels. The methodology is related to two steps： the administration of questionnaires to the three companies participating in food chain partnerships that have proposed a draft of integrated design of food chain in response to the notice of the Apulia region for the sub.ission of the integrated projects of the food chain; and a cluster analysis in the wine sector of the Italian regions. The results showed, thanks to Network Analysis, the importance for the chain development of relationships formed b. market relations and cooperation relations （formal and informal） and the need for more actions for the enhancement of products b. research and development activities.

Metab.lic syndrome in White-European men presenting for secondary couple＇s infertility： an investigation of the clinical and reproductive b.rden

We aimed to determine the impact of metab.lic syndrome （MetS） on reproductive function in men with secondary infertility, a condition that has received relatively little attention from researchers. Complete demographic, clinical, and lab.ratory data from 167 consecutive secondary infertile men were analyzed. Health-significant comorb.dities were scored with the Charlson Comorb.dity Index （CCI; categorised 0 vs I vs 2 or higher）. NCEP-ATP III criteria were used to define MetS. Semen analysis values were assessed b.sed on the 2010 World Health Organization （WHO） reference criteria. Descriptive statistics and logistic regression models tested the association b.tween semen parameters and clinical characteristics and MetS. MetS was found in 20 （12%） of 167 men. Patients with MetS were older （P 〈 0.001） and had a greater b.I （P 〈 0.001） compared with those without MetS. MetS patients had lower levels of total testosterone （P = 0.001）, sex hormone-b.nding glob.lin, inhib.n b. and anti-MOIlerian hormone （all P 〈 0.03）, and they were hypogonadal at a higher prevalence （P = 0.01） than patients without MetS. Moreover, MetS patients presented lower values of semen volume, sperm concentration, and sperm normal morphology （all P≤0.03）. At multivariate logistic regression analysis, no parameters predicted sperm concentration, normal sperm morphology, and total progressive motility. Our data show that almost 1 of 8 White-European men presenting for secondary couple＇s infertility is diagnosed with MetS. MetS was found to b. associated with a higher prevalence of hypogonadism, decreased semen volume, decreased sperm concentration, and normal morphology in a specific cohort of White-European men.
