Mitomycin C and capecitabine: An additional option as an advanced line therapy in patients with metastatic colorectal cancer

BACKGROUND In recent years survival of patients with metastatic colorectal cancer(mCRC),though still limited,has improved significantly;clearly,when the disease becomes refractory to standard regimens,additional treatment options are *** have shown that mitomycin C(MMC),an antitumor antibiotic,and capecitabine,a precursor of 5-fluorouracil,may act synergistically in *** efficacy of MMC/capecitabine has been demonstrated in the first-line setting,but only a few small studies have tested it in the advanced-line setting,with contradictory *** a median of 2 MMC/capecitabine cycles(range 0.5-9.0).Thirty-four patients(28.6%)experienced grade≥3 toxicity,including 2(1.7%)with grade 4;there was no drug-related *** objective response rate was 0.8%,and the disease control rate,24.4%.Median progression-free survival(PFS)was 2.1 mo(range 0.2-20.3),and median overall survival,4.8 mo(range 0.2-27.5).The 6-month overall survival rate was 44%;8.7%of patients remained *** associated with longer PFS were lower gamma-glutamyl transferase level(P=0.030)and primary tumor location in the left colon(P=0.017).Factors associated with longer overall survival were lower gamma-glutamyl transferase level(P=0.022),left-colon tumor location(P=0.044),low-to-moderate histological grade(P=0.012),Eastern Cooperative Oncology Group performance status 0-1(P=0.036),and normal bilirubin level(P=0.047).CONCLUSION MMC/capecitabine is an active,available,and relatively safe regimen for use beyond standard lines of therapy in *** clinical and laboratory parameters can identify patients more likely to benefit.

Cholecystoenteric fistula with and without gallstone ileus: A case series

Background: A cholecystoenteric stula (CEF) is an uncommon complication of gallstone disease. The aim of this study was to present our experience of a series of patients with CEF, presenting with or without gallstone ileus, along with their surgical outcomes. Methods: From 2015 to 2018, 3245 consecutive patients underwent cholecystectomy for gallbladder disease at our institution, of which 15 were diagnosed with a CEF. All electronic medical records were retrospectively reviewed. Results: Fifteen patients presented with CEF. Ten patients presented cholecystoduodenal stula, four pa- tients cholecystocolonic, and one patient cholecystogastric counterparts. Twelve patients were female. The median patient age was 61 years (range 33 86 years). Five patients presented with gallstone ileus treated by laparotomy and enterolithotomy. In ten patients, a laparoscopic approach was attempted, but conversion to open surgery was necessary for eight of them. The median operative time was 140 min (range 60 240 min), and the median operative blood loss was 50 mL (range 10600mL). The procedure-related morbidity and mortality rates were 13.3% and 6.7%, respectively. Conclusions: There is no consensus on the best treatment modality for a CEF, as the treatment outcome is mostly dependent on the surgeon’s expertise and the patient’s condition. Not all CEFs are accompanied by gallstone ileus. For such case, the main purpose is to resolve the intestinal obstruction and, unless necessary, avoidance of the gallbladder area.

Modulation of immunity and inflammatory gene expression in the gut, in inflammatory diseases of the gut and in the liver by probiotics

The potential for the positive manipulation of the gut microbiome through the introduction of beneficial microbes, as also known as probiotics, is currently an active area of investigation. The FAO/WHO define probiotics as live microorganisms that confer a health benefit to the host when administered in adequate amounts. However, dead bacteria and bacterial molecular components may also exhibit probiotic properties. The results of clinical studies have demonstrated the clinical potential of probiotics in many pathologies, such as allergic diseases, diarrhea, inflammatory bowel disease and viral infection. Several mechanisms have been proposed to explain the beneficial effects of probiotics, most of which involve gene expression regulation in specific tissues, particularly the intestine and liver. Therefore, the modulation of gene expression mediated by probiotics is an important issue that warrants further investigation. In the present paper, we performed a systematic review of the probiotic-mediated modulation of gene expression that is associated with the immune system and inflammation. Between January 1990 to February 2014, PubMed was searched for articles that were published in English using the MeSH terms “probiotics' and 'gene expression' combined with “intestines', 'liver', 'enterocytes', 'antigen-presenting cells', 'dendritic cells', 'immune system', and 'inflammation'. Two hundred and five original articles matching these criteria were initially selected, although only those articles that included specific gene expression results (77) were later considered for this review and separated into three major topics: the regulation of immunity and inflammatory gene expression in the gut, in inflammatory diseases of the gut and in the liver. Isolation and characterization of Pseudomonas otitidis WL-13 and its capacity to decolorize triphenylmethane dyes

Pseudomonas otitidis WL-13, which has a high capacity to decolorize triphenylmethane dyes, was isolated from activated sludge obtained from a wastewater treatment plant of a dyeing industry. This strain exhibited a remarkable color-removal capability when tested against several triphenylmethane dyes under both shaking and static conditions at high concentrations of dyes. More than 95% of Malachite Green and Brilliant Green was removed within 12 h at 500 μmol/L dye concentration under shaking conditions. Crystal Violet lost about 13% of its color under the same conditions tested. The rate of decolorization increased when the M9 medium was supplemented with yeast extract. The optimum pH and temperature for color removal were 7-9 and 35-40℃, respectively. The observed changes in the visible spectra and the inspection of bacterial growth indicated the color-removal by the adsorption of dye to the cells during incubation with strains.

Removal of heavy metals using waste eggshell

The removal capacity of toxic heavy metals by the reused eggshell was studied. As a pretreatment process for the preparation of reused material from waste eggshell, calcination was performed in the furnace at 800℃ for 2 h after crushing the dried waste eggshell. Calcination behavior, qualitative and quantitative elemental information, mineral type and surface characteristics before and after calcination of eggshell were examined by thermal gravimetric analysis （TGA）, X-ray fluorescence （XRF）, X-ray diffraction （XRD） and scanning electron microscopy （SEM）, respectively. After calcination, the major inorganic composition was identified as Ca （lime, 99.63%） and K, P and Sr were identified as minor components. When calcined eggshell was applied in the treatment of synthetic wastewater containing heavy metals, a complete removal of Cd as well as above 99% removal of Cr was observed after 10 rain. Although the natural eggshell had some removal capacity of Cd and Cr, a complete removal was not accomplished even after 60 rain due to quite slower removal rate. However, in contrast to Cd and Cr, an efficient removal of Pb was observed with the natural eggshell rather than the calcined eggshell. From the application of the calcined eggshell in the treatment of real electroplating wastewater, the calcined eggshell showed a promising removal capacity of heavy metal ions as well as had a good neutralization capacity in the treatment of strong acidic wastewater.

One of the biggest challenges in the biocompatibility of implantable metals is the prevention of the stress shielding effect,which is related to the coupling of the bone-metal mechanical *** stress shielding phenomenon provokes bone resorption and the consequent adverse effects on prosthesis ***,it can be inhibited by adapting the stiffness of the implant *** the use of titanium(Ti)porous structures is a great alternative not only to inhibit this effect but also to improve the osteointegration of orthopedic and dental implants,a brief description of the techniques used for their manufacturing and a review of the current commercialized implants produced from porous Ti assemblies are compiled in this *** powder metallurgy(PM)with space holder(SH)is a powerful technology used to produce porous Ti structures,it is here discussed its potential for the fabrication of medical devices from the perspectives of both design and *** most important parameters of the technique such as the size and shape of the initial metallic particles,the SH and binder type of materials,the compaction pressure of the green form,and in the sintering stage,the temperature,atmosphere,and time are reviewed according to the bibliography ***,the importance of the porosity and its types together with the influence of the mentioned parameters in the final porosity and,consequently,in the ultimate mechanical properties of the structure are ***,a few examples of the PM-SH application for the manufacturing of orthopedic implants are presented.

Effects of Mg Enhancement and Heat Treatment on Microstructures and Tensile Properties of Al2Ca-Added ADC12 Die Casting Alloys

The effects of Mg enhancement and heat treatment on the microstructures and tensile properties of Al_2Ca-added ADC12 die casting alloys were investigated. 0.3% and 0.5% Mg in the form of a master alloy including a trace amount of Al_2Ca were added to conventional ADC12（383 and AlSi10Cu2Fe） alloy with an initial Mg-content of 0.3% to increase the Mg content to 0.6% and 0.8%, respectively. To avoid heat treatmentinduced surface blisters, shortened solution treatment for 15 min at 490 ℃ and artificial aging for 6 h at 150 ℃ was undertaken. The results show that a 10% improvement in the shape factor of eutectic Si particles was achieved for Al_2Ca-added ADC12 with 0.8% Mg compared to the conventional ADC12 in the as-aged condition. Al_2Ca-added ADC12 with 0.8% Mg exhibited a yield strength of 289 MPa, a tensile strength of 407 MPa, and an elongation of 4.22%.
