Non-Infectious Complications of Non-Tunneled Central Venous Catheterization for Hemodialysis: Incidence and Reasons in Ouagadougou (Burkina Faso)

Goal: Evaluate the importance and reasons of non-infectious complications of non-tunneled central venous catheterization in our hemodialysis unit. Patients and methods: The study, a prospective type, was conducted in the department of nephrology and hemodialysis of Yalgado Ouedraogo University Hospital Center (YO-UHC) in Ouagadougou, Burkina Faso, from 15 February to 30 June 2015. Patients in whom a new central venous catheter (CVC) was inserted during the study period were included. Catheterization-related complications were noted. Results: During the study period, 156 CVCs (9 per week) were placed in femoral (56.4%), internal jugular (40.4%) or subclavian vein (3.2%). There were 114 patients (59.7% of men and 40.3% of women), average age 41.8 ± 17.1 years, low socio-economic level in 64% of cases. At least a non-infectious complication was observed in 67 cases representing 42.9%. They were: puncture failure (40%), arterial puncture (12.2%), puncture of the thoracic duct (1.3%), pneumothorax (1.3%), bleeding related to the catheter insertion (5.8%), hematoma (1.3%), opposite direction (0.6%), dysfunction of the CVC (10.3%), femoralthrombophlebitis (3.2%). Conclusion: Non-infectious complications of non-tunneled central venous catheterization in our hemodialysis unit were frequent and sometimes severe. Their common denominator was the absence of ultrasound guidance. Our study reaffirms the need for equipping with Doppler ultrasound in our hemodialysis units, even in developing countries, for better security of the patient during central venous catheterization.

Factors Associated with Infection of the Central Venous Catheter for Hemodialysis in Ouagadougou (Burkina Faso)

Goal: The goal is to evaluate infections associated with central venous catheters of hemodialysis (CRI) in hemodialysis patients in Ouagadougou. Patients and methods: The study was cross-sectional, from February 15th to June 30th, 2015. It was conducted in Ouagadougou (Burkina Faso). We included patients with a new central venous catheter (CVC) inserted in the hemodialysis unit. The timeout for making an arteriovenous fistula (AVF) for chronic hemodialysis in Ouagadougou may exceed six months. The CVCs used at the time of the study were made of polyurethane and non-tunneled. Sociodemographic, clinical and paraclinical data were collected and analyzed. Results: During the study period, 156 CVCs were installed in 114 patients, and the average age was 41.8 ± 17.1 years. A CRI occurred on 39 CVCs for a rate of 9.5/1000 days-catheter. The frequency of CRI was 28.4%, 20.6% and 20% respectively in femoral, jugular, and subclavian veins. The culture of the tip of the catheter was done in 21 cases. Fifteen germs have been identified: eight cases of Gram-positive cocci (53.3%) and seven cases of Gram-negative bacilli (46.7%). It was essentially staphylococci (eight cases). In univariate analysis, a period of use of the CVC in chronic hemodialysis over 10 days was significantly (p = 0.01;r = 2.91) associated with the catheter-related infection (CRI). Conclusion: The impact of the CRI was very high in our series. The associated factor in the univariate analysis was the long period of use of the CVC. The most diligent production of AVFs at the YO-UHC would contribute to a significant reduction of the CRI.

The Resources, an Essential Element in the Relationship Between Public and Private

This paper＇s objective is to examine the relationship between public and private sectors through the mechanisms of public intervention in the market. With the financial and economic crisis of 2008, there has been a coordination of states （G20） for the implementation of affirmative action in order to support the international financial system. Our hypothesis is that resource dependence is the basis of all interactions between the public and private sectors. The fact that the public sector bails out private sector activity shows that it is beneficial to citizens through employment and taxes repaid by the company and all this contributes to social stability. Here we develop an argument in two phases. The first is to revisit the question of the relationship between the state and the market. The second part develops the hypothesis by focusing on the interdependence of resources. The theory of resource dependence often applied to international relations and inter-organizational business has allowed us to highlight the correlation between public and private sectors

Provenance, depositional process, and tectonic setting of gold placer from the Bétaré-Oya Gold District(East-Cameroon, Central Africa)along the Precambrian Sanaga fault

The Bétaré-Oya Gold District is one of the three gold districts in East Cameroon with varied types of mining. The study site is an open-cast mine located in an alluvial plain. The strata succession globally consists of two positive sequences represented by three facies associations and seven lithofacies: massive clast-supported(Gcm), massive trough-cross bedded(Gtm), and massive matrix-supported(Gmm)conglomerates; massive sands(Sm); and massive(Fm), heterolothic(Fhl), and convolute and heterolithic(Fhlc) clays. The facies analysis indicates deposition after a recent tectonic event creating a depression,followed by a rapid deposition of varied facies and energies of sedimentation with fluvial and gravity inputs. The deposits come from proximal to distal sources but more proximal. The identified heavy mineral or accompanying minerals reveal a plutonic source(tourmaline sometimes folded), a volcanic source(olivine or clino-pyroxene with cracks), and a metamorphic source(sillimanite, andalusite, and staurolite). In the pan concentrates of placer with high silver(Ag) concentration(100 ppm) are also associated with relative abundance of lead(Pb)(17.8 ppm), zinc(Zn)(27 ppm), and low levels of copper(Cu)(3.1 ppm). The concentration and distribution of some element ratios, such as thorium/scandium(Th/Sc), chromium/thorium(Cr/Th), and europium anomaly(Eu/Eu*), rare earth elements(REEs) and aluminum oxide/titanium dioxide(Al2O3/TiO2) vs. silicon dioxide(SiO2) plots for the studied sediment indicate heterogeneous sources of rocks with dominance of felsic sources. The discriminant diagram reveals a recycled or sedimentary provenance of the deposited sediments. The tectonic setting plot indicates a collision tectonic setting which is consistent with the Precambrian history of the study area which belongs to the Pan-African North-Equatorial fold Belt. This article chronicles a failed research *** designed and carried out a psychological intervention aimed at increasing esogastric and lung cancer patients’emotional competencies after *** present the final protocol of the study,a randomized controlled trial in a public hospital,and describe the difficulties encountered and our subsequent reflections,to provide researchers and clinicians with advice for the implementation of such ***,the role of psychology,emotions,and emotional competencies,is still underacknowledged in cancer *** efforts must be made to convince both physicians and patients of the importance of those *** and consequently,even distressed patients sure to benefit from such an intervention,do not take it *** particular,male patients often declined the intervention due to gender stereotypes,and as such creativity is needed to present such interventions in a motivating way for ***,and most importantly,even if there is a good rationale for a psychological intervention and all favorable conditions are present,it is essential to first conduct a feasibility/pilot ***,even the most thorough preparation is no guarantee of anticipating all issues due to important gaps between theory and practice.

Chaos Appearance during Domain Wall Motion under Electronic Transfer in Nanomagnets

In this paper, we study the likelihood of chaos appearance during domain wall motion induced by electronic transfer. Considering a time-varying current density theory, we proceed to a numerical investigation of the dynamics. Using the dissipation parameter, amplitude and frequency of current density as control parameters;we show how periodic regime as well as chaotic regime can be exhibited in nanomagnetic systems. Numerical results allow setting up the periodicity and quasi-periodicity of system and chaotic phenomena occurring during magnetization switching process in nanomagnet through electronic transfer.
