Effects of Obesity and Smoking on Survival in Non-Small Cell Lung Cancer

Species in Neofabraea, Pezicula, and related genera have been reported as saprobes, plant pathogens or endophytes from a wide range of hosts. The asexual morphs of Neofabraea and Pezicula had been placed in Cryptosporiopsis, now a synonym of Pezicula, while Neofabraea was also linked to Phlyctema. Based on morphology and molecular data of the partial large subunit nr DNA（LSU）, the internal transcribed spacer region with intervening 5.8S nrDNA（ITS）, partial β-tubulin region（tub2）, and the partial RNA polymerase II second largest subunit region（rpb2）, the taxonomy and phylogenetic relationships of these fungi were investigated. Five new species were described in Pezicula based on morphology, while a further eight unnamed phylogenetic lineages revealed further diversity in the genus. Based on these results, the generic concept of Neofabraea was also emended. Phlyctema, which was previously associated with Neofabraea, formed a distinct clade, separate from Neofabraea s. str. Two new neofabraea-like genera, Parafabraea and Pseudofabraea were proposed, along with one new combination in Neofabraea s. str. To stabilise the application of these names, an epitype was designated for Pe. carpinea, the type species of Pezicula, and for N. malicorticis, the type species of Neofabraea.

Background: Obesity is an emerging leading cause of morbidity and mortality in the US and the relationship between obesity, tobacco, and survival in NSCLC is unclear. Methods: Data (n = 87,631) were obtained from linkage of the 1996-2007 Florida Cancer Data System to the Agency for Health Care Administration database providing procedure and diagnoses codes. Survival time was calculated from date of diagnosis to date of death. Smoking status was categorized as never, current, and former. Obesity (yes/no) = ICD9 code BMI > 30 kg/m2, cachexia = ICD9 code “wasting syndrome”, & non-obese = non-obese & non cachectic. Cox proportional regression models used to predict survival;demographic, clinical, treatment factors, & comorbidities were included in adjusted models with smoking status and obesity as the main factors. Results: The majority of patients (pts) were either former (49%) or current (40%) smokers, & non-obese (88%). 6.8% of pts were obese & 4.8% of pts were cachectic. There were significant differences between survival curves and median survival (months) for obese vs. non-obese vs. cachectic pts. 目的一直以来,围手术期血红蛋白监测依赖于测量血浆红细胞比容(Hct)的血气分析设备计算饱和度。测量总血红蛋白量(t Hb)的碳氧-血氧定量法,可获得对血液稀释更全面的评估。本研究旨在评估施行大心脏手术的住院患者中用碳氧-血氧定量法检测的t Hb和用电导率检测的Hct,与红细胞(red blood cell,RBC)输注、住院时间(length of stay,LOS)和住院费用的相关性。方法回顾性分析2014年1月至2016年6月行冠状动脉旁路移植术(CABG)和/或瓣膜置换(VR)术患者的临床资料,使用Med Assets出院数据。患者按照检测方式(t Hb和Hct)进行分组,采用详细的账单记录和当代操作术语编码。根据医院特定的成本费用比来计算住院费用。采用多变量logistic回归法来确定RBC输注和资源利用的重要驱动因素。结果本研究纳入18 169例心血管手术患者。其中Hct监测患者占66%,并更有可能进行CABG+VR双重手术(10.4%vs.8.9%,P=0.006 9)。在控制了患者和医院特征及患者合并症后,Hct组较t Hb组有明显更高的RBC输注风险(OR=1.26,95%CI 1.15~1.38,P<0.000 1),更长的LOS(IRR=1.08,P<0.000 1)和更高的费用(IRR=1.15,P<0.000 1)。结论与Hct监测相比,在心血管手术期间进行t Hb监测可以显著减少RBC输注和住院费用、缩短住院时间。

PURPOSE: Biofeedback is an effective treatment for patients with fecal incontinence, yet little is known about how it works or the minimum regime necessary to provide clinical benefit. This study compares the effectiveness of a novel protocol of telephone-assisted biofeedback treatment for patients living in rural and remote areas with the standard face-to-face protocol for patients with fecal incontinence. METHODS: A new treatment program comprising an initial face-to-face assessment and treatment with transanal manometry and ultrasound biofeedback, followed by three treatments conducted via telephone and a final face-to-face assessment, was developed. Standard treatment involved five face-to-face treatment sessions with manometry and ultrasound. Patients from rural areas were offered the telephone-assisted treatment protocol. Data gathered prospectively included incontinence scores, a quality of life index, anal manometry, and external sphincter isometric and isotonic fatigue times. RESULTS: A total of 239 consecutive patients treated between july 2001 and july 2004 were enrolled. There were no significant differences in demographic details, past history, or pretreatment measures of the two groups. Forty-six of 55 patients (84 percent) treated with the telephone protocol and 129 of 184 (70 percent) treated by the standard technique completed treatment. There were substantial, significant improvements after treatment, including 54 percent mean improvement in patient’ s own rating of their incontinence in both groups; a mean decrease of 3.1 and 3.2 on the St. Mark’ s incontinence score (from 7.9 to 4.7 and 7.4 to 4.2 of 13) and relative improvements of 128 and 130 percent in the quality of life index (from 0.29 to 0.65 and 0.3 to 0.69 of 1) for the telephone-assisted and standard groups respectively.
