Bilateral benign reactive lymphoid hyperplasia of the conjunctiva: a case treated with oral doxycycline and review of the literature

Background:To report a case of bilateral benign reactive lymphoid hyperplasia(BRLH)of the conjunctiva treated with oral doxycycline and perform review of the literature evaluating the presentation,treatment and risk of transformation to *** presentation:A case report is described and review of the literature from January 1975 to January 2019 was performed.A 30-year-old man presented with bilateral enlarging fleshy pink medial canthal conjunctival *** biopsy revealed *** doxycycline was initiated(100 mg two times a day)for a total of 2 *** lesions decreased in size significantly at the patient’s two-month follow up *** residual lesion in the right eye was excised along with an adjacent pterygium and the patient has been free of recurrence for the past 1.5 *** lesion in the left eye has remained stable in size after cessation of the oral doxycycline.A total of 235 cases of conjunctival BRLH were identified in our literature *** mean age at diagnosis was 35.2 years(range,5 to 91 years).BRLH lesions were unilateral in 75%of patients and bilateral in 25%of *** patients(2.9%)had a concurrent Epstein-Barr virus(EBV)infection at the time of lesion *** most common treatments were surgical excision(155/235 or 65.9%)and corticosteroids(30/235 or 12.7%),while 14%(33/235)of the patients were observed and 4.6%(11/235)received external beam radiotherapy *** occurred in ten patients(10/235 or 4.2%),of whom five had undergone surgical excision alone,two excision followed by external beam radiotherapy,one excision and oral corticosteroids,one radiotherapy alone and one had been treated with topical ***,only 2 of the 235 reported cases(0.8%)developed malignancy,one localized to the conjunctiva and one ***:Benign reactive lymphoid hyperplasia is one of the lymphoproliferative disorders of the conjunctiva and ocular *** literature review shows that m

Mean nocturnal baseline impedance in gastro-esophageal reflux disease diagnosis:Should we strictly follow the Lyon 2 Consensus?

Clinical practice guidelines drive clinical practice and clinicians rely to them when trying to answer their most common *** of the most important position papers in the field of gastro-esophageal reflux disease(GERD)is the one produced by the Lyon *** an updated second version has been *** nocturnal baseline impedance(MNBI)was proposed by the first Consensus to act as supportive evidence for GERD *** a cut-off of 2292 Ohms was proposed,a value revised in the second *** updated Consensus recommended that an MNBI2500 Ohms can be used to refute *** proposed cut-offs move in the correct direction by diminishing the original cut-off,nevertheless they arise from a study of normal subjects where cut-offs were provided by measuring the mean value±2SD and not in symptomatic ***,data exist that even symptomatic patients with inconclusive disease or reflux hypersensitivity(RH)show lower MNBI values in comparison to normal subjects or patients with functional heartburn(FH).Moreover,according to the data,MNBI,even among symptomatic patients,is affected by age and body mass ***,various studies have proposed different cut-offs by using receiver operating characteristic curve analysis even lower than the one ***,no information is given for patients submitted to on-proton pump inhibitors pH-impedance studies even if new and extremely important data now ***,even if MNBI is an extremely important tool when trying to approach patients with reflux symptoms and could distinguish conclusive GERD from RH or FH,its values should be interpreted with caution.

Critical considerations for the management of acute abdomen in transplant patients

Objective Brain metastases significantly impact the clinical course of patients with hepatocellular carcinoma(HCC).This study aimed to examine the age-related incidence,demographics,and survival of patients with HCC and brain *** Data of HCC patients from 2010 to 2015 in the Surveillance,Epidemiology,and End Results(SEER)Registry were screened for the presence of brain *** were stratified by age and *** logistic and Cox regression analyses were used to identify factors associated with brain metastases and those with overall survival(OS)and liver cancer-specific survival(CSS),*** A total of 141 HCC patients presenting with brain metastases were identified,accounting for 0.35% of all HCC patients and 2.37% of patients with metastatic *** all HCC patients,the incidence rate was the highest among patients aged 30-49 years old(0.47%).Ethnicity was not associated with the presence of brain metastases at the time of HCC ***,African-American patients presented with a significantly lower disease-specific survival[median time:1 month;interquartile range(IQR):0-3.0 months].Initial lung or bone metastasis was independently associated with an increased risk of the presence of brain metastases[odds ratio(OR):12.62,95% confidence interval(CI):8.40-18.97]but was not associated with a worse OS or CSS among those with brain *** This study identified the age-related incidence and risk factors of brain metastases in HCC *** results may contribute to the consideration of brain screening among patients with initial metastatic HCC with lung or bone metastases,and influence the counseling of this patient population regarding their prognosis.

Smarter in the city?Lizards from urban and semi-natural habitats do not differ in a cognitive task in two syntopic species

Urbanization occurs at a global scale,imposing dramatic and abrupt environmental changes that lead to biodiversity ***,some animal species can handle these changes,and thrive in such artifcial *** possible explanation is that urban individuals are equipped with better cognitive abilities,but most studies have focused on birds and mammals and yielded varied *** have received much less attention,despite some lizard species being common city *** Italian wall lizard,Podarcis siculus,and the common wall lizard,Podarcis muralis,are two successful lizards in anthropogenic habitats that thrive in urban *** test for differences in a cognitive skill between urban and semi-natural environments,we investigated inhibitory control through a detour task in syntopic populations of the two species,across 249 lizards that were tested in partially artifcial feld *** inhibitory control is considered essential for higher degrees of cognitive fexibility and other higher-level cognitive *** this task,we confronted lizards with a transparent barrier,separating them from a desired shelter area that they could only reach by controlling their impulse to go straight and instead detour the *** found no differences between lizards in urban and semi-natural environments,nor between species,but females overall performed better than ***,48%of the lizards in our study did not perform a correct trial in any of the 5 trials,hinting at the diffculty of the task for these *** study is among the frst to address lizard cognition,through their inhibitory control,as a potential explanation for success in cities and highlights one should be careful with assuming that urban animals generally have enhanced cognitive performance,as it might be taxa,task,or condition dependent.
