Fatal submucosal invasive gastric adenosquamous carcinoma detected at surveillance after gastric endoscopic submucosal dissection

An 80-year-old man was under annual surveillance esophagogastroduodenoscopy after endoscopic submucosal dissection(ESD) for early gastric cancer(EGC).Two years after the initial ESD, a 0-Ⅱc type metachronous EGC lesion, 8 mm in size, without an ulcer scar, was found in the gastric antrum.The estimated tumor depth was up to the mucosa, and biopsy revealed well and poorly differentiated adenocarcinoma.ESD was performed for this lesion and en bloc resection with negative margins was achieved.Histopathological examination revealed an adenosquamous carcinoma 8 mm in size invading the deep submucosal layer(1600 μm), with lymphovascular invasion, consistent with the diagnosis of non-curative resection.Additional gastrectomy was recommended for this patient; however, two months after the ESD, preoperative computed tomography revealed multiple liver metastases, and the patient was considered as an unsuitable candidate for surgical resection.Systemic chemotherapy was therefore started; however, the patient died of gastric cancer 27 mo after the second ESD.Early gastric adenosquamous carcinoma localized to the mucosa and submucosa is extremely rare and its clinical behavior is not well known.The present report is very significant in that it underscores the distinct possibility of gastric adenosquamous carcinoma being very aggressive and fatal even when detected at an early cancer.

We study conditions of Hormander's L^(2)-estimate and the Ohsawa-Takegoshi extension theorem.Introducing a twisted version of the Hormander-type condition,we show a converse of Hormander's L^(2)-estimate under some regularity assumptions on an n-dimensional domain.This result is a partial generalization of the one-dimensional result obtained by Berndtsson(1998).We also define new positivity notions for vector bundles with singular Hermitian metrics by using these conditions.We investigate these positivity notions and compare them with classical positivity notions.

A cross-flow wind turbine near a structure was tested for the performance. The results showed that the performance of a cross-flow wind turbine near a structure was up to 30% higher than the one without a structure. In addition, we tried to get higher performance of a cross-flow wind turbine by using an Inlet Guide Structure and a Flow Deflector. An Inlet Guide Structure was placed on the edge of a structure and a Flow Deflector was set near a cross-flow wind turbine and can improve ambient wind environments of the wind turbine, the maximum power coefficients were about 15 to 40% higher and the tip speed ratio range showing the high power coefficient was wide and the positive gradients were steep apparently.

Advancement in mid-infrared(MIR)technology has led to promising biomedical applications of MIR spectroscopy,such as liquid biopsy or breath diagnosis.On the contrary,MIR microscopy has been rarely used for live biological samples in an aqueous environment due to the lack of spatial resolution and the large water absorption background.Recently,mid-infrared photothermal(MIP)imaging has proven to be applicable to 2D and 3D single-cell imaging with high spatial resolution inherited from visible light.However,the maximum measurement rate has been limited to several frames s-1,limiting its range of use.Here,we develop a significantly improved wide-field MIP quantitative phase microscope with two orders-of-magnitude higher signal-to-noise ratio than previous MIP imaging techniques and demonstrate live-cell imaging beyond video rate.We first derive optimal system design by numerically simulating thermal conduction following the photothermal effect.Then,we develop the designed system with a homemade nanosecond MIR optical parametric oscillator and a high full-well-capacity image sensor.Our high-speed and high-spatial-resolution MIR microscope has great potential to become a new tool for life science,in particular for live-cell analysis.

The paper aims at reconstructing the conception of descriptive analysis shared by Brentano and the early Husserl.According to this shared conception,the descriptive analysis consists in the articulation of the multi-layered part-whole structure of consciousness.Focusing on the problem of intentional reference,the paper shows how they make different distinctions among parts of consciousness to carry out the descriptive analyses thus defined.Further,it shows how such a difference is closely connected to the two philosophers'views on the nature of intentional reference.

Cerium-doped yttrium aluminum garnet（YAG：Ce） as a yellow phosphor for white light-emitting diodes（LEDs） was synthesized via a facile combustion method using Y2 O3, CeO2, Al2 O3, Al,and NaClO4 as raw materials. The combustion synthesis approach utilizes the strong exothermic oxidation of aluminum to realize a self-sustaining reaction. In this study, we investigated the effects of the ratios of Al2 O3 to AI,fluxes, and coprecipitated materials as raw materials on the luminescence properties of the synthesized YAG：Ce phosphors. When the amount of Al2 O3 x is varied, the combustion reaction proceeds at x ≤ 1.8,with x = 1.725 being the optimum condition for producing a high-performance product. When 5 wt%BaF2 is added, the luminescence intensity is significantly improved owing to a decrease of YAP（YAlO3）formation with improved uniformity. However, the addition of CaF2 and NaF does not improve the luminescence properties. To suppress the segregation of CeO2, we used the coprecipitated material Y2 O3-CeO2 as a raw material. Unlike with separate addition of Y2 O3 and CeO2, Ce ions are uniformly distributed in the coprecipitated material, resulting in improved luminescence properties. The combination of BaF2 and coprecipitated material significantly improves the internal quantum efficiency to83.0%, which is close to that of commercial phosphors.

We report the lithium ionic conductivities of closo –type complex hydrides synthesized from various molar ratios of lithium borohydride(LiBH4) and decaborane(B10H14) as starting materials. The prepared closo –type complex hydrides comprised [B12H12]^2-, [B11H11]^2-, and [B10H10]^2- complex anions. In addition, increasing the LiBH4 content in the starting materials increased the amounts of [B11H11]^2- and [B10H10]^2-, leading to an improved ion conductivity of the prepared sample. The present study offers useful insights into strategies for controlling the complex anion composition in emerging solid electrolytes of closo-type complex hydrides at the molecular level, and improving their ionic conductivities.

Nowadays mandibular advancement appliances (MA- As) are available and distributed widely in the field of dentistry for the treatment of snoring and ob- structtive sleep apnea (OSA). However, a few stud- ies discussed temporomandibular joint (TMJ) discom- fort and masticatory muscle stiffness during wearing MAAs are found. A new appliance that we introduced in this pilot study has shown significant jaw move- ment and could diminish TMJ and masticatory side effects.
