Laparoscopy within the urology field has undergone a noteworthy surge in popularity over the past decade,with particular importance given to renal surgeries such as simple nephrectomy,radical nephrectomy,donor nephrectomy,nephroureterectomy,and partial nephrectomy.1 This shift toward laparoscopic procedures among urologists can be attributed to the many benefits of these procedures,including reduced postoperative pain,shorter hospital stays,and faster recovery times.2 However,as with any evolving surgical technique,laparoscopic procedures in renal surgery bring forth their own set of distinctive challenges and complications.

The effort invested in a software project is probably one of the most important and most analyzed variables in recent years in the process of project management. The determination of the value of this variable when initiating software projects allows us to plan adequately any forthcoming activities. As far as estimation and prediction is concerned there is still a number of unsolved problems and errors. To obtain good results it is essential to take into consideration any previous projects. Estimating the effort with a high grade of reliability is a problem which has not yet been solved and even the project manager has to deal with it since the beginning. In this study, performance of M5-Rules Algorithm, single conjunctive rule learner and decision table majority classifier are experimented for modeling of Effort Estimation of Software Projects and performance of developed models is compared with the existing algorithms namely Halstead, Walston-Felix, Bailey-Basili, Doty in terms of MAE and RMSE. The proposed techniques are run in the WEKA environment for building the model structure for software effort and the formulae of existing models are calculated in the MATLAB environment. The performance evaluation criteria are based on MAE and RMSE. The result shows that the M5-Rules have the best performance and can be used for the effort estimation of all types of software projects.

We are reporting four cases of acquired double orifice mitral valve (DOMV) of rheumatic etiology (rare) presenting as significant mitral stenosis (MS) treated successfully by percutaneous intervention. All four patients are young (3 males, 1 female) who presented with dyspnea of Newyork heart association (NYHA) class II with mean duration of 1.3 years. Typical clinical findings of MS were present in all. Echocardiography confirmed the diagnosis of moderate to severe MS of rheumatic etiology with double orifice of mitral valve of Trowitzsch incomplete bridge variety [1], giving appearance of two equal (like a pair of spectacles in one patient) or unequal size orifices (in three patients) in parasternal short axis view. Color Doppler examination revealed separate jets originating from each orifice, determined severity of the lesion and evaluated the eligibility for balloon mitral valvuloplasty (BMV). BMV was carried out under transeosophagial echo (TEE) guidance (for perfect septal puncture and crossing the separate orifices) using Inoue balloon of appropriate size (in accordance with patient’s height) with intention to break central fibrous strand converting into a single orifice. All four patients underwent successful BMV (clinical & ECHO criteria) after dilating the separate orifices (except case 4) but the fibrous strand could not be broken. Hence, this study shows that good results can be obtained in acquired double orifice mitral valve with significant MS with BMV even without breaking the fibrous strand. Considering rarity of isolated DOMV cases and rheumatic etiology being very rare, this report of four rheumatic acquired DOMV cases with successful BMV done with good result is significant.

Many variations can be applied to traditional mammoplasty techniques to improve outcomes in certain situations. The purpose of this report was to demonstrate the indications and benefits of mammoplasty autoaugmentation using an extended superomedial pedicle. All patients who underwent transfer of an extended superopedicleto other parts of the breast for autoaugmentation were included. Indications were determined and outcomes were assessed. Forty-eight patients were included in the series. The average follow-up was 2.1 years. Indications were categorized into reconstruction of a partial mastectomy defect (oncoplastic group, n = 18), upper pole volume in the contralateral mastopexy (implant reconstruction group, n = 9), and volume improvement and suspension (massive weight loss group, n = 21). The overall complication rate was 15% (n = 7/48), with a revision rate of 10% (5/48). The extended superomedial pedicle is a reliable and versatile adjunct to regular mastopexy techniques for various indications. It gives us the ability to transfer vascularized tissue from the lower pole to areas that require autoaugmentation.

The Coronavirus 2019 (COVID-19) pandemic has widespread implications for clinical practice of otolaryngologists in clinics and hospitals. With various reports of otolaryngology practitioners catching infection, a profound structural reorganization of ENT services in the clinic is mandatory for protecting both patients and healthcare workers. The present study focused on quantifying the cost involved in reorganizing the otolaryngology out-patient services in a third world country during the ongoing Covid-19 pandemic. Though the pandemic has increased the cost of running of an otolaryngology practice world over, the impact is huge in India as penetration of health insurance/social security is minimal. As out of pocket expenditure forms a significant proportion of healthcare spending by majority in India, any transfer of additional cost incurred because of Covid-19 pandemic to the patient will burn a bigger hole in their pocket.

Staphylococcal bullous impetigo in a neonate

An otherwise healthy, full-term neonate presented at day 15 of life to the pediatric emergency with generalized papulo-pustular rash for 2 d. This was finally diagnosed as bullous impetigo caused by Staphylococcus aureus(S. aureus). The skin lesions decreased significantly after starting antibiotic therapy and drainage of blister fluid. There was no recurrence of the lesions on followup. This case of generalized pustular eruption due to S. aureus in a neonate is reported, as it poses a diagnostic dilemma and can have serious consequences if left untreated.

Microbiological Treatment of Effluent for Zinc Hydrometallurgical Plant

Sulfate reducing bacteria were isolated from the soil sample of Roaster-Acid unit of Debari Zinc Smelter of Hindustan Zinc Ltd., India and were adapted for effluent water of Zinc hydrometallurgy plant to sustain zinc concentration of about 500 ppm and iron concentration of about 160 ppm with high acidity (pH < 1.0). Nutrient broth, whey, ethanol and sucrose were tested as carbon source for SRB (sulfate reducing bacteria) out of which whey was found as the best electron donor to expedite the rate of biological treatment with SRB. A two stage anaerobic bioreactor was developed with a treatment capacity of 30 L effluent water in 4 days. The concentration of heavy metals in the treated water was within the permissible limits. The suggested methodology generates no solid waste, is environment friendly and may be commercially implemented for treatment of zinc plant effluent.
